Oxidative stress is one of the important pathogenesis of diabetic nephropathy.In recent years,the role of oxidized low-density lipoprotein(Ox-LDL) in the occurrence and progress of diabetic nephropathy has been more and more concerned about,and becomes a hot spot once again.This paper makes a review of the formation and metabolic mechanism of Ox-LDL,the changes of Ox-LDL in the progress of diabetic nephropathy and its clinical significance,and a number of new treatments for Ox-LDL in recent years to further understand the role of Ox-LDL in the occurrence and progress of diabetic nephropathy,then to provide new ideas for effective clinical treatment of diabetic nephropathy. 2.Teaching practice to overcome neurophobia at clinical practice stage

Neurology is a relatively difficult subject in all medical subjects, to which medical students, interns and low qualification residents generally have a kind of fear, called neurophobia. Through such teaching methods as bedside teaching, problem and case oriented group discussions, mini-lectures and net-work communication, and combined with the specific case and imaging data, our teaching and research section helps students consolidate the basic knowledge of neuroanatomy, grasp the physical examination of the nervous system, and cultivate their thinking of positioning and qualitative diagnosis, so as to enable them to achieve better learning effect in the shortest practice time when they are in clinical neurological depart-ment, and help them to overcome neurophobia.
3.Application of video-assisted thoracoscopic surgery for thoracic traumas

Objective To research the feasibility and superiority of video-assisted thoracoscopic surgery(VATS) in the diagnosis and treatment of thoracic traumas.Methods Video-assisted thoracoscopic surgery was used in 40 patients with thoracic traumas,including thoracic exploration,repair of pulmonary laceration,and evacuation of clotted hemothorax.[WTHZ]Results All the 40 patients were cured,including simple VATS in 31 patients,thoracoscopy-assisted mini-incision surgery in 8 patients,and conversion to open surgery in 1 patient.The time of operation was 79.9?33.1 min.The amount of blood clots and noncondensing blood cleared was 567.5?177.8 ml.The closed thoracic drainage tube was removed at 24~48 h postoperatively,with a drainage volume of 220?45.6 ml.The length of hospital stay was 4~13 d(mean,8.7 d).The sutured wound healed by first intention in all the patients.No postoperative complications were observed.Follow-up reviews in 35 patients for 6~12 months(mean,8.6?2.6 months) revealed uneventful recovery and no trauma-related complications.Conclusions As compared with conventional open surgery,VATS has shown advantages of exact diagnosis,timely management,little invasion,and quick recovery for patients with thoracic traumas.

5.Exploratory on Phase 0 Clinical Pharmacodynamics Trial in Chinese Materia Medica

Shu-Xue-Ning (SXN) injection was used as study subject in order to explore the feasibility of pharmacodynamics experiment of Phase 0 clinical trial in Chinese materia medica (CMM).This paper tried to establish key-technique standards which fit to the evaluation on CMM phase 0 (early) clinical evaluations.According to the established research methods,this research selected volunteers and assured low dose SXN injection inside their body.And then,serum before and after medication were collected for the HAEC in vitro experiment.Blood sample of the clinical dosage was used as control to verify the tendency consistency of pharmacodynamics in low dose and clinical dose.The results showed that in the H2O2 inducing HAEC damage experiment,low dose SXN injection can affect the cell proliferation,protect the shape of cells,reduce the release of ion owing to cell damages and influence the secretion in cells.Complete-sequence RNA gave differential multiterm expression before and after the low dose.It was concluded that compared with the clinical group,low dose medication had similar metabolic tendency.The pharmacodynamics experiment of phase 0 CMM clinical trial is feasible.
6.Latent problems and thinking of the standardization of syndrome differentiation based on the diseases of Western Medicine(WM)

The standardization of syndrome differentiation based on the diseases of WM has become an important academic guidance,which,however,has induced lots of problems.The logical contradiction was analyzed between the results of standardization of syndrome differentiation based on the diseases of TCM and WM,between the results of two large-scale standardizations of syndrome differentiation based on WM and between the results of syndrome differentiation of related diseases of WM.In this paper,the author studied on the origin of symptoms and signs of syndrome differentiation of the diseases of WM and the effect of standardization of syndrome differentiation of the diseases of WM on the nature and location of disease in TCM,and pointed out some important unresolved academic problems.
7.Prognostic observation of surgery or endovascular embolization for high grade intracranial aneurysms after interim conservative treatment

Objective To observe the prognostic effects of the patients with intracranial saccular aneurysm (Hunt-Hess grade Ⅳ- Ⅴ)first treated conservatively for 12 hours and then with surgical treatment and endovascular treatment. Methods The clinical data of 32 patients with intracranial saccular aneurysm (grade Ⅳ,n = 24 and gradeⅤ,n = 8)Hunt-Hess grade Ⅳ-Ⅴadmitted from January 2012 to January 2014 were analyzed retrospectively. Sixteen of them were treated conservatively for 12 hours in hospital,and then they were treated with surgery or embolization (postpone surgery group)and 16 underwent emergency surgery or embolization (immediate surgery group). The neurological prognosis of the patients was evaluated at 1,3 and 6 months before and after treatment. Results There was no significant difference in Glasgow outcome scores between the postpone surgery group and immediate surgery group at 1 month after treatment (3. 7 ± 1. 4,3. 8 ± 1. 2;t = 1. 372,P > 0. 05);there was no significant difference in Rankin prognostic scores at 3 months after treatment (3. 7 ± 1. 7,3. 6 ± 1. 5;t = 1. 361,P > 0. 05);But there was significant difference in prognostic scores at 6 months after treatment between the two groups (3. 5 ± 1. 5, 4. 2 ± 1. 8;t = 2. 234,P < 0. 05). Conclusion Patients with high grade cystic aneurysmal subarachnoid hemorrhage can be treated with conservative treatment. After 12 hours,endovascular embolization or surgical treatment may be performed. The prognosis at 6 months is better than emergent direct surgery or embolization.
8.Effects of enteral immUNonutrition on immune function in patients with multiple trauma

Objective To study the effects of enteral immunonutrition on immune function in patients with multiple trauma. Method A total of 32 patients with multiple trauma admitted to general ICU in ChangZheng hospital, Shanghai from March 2007 to May 2008 were randomly (random number) divided into two groups,enteral immunonutrition group (EIN group, n = 16)and the general formula enteral nutrition group (EN group, n = 16).Two groups of patients on the third day after injury began to accept the EIN and the general formula EN which respectively support at least 14 days. The patients peripheral blood lymphocyte count (TLC), immunoglobulin (IgG,IgM, IgA) ,and T-lymphocyte subsets (CD3, CD4, CD8, CD4/CD8) were detected on the day of the group formation, the seventh and the fortteenth day of the nutritional support. Results TLC, IgG, IgM, IgA, CD4 and CD4/CD8 ratio were significantly higher in EIN Group on the 7th and 14th day than that on the first day ( P <0.05), and extended with the EIN and sustained increase. All immunization indicators of EN group on the 7th day were no significant changes compared with the day of the group formation, and on the 14 th day TLC, IgG, IgM,IgA, CD4 and CD4/CD8 ratio were significantly higher than that on the day of the group formation ( P < 0.05),but were significantly lower than that in the EIN group on the 14th day ( P < 0.05). Conclusions Compared with the general formula EN, EIN is more helpful to restore and increase the humoral and cellular immune function in early post-multitraumatic phase.

Recent studies have showed that nanotechnology pro-vides a new revolution for the treatment of complex,refractory autoimmune or inflammatory diseases.Nanoparticles combined with specific autoantigens can restore the peripheral tolerance and reduce immunopathological damage in a variety of autoimmune disease models.In this paper,we review the progress and trends of nanomedcine for treatment of autoimmune or inflammatory dis-eases as immunomodulatory therapy from the physical and chemi-cal characteristics of nanoparticles,the route of administration, and the role and mechanism of induction or reconstitution of im-mune tolerance,which provides a theoretical basis for seeking new treatments to treat inflammatory diseases.
10.Adaptive Moving Averaging Based Estimation of Single Event-Re lated Potentials

Event-related potentials (ERP) is pertinent to medical research and clinical diagnosis. Estimation of single event-related potentials (sERP) is the objective of ERP processing. A new technique, adaptive moving averaging ba sed method for estimation of sERP, is presented. After analysis of the properties of background noise by crossingzero, the window length of moving averaging is adapt ively set according to the maximum width of the impulse noise for each recorded raw data. The experiments are made with real recorded data and the results demon strate that the performance of sERP estimation is excellent. So the method propo sed is suitable to sERP processing.
