1.Secretory expression of BPI_(m23) recombinant bactericidal protein in Pichia pastoris

Objective:To express and secrete rBPI m23 bactericidal protein in Pichia pastoris expression system. Methods:pPICZ? synBPI m600 recombinant expression vector was constructed with prepared vectors (pUC18 synBPI 414 and PBV220 BPI m420 ) according to Molecular Cloning Laboratory Manual. The Pichia pastoris GS115 were electroporated with linearized pPICZ? synBPI m600 vectors , positive clones were screened on low salt LB medium with Zeocin , then the positive clones were identified by PCR and Mut phenotype analysis. rBPI m23 was induced to express in BMMY ,purified by SP Sepharose beads , identified by SDS PAGE and Western blot and measured with BCA method. Results:①pPICZ? synBPI m600 expression vector was successfully constructed.②Pichia pastoris GS115 strains integrated with synBPI m600 stably were obtained.③SDS PAGE showed the molecular weight of the recombinant protein was approximate 23 kD,and Western blot confirmed that the protein could bind to the anti BPI antibody specially.④The supernate protein concentration was about 2 9 mg/L.Conclusion:rBPI m23 was expressed and secreted effectively in Pichia pastoris GS115.
2.Proliferation of scar tissues following fresh versus preserved amniotic membrane transplantation with trabeculectomy

BACKGROUND: Fresh amniotic membrane has been extensively used in treating ocular surface disease, which can inhibit fibrous tissue proliferation, inhibit neovascularization, and relieve inflammation. However, its application in treating glaucoma is rarely reported.OBJECTIVE: To explore the antiproliferative effect of fresh amniotic membrane on postoperative scar and compared with preserved amniotic membrane.METHODS: A total of 36 New Zealand white rabbits were randomly divided into 3 groups. Fresh amniotic membrane and preserved amniotic membrane combined with trabeculectomy separately underwent fresh amniotic membrane and preserved amniotic membrane transplantation. The control group was subjected to trabeculectomy alone. After 1, 2, 3, and 4 weeks, the morphology and function of filtering bleb were checked, and the platelet-derived growth factor (PDGF) of surrounding tissue was determined by immunohistochemical method.RESULTS AND CONCLUSION: Fresh amniotic membrane and preserved amniotic membrane groups displayed bulged filtering bleb with good filtering function. Pathological observation showed that fibroblasts of the filtration pathway had less, but more sparse scar tissues than control group, but inflammatory infiltration was observed in all groups. The cavity of different sizes and shapes were detected in filtration pathway of control group, which was replaced by fibrous tissue hyperplasia, with a large number of fibroblasts. Immunohistochemical results showed that the expression of PDGF was significantly less in fresh amniotic membrane and preserved amniotic membrane groups compared with the control group, and the PDGF expression was less in fresh amniotic membrane group than the preserved amniotic membrane group. Fresh and preserved amniotic membrane can improve the filtering bleb function, reduce scar formation, and maintain the patency of filtering pathway. Moreover, the effects of fresh amniotic membrane is better than preserved amniotic membrane.

4.Research and Development of One-off Multifunctional Instrument for Blood Collection and Transfusion

Objective To develop an one-off multifunctional instrument for blood collection and transfusion.Methods Blood collection pipe,blood sample pipe and blood transfusion pipe were united by means of three-way pipe.Different venipuncture needles could be selected by means of different connectors.Plastic clips and connectors were used to cut off and connect pipes.It was also equipped with pipe warming apparatus,manual pipe seal apparatus and newest venipuncture needles.Results The one-off multifunctional instrument for blood collection and transfusion possessed advanced technology,convenience and good application value.Conclusion The one-off multifunctional instrument for blood collection and transfusion can be applied into blood medical service,all kinds of surgical operations,emergency care,abrupt affairs,and it meets the requirements of war-readiness and medical service for troops in different periods.
5.Effect of nurse hierarchical management on nurse core competence and responsibility system nursing level

Objective To study the effect of nurse hierarchical management on nurse core competence and responsibility system nursing level.Methods A total of 79 nursing staff in our hospitalwere selected as research objects.Nurses before the implementation of the new nursing management measures (March 2015 to March 2016) were carried out nurse conventional hierarchical management,and those after implementation (April 2016 to April 2017) took new hierarchical management model.The quality of nursing management,the core competency score and the satisfaction of doctors and patients were compared before and after the implementation of new hierarchical management model.Results After implementation,the core competence scores of nursing staff in medical records writing ability and language communication ability were significantly lower than implementation before,the difference was statistically significant (P < 0.05).After the implementation,the nursing management quality in informing precautions after discharge,obligation to inform before operation,and initiative to carry out life care were significantly higher than implementation before (P <0.05).Conclusion Nurse level management can improve core competence of nurses,enhance the level of responsibility system and the application effect is more obvious,so it is worthy of promotion.

7.Application of Health Care Failure Mode and Effect Analysis in the construction and evaluation of Hospital Oral Administration Security System

Objective To standardize the procedure and ensure the safety of oral administration,and to achieve the "three-accurate",that is right medicine,right time,and right patients. Methods The concept of "Health Care Failure Mode and Effect analysis" was used to analyze the procedure of in-patients' oral administration,and adjust the system according to insecurity factors to improve the hospital oral administration system constantly. Results The systematic reconstruction of the oral administration improved nurses' and patients' satisfaction,reduced the error rate of in-patients' oral administration and improved the safety of oral administration. Conclusion The application of"Health Care Failure Mode and Effect analysis" to forward-looking analysis in-patient oral administration system,combined with the actual situation of our hospital to develop and implement plans are the guarantees of oral administration security.
8.The influence of specialized nurses′ assistance mode on the maintenance ability of PICC catheter in nursing hospital from the perspective of combination of medical care and nursing care

Objective:To evaluate the effect of nursing specialist nurses′ assistance on the ability of nursing home nurses to maintain venous catheter under the vision of combination of medical care and nursing care, providing reference for carrying out specialized nursing assistance work in grassroots hospitals.Methods:This study was an experimental study. According to Miller′s pyramid theory, a model of specialist nurse assistance was established with specialized technology as the core. From January to December 2021, three nursing hospitals in Suzhou were assisted. Convenience sampling method was used to investigate the ability of intravenous catheter maintenance, the incidence of central venous catheter maintenance defects, the incidence of central venous catheter complications and unplanned extubation, and the satisfaction of doctors and patients before and after help.Results:Totally 86 nurses were included, aged (33.17 ± 2.31) years old. After the help, the theoretical and operational skill scores of nurses in the nursing home were (89.77 ± 2.11) and (92.75 ± 1.62) points, respectively, which were significantly higher than those before the help (60.84 ± 2.71) and (79.81 ± 1.68) points, and the differences were statistically significant ( t=-84.96, -52.12, both P<0.05). Among the maintenance defects of central venous catheter in nursing homes, incomplete labeling, film wrapping, blood seepage, catheter discounting, non-standard film fixing, and incomplete information of extended care were 9.95% (21/211), 8.53% (18/211), 2.84% (6/211), 6.16% (13/211), 4.74% (10/211), 2.37% (5/211) after help, all lower than 50.57% (89/176), 43.18% (76/176), 14.20% (25/176), 25.57% (45/176), 18.18% (32/176), 11.93% (21/176) before help. The differences were statistically significant ( χ2 values were 14.00 to 77.81, all P<0.05). The incidence of medical adhesive related skin injury, catheter obstruction complications and unplanned extubation were 4.44% (4/90), 3.33% (3/90) and 2.22% (2/90), respectively, which were lower than 20.51% (16/78), 15.38% (12/78) and 11.54% (9/78) before help. The differences were statistically significant ( χ2=8.81, 6.06, 4.50, all P<0.05). Doctors′ satisfaction with nurses and patients′ satisfaction with nursing work were 96.43%(27/28) and 93.75%(45/48), respectively, both higher than 71.43%(20/28) and 72.73%(32/44) before help, and the differences were statistically significant respectively ( χ2=4.77, 5.97, both P<0.05). Conclusions:From the perspective of combination of medical care and nursing care, the maintenance and assistance of intravenous catheters by the team of intravenous therapy nurses in nursing hospitals can effectively reduce the incidence of central venous catheter complications and unplanned extubation, improve the maintenance ability of intravenous catheters of nurses in nursing hospitals, and help to improve the quality of nursing care in nursing hospitals.
9.Preliminary study on the aerobes distribution of nasal cavity from the healthy children and adults.

OBJECTIVE:
To study whether there are differences in the bacteria distribution from the nasal cavity of the healthy children, teenagers and adults
METHOD:
The cotton swab specimens were taken from the nasal cavity of the healthy children, teenagers and adults for aerobic culture training.
RESULT:
Corynebacterium, coagulase-negative staphylococci, Staphylococcus aureus, Gram-negative Neisseria, alpha-hemolytic streptococcus and gram-negative bacillus with, six species of bacteria in total, were cultivated from the nasal cavity of 40 healthy children. The positive rate of bacterial culture was 80.0%; Corynebacterium, coagulase-negative staphylococci and Staphylococcus aureus were cultivated from the nasal cavity of 40 healthy teenagers and 56 healthy adults with the positive rate of bacterial culture was 90.0% from the healthy teenagers group and was 92.9% from the healthy adults group.
CONCLUSION
There were obvious difference between the nasal cavity of the healthy children and adults with no obvious difference between the bacteria distribution from the nasal cavity of healthy teenagers and adults.

10.The effects of linoleic acid on intestinal flora in mice were analyzed based on 16sRNA sequencing

Objective To investigate the effects of linoleic acid(LA)on the diversity and structure of intestinal flora in mice.Methods Twelve SPF grade male C57BL/6J mice at 7 weeks were randomly divided into control group(CTRL group)and linoleic acid group(LA group).One day before the linoleic acid diet was supplemented,the normal food was removed from the LA group and the mice in the LA group were fasted for one night,so that the LA diet was more acceptable to the mice in the LA group,and LA was given on the day of the experiment recording,and the feed was updated at any time to ensure that the mice could eat freely until the end of modeling.After 12 weeks of modeling,mouse feces were collected,and mixed samples were collected for every two mice feces,and then 16sRNA high-throughput sequencing was performed to analyze intestinal flora structure,Alpha and Beta diversity.Results 16sRNA high-throughput sequencing showed that LA intervention damaged the richness and diversity of intestinal flora.The results of principal component analysis showed that the composition of flora in CTRL group was different from that in LA group.At gate level,the relative abundance of Actinobacteria in LA group increased(P<0.01).At the genus level,the relative abundance of L.Duchennei in the LA group decreased(P<0.05),but the relative abundance of Bifidobacterium,Faecalibaculum and Erysipelotrichaceae in the LA group increased(all P<0.01).Conclusion LA intervention could reduce the richness and diversity of intestinal flora in mice,and adjust the structure of intestinal flora.There were significant differences between beneficial bacteria and pathogenic bacte-ria in intestinal flora after LA intervention,which provided certain basis for the treatment of bioactive compounds of linoleic acid and the therapeutic adjustment of intestinal microorganisms as targets.
