1.Latrogenic injuries of radial nerve:A report of 36 cases

Objective:To investigate the causes, the prevention and the treatment of iatrogenic radial nerve injuries.Methods: From March 1987 to March 2000 ,the authors treated 36 cases of iatrogenic radial nerve injuries .Of the 36 cases ,33 cases occurred after the orthopaedic operation ,3 cases occurred after the manipulative reduction. 32 cases were treated surgically, and the other 4 cases were treated non-surgically. 8 cases were repaired by epineurial suturing or by epi-fascicular suturing to connect the severed ends of the nerves. 19 cases were treated with neurolysis operation, 8 cases with tendon transfer and 3 cases with palliative treatment. Results: 27 cases were followed up for 1 year to 6 years. The curative effect was assessed according to Zhang Xianzhong"s norm. The recovery of 20 cases were excellent, 4 cases were good, 3 cases were poor.Conclusion:The iatrogenic radial nerve injury can be avoided if the surgeon take a high responsibility and grasp a precise knowledge of pertinent anatomy. At the same time more attention should be paid in operation. Once incurred, the damage can be minimized if it is recognised and treated earlier.
2.A study on relationship between juvenile transient synovitis and enterovirus

Objective To study the relationship between juvenile transient synovitis of the hip and enterovirus.Methods EV-IgM antibody was detected with ELISA in blood and arthrocentesis. At same time, sample with coxsackievirus B(CVB) IgM positive was tested by serptype.Results IgM antibody was detected in 38.27% in blood of the patients and in 76.19% in arthrocentesis.CVB-IgM was found positive in 16.05% of blood and in 42.86% of arthrocentesis.AdV-IgM was in 11.73% and in 23.81%,respectively.Most of the CVB-IgM positive individuals were infected with CVB3.Conclusion The results of this study suggest that there may be a significant correlation between juvenile transient synovitis and CVB, especially CVB3.
3.Role of SLB elements presented at 5 UTR of dengue virus on translation and RNA synthesis

Objective To investigate the effects of SLB elements presented at 5' UTR of dengue virus genome on viral translation and RNA replication.Methods The 5 end of RNA secondary structures of dengue virus genome were predicted using mfold 3.2,and three mutants with different modifications in SLB elements were designed:SLB1,part deletion of the 5' UAR sequence(82-87nt);SLB2,mutation of the sequence on stem(Mut 77G/C);SLB3,mutation of the sequence on stem(M77-78AG/GA),respectively.The mutants described above were constructed by OL-PCR based on DEN-R.luc2A-RP,respectively.Replicon RNAs corresponding to DEN-R.luc2A-RP,DEN-R.luc2AGDD-RP and the 3 mutants mentioned above were in vitro transcribed and equal amounts of RNA were transfected into BHK cells with Lipofectamine 2000.After RNA transfection,the replicons were detected and characterized by RT-PCR,IFA,Renilla Luciferase assay system and real time RT-PCR,respectively.Results It was shown that SLB1 mutant did not significantly affect the translation of the input RNA,but seriously compromised RNA synthesis;SLB2 mutant did not significantly affect the translation of the input RNA either,but its RNA replication was abolished;both the translation and replication of SLB3 mutant were abolished.Conclusion Both the nucleotide sequence and the RNA secondary structure of the second loop and short stem of SLB element are highly conserved.SLB element may play an essential role on viral translation and RNA replication.The findings of present study may set a foundation for elucidating the role of SLB during viral translation and replication.
4.Effects of bupivacaine on short-circuit currents in human alveolar epithelial cells

Aim Bupivacaine is a kind of long-acting amide local anesthetics.This paper aims to explore the effects of bupivacaine on the short-circuit currents in human alveolar epithelial monolayers and study the possible mechanisms.Methods Short-circuit currents were recorded by ussing-chamber setup.Amiloride-sensitive currents were defined as the difference be-tween the total current and the amiloride-resistant cur-rent.ERK1 /2 phosphorylation protein levels were ana-lyzed by Western blot at 0,1 5,30 and 60 min after administration of 1 00 μmol·L -1 bupivacaine.Results Bupivacaine could inhibit the short-circuit currents in H441 monolayers dose-dependently,which could be inhibited by amiloride.Western blot analysis showed that bupivacaine increased the level of ERK1 /2 phos-phorylation.Conclusion These data demonstrate that bupivacaine can reduce the alveolar ion transport by in-hibiting the amiloride-sensitive currents,possibly by the enhancement of ERK1 /2 phosphorylation. The effects of alveolar fluid clearance following application of bupivacaine should be considered clinically when the patient is complicated with lung injury.
5.Effect of Modified Liujunzi Decoction on Syndrome of Spleen Deficiency and Phlegm Stagnation in Patients with Type 2 Diabetes Mellitus

Objective To observe the effects of modified Liujunzi Decoction on improving isletβ-cell function of patients with the syndrome of spleen-deficiency and phlegm-stagnation of type2 diabetes mellitus.Methods Totally 72 patients were selected and divided into treatment group and control group randomly, with 36 cases in each group. Both groups were under diet adjustment and appropriate exercise and were given metformin hydrochloricle tablets, 0.5 g each time (1.0 g each time after 2 weeks), twice a day; acarbose, 50 mg each time, three times a day. The treatment group was given modified Liujunzi Decoction additionally, 200 mL each time, three times a day, orally. The course of treatment of the two groups was three months. Blood glucose, C peptide, INS, HbA1c, HOMA-IR, HOMA-β andΔI30/ΔG30 after 30 minutes of glucagon loading was recorded.Results Compared with before treatment, FBG, 30 min PBG, 2h PBG, HbA1c, FINS, fasting C-peptide, and HOMA-IR decreased in both groups, and the 30 min INS, 30 min C peptide, andΔI30/ΔG30 increased in the treatment group, with statistical significance (P<0.05). After treatment, there was statistical significance in FBG, 30 min PBG, 2 h PBG, HbA1c, fasting C-peptide, FINS, 30 min INS, 30 min C peptide, HOMA-IR, andΔI30/ΔG30 between the two groups (P<0.05).Conclusion Modified Liujunzi Decoction combined with Western medicine can better improve isletβ-cell function of patients with the syndrome of spleen-deficiency and phlegm-stagnation of type2 diabetes mellitus than Western medicine merely.
6.Effects of Huazhuo Granule on Expressions of PI-3K and GLUT4 mRNA in Rats with Type 2 Diabetes

Objective To observe the effects of Huazhuo Granule on expression of phosphatidylinositol-3-kinase (PI-3K) and glucose transporter 4 (GLUT4) in skeletal muscle type 2 diabetes (T2DM) rats;To discuss its effects on activating signal path of PI-3K and GLUT4. Methods High-fat diet combined with low-dose streptozotocin intraperitoneal injection was used to establish T2DM rat models. The rats were randomly divided into blank group, Huazhuo Granule group, rosiglitazone group, and model group. Each treatment group was given relevant medicine for gavage. Model group and blank control group were given normal saline for gavage. The general state, weight, and food ration of rats were observed. After ten-week medicine intervention, 10%chloral hydrate was used for anesthesia to detect the levels of FBG, OGTT (2 h PG), and FINS. RT-PCR was used to detect the expressions of PI-3K and GLUT4 mRNA of rat skeletal muscle in all groups. Results Compared with model group, the levels of FBG, 2 h PG, and FINS decreased (P<0.05);the expressions of PI-3K and GLUT4 mRNA of rat skeletal muscle increased significantly (P<0.05). Conclusion Huazhuo Granule has the effects on increasing insulin sensitivity, which is related to activating insulin signal path of PI-3K and GLUT4 of skeletal muscle tissues.
7.AO spiked washer and screw implantation for the treatment of small or comminuted tibial avulsion fractures of the posterior cruciate ligament in 11 patients

Eleven patients with small or comminuted displaced tibial avulsion fractures of posterior cruciate ligament admitted in the Department of Orthopaedics,Second Affiliated Hospital,Fujian Medical University between January 2003 and January 2008 were selected.With diameter of 4.0 mm,the length of 35 mm or 40 mm cancellous bone screw,and internal diameter of 4.0 mm spiked washer was implanted for fixation.For cases of large comminuted facture,two cancellous bone screws and spiked washer were used.Postoperatively,the limb was immobilizated in a cast plaster at a 30 degrees flexion for 2-4 weeks,followed by active exercise rehabilitation.All 11 patients were followed up with an average of 18 months(ranged 7 to 36 months).No electrolysis,corrosion reaction to metal material,or allergic response to the spiked washer were detected in patients.At 2 months,all patients achieved bony union,no fragment redisplacement,no knee laxity or instability was detected,and the affected knees regained full range of motion.The knee function was significantly improved.
8.Correlation between expressions of PTEN and C-myc in colorectal carcinoma

Objective To investigate the correlation between the expressions of tumour suppressor gene PTEN and oncogene C-myc in colorectal carcinoma.Methods The expressions of PTEN and C-myc protein in normal colorectal mucosa(n=8), adjacent non-cancerous tissues(n=10) and primary colorectal carcinoma tissues(n=60) were observed by S-P immunohistochemical assay.Results Of 60 colorectal carcinoma tissues,C-myc protein was detected in 46 cases.The expression rate of C-myc in the primary colorectal carcinoma tissues(76.70%) was significantly higher than that in the normal(0) and adjacent non-cancerous tissues(10%)(P0.05).The positive expression rate of PTEN protein in the primary colorectal carcinoma tissues(25.00%,15/60) was significantly lower than that in the normal(100%) and adjacent non-cancerous tissues(90%)(P

OBJECTIVEWe detected and analyzed the clinical values of total protein (TP), albumin (ALB), globulin (GLB), and ALB/GLB ratio (A/G) of whole unstimulated saliva of healthy people to determine the time of day when saliva composition is relatively stable. We compared the protein concentration and A/G of whole unstimulated saliva of patients with chronic periodontitis with those of healthy volunteers to provide references for diagnostic methods and clinical applications of saliva.
METHODSThe whole saliva of 37 healthy subjects were collected at 8:00, 9:30, 11:30, 13:00, 16:30, and 21:00. Meanwhile, the whole saliva of 24 patients with periodontitis was collected in the morning. Bicinchoninic acid method was used to detect the TP content. Saliva ALB was detected by GF-D800 semi-automatic biochemical analyzer, and the GLB and A/G were calculated. Finally, the results were statistically analyzed using SPSS 19.0.
RESULTSSalivary protein compositions were stable in the morning on an empty stomach. Healthy people: TP, (1 354.35±389.52) µg.mL-1; ALB, (139.55±27.19) µg.mL-1; GLB, (1 211.80±360.73) µg.mL-1; A/G, 0.126 3±0.041 7. Subjects with chronic periodontitis: TP, (2 611.56±231.62) µg.mL-1; ALB, (296.27±17.34) µg.mL-1; GLB, (2 315.69±221.67) µg.mL-1; A/G, 0.156 2±0.017 3. The contents of TP, ALB, and GLB in healthy individuals at different periods within a day showed significant differences (P<0.05), which were mainly reflected in the levels before and after meals. No significant difference was detected in A/G. The concentrations of TP, ALB, and GLB were significantly increased in patients with chronic periodontitis compared with those in healthy volunteers. However, no significant difference existed in A/G.
CONCLUSIONSalivary protein compositions are more stable in the morning than in other periods. Thus, mornings can be set as the time of specimen collection in future research. The concentrations of TP, ALB, and GLB in patients with chronic periodontitis are higher than those in healthy people.
