1.The impact of difficult embryo transfer on the outcome of in vitro fertilization-embryo transfer

Objective To evaluate the effect of the difficult embryo transfer on the clinical pregnancy outcome of in vitro fertili-zation-embryo transfer(IVF-ET) .Methods There were 209 fresh cycles of difficultly transferring and 2 489 fresh cycles of easily embryo transferring between January 2011 and December 2012 .The clinical outcome was compared .Results There were statistical-ly significant differences in the catheter blood staining rates (51 .20% vs 27 .68% ,P< 0 .05) ,implantation rate(31 .14% vs 35 . 54% ,P<0 .05) ,and clinical pregnancy rate (46 .41% vs 55 .56% ,P<0 .05)between the two groups .There was no significant difference in the rates of ectopic pregnancy and miscarriage between the two groups (P>0 .05) .Conclusion Difficulty ET will in-fluence the clinical pregnancy .Therefore ,all efforts should be made to avoid the difficult transfer in order to increase the pregnant rate .
2.The relationship between expression of Id-2 and MMP-9 and clinical pathology index in rectal cancer

Objective To observe the expression level of inhibitor of differentiation 2 (Id‐2) and matrix metalloproteinases‐9 (MMP‐9) in rectal cancer ,analysis the correlation of the expression level of them ,to study the relationship between the expression level of them and the clinical pathology indicators of rectal cancer .Methods Rectal cancer tissues and normal tissue adjacent to rec‐tal cancer were obtained from the rectal cancer resection of 56 patients with rectal cancer ,using immunohistochemical method to ob‐serve the expression level of Id‐2 and MMP‐9 in normal tissue adjacent to rectal cancer and rectal cancer and Spearman correlation test to detect the correlation between the expression level of Id‐2 and MMP‐9 ;then we analyzed the relationships between the ex‐pression level of Id‐2 and MMP‐9 and the index of rectal cancer clinical pathology .Results The positive expression rate of Id‐2 in the in rectal cancer tissues is more higher than that of normal tissue of adjacent to rectal cancer (73 .21% vs .48 .21% ,P<0 .05) . The positive expression rate of MMP‐9 in the in rectal cancer tissues is higher than that of normal tissue of adjacent to rectal cancer (71 .43% vs .44 .64% ,P<0 .05) .Spearman correlation test showed that there is the positive correlation between the expression level of Id‐2 and MMP‐9 (r=0 .393 ,P=0 .003) .The expression levels of Id‐2 and MMP‐9 in rectal cancer were correlated with the degree of tumor differentiation ,TNM stage and lymph node metastasis (P<0 .05) ,but had no differences between the elements of age and sex (P>0 .05) .Conclusion There is a close relationship between the expression levels of Id‐2 and MMP‐9 in rectal cancer and the occurrence and development of rectal cancer .Rectal cancer with the higher Id‐2 expression level may be the ways to achieve tumor invasion and metastasis through MMP‐9 as a facilitator .
3.Clinical Characteristica of 21 Cases Series with Anti-γ-aminobutyric Acid B Receptor Encephalitis

Objective To summarize the clinical characteristics,treatment and outcome of 21 patients with anti-γ-Aminobutyric acid-B receptor (GABA-B) encephalitis. Methods The clinical data of inpatients with anti-GABABR encephalitis at Neurology Department of Beijing Youanmen Hospital from October 2012 to December 2019 were retrospectively collected. all patients were followed up by outpatient visits or telephone. The good prognosis was defined as a mRS of 0 to 2 points. The clinical feature,treatment and followed-up outcome were analyzed. Results A total of 21 patients were included in this analysis,with a median age of 57 years (range:11~77 years) and 14 males. The major clinical manifestations included seizures (21/21),cognitive impairment (19/21),psychiatric symptoms (19/21),involuntary movement (7/21),speech impairment/silence (14/21),sleep disorders(13/21),disturbance of consciousness(13/21),involuntary movement(7/21),autonomic nervous dysfunction(6/21)and ataxia(1/21). Lung cancer was detected in 9(9/21) patients. Electroencephalogram (EEG) showed mild to moderate abnormal changes in 11(11/14)cases. The head magnetic resonance imaging showed abnormal signals in 12(12/21)cases,with the most involved area in temporal lobe. All the 21 patients showed anti-GABABR positive in serum and cerebrospinal fluid. 17 patients received the first-line immunotherapy,except 3 patients showing no effect,all the other patients responded well to the treatment. During a median follow-up of 16(range:3~30 months),11 cases had good prognosis,9 cases died,and 4 cases had recurrent encephalitis. Conclusion The cases series with anti-GABABR encephalitis and limbic encephalitis presentation were more common in middle-aged and elderly men. The main clinical symptoms were epilepsy and neuropsychiatric disorders,and most epilepsy were refractory epilepsy. The proportion of concomitant tumor in the patients with this disorder was high,especially lung cancer. Most of these patients responded well to immunotherapy and had a good prognosis,and the patients with lung cancer had a relatively poor prognosis.
