OBJECTIVE To observe the curative effect of continuous lumbar cisterna drainage for severe intracranial infection. METHODS We cured 24 patients of severe intracranial infection with the method of continuous lumbar cisterna drainage and injecting sensitive antibiotics to subarachnoid space. RESULTS Making use of continuous lumbar cisterna drainage and injecting sensitive antibiotics to subarachnoid space turned to be an effective method for severe intracranial infection,all patients had been cured. CONCLUSIONS In patients of severe intracranial infection,the responsible agents have been transformed,we conclude that it improves the outcome obviously to drain the CSF and inject sensitive antibiotics to subarachnoid space.

Objective Studing the relationship among matrixmetal-loproteinnse2 (MMP2), the tissue inhibitors of metalloprotein-ase2 (TIMP2) and chnicopathological characteristics such as histological grade, clinicopathological stage of the rectal cancer. Assessing the significance of MMP2 and TIMP2 expression in rec-tal cancer for prognosis and treatment. Methods To test the expression level of MMP2 and TIMP2 in rectal caneer patients with immunohistochemisty method. Results were analysised with χ2 test. The Kaplan-Meier (Log-rank test) was used for univariate survival analysis. Results The positive rates of MMP2 and T1MP2 ex-pression in rectal cance are higher in comparison with normal tissues. With MMP2 have a trendence of increas-ing the positive expression of TIMP2 decreases. Conclusion MMP2 and TIMP2 relate obviously with the clini-copathological character of the rectal cancer. They may play an important role in the proliferation, invasion and of the cancer. Testing the level of expression of MMP2 and TIMP2 may be helpful to prejudge the prognosis of the patients with rectal cancer. Some new therapic methods relating to MMP2 and TIMP2 may give patients with rectal cancer a more satisfying treatment.

Objective: To study the effects of maternity insurance lump capitation on obstetric index in Yinchuan , discuss its impact on decreasing cesarean section rate and improving reasonable check and rational drug use. Methods: The relevant indexes of inpatient parturient in Yinchuan fixed-point hospitals in 2011 are analyzed by comparing with the data from 2008 to 2010. Results:After the application of capitation lump policy, the cesarean section rate of the research object decrease from 54.14% to 35.38%, average hospitalization expenditure decreased by 9.52%, per drug expenditure decreased to 45.24%, average length of stay decreased by 0.8 day and per medical service cost increased by 9.39%. Conclusion: Since the application of capitation lump mode, the effects on the indexes of cesarean section rate, medical and medicine cost are obvious.

Objective To analyze the clinical effects aftet one year of LASIK for myopia. Methods According to the preoperative diopters of the myopia, the patients were divided into three groups, group Ⅰ: 57 eyes( < - 6.00D), group Ⅱ: 109 eyes( - 6.25～ - 10.00D) ,group Ⅲ :31 eyes( > - 10.25D～ - 15.00D). The patients were followed up 1 year. Re- sults In groups,the percentage of the uncorrected visual acuity equal or more than 1.0 was 94.3%,80.3%,42.0% re- spectivdy. The percentage of there sidual refractive error within ±0.50D was 99.3% ,85.7% ,59.7%. The spherical e- quivalent was stable in 3 months after LASIK. The complications are few. Conclusion LASIK is a safe,effective and good predictable method for treatment of myopia.

Objective To evaluate the curative effect of LASIK for correcting compound myopic astigmatism.Methods One hundred and sixteen eyes in 68 patients with compound myopic astigmatism had been corrected with LASIK.All patients were followed up for more than 12 months.Meanwhile,all eyes were divided into two groups according to the preoperative astigmatism:group I (-2.25～ -3.00D),group Ⅱ (-3.2.5～ -6.00D).Results In group I :the average refractive cylinder was (- 2.51±0.68) D preoperafion, and had the average of (- 0.51±0.25) D residual astegmatism postoperation.In group Ⅱ :the average refractive cylinder was (- 3.85±0.46) D preoperation, and had the average of (- 0.52±0.22)D residual astigmatism postoperation.There was no statistical difference in astigmatism between the two groups after 12 months.In group Ⅰ and Ⅱ the visual acuity after operation was better than the corrected visual acuity before operation.Conclusion For compound myopic astigmatism LASIK was safe and effective.

Objective To evaluate the efficacy of the retroperitoneal laparoscopic pyelolithotomy (RLP) and the percutaneous nephrolithotomy (PCNL) in treating renal pelvic stone.Methods The data of 89 patients diagnosed as renal pelvic stone were retrospectively reviewed from January 2009 to July 2016,of whom 43 patients underwent RLP and 46 underwent PCNL.Statistical analysis was performed regarding operation time,blood loss,mean hospital stay,complication rate,and stone-free rate.Results The operation time in RLP group and PCNL group was (117.5 ± 16.7) min and (118.3 ± 16.6) min,respectively,and there was no significant difference (P =0.547).For the two groups,the mean hospital stay was (4.5 ± 0.5) d and (6.1 ± 0.9) d,the mean hemoglobin decrease was (4.5 ± 1.2) g/L and (18.1 ± 3.4) g/L,the post-operative blood transfusion rate was 2.3％ and 14.0％,the post-operative septic shock rate was 0 and 9.3％,respectively,with significant difference between the two groups (P ＜ 0.05).The stone-free rate in both groups was 97.7％ and 95.3％ with no significant difference (P =0.557).Conclusions RLP has the advantages of quick recovery,less blood loss and lower complication rate than PCNL.It could be a minimally invasive option for the treatment of renal pelvic stone.

Objective To investigate the effect of atorvastatin on the expressions of solubility P-selectin(sP-selectin) and high sensitive C reactive protein(hs-CRP) in patients with percutaneous coronary intervention (PCI),and to explore the protective effect of atorvastatin on myocardium.Methods A total of 100 acute coronary syndrome patients having underwent successful PCI were enrolled in this study.All patients were divided into 3 groups by random digits table method:standard treatment group (group A,30 patients,atorvastatin 20 mg),low-dose atorvastatin pretreatment group (group B,35 patients,preoperative 12 h and 2 h atorvastatin 40 mg and 20 mg,respectively) and high-dose atorvastatin pretreatment group (group C,35 patients,preoperative 12 h and 2 h atorvastatin 80 mg and 40 mg,respectively).The level of sP-selectin was measured with ELISA.The level of hs-CRP was measured with latex enhanced immunoturbidimetry.Results Before operation,the levels of sP-selectin in group A,B,C were (2.32 ±0.40),(2.51 ± 0.33),(2.47 ± 0.28) μ g/L.After 6 and 12 h of operation,the levels of sP-selectin in group A,B,C were (4.12 ± 0.75),(3.34 ± 0.42),(3.31 ± 0.46) μ g/L and (5.64 ± 1.07),(4.08 ± 0.74),(3.84 ±0.48) μg/L.The levels of sP-selectin in group A,B,C after operation were significantly higher than that before operation (P ＜ 0.05).At the same time point,the levels of sP-selectin in group B,C after operation were significantly lower than that in group A (P＜ 0.05).Before operation,the levels of hs-CRP in group A,B,C were (4.32 ±0.51),(4.46 ±0.57),(4.29 ±0.43) mg/L,after 6 and 12 h of operation,the levels of hs-CRP in group A,B,C were (8.91 ± 1.34),(7.44 ± 1.06),(7.28 ±0.95) mg/L and (13.66 ± 1.49),(8.08 ± 1.14),(7.92 ± 1.04) mg/L.The levels of hs-CRP in group A,B,C after operation were significantly higher than that before operation (P＜ 0.05).At the same time point,the levels of hs-CRP in group B,C after operation were significantly lower than that in group A (P ＜ 0.05).The analysis of Pearson correlation showed,the level of sP-selectin was positively related with hs-CRP (r =0.753,P ＜0.01).The incidence of perioperative myocardial infarction was 23.3％ (7/30),5.7％ (2/35) and 2.9％ (1/35) in group A,B and C,respectively.The incidence in group A was significantly higher than that in group B and C (P ＜ 0.05).But there was no significant difference between group B and group C (P＞ 0.05).Conclusion The atorvastatin pretreatment can decrease the levels of sP-selectin and hs-CRP in patients after PCI,which play an important role in myocardium protection.

Objective To study the impact of capitation payment on obstetric indicators for the maternity insurance of urban workers in Yinchuan city.Methods Collection of indicators on lying-in women hospitalized at the obstetrics departments of designated hospitals in Yinchuan,in the period of 2011 to 2012 when the capitation payment was put in place.Such indicators include the percentage of uterine-incision delivery,diagnostics and therapeutic expenses,drug expenses,and average days of stay,along with mortality of pregnant and lying-in women and that of newborns,which are used as indicators to measure quality of care.Results The capitation payment policy has witnessed drops in the percentage of uterine-incision delivery,cost per inpatient,drug expenses per inpatient and average days of stay among urban workers covered by the insurance.The drops amount to 10％ for cost per inpatient and 45％ for drug expenses per inpatient.The differences found in pregnant and lying-in women are not statistically significant.Conclusion Capitation payment is conducive to dropping the percentage of uterine-incision delivery and medical expenses,and saving medical insurance payment,for the sake of optimal use of healthcare resources.

Objective To study clinical outcomes of cross-leg fibula flap for difficult reconstruction of an extensive injury in the lower extremity,which included large soft-tissue defects and long infected bone and tibia defects combined with vascular injury.Methods From September 2004 to September 2008,8 cases with an extensive injury in the lower extremity,which included large soft-tissue defects and long infected bone and tibia defects with vascular injury,were retrospectively analyzed,including 6 males and 2 females with an average age of 36 years (range,19-55 years).The course of disease ranged from 2 weeks to 3 months (average,2 months).The bone defect length ranged from 8 to 20 cm (average,13 cm),and the skin defect area varied from 10 cm×7 cm to 22 cm×12 cm.After thorough debridement,cross-leg fibula flap was used to repair the tibial defect and concomitant soft tissue defect.The pedicles were divided at 4-6 weeks after the initial surgery.The important technical considerations were outlined.Results All 8 patients were followed up for 6 to 36 months (average,24 months).The flaps survived in all 8 patients,and no infection and osteomyelitis happened.The flap transplantations healed at two weeks after operation with good shape.The fractures healed in all of patients with an average period of 11 months (range,6-15 months).All of patients recovered to walk without aid.According to Edwards classification of tibial fractures,4 cases were classified as good,2 as acceptable,and 2 as poor.Conclusion The cross-leg fibula flap is relatively simple,liable for the treatment of an extensive injury in the lower extremity.

Objective To explore the locating, parameter measurement and 3D display of nucleus accumbens in human brain in terms of digital anatomy .Methods The raw data of the head specimen of a 45-year-old male adult with 0.5mm as the section spacing was collected by using digital milling machine .Three hundreds images of continual cross sections containing brain were chosen and the segmentation of the caudate nucleus , putamen and nucleus accumbens was accomplished with Photoshop CS .The nucleus accumbens on the images of continual coronal section reconstruction were distinguished according to Harvard Medical School ’ s segment method to calculate the volume of nucleus accumbens and collect the correlative location information .The caudate nucleus , putamen and nucleus accumbens were 3D visualize with the software of Amira 3.1.1.Results The nucleus accumbens , the adjoining structure and the lesion target of nucleus accumbens were all clearly visible .The left nucleus accumbens volume was 972.5mm3 , and the right was 830.6mm3 .The 3D coordinate value was the left ( -11.0, 24.4, 1.3) and the right (9.3, 23.9, 1.7).Conclusion The digital anatomy of nucleus accumbens can distinctly display the nucleus accumbens , form and confirm it ’ s volume, location and adjoining area , which is useful to clinician .
