Based on the practical reconstruction of Ultrasonograph Information Management System in Nanjing General Hospital,this paper expounds the hardware architecture,software function and main technology characteristics in detail. The practical application shows that Ultrasonograph Information Management System can realize high-quality display,rapid capture,replay and storage for images,and can provide the functions of quick & accurate research,retrieval and auto statistics,thus effectively improving the work efficiency of ultrasonic diagnoses.

To suit the needs of educating medical talents in knowledge economy times,quality-oriented education idea should be introduced in biochemistry teaching. In the article, the author explored the effective paths of promoting the students' quality in biochemistry teaching on many respects such as the improvement of teachers' comprehensive ability, biochemistry teaching in class and supplementary class as well as application of multi- media technique, in order to generally boost the educational quality and reach the goal of medical personnel training.

Objective To evaluate the diagnosis and differential diagnosis methods for chronic lymphocytic thyroiditis(CLT).Methods One hundred and eight patients with CLT were treated in our department during recent 20 years,the clinical data were analyzed retrospectively.The diagnostic methods included thyroid function measurement,anti-thyroid antibody measurement,image detection,fine needle aspiration,diagnostic drug therapy and biopsy exam during operation.The preoperative examinations and diagnoses of 94 patients,who had received operation,were compared with postoperative pathological results.Results Forteen of the 108 cases did not receive any operation,of whom,9 had a definite diagnosis of CLT by fine needle biopsy under color ultrasound and 5 by drug(prednisone) diagnostic treatment.Before operation,57 patients were diagnosed as nodular goiter,16 as thyroid cancer and 13 as nodular goiter associated with hyperthyroidism.Only 8 patents were diagnosed as CLT with nodular goiter.Postoperative pathological reports were simple CLT in 59 cases(62.7%),CLT with nodular goiter in 19(20.2%),CLT with hyperthyriodism in 10(10.6%)and CLT with thyroid cancer in 6(6.3%).The preoperative misdiagnosis rate reached 91.5%(86cases).Conclusions It is easy for CLT to be misdiagnosed because of the complex clinical manifestations.Careful analysis of examination results,especially thyroid function measurement,anti-thyroid antibody measurement and image detection is significant for confirming the diagnosis.The fine needle biopsy and diagnostic drug treatment are better methods for CLT diagnosis.Unwarranted operation should be avoided in patients with CLT.

Helicobacter pylori (H. pylori) is one of the most common pathogens in human and it is closely related to gastrointestinal diseases. It is essential for us to understand the transmission process of H. pylori to prevent its spreading. The oral cavity has been proposed as a reservoir for gastric H. pylori, which has been detected by culture and polymerase chain reaction (PCR) in both dental plaque and saliva. Some researchers have proposed H. pylori in oral cavity may play an important role in its transmission and reinfection. Oral-oral or fecal-oral transmission are thought to be the most possible transmit way. This review will discuss the evidence for the role of the oral cavity in the transmission of H. pylori, the difficulties encountered in addressing this topic and possible directions for future research. Oral H. pylori may also play a role in the diagnosis and prevention of deceases related to H. pylori such as gastritis, gastric ulcer and gastric carcinoma. The recent progresses in this area are also reviewed. Moreover, we also discussed the relationship between oral H. pylori and oral deceases like periodontal disease and oral ulcer.

Objective To investigate the effect of cryotherapy of employing Argon/Helium assisted with TACE for the unresectable primary liver carcinoma. Methods 124 cases with primary liver carcinoma were randomly divided into two groups:60 cases were treated by TACE and cryotherapy; the other 64 cases were simply done by TACE as control. In general, TACE was undertaken once a month and altogether three times for a course. Cryotherapy was undergone 1-3 times for a course. Results The total effective rates (CR+PR) were 45.3% for the control group and 68.3% for the combined therapy group, with an obvious difference between the two groups, 0.5,1,1.5 years survival rate were 81.3%,62.5%,43.8% respectively in the control group;93.3%,83.3%,63.3% respectively for the combined group. There was an obvious difference between the two groups of 1,1.5 years of survival rates. Conclusions Cryotherapy of employing Argon/Helium assisted with TACE for the unresectable primary liver carcinoma is feasible with raising the effective rate and prolonging survival time.

By means of 3H-Thymidine and3H-Leucine incorporation, we observed that CGRP inhibited the multiplication of SHR's aortic smooth muscle cells (VSMCs) and the proliferation of VSMCs stimulated by Endothelin. These results suggest that CGRP may be a natu-ral antagonist of Endothelin in human body.

Objective To design phacoemulsification handpiece with small size and high amplitude amplification coefficient.Methods By introducing the electro-mechanical vibration characteristics of piezoelectric transducer and ultrasonic horn,and adopting the equivalent circuit method and analytical method,respectively,the size of the ultrasonic horn,quarter wavelength piezoelectric transducer and ultrasonic horn were designed.Then,modal and harmonic analysis in finite element analysis software ANSYS11.0 were used to simulate the handpiece,and to optimize the size of the handpiece after adding on needle.Results The results indicated that the longitudinal vibration frequency and amplification coefficient were very close to theoretical design.The errors were 1.4％ and 5.8％,respectively.When piezoelectric ceramics were loaded with 300 V sinusoidal voltage,the displacement amplitude at the end of the needle reached 125.5 μm,which could meet the phacoemulsification surgery needs.Conclusion This design greatly reduced the size of the handpiece,and obtained high amplitude amplification coefficient.The simulation results sufficiently validated the theoretical design.

OBJECTIVE: To explore the relationship of the local recurrence with the expression of protein Survivin and MMP-2 in the primary lesions and the surgical margins of laryngeal carcinoma. METHOD: The primary lesions and the surgical margins of laryngeal carcinoma of 48 patients were made into serial sections. Immunochemical methods was used to detect the expression of protein Survivin and MMP-2 in the primary lesion and the surgical margins of laryngeal carcinoma of 48 patients. RESULT: The positive expression for Survivin and MMP-2 in the primary lesion was 70.83% (34/48) and 66.67% (32/48) respectively, and the positive expression of Survivin and MMP-2 in the surgical margins of laryngeal carcinoma was 47.92% (23/48) and 37.50% (18/48), which in the primary lesion was significantly higher than those of the surgical margins of laryngeal carcinoma (P < 0.05). The recurrence rates of primary lesion positive for Survivin (34 cases) and MMP-2 (32 cases) were 26.47% (9/34) and 25.00% (8/32), which were higher than negative for them 7.14%(1/14) and 12.50% (2/16) (P > 0.05). The recurrence rates of those with Survivin (23 cases) and MMP-2 (18 cases) positive surgical margins were 34.78% (8/23) and 38.89% (7/18) respectively, which were significantly higher than those with negative ones 8.00% (2/25) and 10.00% (3/30) (P < 0.05). Logistic analysis showed that the expression of Survivin and MMP-2 protein in the surgical margins of laryngeal carcinoma was positively associated with the recurrence rates. CONCLUSION Laryngeal carcinoma patients with Survivin-positive or MMP-2-positive margin would have a higher recurrence rate. Survivin and MMP-2 protein can be used as biomarkers for local recurrence of laryngeal carcinoma after operation.

Objective To explore the effects of gadopentetate dimeglumine injection on renal function. Methods The study included 623 consecutive patients. Their serum creatinine concentrations before and within 3 days after injection of gadopentetate dimeglumine were analyzed. Their eGFR ( estimated glomerular filtration rate) were calculated. Patients were divided into two groups according to their injection dose: group received single dose and group received double dose. Patients in each group were subdivided into three subgroups according to their eGFR: subgroup of normal renal function, subgroup of mild renal insufficiency and subgroup of moderate renal insufficiency. Paried sample t test and group design paried sample Rank Sum test were used for statistical analysis. Results There was no adverse reaction oroccurrence of acute renal insufficiency. The mean level of serum creatinine dropped from (74. 0 ±17. 2) μ mol/L to (71.5 ± 19. 0) μ mol/L (t = 5. 39, P ＜ 0. 05 ) in 623 cases after injection of gadopentetate dimeglumine. However, the mean level of serum creatinine increased from (89.6 ± 12. 2 ) μmol/L to (92. 1 ± 14. 6) μmol/L ( t = 0. 68, P ＞ 0. 05 ) in subgroup of mild renal insufficiency under group received double dose and increased from ( 118. 3 ± 15. 3 ) μmol/L to ( 135.7 ± 8. 5 ) μ mol/L ( t = 2. 02, P ＜ 0. 05 ) in subgroup of moderate renal insufficiency under group received double dose, the mean level of serum creatinine dropped in all other subgroups. Conclusions Single dose gadopentetate dimeglumine is safe for patients with normal renal function and mild, moderate renal insufficiency in short term, but patients with abnormal renal function should be followed up after double dose injection.
