1.Research of the effect of periodic telephone follow-up on primiparas′ exclusive breastfeeding ;during puerperal period

Objective To investigate the positive influence of telephone follow-up on primiparas′exclusive breastfeeding during puerperal period. Methods By using convenience sampling method, 90 primiparas were selected who had given birth in the integration ward of our hospital from October to December 2015 as the control group, and 60 primiparas were selected who had given birth in the integration ward of our hospital from December 2015 to January 2016 as the experimental group. For the experimental group, every 2 weeks, a telephone follow-up was executed according to primiparas′parturition time after leaving hospital. For the control group, telephone follow-up was not executed. The influence of periodic telephone follow-up on primiparas′ exclusive breastfeeding was observed. Results The exclusive breastfeeding rate of primiparas in the experimental group was increased as the telephone follow-up was executed. It was up to 91.7%(55/60) 6 weeks since parturition. The exclusive breastfeeding rate of primiparas in the control group was 71.1% (64/90). The difference was significant (χ2=9.28, P<0.01). Conclusions Periodic telephone follow- up according to necessity provides breastfeeding publicity and guidance after parturition, maintaining the continuous education and caring for the primiparas after leaving hospital which efficiently increased the pure breastfeeding rate of primiparas during puerperal period.
2.Prediction of the pharmacokinetic drug-drug interaction of pravastatin and pitavastatin with cyclosporine by a digital liver model based on metabolism and transporter.

Information of metabolic enzymes and transporters, physiological parameters of animals and demography of Chinese people were integrated to establish a digital liver model (DLM) based on metabolism and transporter and coded with VBA. Clearance and drug-drug interaction (DDI) of candidate drugs in animal and human could be predicted based on the pharmacokinetic data obtained from in vitro and in vivo experiments. Pravastatin and pitavastatin were selected as the samples to examine this model, where their clearance and their DDI with cyclosporine were predicted. The results showed that the predicted values of median parameters in same species were within twofold of observed values for 83.3% (5/6). The program's successful prediction in DDI tendency might indicate its application in optimizing the dosage regimen and reducing the risk of clinical trial.
3.Knowledge-attitude-practice(KAP)-based health management of type 2 diabetes among community residents

Objective To discuss the effectiveness of knowledge-attitude-practice(KAP)-based health management on type 2 diabetes in community residents.Methods From June 2010 to December 2011,930 type 2 diabetes patients living in Mishi Lane Community were enrolled and assigned to the intervention group(n=494) and the control group(n=436).The KAP mode was used for the intervention group,and the routine management mode was applied to the control group.Fasting blood glucose(FBG),PBG2 h,HbAlc,blood pressure(BP),triglyceride(TG) and body mass index(BMI) were tested and recorded.Student's t test or Chi-square test was used for data analysis.Results Age(t=0.124,P>0.05),gender(x2=2.0,P>0.05) and other demographics showed no statistically significant difference between the two groups(all P>0.05).After one-year KAP-based health management,HbA 1 c of the intervention group reduced to(6.1 ± 1.2)%,FBG and PBG2 h reduced by 1.7 and 3.23 mmol/L,respectively(t=4.926,P<0.05; t=4.306,P<0.05; t=4.523,P< 0.05).Diet,physical exercises,medication,regular inspection,unhealthy habits and questionare test awaveness of the intervention group were improved(86.03%,82.19%,85.63%,70.45% 76.11% and 88.46%,respectively.Drinking,dietary structure,overweight and physical exercises of the intervention group were also improved(14.17%,15.79%,70.65% and 68.83%,respectively; all P<0.001).Conclusion The KAPbased health management is proved to be effective in the control of community diabetic patients.
4.Community-based intensive health management for type Ⅱ diabetics

Objective To discuss the effectiveness of community-based intensified health management on type Ⅱ diabetes patients.Metbods From June 2010 to December 2011,1 024 type Ⅱ diabetes patients in Mishi Lane community were enrolled and assigned to the intervention group (n=501,obtaining intensive health management) and the control group (n=523,obtaining routine community health management).Fastingbloodglucose (FBG),2-hour postprandial blood glucose (PBG2h),HbA1c,blood pressure (BP),triglyceride (TG) and body mass index (BMI) were tested and recorded before and after the intervention.Student's t test or Chi-square test were used for data analysis.Results Differences of age (t=0.124,P>0.05) and gender (x2=2.0,P>0.05) were not statistically significant (both P>0.05).Following the intensive health management,the control rates of HbA1c,BP,TG and BMI were increased to 75.4%,30.4%,85.6% and 49.0%,respectively (x2 values were 4.54,4.71,6.54 and 3.67; all P<0.05).TG,FBG and PBG2h of the intervention group were (1.5 ± 0.9),(6.2 ± 1.3) and (8.3 ± 2.4) mmol/L,respectively,which were significantly lower than those of the control group (all P<0.05).Conclusion Community-based intensive health management could be effective for diabetes patients.
5.Study on the evaluation index system of the quality of health education among the elderly in the community

Objective To establish a scientific and feasible evaluation index system of the quality of health education among the elderly in the community.Methods Based on the results of literature review,theory analysis,cross-sectional study and interviews,a draft evaluation index system was formulated.Then Delphi method was conducted on 28 specialists around the country,the weight of each index was determined by hierarchy analysis.Results After three rounds of Delphi procedure,the response rates of the three round consultation were all above 85%,and the authority coefficient and coordination coefficients was 0.858,0.210,respectively.An evaluation index system which covered three first-level items,7 second-level items and 35 third-level items was formulated.Conclusions Based on survey of the elderly and interviews of health education staffs in the community,and using the method of Delphi to collect experts' suggestion,a strong maneuverability and high reliability quality evaluation index system was formulated,which provided guidance for community health education of the elderly,and promoted its development towards scientization and standardization.

Objective:To explore the relationship between the pathophysiological changes in TCM spleen de ciency syndromes and precancerous lesion of gastric mucosa.Method:160 cases with spleen de ciency syndromes accompanied by gastric mucosa intestinal metaplasia,IM and / or atypical hyperplasia,AHP,(spleen de ciency is divided into spleen qi de ciency or SQD,spleen yin de ciency or SyinD and spleen de ciency with qi stagnation or SDQS) and 22 cases in health control group were taken as the study object.The detection was conducted on such trace elements as Zn,Cu of gastric mucosa epithelial cell nuclei and mitochondrion under the direct view of CM200FEG –TEM by adopting 9100/60 -Energy Dispersion X-ray Analyzers;detection was conducted on epithelial cell nucleus DNA by using IBAS2000 image analyzer;detection was conducted on the expression of P53,Ki67,CerbB2,P21ras in gastric mucosa tissue slices by adopting ElivisionTM Plus'.Result:The quantitative changes of gastric mucosa epithelial cell nucleus DNA,Zn,Cu and the positive expression rates of P53,P21ras,CerbB2 and Ki67 increased in the sequence of SQD group,SyinD group and SDQS group,while mitochondrial Cu,Zn decreased in the same sequence,there was signi cant di erence between the groups(P
7.Effect of positive experience intervention on anxiety, depression and quality of life among rural empty nesters

Objective To evaluate the effect of positive experience intervention on anxiety, depression and quality of life among rural empty nesters. Methods A total of 115 rural empty nesters were divided into the intervention group (n=58) and the control group (n=57). Both groups received routine community activities. In addition, participants in the intervention group received positive experience intervention. Geriatric Depression Scale, Self-Rating Anxiety Scale and 12-item Short-Form Health Survey were adopted to measure the related items before and after the intervention. Results The intervention group showed a significant decrease in anxiety (1.54±0.07) and depression (7.28±3.82), compared with the control group (1.64±0.09, 11.81±6.38), the difference was significant (t=-5.960,-4.613, P<0.01). The Physical Component Summary (PCS) and Mental Component Summary (MCS) was 47.01±6.47 and 52.13±6.22 in the intervention group, which were significantly higher than those in the control group (41.37±8.71, 48.71±6.13), the differences were significant (t=3.937, 2.970, P < 0.01). Conclusions Positive experience intervention can decrease anxiety and depression, and improve quality of life of rural empty nesters.

Objective To investigate the quality control circle's effect assessment about improving the successful rate of intervening adding the non-doctor advised formulated milk powder for the newborn baby rooming-in in obstetric ward, decrease the nonmedical indication formula milkadding,and improve the rate of exclusive breastfeeding. Methods Found quality control circle group,analyze all the different elements which influence the success of nurse's intervening of adding the non-doctor advised formulated milk powder for the newborn baby in room-in system,target the theme,make the policy and implement it, compare the success rate of intervening adding the non-doctor advised formulated milk powder for the newborn baby rooming-in before and after quality circle's activity. Results Increase the success rate of nurse's intervening of adding the non-doctor advised formulated milk powder for the newborn baby rooming-in from 32.5%(13/40)before improvement to 70.7%(341/482), the target rate was 96.7%. Conclusions The quality circle's activity increases the success rate of nurse's intervening of adding the non-doctor advised formulated milk powder for the newborn baby rooming-in, while it enhances the comprehensive capability of nurse,and strengthens the clinical nursing specialty's quality.

Senescence of activated hepatic stellate cells (aHSCs) is a stable growth arrest that is implicated in liver fibrosis regression. Senescent cells often accompanied by a multi-faceted senescence-associated secretory phenotype (SASP). But little is known about how alanine-serine-cysteine transporter type-2 (ASCT2), a high affinity glutamine transporter, affects HSC senescence and SASP during liver fibrosis. Here, we identified ASCT2 is mainly elevated in aHSCs and positively correlated with liver fibrosis in human and mouse fibrotic livers. We first discovered ASCT2 inhibition induced HSCs to senescence in vitro and in vivo. The proinflammatory SASP were restricted by ASCT2 inhibition at senescence initiation to prevent paracrine migration. Mechanically, ASCT2 was a direct target of glutaminolysis-dependent proinflammatory SASP, interfering IL-1α/NF-κB feedback loop via interacting with precursor IL-1α at Lys82. From a translational perspective, atractylenolide III is identified as ASCT2 inhibitor through directly bound to Asn230 of ASCT2. The presence of -OH group in atractylenolide III is suggested to be favorable for the inhibition of ASCT2. Importantly, atractylenolide III could be utilized to treat liver fibrosis mice. Taken together, ASCT2 controlled HSC senescence while modifying the proinflammatory SASP. Targeting ASCT2 by atractylenolide III could be a therapeutic candidate for liver fibrosis.
