1.Prediction of the pharmacokinetic drug-drug interaction of pravastatin and pitavastatin with cyclosporine by a digital liver model based on metabolism and transporter.

Information of metabolic enzymes and transporters, physiological parameters of animals and demography of Chinese people were integrated to establish a digital liver model (DLM) based on metabolism and transporter and coded with VBA. Clearance and drug-drug interaction (DDI) of candidate drugs in animal and human could be predicted based on the pharmacokinetic data obtained from in vitro and in vivo experiments. Pravastatin and pitavastatin were selected as the samples to examine this model, where their clearance and their DDI with cyclosporine were predicted. The results showed that the predicted values of median parameters in same species were within twofold of observed values for 83.3% (5/6). The program's successful prediction in DDI tendency might indicate its application in optimizing the dosage regimen and reducing the risk of clinical trial.
2.Retrospective analysis of hepatitis B virus serological markers in 70 582 inpatients

Objective To analyze the detection rate of HBV serological makers in non-hepatic inpatients in the past six years. Methods Serum samples of 70 582 non-hepatic inpatients from three large hospitals were collected during 2003 to 2008. Serological markers of HBV ( HBsAg, anti-HBs, HBeAg, antiHBe and anti-HBc) were detected by the AxSYM MEIA system (Abbott Laboratories,Abbott Park,IL).Combining the test results of serological makers with other clinical data, several analysis models for this retrospective study were set up to evaluate the year-to-year changes in serological makers and the detection rates of each model. Results The order from high to low of detection rate of the 5 HBV serological markers was anti-HBc (55. 17% ), anti-HBs (49. 57% ), anti-HBe (28.42%), HBsAg ( 8. 92% ) and HBeAg (2. 12% ), and all of them had a downward trend in the past six years. The positive rate of HBsAg went down from 9. 30% (2003) to 8.70% (2008). The positive rate of HBsAg among people who were born after 1992 (2. 28% ) were significantly lower than that of the overall population (8. 92% ) and fell from 3.57%(2003) to 1.85% (2008). Each detection rate of all serological makers had male sexual side effect [HBsAg ( 12. 38%/7. 25% ), HBeAg ( 2. 72%/1.58% ), anti-HBc ( 56. 57%/53.43% ), anti-HBe (41.50%/28. 35% ) and anti-HBs (65.48%/50. 00% ), male/female]. The differences were statistically significant (Chi-square values of HBsAg, HBeAg, anti-HBc, anti-HBe and anti-HBs were 509.74,105.78, 69.66, 1 321.61 and 1 726.91, respectively; all P < 0. 01).Twenty-six models of HBV serological makers from 70 582 inpatients were summed up, and 8 models had positive rates geater than or equal to 1%. The "All Negative" model ranked No. 1 and had no significant change from year to year. During the past six years, models representing "A11 Negative" and "anti-HBs Positive alone" were mainly in individuals younger than or equal to 20-year-old, while the models representing "anti-HBc and/or anti-HBe,anti-HBs Positive" were mostly in people older than 20-year-old. The distribution curve of models representing "HBsAg, HbeAg and anti-HBc Positive" and "HBsAg, anti-HBc, anti-HBe Positive"etc. showed a bell-shape, covering the population from 20-year-old to 70-year-old. Conclusions The slowlydescending tendency of the detection rates of HBV serological makers was observed during the past six years.The detection rates of HBV in the younger generation decreased significantly. However, the HBV infection rates of overall population is still high, so it is a high time that we made continuous improvement for the serum HBV screening technique in order to reduce the HBV infection ratess.
3.Knowledge-attitude-practice(KAP)-based health management of type 2 diabetes among community residents

Objective To discuss the effectiveness of knowledge-attitude-practice(KAP)-based health management on type 2 diabetes in community residents.Methods From June 2010 to December 2011,930 type 2 diabetes patients living in Mishi Lane Community were enrolled and assigned to the intervention group(n=494) and the control group(n=436).The KAP mode was used for the intervention group,and the routine management mode was applied to the control group.Fasting blood glucose(FBG),PBG2 h,HbAlc,blood pressure(BP),triglyceride(TG) and body mass index(BMI) were tested and recorded.Student's t test or Chi-square test was used for data analysis.Results Age(t=0.124,P>0.05),gender(x2=2.0,P>0.05) and other demographics showed no statistically significant difference between the two groups(all P>0.05).After one-year KAP-based health management,HbA 1 c of the intervention group reduced to(6.1 ± 1.2)%,FBG and PBG2 h reduced by 1.7 and 3.23 mmol/L,respectively(t=4.926,P<0.05; t=4.306,P<0.05; t=4.523,P< 0.05).Diet,physical exercises,medication,regular inspection,unhealthy habits and questionare test awaveness of the intervention group were improved(86.03%,82.19%,85.63%,70.45% 76.11% and 88.46%,respectively.Drinking,dietary structure,overweight and physical exercises of the intervention group were also improved(14.17%,15.79%,70.65% and 68.83%,respectively; all P<0.001).Conclusion The KAPbased health management is proved to be effective in the control of community diabetic patients.
4.Community-based intensive health management for type Ⅱ diabetics

Objective To discuss the effectiveness of community-based intensified health management on type Ⅱ diabetes patients.Metbods From June 2010 to December 2011,1 024 type Ⅱ diabetes patients in Mishi Lane community were enrolled and assigned to the intervention group (n=501,obtaining intensive health management) and the control group (n=523,obtaining routine community health management).Fastingbloodglucose (FBG),2-hour postprandial blood glucose (PBG2h),HbA1c,blood pressure (BP),triglyceride (TG) and body mass index (BMI) were tested and recorded before and after the intervention.Student's t test or Chi-square test were used for data analysis.Results Differences of age (t=0.124,P>0.05) and gender (x2=2.0,P>0.05) were not statistically significant (both P>0.05).Following the intensive health management,the control rates of HbA1c,BP,TG and BMI were increased to 75.4%,30.4%,85.6% and 49.0%,respectively (x2 values were 4.54,4.71,6.54 and 3.67; all P<0.05).TG,FBG and PBG2h of the intervention group were (1.5 ± 0.9),(6.2 ± 1.3) and (8.3 ± 2.4) mmol/L,respectively,which were significantly lower than those of the control group (all P<0.05).Conclusion Community-based intensive health management could be effective for diabetes patients.
5.Analysis of contact allergens in 1065 patients with dermatitis or eczema in Wuxi city

Objective To determine contact allergens and their clinical significance in patients with dermatitis or eczema in Wuxi city.MethodsPatch test was conducted in 1065 patients with a clinical diagnosis of dermatitis or eczema.A statistical analysis was carried out.Results Positive patch test reactions were observed in 83.19% of the patients,with no statistical differences in the frequency of positive patch test reactions between female and male patients or between patients of different age groups.The most frequent 6 allergens were potassium dichromate,cobalt chloride,nickle sulfate,formaldehyde,carba mix and fragrance mix in these patients.Female patients showed a higher frequency of positive patch test reaction to nickle sulfate and formaldehyde,but a lower frequency to potassium dichromate and carba mix compared with male patients (all P< 0.01 ); young,middle-aged,and old patients exhibited an increased frequency of positive reaction to cobaly chloride compared with child patients,while young and middle-aged patients showed a decreased frequency to nickel sulfate compared with child patients (all P < 0.05).Conclusion Potassium dichromate,cobalt chloride,nickle sulfate,formaldehyde,carba mix and fragrance mix are the main allergens causing dermatitis and eczema in Wuxi city.
6.Effect of acupuncture at points selected from different regions on SCF-kit signaling pathway in diabetic gastroparesis rats

Objective: To observe the effect of acupuncture at points selected from different regions on the positive expression of interstitial cells of Cajal (ICC) and the stem cell factor (SCF) in gastric antrum tissues in diabetic gastroparesis (DGP) rats, and to explore the influence of region-based point selection on the acupoint combination efficacy. Methods: Sixty Sprague-Dawley (SD) rats were randomly divided into a normal group (group A), a model group (group B), a group of Zusanli (ST 36) plus Zhongwan (CV 12) (group C), a group of Zusanli (ST 36) plus Neiguan (PC 6) (group D), and a group of Zusanli (ST 36) plus non-meridian non-acupoint points (group E), based on the random number table (n=12). DGP rat model was established by single intraperitoneal injection of 2% streptozotocin and common diet. After successful modeling, the rats were treated once a day for 4 weeks. Positive ICC and SCF expressions were measured by immunohistochemistry. Results: Compared with group A, the gastrointestinal propulsion rate of group B showed a statistically significant decrease (P<0.05). Compared with group B, the gastrointestinal propulsion rate and the expression of ICC in the gastric antrum were significantly higher in group C, group D and group E, and the between-group differences were statistically significant (P<0.05); the expression of C-kit protein in group C was statistically significantly higher than that in group D and group E (P<0.05). The expression of SCF protein was significantly increased in group C than in group B, and the difference was statistically significant (P<0.05). Conclusion: Acupuncture can improve the symptoms of delayed gastric emptying in DGP model rats, and regulate the expression of ICC and SCF in gastric antrum tissues. The effect of Zusanli (ST 36) plus Zhongwan (CV 12) in the gastric region is superior to that of the Zusanli (ST 36) plus distal Neiguan (PC 6) or non-meridian non-acupoint point, indicating that region-based point selection is the key factor affecting the effect of acupoint combination.
7.Application of ultrasound-guided stellate ganglion block in patients with aneurysmal intracerebral hemorrhage

Objective To explore the application value of ultrasound-guided stellate ganglion block in patients with aneurysmal cerebral hemorrhage.Methods Eighty patients with subarachnoid hemorrhage caused by aneurysm from November 2017 to March 2019 in Hangzhou Hospital of Zhejiang Medical Health Group were selected.The patients were divided into observation group and control group by random digits table method with 40 cases in each group.All patients were treated with craniotomy hematoma removal.At the conclusion of the surgical procedure,the control group was treated with nimodipine,while the observation group was treated with ultrasound-guided right stellate ganglion block on the basis of nimodipine.The blood flow velocity of middle cerebral artery before operation and 6,24 h after operation by transcranial Doppler sonography were detected to reflect cerebral vasospasm status;the blood samples from the radial artery and internal jugular bulb 24 h after operation were used to detect and count cerebral oxygen metabolism indexes,including the blood oxygen saturation of internal jugular bulb,arteriovenous oxygen content difference and cerebral oxygen uptake rate;the IgG,IgA and IgM 24 h after operation were detected.The patients were followed up at 1 week after operation,and the complication was observed.Results There was no statistical difference in the blood flow velocity of middle cerebral artery before operation between 2 groups (P>0.05);the blood flow velocity of middle cerebral artery 6 and 24 h after operation in observation group was significantly lower than that in control group:(100.8± 8.2) cm/s vs.(123.5 ± 9.9) cm/s and (89.7 ± 5.3) cm/s vs.(118.9 ± 7.1) cm/s,and there was statistical difference (P<0.01).The blood oxygen saturation of internal jugular bulb,cerebral oxygen uptake rate,IgG,IgA and IgM 24 h after operation in observation group were significantly higher than those in control group:0.704 ± 0.035 vs.0.598 ± 0.058,(57.5 ± 6.5)% vs.(49.7 ± 3.6)%,(12.5 ± 0.3) mg/L vs.(5.0 ± 0.1) mg/L,(5.5 ± 0.3) mg/L vs.(2.1 ± 0.1) mg/L and (4.3 ± 0.3) mg/L vs.(1.9 ± 0.2) rg/L,the arteriovenous oxygen content difference and incidence of complication were significantly lower than those in control group:(40.8 ± 3.2) ml/L vs.(58.3 ± 8.6) ml/L and 5.0% (2/40) vs.25.0% (10/40),and there were statistical differences (P<0.01 or <0.05).Conclusions For patients with aneurysmal intracerebral hemorrhage,ultrasound-guided stellate ganglion block therapy can effectively improve cerebral blood flow,ensure cerebral oxygen supply,improve humoral immunity,reduce the incidence of postoperative complication,and achieve the purpose of improving clinical therapeutic effect.
8.Influence of intravenous-inhalation combined anesthesia and total intravenous anesthesia on postoperative immune function and inflammatory mediators in patients having underwent modified radical mastectomy during same anesthetic depth

Objective To study the influence of intravenous-inhalation combined anesthesia and total intravenous anesthesia on postoperative immune function and inflammatory mediators in patients having underwent modified radical mastectomy during same anesthetic depth. Methods The clinical data of 106 patients having underwent modified radical mastectomy from August 2013 to December 2016 were retrospectively analyzed. In the patients, intravenous-inhalation combined anesthesia was in 56 cases (combined anesthesia group), and total intravenous anesthesia was in 50 cases (intravenous anesthesia group). The peripheral blood cellular immune indexes CD4+, CD8+ and CD4+/CD8+; serum humoral immunity indexes IgA, IgG and IgM; serum inflammatory mediators interleukin (IL)-6, IL-10, tumor necrosis factor(TNF)-α and C-reactive protein(CRP)2 h after operation were compared between 2 groups.Results The CD4+and CD4+/CD8+levels 2 h after operation in combined anesthesia group were significantly higher than those in intravenous anesthesia group (0.31 ± 0.04 vs. 0.29 ± 0.03 and 1.28 ± 0.15 vs.1.17 ± 0.13),the CD8+was significantly lower than that in intravenous anesthesia group (0.34 ± 0.04 vs. 0.36 ± 0.04), and there were statistical differences (P<0.01 or <0.05). The IgA, IgG and IgM levels 2 h after operation in combined anesthesia group were significantly higher than those in intravenous anesthesia group:(1.18 ± 0.15)g/L vs.(0.97 ± 0.11)g/L,(7.96 ± 0.88)g/L vs.(7.18 ± 0.85)g/L and (0.82 ± 0.09) g/L vs. (0.72 ± 0.08) g/L, and there were statistical differences (P<0.05). The IL-6, IL-10,TNF-α and CRP levels 2 h after operation in combined anesthesia group were significantly lower than those in intravenous anesthesia group:(39.68 ± 4.52)ng/L vs.(50.37 ± 6.18)ng/L,(7.05 ± 0.88)ng/L vs. (11.19 ± 2.41) ng/L, (62.38 ± 7.19) ng/L vs. (78.54 ± 9.17) ng/L and (22.53 ± 3.18) mg/L vs. (39.47 ± 4.51)mg/L,and there were statistical differences(P<0.01).Conclusions Compared with total intravenous anesthesia, the intravenous-inhalation combined anesthesia at the same anesthesia depth is helpful to reduce the postoperative immune suppression and systemic inflammatory response in patients undergoing modified radical mastectomy.
9.Serum proteomic-based analysis identifying autoantibody against ENO1 as a potential diagnostic biomarker in nasopharyngeal carcinoma

Objective:To screen and identify autoantibody biomarker to diagnose patients with nasopharyngeal carcinoma (NPC).Methods:Candidate autoantibodies against tumor-associated antigens were identified from NPC CNE2 cells using serological proteome analysis. Levels of candidate autoantibody biomarkers were measured by enzyme-linked immunosorbent assay (ELISA) in 50 patients with NPC and 80 normal controls recruited from the Cancer Hospital of Shantou University Medical College between July 2014 and January 2015. Receiver operating characteristic curve (ROC) was employed to evaluate diagnostic efficacy.Results:Serological proteome analysis showed that sera from patients with NPC yielded a positive spot, of which was identified as enolase 1 (ENO1). ELISA results showed that the level of serum autoantibody against ENO1 in patients with NPC was significantly higher than that in normal controls [0.165 (0.088, 0.378) vs. 0.100 (0.054, 0.117), Z=4.077, P<0.001]. With the optimum diagnostic cutoff of 0.164, ROC curve showed the diagnostic sensitivity and specificity of autoantibodies against ENO1 were 52.0% and 90.0%, respectively. Measurement of autoantibody against ENO1 demonstrated a positive rate of 75.0% for early stage NPC. Conclusion:Autoantibody against ENO1 may be a potential diagnostic biomarker for NPC.
10.Clinical efficacy of the folding lateral position fixation frame with adjustable angles in the postural nursing of critically ill patients

Objective:To investigate the clinical efficacy of the folding lateral position fixation frame with adjustable angles.Methods:A total of 60 patients from the Department of Intensive Care Unit (ICU) between January and June in 2019 with consciousness disorder or sedation and no ability to change postures on their own were selected and randomized into treatment group ( n=30) and control group ( n=30). Patients in the treatment group were backed by the fixation fame in lateral position of 80 degrees, while those in the control group were backed by sponge triangle pillow in lateral position of 80 degrees. After 2 hours, the angles of the mattress surface and the body of patients in two groups were measured with a SanNiang digital angle ruler. The efficacy rates of maintaining lateral positions of patients in both groups were calculated. The occurrence rates of skin ulcers in both groups were observed as well. Results:The efficacy rate of maintaining a lateral position was 93.3% (28/30) in the treatment group and 33.3% (10/30) in the control group. The differences between two groups were statistically significant ( χ2 value was 23.25, P=0.000). No skin ulcers were observed in patients in the two groups. Conclusion:The folding lateral position fixation frame with adjustable angles helps to maintain the lateral position efficiently, which can smooth sputum drainage, reduce the imbalance between ventilation and blood flow, increase oxygenation and prevent pulmonary infection. Compared with sponge triangle pillow, the frame can bear more weights, which is suitable for patients with varying weights. Beside, the frame is small in size and foldable, which is convenient for storing and sterilizing. It is highly cost effective and can meet different clinical needs.
