1.Verapamil attenuates the progression of pacing-induced ion channel remodeling in atrial myocytes

Objective To study the potential effect of verapamil for the early changes of L-type calcium channel ?1c and potassium channel Kv4.3 in a rapid paced primary cultured atrial myocyte model for atrial fibrillation. Methods Primary rat atrial myocytes were cultured and established a rapid paced primary cultured atrial myocyte model. Atrial cells were divided into four groups: control group, rapid pacing group, verapamil with rapid pacing group and verapamil without rapid pacing group. Durations of rapid pacing were 24 h. The mRNA and protein expressions of L-type calcium channel ?1c and potassium channel Kv4.3 were measured by RT-PCR and Western blotting respectively. Results It was found that 24 h pacing significantly reduced mRNA and protein expressions of L-type calcium channel ?1c and potassium channel Kv4.3 as compared to controls (P0.05). Conclusion Expressions of L-type calcium channel ?1c and potassium channel Kv4.3 were reduced in the rapid pacing atrial myocytes, suggesting ion channel remodeling of atrial myocytes. However, verapamil can attenuate the progression of ion channel remodeling of atrial myocytes at least in early phase.
2.The effects of individualized therapeutic programs on chronic hepatitis C and the influential factors of virological response

Objective To investigate the effect of individualized therapeutic programs with combination of interferon and ribavirin (RBV) in chronic hepatitis C (CHC) and study the influential factors of virological response rates.Methods A total of 139 patients with CHC were enrolled and given the intensive treatment doses of interferon and RBV according to their basic clinical condition.At the treatment of 0,4,12,24 weeks,the end of treatment and 24 weeks after treatment stop,the serum HCV RNA was determined.Timely adjustment to dosage and time periods was made according to the virological response to treatment,and the predictive value of rapid virological response (RVR) and complete early virological response (cEVR) for sustained virological response (SVR) were analyzed.Results At the 4th week of treatment,the level of serum HCV RNA was monitored in 120 patients,and 84.2% (101/120) of patients obtained RVR; among them,90.7% (88/97) obtained SVR.The virus load of patients obtained RVR at pretherapy was lower than that of patients didn't obtained RVR [(5.883±1.246) lg copies/ml vs (6.502±0.693)lg copies/ml,P =0.034].The RVR rate of initial treatment patients with PEG-IFNα-2a [87.8%(79/90)]was significantly higher than that of retreatment patients with PEG-IFNα-2a [65.0% (13/20)](P =0.031).At the 12th week of treatment,the level of serum HCV RNA was monitored in 132 patients,and 92.4% (122/132) of patients obtained cEVR; among them,90.8% (108/119) obtained SVR.The SVR rate of patients obtained cEVR was significantly higher than that of patients didn't obtained cEVR (5/9) (P =0.007).There was no significant difference between the cEVR rate of initial treatment patients [94.7% (90/95)]and retreatment patients [85% (17/20)]with PEG-IFNα-2a (P =0.158).Conclusions cEVR was predictor of SVR.Individualized therapy can increase the obtaining probability of RVR,cEVR and SVR.Adjusting drug dose timely and extending treatment period of HCV RNA-negative based on virological response to treatment are important in CHC individualized therapy.
3.Simulation of intestinal content sensory nerves with ultrasonic detection system

BACKGROUND:The existed artificial anal sphincter cannot sense whether there are intestinal contents in the bowel and the state of intestinal contents, and the previous studies on the intestinal content signal did not included the different parts of colon and the signals of different intestinal content status.
OBJECTIVE:To compare the differences of ultrasound signals of different intestinal content status in different parts of New Zealand rabbits detected with transmission-type ultrasonic signal detection system.
METHODS:Twenty healthy New Zealand rabbits were used;the rabbits were divided into groups according to distal part of proximal colon, distal colon and distal rectum. Five positions of solid stool particles were selected randomly as the detection point in each group. The voltage signals of stool were measured under solid, fluid, gaseous, and deflation status of the lumen.
RESULTS AND CONCLUSION:In rectum, there were no significant differences in voltage signals between solid and gaseous conditions, while there were significant differences between fluid and deflation conditions (P=0.000). Multiple comparison showed there were significant differences between four conditions in distal part of proximal colon and distal colon (P=0). There were significant differences in voltage singles between rectum, distal colon and proximal colon under solid and fluid conditions (P=0). Multiple comparison showed there were significant differences in any part of large bowel for solid and fluid status (P=0). There were no significant differences in voltage singles between rectum, distal colon and proximal colon under gaseous and deflation conditions. There was interactive effect of intestinal contents between three parts of large bowel under the four conditions (P=0.000). Transmission-type ultrasonic detection system can distinguish artificial intestinal contents sensory nerves of various intestinal contents. And the application of ultrasonic detection system to detect the intestinal contents under different conditions can provide a new effective method for the research on artificial intestinal content sensory nerves.
4.Laparoscopic right hemihepatectomy under selective hemihepatic inflow occlusion

Objective To evaluate the feasibility and safety of laparoscopic right hemihepatectomy under selective hemihepatic inflow occlusion.Methods In performing this procedure,first dissect the right hepatic portal,and under selective hemihepatic inflow occlusion,laparoscopie Peng's multifunctional operative dissector (LPMOD) was used to transect the hepatic parenchyma by stepwise curettage and aspiration.Results Procedures were successful in all six patients of benign liver diseases,though a small subcostal auxiliary incision was needed in one case to control the middle hepatic vein branch hemorrhage.Operation time was 300-540 min[mean,( 399 ± 75 ) min].The time of hepatic portal dissect was 30-75 min[mean,(51 ± 16) min].The time of liver parenchyma transection was 60-160 min[mean,( 116 ± 32) min].Intraoperative blood loss was 600-3000 ml[mean,( 1486 ± 809 ) ml].The level of ALT increased to (302-557) U/L[mean,(386 ± 85 ) U/L]after the operation,and back to normal level in 5-11 d[mean,( 7.1 ± 2.0) d].The postoperative hospital stay was 10-18 d[mean,( 12.4 ± 2.6 ) d].No severe complication occurred after the operation.Conclusions Laparoscopic right hemihepatectomy under selective hemihepatic inflow occlusion is safe and feasible.
5.Current status and problems of coagulation factor Ⅷ and Ⅸ assay

Objective To investigate current status and problems of coagulation factor Ⅷ and Ⅸ assay in domestic laboratories so as to provide the reference for implementing the standardization and quality improvement.Methods A questionnaire survey was carried out in 76 laboratories,and quality control materials were distributed to 54 laboratories for activity assay.The questionnaire information was analyzed statistically.Test results of quality control materials were classified into three groups according to the reagents and the ranked grading analysis were used to evaluate the performance.Results This research was investigative study.The amount of sample was less than 30 per month in 72% (52/72)of laboratories.The frequencies of calibration were different,and 33% (24/72) of laboratories did not perform calibration in a different assay batch.39% (28/72)of laboratories did not run internal quality control,and about 21% (15/ 72) of laboratories just performed the normal level quality control.Individual laboratories showed a high cumulative CV (> 30%) of intemal quality control.For normal FⅧ and FⅨ control materials,the CV of results were 11.3%-18.2% and 11.3%-17.9% respectively as well as 15.3%-20.3% and 19.5%-21% for abnormal.Of the three groups,the proportions of laboratories which the FⅧ test results out with consensus were18%,24% and 22% as well as 20%,24% and 28% for FⅨ.Conclusions The key requirements for quality control of coagulation factors active assay remain to be addressed and implemented.The repeatability and comparability in some laboratories are not satisfactory to meet the clinical needs.With the purpose of promoting quality improvement,we need to develop guidelines,organize related training and establish a national external quality assessment scheme.
6.Investigations into the culture condition for human epidermal melanoblasts in vitro

Objective To study the in vitro culture condition for melanoblasts from human foreskin tissue. Methods The skin tissue taken from foreskin of children was treated with 0.5% dispase Ⅱ to separate epidermis from dermis, then with trypsin to obtain single cell suspension, which was cultured in modified medium for melanoblasts, i.e., MCDB254 medium supplied with several cell growth factors. Finally, melanoblasts were obtained based on the difference of adhesion speed. The morphology and proliferation of cultured melanoblasts were observed under a light microscope. DOPA staining, immunostaining with anti- S-100 and -tyrosinase related protein 2 (TRP2) antibodies, and transmission electron microscopy were per- formed to identify the cultured melanoblasts. Results The cultured human melanocytes displayed a match-like shape, scattered arrangement, syrmnetric double poles, slim cell body, highly refractive nuclei; meanwhile, the melanoblasts exhibited plentiful cytoplasm, large volume, bipolar or irregular shape and clonal growth. Additionally, the melanocytes were positive for TRP2, S-100 and Dopa staining, while the melanoblasts were positive only for TRP2. Electron microscopy revealed the presence of mature melanin granules (stage Ⅲ-Ⅳ ) in melanocytes but immature melanin granules (stage Ⅰ ) in melanoblasts. Conclu- sion Stable pure culture of melanoblasts has been realized with the reformed medium, which may lay a foundation for the investigation into the mechanism of epidermal pigmentation.
7.Application of association rules to risk prediction of sudden deafness

Objective To apply data mining to risk prediction of sudden deafness, and form the association rules.Methods The clinical data of 517 patients with sudden deafness was collected, including the characteristics of 19 attributes: sex, age, season, hypertension, diabetes, heart disease, hypercholesterolemia, atherosclerosis, long-term smoking, alcoholism, mental tension, insomnia, weakness, bedridden, infection, congenital malformation, trauma, tumour and autoimmune diseases. The source database were cleaned, then mapped for mining database. Minimum support to 0.1 and minimum confidence level to 0.9 were set for analysis of association rules. Results One hundred and six strong association rules were formed, and the rules contained the relation between the incidence of sudden deafness and the characteristics of 19 attributes. Conclusion This method is conducive to make the abstract theory of mathematical statistics into useful association rules to guide the practice of disease prevention and control.
8.Clinical application of 2000 ArthroCare System in knee arthroscopic surgery

Objective To evaluate the clinical application of 2000 ArthroCare System in knee arthroscopic surgery. Methods 221 cases of knee problems were treated with 2000ArthroCare System. The disorders of the 221 cases diagnosed by the arthroscopy were as follows: 73 cases of osteoarthritis, 49 meniscus tear, 29 degenerative cartilage injury, 11 plica synovitis, 11 Kaschin Beck disease, 8 ACL, 5 osteochondritis dissecans, and 2 TKA brisement. The operative procedures, such as meniscectomy, meniscoplasty, fitting of cartilage and ligament, synovectomy, and release of lateral patellar retinaculum, were done with 2000ArthroCare System and arthroscopy. Results The Lysholm Knee scores were 43.92 preoperatively, 81.96 three months postoperatively, and 92.06 six months postoperatively. Conclusion Knee problems can be effectively released with 2000 ArthroCare system vaporization under arthroscopic guidance. The advantages of this procedure are very limited tissue damage, mild reaction, less blood loss, early rehabilitation, and fine functional recovery.

Objective To understand the molluscicidal effects of water extract of Ginkgo biloba's peel on Oncomelania hupensis Methods Immersion methods with a series of six concentrations from 0.3 percent to 3 percent were used to determine the molluscicidal effect. Results The LC 50 of the water extract of 1.0 percent was 28.8 mg/L for 24 hours immersion,12.6 mg/L for 48 hours,and 0.48 mg/L for 72 hours, respectively. The LC 50 of the extract of 2 percent was 19.9 mg/L for 24 hours,11.2 mg/L for 48 hours and 0.48 mg/L for 72 hours,respectively.The LC 50 of the extract of 3 percent was 19.1 mg/L for 24 hours,10.9 mg/L for 48 hours and 0.48 mg/L for 72 hours,respectively. The higher of the concentration, the better of the effect. Meantime, it's poisoning to crops and fishes was very low. Conclusion Ginkgo biloba's peel has molluscicidal effect. It is a good candidate of potential plant molluscicides.
