1.CT Evaluation of Multichannel Cochlear Implantation

Objective To investigate the value of CT as preoperative and postoperative imaging technique for cochlear implant candidates.Methods Axial high-resolution temporal bone CT and three-dimensional reconstruction of inner ear were performed in 44 patients with sensorineural hearing loss. Cochlear view radiography was performed routinely in postoperation cases. CT scanning was performed in 3 postoperation cases.Results There were 1 case (2 ears) with Mondini malformation type Ⅰ, 2 case (4 ears)with Mondini malformation type Ⅱ in 32 patients with prespeech deafness. There were 2 case (4 ears) with chronic suppurative otitis meadia, 1 case (2 ears) with inner ear ossification,3 cases (6 ears)with large vestibular aqueduct syndrome,1 case (2 ear)with cochlear sclerosis in 12 patients with postspeech deafness. 1 case with intracochlear electrode knot and otitis meadia was found by CT.Conclusion CT plays an important role in selecting patients, observing implanted electrode and finding postoperative complication.
2.Significance of high-resolution spiral CT and X-ray examination in the evaluation of preoperative selection and postoperative outcome of patients with multichannel cochlear implantation

BACKGROUND:Artificial electronic cochlear (AEC) is a high-tech product used as a therapeutic substitute for pathological cochlear, and the implantative state of AEC can be assessed with the aid of imaging techniques.OBJECTIVE: To investigate the significance of CT and X-ray examination in the evaluation of preoperative selection and postoperative therapeutic result of patients with AEC implantation.DESIGN: Self-control observaton.SETTING:Auditory Rebuilding Ward of Otolaryngology Department, Zhujiang Hospital, Southern Medical University.PARTICIPANTS:Forty-four patients prepared for AEC implantation because of severe sensorineural hearing loss or extremely severer sensorineural hearing loss were recruited from the Otolaryngological Department, Zhujiang Hospital, Southern Medical University from January 1999 to June 2003.Thirty-two patients were with prespeech hearing loss, aged 2.5-12 and 12patients with postspeech hearing loss, aged 10-42.METHODS: Axial high-resolution temporal bone CT and three-dimensional reconstruction of inner ear were performed on 44 patients. They lay in supine position and received bilateral symmetric scanning with view of 9.6 cm. The baseline was set at the level of upper limit of auditory frame, and temporal bone was axially scanned. The scanning parameters were:thickness of 1 mm,spiral distance of 1.0 and interval of 0.5 mm; inserting rebuilding was performed after scanning with thickness of 1mm and interval of 0.1 mm. The improved routine cochlear axial stenves-X-ray image was performed after operation, and three patients underwent spiral CT temporal volume scanning due to suspicious postoperative complications.Postoperative inner ear and the arrangement and depth of implanted AEC.RESULTS: Totally 44 patients were remained in the result analysis perstructure: Of the 32 patients with prespeech deafness, 1 case (2 ears) was diagnosed as type Ⅰ Mondini malformation and 2 cases (4 ears) as type ⅡMondini malformation. Of the 12 patients with postspeech deafness, chronic suppurative otitis meadia was observed in 2 cases (4 ears ) , inner ear ossification in 1case (2 ears ), large vestibular aqueduct syndrome in 3 cases(6the arrangement and depth of implanted AEC :image of X-ray indicated that the electrode was implanted right in cochlea,displaying planar spiral shape and located at the bottom of inner-ear channel. Postoperative CT scanning was performed on 3 cases and revealed that electron was implanted into the bottom surroundings through round window and the electrode got twisted and infected in one cases.three-dimensional reconstruction of inner ear can be adopted before AEC implantation for screening out acute and chronic suppurative and severe ray image can afford clues to know the position and depth of implanted AEC and find out the twist and olithy of electrode, if any CT and cochlea image are supplemented for preoperative case selection and postoperative therapeutic evaluation, helping AEC to exert auditory function to the maximual degree.

Based on the practical reconstruction of Ultrasonograph Information Management System in Nanjing General Hospital,this paper expounds the hardware architecture,software function and main technology characteristics in detail. The practical application shows that Ultrasonograph Information Management System can realize high-quality display,rapid capture,replay and storage for images,and can provide the functions of quick & accurate research,retrieval and auto statistics,thus effectively improving the work efficiency of ultrasonic diagnoses.
4.Application of combination of parenteral nutrition with enteral nutrition in perioperative period of total gastrectomy

ObjectiveTo determine the effects of combination of parenteral nutrition(PN) with enteral nutrition (EN) on patients with gastric cancer in perioperative period of total gastrectomy . MethodsIn 52 patients with advanced gastric cancer, 41 had PN support preoperatively and all 52 patients received combination of PN with EN after the total gastrectomy. Results48 of the 52 patients whose intestinal function was recovered within 3 days postoperatively. When using EN, 5 cases felt borborygmus,abdominal pain or mild diarrhea, but these symptoms decreased by slowing down of the infusion speed. 6.Clinical analysis of different routes of administration of methotrexate combined with hysteroscopy in cesarean scar pregnancy

Objective To investigate the feasibility of methotrexate incision and different methods of administration combined with hysteroscopic surgery for cesarean scarpregnancy (CSP).Methods Retrospective analysis of 106 patients with CSP in our hospital from January 2001 to December 2015 were.The patients were divided into two groups, group A of 66 cases: methotrexate ( MTX) incision and systemic administration combined with hysteroscopic surgery group B of 40 cases: methotrexate combined with hysteroscopy surgery.The operation time, intraoperative blood loss, preoperative hospital stay, the time of blood HCG decreased to normal, the total amount of MTX and the abnormal rate of liver function were compared between the two groups, and the factors affecting the prognosis of CSP were analyzed.Results The preoperative hospitalization time and abnormal liver function of the two groups were compared, the difference was statistically significant(P<0.05).A group of patients with success and failure, blood HCG before treatment, gestational sac and bladder wall thickness of the myometrium (MT), the difference was statistically significant (P<0.05).The median gestational sac diameter ( GS) of patients with successful treatment was 17.1 mm, and the median number of patients was 6.2 mm, and the difference was statistically significant between the two groups ( GS ) , the difference of GS between the two groups was statistically significant ( P<0.05 ) . Conclusion Methotrexate incision and systemic administration methods combined with hysteroscopy treatment compared with systemic administration combined with hysteroscopy is a safe and effective method and the hospitalization time is short, but the MT >3 mm, before treatment, blood HCG≤20000IU/L, GS≤40 mm CSP more secure.
7.Oral microbiota: a promising predictor of human oral and systemic diseases.

A human oral microbiota is the ecological community of commensal, symbiotic, and pathogenic microorganisms found in human oral cavity. Oral microbiota exists mostly in the form of a biofilm and maintains a dynamic ecological equilibrium with the host body. However, the disturbance of this ecological balance inevitably causes oral infectious diseases, such as dental caries, apical periodontitis, periodontal diseases, pericoronitis, and craniofacial bone osteomyelitis. Oral microbiota is also correlated with many systemic diseases, including cancer, diabetes mellitus, rheumatoid arthritis, cardiovascular diseases, and preterm birth. Hence, oral microbiota has been considered as a potential biomarker of human diseases. The "Human Microbiome Project" and other metagenomic projects worldwide have advanced our knowledge of the human oral microbiota. The integration of these metadata has been the frontier of oral microbiology to improve clinical translation. By reviewing recent progress on studies involving oral microbiota-related oral and systemic diseases, we aimed to propose the essential role of oral microbiota in the prediction of the onset, progression, and prognosis of oral and systemic diseases. An oral microbiota-based prediction model helps develop a new paradigm of personalized medicine and benefits the human health in the post-metagenomics era.

8.Precision stomatology: current status and challenges.

The completion of human genome project and the progress in medical practice have inevitably lead to the development of precision medicine, which is a medical model that proposes the customization of medical care including medical decisions, practices, and/or medical products with patient's genetic background, environmental factors and life behavior being taken into account. The current work proposed precision stomatology for the first time, and by integrating data reported in recent literature, we described the current practice of precision stomatology in multiple disciplines in modem dentistry. The clinical significance of precision stomatology and its future challenges have also been discussed.

9.Investigation and analysis on patients of extended stay at 11 hospitals in Beijing

Objective To investigate and analyze the status quo of the patients of extended stay at 11 clinical hospitals of a general university in Beijing, and explore the countermeasures and solutions.Methods Contact with the medical affairs departments of these hospitals to collect information about such patients of extended stay.Results A total of 43 cases of patients were identified as extended stay at these hospitals, accounting for 0.00% to 0.93% of total beds.Their stay ranged from 1 month to 15 years, including 17 cases with medical disputes (39.5%), and 26 cases without (60.5%).Conclusions The phenomenon of extended stay at hospitals deserve enough attention and appropriate actions accordingly.
10.Value of Hemoglobin A2 Level Determined by Capillary Electrophoresis in Screening and Diagnosis of Thalassemia

Objective To evaluate the value of HbA2 level determined by capillary electrophoresis (Hb-CE)in screening and diagnosis of thalassemia.Methods HbA2 level of 249 thalassaemia carriers and 142 healthy controls confirmed by molecular biological detection were determined by Hb-CE method.The thalassaemia carrier subjects were divided into different groups and subgroups according to their results of gene detection.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value for the diagnosis ofα-thalassemia,β-thalassaemia,α,β-thalassaemia were calculated under different HbA2 cut-off value.Results Mean value of HbA2 in healthy controls was (3.03±0.27)%.Mean values of HbA2 inα-thalassemia group and its subgroups of silentα-thalassemia,standardα-thalassemia and hemoglobin H disease were (2.38± 0.55)%,(2.61±0.46)%,(2.47 ± 0.32)% and (1.07 ± 0.17)%,respectively.Mean values of HbA2 inβ-thalassaemia group and itsβ0 subgroup,β+ subgroup were (5.65±0.47)%,(5.71±0.48)% and (5.56±0.43)%.Mean value of HbA2 in compoundαandβ-thalassaemia group was (5.7±0.82)%.Compared with healthy controls,HbA2 level inα-thalassemia group,silentα-thalassemia subgroup,standardα-thalassemia subgroup and hemoglobin H disease group decreased signifi-cantly (t values of 11.73,5.02,12.91 and 33.46,respectively,P<0.01).HbA2 level in hemaglobin H disease was signifi-cantly lower than silent and standardα-thalassemia subgroups (t values of 15.62 and 21.31,respectively,P<0.01),but there were no differences in HbA2 level between silent and standardα-thalassemia subgroups (t=1.50,P>0.05).HbA2 level inβ-thalassaemia group,β0 subgroup,β+ subgroup and compoundαandβ-thalassaemia group increased significantly (t values of 55.12,44.33,38.94 and 9.10,respectively,P<0.01),but there were no differences in HbA2 level betweenβ0 andβ+ subgroups (t=1.79,P>0.05).Of 249 thalassemia carriers,all 124β-thalassaemia carriers were distinguished with ele-vated HbA2 level (>3.5%)determined by Hb-CE and only 57 were distinguished from 117α-thalassemia carriers by Hb-CE.Under the cut-off value of 2.5%,the sensitivity,specificity,positive predictive value,negative predictive value and accu-racy for the diagnosis ofα-thalassemia were 48.72%,97.18%,93.44%,69.70%,75.29%,respectively.Under the cut-off value of 3.5%,they were 100.00%,98.59%,98.41%,100%,and 99.25% for the diagnosis ofβ-thalassaemia,respectively. The analysis of ROC curve showed that the optimal HbA2 cut-off values for diagnosis ofα,β-thalassaemia by capillary elec-trophoresis were 2.8% and 3.7%,respectively.Conclusion When no abnormal bands,the elevated HbA2 (>3.7% in this study)determined by Hb-CE could be used as a marker forβ-thalassaemia diagnosis,but theβ-thalassaemia co-existingα-thalassemia could not be differentiated fromβ-thalassaemia diagnosis.Decreased HbA2 level (<1.5% in this study)and HbH band could be used for the diagnosis of hemoglobin H disease.Only HbA2 determination by Hb-CE has no clinical sig-nificance for the screen and diagnosis ofα-thalassemia.
