Objective To construct a basic competency framework for advanced practice nurses.Methods A primary index system was formed through literature review and expert consultation,the weight of each index was determined by hierarchy analysis.Results The expert authority coefficient was 0.857,Cronbrach alpha of the total system was 0.937.The index system consisted of 5 first-level items,11 second-level items and 46 third-level items.Conclusions The 62-item competency framework for APN is reliable and valid,which can provide quantified standards for selecting,training and evaluating advanced practice nurses.

Objective To determine the relationship between time management disposition and subjective well-being among the head nurses.Methods Totally 388 cases in third-grade class-A hospitals in Chengdu were investigated with Time Management Disposition Scale and Subjective Well-being Scale.Results The higher the scores in time management disposition,the higher the scores of the head nurses in subjective well-being.There was a significant positive correlation between time management disposition and subjective well-being.Among which,time efficacy feeling and time monitoring view had significant predictable effect toward subjective well-being.Conclusions Time management disposition had significant influences on subjective well-being.It indicates that we can improve subjective well-being and promote mental health of the head nurses to a certain extent through strengthening time management training to improve time management disposition level.

Healthy male Wistar rats were prepared as model of precancerous lesion of gastric mucosa. The tongue, pH of tongue surface, dropped cells from tongue fur, histology of tongue. and corresponding gastric mucosa, pH of gastric juice, mucosal histological changes in both groups were observed. Results revealed that, in the rats with precancerous lesion, the tongues were bare or only with little fur, dark purplish tongue with petechia. pH of both tongue and gastric juice were high, which were both coincided with the syndrome type of asthenic gastric - qi and blockage of collaterals by stagnant blood.

Objective To investigate the causes of upper gastrointestinal bleeding (UGB), and some relative factors. Methods The UGB in 1 869 patients examined endoscopically between 1982－ 1996. Results 1.The most common causes of UGB were peptic ulcer (31.5％ ), gastric cancer(19.3％ ), and AGML(17.5％ ), and the rate of severe bleeding was highest in esophageal varices(49.4％ ). 2.Between 1982－ 1986,1987－ 1991,and 1992－ 1996, the rates of UGB were 22.3％ ,12.0％ ,and 13.4％ respectively. 3. Objective To evaluate the results of lung volume reduction surgery (LVRS) using domestic staplers and autologous tissue. Methods Thirteen male patients received LVRS from June 2000 to March 2006 in this hospital. The age range was 56~68 years (mean, 60.5 years). The patients had a history of chronic obstructive pulmonary disease (COPD) for 2~16 years (mean, 11 years). The operation was performed under general anesthesia, with single lung ventilation. A muscle sparing thoracotomy in the 5th intercostal space was conducted. The “target area” was identified by combination of observation and palpation during operation and preoperative CT scans. The cutting edge was stapled with domestic staplers buttressed with autologous tissue. Results All the procedures were successfully accomplished. Follow-up observations for 8 months ~ 5 years showed no dyspnea and improved activities. At 6 months after operation, the FEV_1 and the PaO_2 were increased by 94.4%?21.2% and 12.5%?3.1%, while the RV and TLC were decreased by 24.1%?7.8% and 20.8%?5.1%, respectively. BACKGROUND: Xiongbitong capsule (capsule) increases blood flow in myocardial ischemic area in patients with coronary heart disease, lessens the size of myocardial infarction and improves ischemic S-T alternation in electrocardiogram (ECG).OBJECTIVE: To observe the effect of the capsule on NO metabolism on arterial wall in rabbits and explore the improvement of such drug on myocardial blood flow and protection on myocardium.DESIGN: Randomized group division and control experiment were designed.SETTING: Department of First Internal Medicine, Hospital of Traditional Chinese Medicine, Weifang, Shandong.MATERIALS: The experiment was performed in Teaching-Research Room of Physiology of Weifang Medical College from April 2003 to February 2004. Totally 30 healthy adult rabbits were employed, of either sex and randomized into 3 groups, named normal control, model group and treatment group, 3 rabbits in each one.METHODS: ① Preparation of hyperlipemia (HP) model: In normal control, common granular forage was used. In model group, modeling forage was applied (common granular forage + 3% pig oil + 1% cholesterol powder). In treatment group, besides modeling forage, at the end of the 6th week, the capsule [composed of taizishen (Radix Pseudstellariae), yunling (Poria), gualou (Frnctus Trichosanthis), qingxia (Rhizoma Pinelliae), juhong (Exocarpium Citri Grandis), shichangpu (Rhizoma Acori Graminei), yujin (Radix Curcumaea), zhifuhua (Flos Inulae), jiangxiang (Lignum Dalbergiae Odoriferae), danggui (Radix Angelicae Sinensis), maidong (Radix Liriopis Platyphyllae) and wuweizi (Frnctus Schisandrae).] was administrated every day for gastric perfusion at 1 pill/kg with 10 mL physiological saline for 3 weeks. The intervention was applied totally for 8 weeks. ② Nitrieoxide synthase (NOS) and NO metabolic product kits were provided from Radiation Medical Institute of Military Academy of Medical Sciences. The operation was based on the instrnction. NOS activity was calculated and expressed by NO amount (nmol) produced in one-gram tissue per minutes.Spectrophotometer was used to measure the optic density of sample at 545nm. Sodium nitrite solution was used to prepare standard curve. The concentration of serum NO metabolic products, NO2- and NO3- was expressed with μmol/L. ③ T test was applied for difference comparison among measurement data.MAIN OUTCOME MEASURES: Comparison of NOS activity and concentration of serum NO metabolic products in vessel-wall tissue in eachgroup.RESULTS: Totally 30 rabbits all entered result analysis. For NOS activity and concentration of serum NO metabolic products in arterial-wall tissue,the results in model group and treatment group were lower remarkably than the control (P ＜ 0.05-0.01) and the results in treatment group were higher remarkably than model group (P ＜ 0.05).CONCLUSION: ① NO system of arterial wall is damaged in HP rabbits and NO metabolic level is decreased in serum. ② Xiongbitong capsule improves the above situation and brings the resistance of myocardial ischemia and protection of myocardium into play.

Objective To explore the clinical value of application research in business process reengineering in transferring of inpatients for check-up. Methods We reorganized transferring of inpatients for check-up and service tache by introducing the Systematic Redesign method from the therory of Business Process Reengineering. The reengineered process was adopted and applied in the transferring processes. We randomly selected patients for check-up(126 cases for electrocardiogram,130 cases for Xrayand 128 cases for CT examination)and investigated the time cost for check-up and patients satisfaction rate. The above results were compared with those patients(112 cases for electrocardiogram,112 cases for Xrayand 120 cases for CT examination)before use of reengineered process. Results The time cost for the above mentioned check-up was shorter and patients satisfaction rate were alleviated compared with those before use of reengineered process(U=14.55,12.26,13.57;x2=17.37,14.65,10.75,P＜0.01).Conclusion Application of business process reengineering in the transferring process of inpatients for check-up proved pivotal important in that it could shorten time cost for transferring process and improve patients satisfaction rate.It possessed realistic significance for increasing benefit and competitions for hospitals.

Objective To compare the therapeutic effect and clinical tolerance of endoscopic bipolar coagulation hemostasis(EBCH) and nasal packing(NP) in treatment of epistaxis.Methods The clinical data of 76 epistaxis patients treated with EBCH and 50 epistaxis patients treated with NP between Januaray 2009 and December 2011 were retrospectively analyzed.The two groups were matched in age,gender and bleeding sites.One time control rate of epistaxis,body tolerance,hospitalization time and recurrence rate were compared between the two groups.Results The one time control rate,body tolerance,hospitalization time and recurrence rate were 98.7％ (75/76),(2.91 ± 0.07),(4.11 ± 0.07) d and 2.63 ％ (2/76) respectively in EBCH group.The one time control rate,body tolerance,hospitalization time and recurrence rate were 72.0％ (36/50),(7.96 ± 0.10),(6.72 ± 0.13) d and 16.0％ (8/50) respectively in NP group.There were significant differences between the two groups.Conclusion The treatment of epistaxis with EBCH is better than NP on the therapeutic effect and clinical tolerance.The former method is a potential preferable choice for the treatment of clinical emergent epistaxis.

Objective To observe the protective effect of Naoli Zhibao(NZ) on mice with learning and memory impairment.Methods (1) Thirty-two NIH mice were randomized into pseudo-operation group,model group,and low-and high-dose NZ groups(at the dose of 4 and 8 g?kg-1?d-1 respectively).Except the pseudo-operation group,the mice in other groups received the operation of cerebral ischemia/reperfusion.After treatment for 10 days,the learning and memory abilities of the mice were observed by step-down test,and the contents of malondialdehyde(MDA) and nitric oxide(NO) as well as the activities of superoxide dismustase(SOD) and glutathione peroxidase(GSH-Px) were detected.Meanwhile,the free amino acid content in mice cerebral cortex was measured with high performance liquid chromatography.(2) Forty NIH mice aged 9 months were randomized into normal control group,mimetic aging group,and low-and high-dose NZ groups(at the dose of 4 and 8 g?kg-1?d-1 respectively).The mice models of mimetic aging were induced by intraperitoneal injection of D-galactose 0.5mL for 6 weeks,and the mice in NZ groups were also given gastric gavage of NZ for 6 weeks.After treatment,the learning and memory abilities of the mice were observed by step-down test,and the contents of epinephrine(E),norepinephrine(NE) and tryptophan(TP) in mice cerebral cortex were measured.Results (1) NZ significantly improved the memory ability of mice with cerebral ischemia/reperfusion injury(P

Objective To design a questionnaire to explore nursing care behavior demand from patients'vision, and verify its reliability and validity. Methods The nursing care behavior index entry pool were summed up through literature analysis and discussion, and then a questionnaire containing 40 items was designed through per-investigation. A total of 150 patients from 30 departments in general hospital of Shenyang military region were selected for questionnaire survey using the convenience sampling method. The SPSS 19.0 was used to test the reliability and validity. Results Totally 149 effective questionnaires were recycled. Finally the questionnaires contained 6 dimensions and 35 items. Six factors were extracted with exploratory factor analysis, which explained 57.976% variance respectively. Total questionnaire Cronbach alpha coefficient was 0.892, the dimension Cronbach alpha coefficient was 0.747-0.914; Total remeasuring reliability was 0.824, the remeasuring reliability was 0.733-0.867. Binary validity was 0.806, binary coefficient was 0.748-0.872. Conclusions This study scientifically and reliably built a nursing care behavior demand questionnaire from patients′vision, which can be used to survey patients′satisfaction on nursing humanities.
