1.Expression of human papillomavirus DNA in keratoacanthoma tissue

Objective To detect the infection rate of human papillomavirus (HPV) types in keratoacanthoma (KA) tissue, and to study the distribution of HPV DNA-positive cells. Methods In situ hybridization was performed to detect HPV subtypes 6/11, 16/18, and 31/33 in paraffin-embeded tissue samples from 46 patients with KA and 34 nomal human controls. Results Mucosal HPV DNA was positive in 31 (67.39%)of the 46 KA samples, and mixed infection rate of HPV amounted to 83.87% (26/31) in these positive samples.The DNA of tested mucosal HPV types existed mainly in episomal status. Positive reactions were observed within or at the edge of nuclei. HPV infected cells showed a stripe-, slice-like or focal distribution mainly in superficial middle lamina of spinous layer at the bottom or lateral margin of crater-shaped epidermal depression. No mucosal HPV DNA was noted in normal control samples. Conclusions Compared with normal controls, patients with KA are infected with mucosal HPV at a higher frequency, and mucosal HPV DNA mainly exists in episomal status in KA tissue, hinting that HPV infection play a certain role in the pathogenesis of KA.
2.Detection of T cell receptor gamma gene rearrangements in mycosis fumgoides

Objective To assess the diagnostic significance of T cell receptor gamma gene rearrangemerits in mycosis fungoides (MF), so as to develop a sensitive diagnosis tool. Methods A total of 50 specimens were collected, including 33 skin lesion specimens and 2 lymph specimens from 30 patients with MF,15 skin lesion specimens from 15 patients with inflammatory dermatoses. PCR was performed with specific primers targeting TCR V gamma 8, 9, 10, 11 to detect T cell receptor gamma gene rearrangement. Results Monoclonai rearrangements of TCR gene was observed in 88% (29/33) of specimens from patients with MF and 33% (5/15) of samples from patients with inflammatory dermatoses. Conclusions The detection of TCR gene rearrangements, as an ancillary test, is useful in the diagnosis and differential diagnosis of MF.
3.Application of Cox model for analyzing prognosis factors of stage Ⅳ gastric cancer

Objective To investigate the related prognostic factors of stage Ⅳ gastric cancer.Methods The clinical data of 248 patients with stage Ⅳ gastric cancer and intact follow up data in the Tumor Prevention and Treatment Center of Sun Yat-Sen University from 2000 to 2010 were retrospectively summarized.The twelve clinicopathological parameters served as the observation indicators,including age,sex,body mass reduction,H b,CEA,CA19-9,Borrmann type,tumor location,tumor size,pathological pattern,operative mode,metastatic sites and therapeutic model.The survival curve was drawn by using the Kaplan-Meier method.The median survival time was calculated.The univariate analysis was conducted with Log-rank test.The prognosis multivariate analysis was conducted by the Cox's proportional hazards regression analysis.Results MST in the patients of whole group was 254 d.The univariate analysis showed that sex,Borrmann type and therapeutic mode were the related factors afecting gastric cancer prognosis,while the Cox regression model revealed that above 3 indicators were also independent factors affecting the prognosis of the patients with stage Ⅳ gastric cancer in this group(P<0.05).Conclusion The treatment mode is an important independent factor affecting the survival of stage Ⅳ gastric cancer,the translational medicine model of palliative chemotherapy combined with palliative operation conduces to improve the prognosis in the patients with stage Ⅳ gastric cancer.
4.Application value of measurement of the regional pulmonary ventilation by electric impedance pneumograph in the clinical diagnosis of pneumoconiosis.

OBJECTIVETo study the value of clinical application and distribution character of regional pulmonary ventilation of patients with various type of pneumoconiosis.
METHODS132 patients with various type of pneumoconiosis were observed by electric impedance pneumograph(EIPV) and routine lung function test. The results were compared with the measured value of 100 healthy cases.
RESULTSThe regional pulmonary ventilation of the three kinds of pneumoconiosis(founder's pneumoconiosis, silicosis and asbestosis) was increased in both side of upper pulmonary region and decreased in both of lower pulmonary region. The ventilation distribution showed that the right pulmonary ventilation increased predominantly, accounted for 53.03% (70/132) of total lung ventilation. There was significant difference(P < 0.01) compared with the normal healthy men's EIPV, but no difference(P > 0.05) among various type of pneumoconiosis. There was also no difference(P > 0.05) between pneumoconiosis patients with normal ventilation function and various type of ventilation obstacle of pneumoconiosis. One silicosis patient complicated by atelectasis of the left upper lobe, another silicosis patient complicated by lung cancer and their EIPV accorded with their chest X-rays.
CONCLUSIONEIPV could not be measured by routine lung function test. It may be the supplement to latter parameter and as a substitute for radio imaging of pulmonary ventilation in pneumoconiosis complicated by other regional pulmonary disorder.

5.Effect of Salmon Calcitonin on Osteoporosis Induced by Spinal Cord Injury

Objective To explore the effect of salmon calcitonin on osteoporosis induced by spinal cord injury. Methods 100 patients with osteoporosis induced by spinal cord injury from September 2011 to September 2014 in our department were included. They were randomly divided into control group (n=50) and observation group (n=50). The control group received vitamin D3 only, while the observation group received vitamin D3 combined with salmon calcitonin on the basis of rehabilitation physiotherapy, for 6 months. Visual Analogue Scale (VAS) of pain was evaluated in different periods. The bone mineral density (BMD) of lumbar spine and femoral neck, the parathyroid hormone (PTH), bone gla protein (BGP) and 1,25- dihydroxy vitamin D3 (1,25-(OH)2D3) were tested and recorded. Results The VAS score was lower in the observation group than in the control group 1, 2, 3 and 6 months after treatment (P<0.001). The BMD of lumbar spine and femoral neck was significantly higher, the PTH and BGP were significantly lower and the 1,25-(OH)2D3 was significantly higher in the observation group than in the control group after treatment (P<0.001). Conclusion Combination of salmon calcitonin can effectively reduce the bone pain and improve the BMD in patients with osteoporosis induced by spinal cord injury.
6.Effect of rapamycin on the biological behaviors of human non-Hodgkin lymphoma Raji cells and its mechanism

Objective To investigate the effects of rapamycin on the biological behaviors of human non-Hodgkin lymphoma Raji cells with different concentrations and time, and to explore its mechanism. Methods Different concentrations (0, 10, 50, 100, 250, 500 nmol/L) of rapamycin were used in Raji cells for 24, 48, 72 h respectively. The apoptosis of Raji cells was detected by using CCK-8 method, and flow cytometry was used to detect the cell apoptosis and cycle of Raji cells. The enzymatic activity of Caspase-3 and Caspase-9 in Raji cells was detected by Caspase-3 and Caspase-9 activity testing kit. The expressions of bcl-2, p53 protein and mRNA were detected by Western blot method and reverse transcription-polymerase chain reaction (RT-PCR). Results The proliferative inhibition rate of Raji cells was increased from (23.7 ± 4.2)%to (51.7±3.7)%, the cell apoptosis rate was increased from (4.9±1.9)%to (20.5±1.5)%, the proportion of G0/G1 was increased from (40.8±1.4) %to (63.6±1.7) %, the Caspase-3 enzyme activity of Raji cell in 24 h was increased from 0.16±0.05 to 1.08±0.04, Caspase-9 enzyme activity was increased from 0.19±0.04 to 1.34± 0.06 after 24 h with the increasing concentration of rapamycin from 0 nmol/L to 500 nmol/L (P<0.01). The mRNA of bcl-2 was decreased from 0.90±0.03 to 0.46±0.03, and mRNA of p53 was increased from 2.51±0.41 to 5.85±0.21. The protein expression of bcl-2 was reduced and the protein expression of p53 was increased. The experimental results of Raji cells in 48 h and 72 h were consistent with the experimental results of 24 h. Conclusion Rapamycin may inhibit the proliferation of Raji cells through Caspase-3, Caspase-9, bcl-2, p53 and induce its cell apoptosis.
7.Diagnostic value of endoscopic ultrasonography combined with liver stiffness measurement and liver histopathology for early liver cirrhosis

Objective To evaluate the value of endoscopic ultrasonography( EUS) alone for early liver cirrhosis and the diagnostic efficacy of EUS combined with liver histopathology ( LH) and liver stiffness measurement ( LSM) for early liver cirrhosis. Methods Data of 226 patients with chronic liver lesions who underwent endoscopy and EUS at Tianjin Second People's Hospital were collected to assess esophageal and gastric varices. Liver fibrosis was assessed by LH and LSM was determined by Fibroscan. Models of EUS-LSM, EUS-LH, LSM-LH, ELL ( EUS, LSM and LH) were constructed to predict early liver cirrhosis. The ROC curve and AUROC were used to evaluate the efficacy of different models in the diagnosis of early liver cirrhosis. Results A total of 149 patients were diagnosed as chronic liver disease and 77 patients were diagnosed as early liver cirrhosis ( Child-Pugh A grade) by clinical evaluation. Ratio of varices found by EUS was significantly higher than that by endoscopy [ 68. 8% ( 53/77) VS 32. 5% ( 25/77) , P<0. 05] . The cut-off value was 8. 65 kPa by LSM to predict early liver cirrhosis. Pseudolobules were confirmed by LH in 42 ( 54. 5%) patients in the early liver cirrhosis group. The AUROC of ELL was 0. 919 ( 95%CI: 0. 875-0. 951),sensitivity=0. 792,specificity=0. 913,PPV=0. 824,NPV=0. 895,+LR=9. 08,-LR=0. 23, accuracy=0. 872, and ELL was superior to EUS ( P<0. 0001) , LSM ( P<0. 0001) , LH ( P<0. 0001) , EUS-LSM (P<0. 0001), EUS-LH (P=0. 0134) and LSM-LH (P=0. 0022) in the diagnosis of early liver cirrhosis. Conclusion EUS is superior to endoscopy in detecting the varices for early liver cirrhosis. Combination of EUS with LSM and LH can improve diagnostic efficacy for early liver cirrhosis.
8.Value of delta radiomic based on contrast enhanced MRI to predict pathological complete response after neoadjuvant therapy for breast cancer

Objective:To investigate the value of delta radiomics based on longitudinal changes of dynamic contrast enhanced MRI (DCE-MRI) in predicting pathological complete response (pCR) after neoadjuvant therapy (NAT) for breast cancer.Methods:The clinicopathological and imaging data of 117 patients with breast cancer confirmed by surgical pathology from April 2019 to November 2021 at Jiangxi Cancer Hospital were analyzed retrospectively. All patients were female with 23?74 (48±10) years old. The patients were randomly divided into training (81 cases) and test sets (36 cases) at the ratio of 7∶3 according to the number of random seeds in the software. All patients underwent DCE-MRI before and after early NAT (2 courses). The maximum diameter relative regression value of breast tumors before and after early NAT (D%) was calculated and used to construct a conventional imaging model. The delta radiomic features were extracted based on pre-NAT and early-NAT (2 courses) DCE-MRI and selected by redundancy analysis and least absolute shrinkage and selection operator algorithm. A ten-fold cross-validation method was used to construct the delta radiomic model and Radscore was calculated for each patient. All patients were classified into pCR group and non-pCR group according to the surgical pathology after NAT. Significant clinicopathological variables were selected by univariate analysis and stepwise regression method. They were integrated with D% and Radscore to build the combined model and nomogram. The model performance in predicting pCR after NAT in breast cancer was evaluated by the receiver operating characteristic curve and the area under the curve (AUC), and the clinical utility of the models was compared by using clinical decision curves.Results:The combined model had the best diagnostic performance among the three models, with an AUC of 0.90 in the training set and 0.87 in the test set. The Radscore had the highest weight in the nomogram. In the training set, the diagnostic performance of the combined model and delta radiomics model were better than that of the conventional imaging model ( Z=?3.48, P=0.001; Z=2.54, P=0.011). The clinical decision curves showed an overall greater clinical benefit of the combined model compared with the conventional imaging model and delta radiomic model. Conclusions:The addition of significant clinicopathological variables and Radscore of delta radiomic model which represents the longitudinal changes in tumor heterogeneity to the conventional imaging model may improve the predictive ability of pCR. The delta radiomic may serve as a noninvasive biomarker for early prediction of NAT response.
