1.Relationship among hypertensive cardiac change, kidney disease and hypertensive retinopathy

Objective:To analyze the relationship among hypertensive cardiac change,kidney disease and hyperten-sive retinopathy (HRP).Methods:A total of 100 patients with hypertension,who were diagnosed and treated in our hospital from Jan 2012 to Dec 2013,were enrolled,including 90 HRP patients (34 cases of class Ⅰ,49 cases of class Ⅱ,six cases of class Ⅲ and one case of class Ⅳ)and 10 cases with normal fundus (normal control group). Clinical data of all patients were analyzed,left ventricular mass index (LVMI),carotid intima-media thickness (IMT)and glomerular filtration rate (GFR)etc.were measured.Results:Compared with normal control group, there were significant thickening in IMT [(0.62±0.30)mm vs.(0.87±0.16)mm vs.(1.10±0.28)mm]in HRP class Ⅰ+Ⅱ group,class Ⅲ+Ⅳ group,the IMT of class Ⅲ+Ⅳ group was significantly thickened than that of class Ⅰ+Ⅱ group (P <0.01 all);Compared with normal control group,the incidence rates of cardiac dilation (0%vs.12.05% vs.71.43%)and kidney function damage (0% vs.24.10% vs.85.71%)significantly rose in HRP class Ⅰ+Ⅱ group,class Ⅲ+Ⅳ group,the incidence rates of cardiac dilation and kidney function damage in HRP class Ⅲ+Ⅳ group significantly rose than those of class Ⅰ+Ⅱ group (P <0.01 all).Conclusion:Severity of hyper-tension retinopathy is closely related to hypertensive cardiac dilation and kidney function damage.
2.Cerebral hemodynamic change for patients with hypertension complicated retinopathy

Objective:To explore the cerebral hemodynamic changes of patients with hypertension complicated reti-nopathy.Methods:A total of 77 patients with hypertension complicated retinopathy were enrolled as study group, another 70 patients,who received physical examination during the same period and its results were normal,were treated as normal control group.Index changes of middle cerebral artery (MCA)and anterior cerebral artery (ACA)were compared between two groups,and hemodynamic difference of ophthalmic artery (OA)and central retinal artery (CRA)were recorded.Results:Compared with normal control group:① Mean velocity of MCA and ACA [(64.28±13.29)cm/s,(20.12±7.21)cm/s vs.(129.18±20.14)cm/s,(89.35±10.98)cm/s]signifi-cantly rose in study group ,P <0.01 all;②There were significant reductions in peak systolic velocity (PSV)and end-diastolic velocity (EDV)of OA and CRA,P <0.05 all,and resistance index significantly rose in all classes of study group (P <0.05 all);③There were significant reductions in mean blood flow velocity [(21.62±7.36)ml/s vs.(13.47±5.13)ml/s],maximum blood flow velocity and minimum blood flow velocity ,and significant rise in pulse wave velocity,peripheral resistance,critical pressure,vascular characteristic impedance [(17.35±5.36)kPa ·s-1 ·m-1 vs.(31.68±6.47)kPa· s-1 · m-1 ],and dynamic resistance [(30.59 ± 6.98)kPa· s-1 · m-1 vs. (64.48±12.38)kPa·s-1 ·m-1 ],in basilar artery in study group,P <0.01 all.Conclusion:In patients with hy-pertension complicated retinopathy,cerebral blood supply indexes reduce along with blood pressure rises,CRA blood flow changes can be regarded as a judging basis for disease course development,which deserves highly atten-tion for patients and physicians.
3.Characterization of naive and memory T cell after immunization with H1N1 influenza vaccine for children

Objective:To elucidate the characterization of CD8+T cell in H1N1 influenza vaccine for children.Methods:PBMCs were isolated from 31 children aged from 3 to 6 years old who had accepted H 1N1 influenza vaccine during December 2009 to January 2010.The lymphocytes were joined with the H 1N1 influenza vaccine as experimental group and cultured .The experiment set without vaccine group as control group .At last we detected the surface molecules by FCM .The CCK-8 assay was added to detecting cellular proliferation and cellular proliferation index were detected by CCK-8.Results: CD8+T cells of PBMC in the two groups were 13.41%and 9.41%,P>0.05.CD8+CD45RAA+naive T cells in the two groups were up to more than 80%,P>0.05.The proportion of CD8+CD45ROA+memory T cells in two groups were up to 17%-19%,P>0.05.Two subsets of CD8+CD45ROA+memory T cells :CCR7+and CD62L+single positive memory T cell subsets in the experimental group were significantly lower than that of the control group,P<0.05.The CCK-8 assay was added to detect cellular proliferation .Only 51.16% of which cellular proliferation index was greater than 0.8,with none was greater than 1 in this study.Conclusion:This study showed that the CD4+T cells were low-level,naive T cells (CD8+CD45RAA+)were higher,with antigen stimulation and response.H1N1 vaccination specific memory T cells were few in number , specific memory T cell subsets were diversity , control memory cells were the main phenotypic characteristics .Cellular proliferation index showed that the proliferation of specific CD 8+T cells vaccine was poor .
4.Study of bletilla striata gelatin loaded macromolecular nerve growth fac-tor in treatment of deep pressure sore

Objective To observe the clinical efficacy of bletilla striata gelantin (BSG) loaded exogenous high molecular weight (HMW) nerve growth factor (NGF) in the promotion of deep pressure sore healing. Methods The patients with deep pressure sore were randomly divided into four group: group A, as the control group, used iodophor-soaked sterile gauze for continuous wet compressing; group B, group C and group D, as the experiment group, received external application of BSG, HMW-NGF and HMW-NGF+BSG 3 times per day. The observation indicators included the pressure sore healing velocity, healing time, general reactions and local skin inflammatory reactions during medication on day 3, 7, 14, 21 and 30 of medication. Results BSG, HMW-NGF and HMW-NGF+BSG could all promote the wound healing of pressure sore and HMW-NGF+BSG has the most remarkable effect (healing days: A∶B∶C∶D=20.16 ± 4.58∶15.43±1.72∶15.80±1.48∶11.86±1.35d). Conclusion HMW-NGF+BSG can obviously accelerate the wound healing of pressure sore.
5.MRI measurement for normal fetal brain biometric parameters in second and third trimesters

Objective To analyze the correlations of commonly used biometric parameters of normal fetal brain with fetal gender and gestational age (GA) and to evaluate the feasibility of establishing MRI reference standards for normal fetal brain biometry in second and third trimesters. Methods MRI measurements of 263 fetuses without central nervous system abnormalities were retrospectively collected from the Obstetrics and Gynecology Hospital of Fudan University from June 2012 to April 2017. MRI measurements of fetal biparietal diameter (BPD), occipital-frontal diameter (OFD), head circumference cross-sectional area (HCS), transverse cerebellar diameter (TCD), anterior-posterior diameter of the vermis (APDV), vermian height (VH) and vermian cross-sectional area (VS) were obtained in every standard plane. Head circumference (HC) and (BPD+OFD)/2 were calculated. Spearman correlation analysis, independent samples t-test, Pearson correlation analysis and linear regression analysis were used for statistic analysis. Results Fetal BPD, OFD and HCS were statistically correlated with gender in second and third trimesters (r= - 0.155, - 0.149 and- 0.159; P=0.032, 0.038 and 0.027). The mean values of BPD, OFD and HCS of male fetuses were greater than those of female ones [(7.25±1.22) vs (6.87±1.28) cm, (8.59±1.38) vs (8.18±1.41) cm, (53.62±16.82) vs (48.48±16.84) cm2; t=2.101, 2.011 and 2.102; all P<0.05] and the mean differences between them were 0.38 cm, 0.41 cm and 5.14 cm2, respectively. BPD, OFD, HCS, HC, (BPD+OFD)/2, TCD, APDV, VH and VS were positively correlated with gestational age (r=0.950, 0.947, 0.962, 0.957, 0.957, 0.976, 0.931, 0.943 and 0.960, respectively; all P<0.001). Among them, TCD (r=0.976) was the closest relating factor, followed by HCS (r=0.962) and VS (r=0.960). The linear regression equations were as follows: BPD (cm)=0.276×GA-0.453, R2=0.903; OFD (cm)=0.308×GA+0.010, R2=0.896; (BPD+OFD)/2 (cm)=0.292×GA - 0.222, R2=0.916;HC (cm)=0.473×GA - 0.359, R2=0.916; HCS (cm2)=3.795×GA - 52.232, R2=0.926; TCD (cm)=0.180× GA-1.858, R2=0.952; APDV (cm)=0.047×GA-0.353, R2=0.867; VH (cm)=0.071×GA-0.592, R2=0.890;VS (cm2)=0.143×GA-2.396, R2=0.919, all P<0.001. Conclusions Some fetal brain biometric parameters are correlated with fetal gender. It is essential and conducive to establish different reference values for male and female fetuses to achieve accurate prenatal evaluation and diagnosis. Fetal BPD, OFD, HCS, HC, (BPD+OFD)/2, TCD, APDV, VH and VS have strong linear relationship with gestational age in second and third trimester and can be served as good indicators for evaluating fetal brain growth and development. MRI quantitative measurement of fetal brain biometric parameters can provide more reliable reference data for prenatal evaluation of fetal brain growth and development. It is feasible to establish MRI reference standards for fetal brain biometry.
6.Efficacy prediction and molecular mechanism of thyroid hormone levels and multi-target TKI therapy for renal cell carcinoma and hepatocellular carcinoma

Hypothyroidism is a common adverse reaction in clinical multi-target tyrosine kinase inhibitor (TKI) and may be used as a predictor of efficacy of TKI drugs.Studies show that hypothyroidism is significantly associated with good prognosis in patients with renal cell carcinoma treated with multi-target TKI,which may be related to inhibition of vascular endothelial growth factor receptor pathway,potential off-target effects and inhibition of cell proliferation.Whereas in the patients with hepatocellular carcinoma who received multi-target TKI therapy,hypothyroidism is an indicator of poor prognosis,which is related to the T3/TR pathway regulating liver cancer growth and invasion.The predictive function and mechanism of thyroid hormone levels in different tumor patients receiving multi-target TKI drugs are different,and further research is still needed.
