ObjectiveTo assess the relationship between the location,quantity and leukodystrophy of cerebral infarction and vascular cognitive impairment ( VCI ),and to compare the differences of the severity and domains of cognitive impairment among the different types of cerebral infarction,which will provide the pathophysiological evidence to the mechanism for the development of VCI. MethodThe patients suffering from cerebral infarction or leukodystrophy were recruited from either the ward or clinic in Tianjin General Hospital from April 2009 to April 2010,then assessed the cognition,and classified into different groups according to the location,size,quantity,whether or not they have leukodystrophy and the severity.The Logistic regression analysis was taken with whether or not the patients have had developed to the VCI as the dependent variable,and with the location,size,quantity,whether or not they have leukodystrophy and the severity as the independent variables.The differences of the score of NIHSS,Bathel index and the level of cognitive impairment were compared among patients with different types of lesions.Moreover,the difference of the score of MoCA for each cognitive domain were compared among patients with different types of lesions.ResultsThere was a relationship between key position infarction in the subcortex and white leukodystrophy with VCI.The position infarction in the subcortex and white leukodystrophy ( grade Ⅱ and Ⅲ ) might be the independent risk factors for the VCI( OR =1.752,2.135,3.753; 95％ CI 1.533-3.821,1.541-6.787,1.432-6.821,all P ＜0.05).There were significant differences in the domains of excusive ability,attention,calculation,fluent language and orientation.The average scores of the domain of excusive ability in the subcortical group (3.40 ± 1.90) and the mix group (3.83 ± 1.27) were significantly lower than the cortical disease group(4.28 ± 0.89,t =0.050,0.857,both P ＜ 0.05 ).The average scores of the domains of attention,calculation,fluent and orientation in the mix group were significantly lower than the subcortical group and cortical disease group.ConclusionsThere is a significant association of the key infarction in the subcortex and leukodystrophy with VCI.The patients with both the subcortex infarction and leukodystrophy are susceptible to VCI.The development of VCI is depended on the position of infarction instead of the quantity and size of infarction.The development of VCI is not only related to the white matter disease but also related to the severity of the leukodystrophy.The data highlights the need to small vessel disease.The patients with subcortical lesions mainly impair the executive ability.The patients with mix lesions may have more extensive domains and more severe cognitive impairment.

Background Proliferative vitreo-retinal diseases include all the overly proliferative diseases of eyes.Many kinds of cytokines play important roles in their pathogenesis.Hypoxia-inducible factor 1α(HIF-1α)participates in the pathogenesis of a lot of ischemic diseases.HIF-1α may play an important role in the pathogenesis of proliferative vitreo-retinal diseases. Objective Present study was to observe the expression of HIF-1α in vitreous body in the eyes with proliferative vitreo-retinal diseases.The vitreous samples were collected during the vitrectomy from 71 eyes of 69 patients(experimental group)including 39 eyes with proliferative diabetic retinopathy(PDR)and 32 eyes with proliferative Vitreoretinopathy(PVR).Sixteen eyes of 16 patients as the cases control group,including 14 eyes with macular hole(MH)and 2 eyes with epiretinal macular membrane(ERM).What is more,there were 8 vitreous fluid samples of the normal control group.The concentrations of HIF-1α in vitreous body were detected by enzyme-linked immunosorbent assay(ELISA).The written informed consent was obtained from each patient before any medical procedure. Results The concentrations of HIF-1α in vitreous body in the eyes of experimental group,the cases control group and the normal control group were(294.08±2.97)、(260.41±8.29)、(16.38±3.56)mg/L respectively,showing a significant difference among the three groups(F=248.77,P=0.00).The concentrations of HIF-1α in vitreous body were obviously increased in the eyes of experimental group in comparison with the normal control group and the cases control group respectively(t=22.25,P=0.00;t=2.70,P=0.00 respectively).The concentrations of HIF-1α in vitreous body were increased in the eyes of the cases control group in comparison with the normal control group eyes(t=14.21,P=0.00).There was inferior correlation between the concentrations of HIF-1α in vitreous body in the eyes of experimental group and the course of ophthalmopathy. Conclusion Intravitreous HIF-1α level regulates in the eyes with proliferative vitreo-retinal diseases. Expression of HIF-1α in vitreous had inferior correlation with the course of ophthalmopathy. HIF-1α may play an important role in the pathogenesis of proliferative vitreo-retinal diseases.

Objective To investigate variation in the carotid bifurcation geometry of adults of different age by MR angiography images combining image post-processing technique. Methods Images of the carotid bifurcations of 27 young adults (≤40 years old) and 30 older subjects ( >40 years old) were acquired via contrast-enhanced MR angiography. Three dimensional (3D) geometries of the bifurcations were reconstructed and geometric parameters were measured by post-processing technique. Results The geometric parameters of the young versus older groups were as follows: bifurcation angle (70.268°± 16.050° versus 58.857°±13.294°), ICA angle (36.893°±11.837° versus 30.275°± 9.533°), ICA planarity (6.453°±5.009° versus 6.263°±4.250°),CCA tortuosity (0.023°0.011 versus 0.014± 0.005), ICA tortuosity (0.070±0.042 versus 0.046±0.022), ICA/CCA diameter ratio (0.693± 0.132 versus 0.728±0.106), ECA/CCA diameter ratio (0.750±0.123 versus 0.809±0.122), ECA/ ICA diameter ratio (1.103±0.201 versus 1.127±0.195), bifurcation area ratio (1.057±0.281 versus 1.291±0.252). There was significant statistical difference between young group and older group in bifurcation angle, ICA angle, CCA tortuosity, ICA tortuosity, ECA/CCA and bifurcation area ratio (F= 17.16,11.74,23.02,13.38,6.54,22.80,respectively, P<0.05). Conclusions MR angiography images combined with image post-processing technique can reconstruct 3D carotid bifurcation geometry and measure the geometric parameters of carotid bifurcation in vivo individually. It provides a new and convenient method to investigate the relationship of vascular geometry and flow condition with atherosclerotic pathological changes.

Objective To investigate the visualization and quantitation of hemodynamic variables at carotid artery bifurcation in vivo by combining computational fluid dynamics(CFD)and vascular imaging. Methods A healthy volunteer underwent CT angiography of left carotid artery by SIEMENS multi-slice CT. Parameters of hemodynamics at this carotid bifurcation were calculated and visualized by combining vascular imaging post-processing and CFD.Results (1) The average range of flow velocity was 0.04～0.36 m/s. A region of high velocity was seen at medial wall of internal carotid artery (ICA) and medial wall of external carotid artery (ECA), respectively. The largest contiguous region of low velocity occurred at the carotid bulb. (2)The average range of absolute pressure, static pressure and dynamic pressure was 100 266.70～101 615.90 Pa, -1058.34～290.88 Pa, and 6.12～553.25 Pa, respectively. (3) The average range of wall shear stress (WSS) at the bifurcation was 0.59～5.35 Pa. There was a large region of low WSS at carotid bulb and posterior wall of ICA, with the lowest value of 0.25 Pa. Also there was a small region of low WSS at anterior and lateral wall of ECA.Conclusion CFD combined with vascular imaging can calculate and visualize the parameters of hemodynamics at carotid bifurcation in vivo individually. It is an interdisciplinary science of computer, radiology and hemodynamics and provides a new method to investigate the relationship of vascular geometry and flow condition with atherosclerotic pathological changes.

Objective To investigate flow patterns at carotid bifurcation in vivo by combining computational fluid dynamics(CFD)and MR angiography imaging.Methods Seven subjects underwent contrast-enhanced MR angiography of carotid artery in Siemens 3.0 T MR.Flow patterns of the carotid artery bifurcation were calculated and visualized by combining MR vascular imaging post-processing and CFD. Results The flow patterns of the carotid bifurcations in 7 subjects were varied with different phases of a cardiac cycle.The turbulent flow and back flow occurred at bifurcation and proximal of internal carotid artery (ICA)and external carotid artery(ECA),their occurrence and conformation were varied with different phase of a cardiac cycle.The turbulent flow and back flow faded out quickly when the blood flow to the distal of ICA and ECA.Conclusion CFD combined with MR angiography can be utilized to visualize the cyclical change of flow patterns of carotid bifurcation with different phases of a cardiac cycle.
