1.Long-term outcomes of drug-eluting stents implanted in distal unprotected left main coronary artery lesions

Objective To evaluate the long-term clinical outcome of percutaneous coronary intervention ( PCI) with drug-eluting stents ( DES) for distal unprotected left main coronary artery (ULMCA) lesions. Methods Between Jan 2005 and Dec 2009, 111 patients with distal ULMCA disease who underwent drug-eluting stents implantation were enrolled in this retrospective study. The primary end points were major adverse cardiac and cerebrovascular event (MACCE) at follow-up, including death, non-fatal myocardial infarction, cerebrovascular event and target lesion revascularization ( TLR) . Results Patients were 65. 6 ± 10 years old. 21. 6% were diabetic. 31. 5% of the left main bifurcations were classed as Medina 1, 1, 1. Provisional T stent technique was performed for 79. 3% of the population. 30. 6% of cases underwent angiography follow-up. The median follow-up time was 2. 3 years, the MACCE-free survival was 87. 4% and the estimated freedom from TLR was 94. 6% . Diabetes mellitus was identified as the predictor of TLR. Conclusions PCI with DES for distal ULMCA disease was safe and effective when the strategy was made based on the case-by-case assessment.
2.Effects of Gymnadenia conopsea alcohol extract on collagen synthesis in rat lungs exposed to silica and its mechanism of antioxidative stress

OBJECTIVE: To explore the effects of Gymnadenia conopsea alcohol extract (GcAE) on the collagen synthesis in rat lungs exposed to silica and the influence on antioxidase activities, level of lipid peroxidation (LPO). METHODS: One hundred and twenty rats were randomly divided into control group, silica group, and GcAE-treated group. Silicotic animal models were established by direct tracheal instillation of silica into rat lungs surgically. From the second day of model establishment, rats in GcAE-treated group were orally given GcAE [8 g/(kg x d) corresponding to raw herb]. At 7, 14, 21, 28 and 60 days after establishment of the animal model, eight rats in each group were sacrificed, and samples were collected. The malondialdehyde (MDA) content, superoxide dismutase (SOD) and glutathione peroxidase (GPx) activities in plasma were assayed by a spectrophotometer. Types I and III collagen were detected by Sirius red polarization and microscopy, and measuered by Image-Pro Plus Version 4.5 for Windows software. RESULTS: GcAE could reduce the lung/body weight ratio of rats exposed to silica, the synthesis of types I and III collagen of the lungs and the level of lipid peroxidation, increase the activities of SOD and GPx. CONCLUSION: GcAE can ameliorate the silica-induced pulmonary fibrosis by increasing the activities of antioxidase and alleviating the damage of lipid peroxidation to the lungs.
3.Three methods for microbiological monitoring on dialysate and dialysis water for hemodialysis

Objective To evaluate detection effect of three methods on monitoring microbes in dialysate and dialysis water for hemodialysis.Methods Seventy-two dialysate and dialysis water specimens were collected from 36 medical institutions,specimens were cultured with three methods: blood agar plate incubated at 35℃ for 72 hours,Tryptic soy agar(TSA)plate incubated at 35℃ for 72 hours,and Reasoner's 2A agar(R2A agar)plate incubated at 23℃ for 168 hours,colony counts,isolation of colony,and detection rate of colony exceeding action level(≥50 CFU/mL)were compared among three methods.Results The colony isolation rates of microbes in dialysate and dialysis water detected by blood agar plate,TSA plate and R2A plate were 40.28%,63.89%,and 69.44%respectively,difference was significant(x2=14.16,P<0.05);pairwise comparison showed that isolation rates of colony on R2A agar plate and TSA plate were higher than blood agar plate.There was significant difference in isolated colony count between blood agar plate and R2A agar plate,TSA plate and R2A agar plate respectively(Z=-4.515,-6.970 respectively,both P<0.05).The rates of isolated colony exceeding action level in dialysate and dialysis water detected by blood agar plate,TSA plate,and R2A agar plate were 1.39%,4.17%,and 20.83%respectively,difference was significant(x2=19.83,P<0.05),detection rate of R2A agar plate was higher than the other two methods.Conclusion The detection rate of colony by R2A agar plate and TSA plate are better than blood agar plate,detection rate of colony exceeding action level by R2A agar plate is higher than TSA plate and blood agar plate,R2A agar plate for microbial monitoring(23℃,168 h)on dialysate and dialysis water is superior to the other two methods.
4.Emergency treatment of large amputated ear defect with auricular cartilage replantation.

OBJECTIVETo investigate the emergency treatment for large amputated ear defect.
METHODSFrom Feb. 2010 to Oct. 2013, 5 cases with large unilateral amputated ear defects were treated. The amputated auricular cartilage was replanted subcutaneously in mastoid area at the first stage. Cranioauricular sulcus was reconstructed and skin grafting was performed at the second stage.
RESULTSAll cases were followed up for 3-6 months with satisfactory result. The reconstructed helix, scapha and cavity of auricular concha had similar color and elasticity as the ear at healthy side. Scar at donor sites, such as inguinal region and armpit, was inconspicuous.
CONCLUSIONSThe method of auricular cartilage replantation is an effective way for emergency treatment of large amputated auricular defect with less morbidity at donor site.

5.Preparation of diatomite based nano-composite dental ceramic powders by layer-by-layer technique

Objective To prepare a novel bioactive and degradable scaffold with mineralized collagenpolyose based composite by biomimetic synthesis for bone tissue engineering and explore the compatibility of osteoblast culturing on the scaffold. Methods Two kinds of polyelectrolyte were assembled on the surface of diatomite particles in order to adsorbe on nano-zirconia through opposite charges. Zeta potential,particle size and size distribution were compared before and after the modification of diatomite; IR was used to analysis molecular structure of functional group changes on the surface of diatomite particles, nano-composite powder morphology was observed by SEM. Results Two kinds of the polyelectrolyte were successfully assembled on the surface of diatomite powders. Particle size and size distribution were significantly reduced, d (0.5) reduce from 16.421 μm to 0.420 μm. SEM showed the dispersion of the modified diatomite was improved and had a good adsorption with nano-zirconia. Conclusion Layer-by-layer technique could enhance the dispersion of diatomite-based dental ceramic powders as well as a good adsorption of nano-composite ceramic powder.
6.Application of Perclose Proglide vascular closure devices in endovascular repair of thoracic aortic dissection

Objective To investigate the application value of the Perclose Proglide vascular devices in the thoracic endovascular aortic repair (TEVAR) of aortic dissection.Methods Retrospective analysis of 106 patients who underwent TEVAR for Standford B type aortic dissection were performed.The femoral lumen was measured by CTA be fore,1 month and 1 year after TEVAR.Results A total of 223 Perclose Proglide vascular closure devices were used in the 106 patients,including 97 patients with 2 devices,7 patients with 3 devices,2 patients with 4 devices.The puncture femoral artery diameters had no significant differences between before and 1 month,1 year after TEVAR (all P >0.05).Conclusion Per close Proglide vascular closure devices can be effectively and safely used in the TEVAR,which has little influence on the femoral artery diameter,and is worth to be applied in the clinics extensively.
7.Differential diagnosis in bloodstream infection with Trichosporon asahii and Geotrichum capitatum

Objective To provide reference for establishing diagnosis and differential diagnosis methods of rare yeast-like fungal bloodstream infection for clinical microbiology laboratory.Methods Trichosporon asahii (T.asa-hii)and Geotrichum capitatum (G.capitatum)bloodstream infection was diagnosed and differentially diagnosed through clinical data analysis,morphological examination,biochemical reactions,and molecular biology technology. Results Two types of yeast-like fungal bloodstream infections in case 1 and case 2 both occurred in leukemia agranulocytosis phase after chemotherapy,such infections were serious and highly similar.The cultivated colonies on blood agar plates of case 1 and case 2 were performed gram stain and microscopic examination.Hyphae,arthro-spores and microconidia were visible in the former,thickness of hyphae branches and length of arthrospores were different,most presented rectangular and barrel shape;the latter can be seen hyphae with transparent septum bro-ken up into arthrospores,presented rectangular shape,did not produce blastoconidia.Identification with API 20C AUX showed that they were T.asahii and G.capitatum.The PCR product sequences were compared with NCBI, suggesting that T.asahii and G.capitatum were at sexual stage.Conclusion Comprehensive application of a varie-ty of technical methods is helpful for improving the diagnosis accuracy of bloodstream infection with yeast-like fungi, identifying Trichosporon and Geotrichum to the species level may help physicians to understand such rare fungal in-fection,choose antifungal agents rationally,and improve clinical prognosis.
8.Effect of adenosine on epileptiform discharge of hippocampal neurons from lithium chloride-pilocarpine induced epileptic rats

Objective To use the whole-cell patch clamp recording to observe the effect of adenosine on hippocampus pyramidal neurons epileptiform discharge from lithium chloride-pilocarpine induced epileptic rats.Methods Twenty adult male and female SD rats (weighing about 200 g) were selected and were narcotized by 10% chloral hydrate intraperitoneal injection.The whole brain was removed, then chopped into 350 μm thick slices.The brain slices were transferred to 37 ℃ preheated artificial cerebrospinal fluid for 1 h and then transferred to 23 ℃ for 30 min.Action potentials (AP) and evoked excitatory postsynaptic currents (eEPSCs) of brain slices neurons were recorded by whole-cell patch clamp, in addition, 25 μmol/L adenosine was used to observe its effect on AP and eEPSCs.Results With the application of current clamp technique, the application of adenosine (25 μmol/L) significandy decreased the numbers of AP in epileptic neurons (P < 0.01) compared with the normal neurons ((0.50 ± 0.06) nA), and eEPSCs amplitude of epileptic neurons increased significantly ((1.44 ± 0.06) nA;independent sample t test, t =30.99, P < 0.01) by voltage clamp.Bath application of adenosine (25 μmol/L) significantly reduced eEPSCs amplitude ((0.66 × 0.06) nA), while application of A1 receptor inhibitor DPCPX partially reversed this effect ((0.92 × 0.06) nA;one-way analysis of variance and q test, F =266.3, q =9.81, P < 0.01).Conclusion Adenosine possesses a strong inhibitory effect on epileptic hippocampal brain slices discharge, which is mediated by its effect on eEPSCs.
9.The application of ultrasound-CT double-guided radiofrequency ablation for hepatic tumors:preliminary experience in 15 cases

Objective To discuss the clinical application of ultrasound-CT double-guided radiofre-quency ablation(RFA) in treating hepatic tumors. Methods Fifteen patients with 20 liver tumor lesions were included in this study. Ultrasound-CT double-guided radiofrequency ablation was employed in all patients. First, under ultrasound guidance the electrode of RFA was inserted to the site close to the lesion, then, guided by CT scanning the accurate positioning of the electrode was accomplished and RFA procedure was completed. Results The accurate puncturing of the electrode was achieved in all 20 hepatic lesions, and the RFA procedure was successfully performed in a short time. Follow-up examination showed that there was no obvious residual tumor tissue, and no RFA-related complications occurred in all the 15 patients. Conclusion Under ultrasound-CT double-guidance, the percutaneous transhepatic puncturing can be more accurately accomplished, which can ensure a successful RFA procedure. This technique is especially useful when the hepatic tumor is incomplete or unclear on ultrasonograph, and it can improve the puncturing accuracy and reduce the complications as well.
10.Morphologic characteristics of atherosclerotic basilar arteries at 3.0T high-resolution MRI

Objective To explore the remodeling modes and the plaque distribution of atherosclerotic BA at 3.0T high resolution MRI.Methods 90 symptomatic patients with atherosclerotic stenosis of BA on digital subtraction angiography (DSA) (50 %-99 %) were recruited consecutively.Luminal area,vessel area of maximal narrow sites and the reference sites were measured.The differences of involved imaging parameters between negative group and positive group were analyzed.Results 51 patients with required imaging quality were enrolled finally.Among the 51 patients,the rate of positive remodeling cases was 72.5% (37/51) and negative remodeling took over 27.5% (14/51).Compared with the negative remodeling group,the positive remodeling group had greater plaque size,larger plaque burden percentage,and higher maximal wall thickness at maximal lumen narrowing sites.The plaques were mainly located at ventral wall of the vessel.Conclusion 3.0T high-resolution MR imaging could be applied in assessing the remodeling modes and plaque distribution of BA stenosis.
