1.Study on the Optimization of Prescription and Preparation of Matrine Liposomes

OBJECTIVE:To optimize the prescription and preparation of matrine liposomes.METHODS:The liposomes were prepared by the reverse-phase evaporation technique;the envelope efficiency and the diameter of matrine liposomes were taken as criteria,the prescription and preparation of the liposomes were optimized by orthogonal design.RESULTS:The best prescription consisted of egg phosphatidylcholin(80mg),the oil phase cholesterol(15mg),the water phase phosphate buffer so?lution(pH=7.0)and matrine(30mg);the best preparation technics was as follows:chloroform(30ml),ultrasonic time was10min rotary evaporating temperature at48℃.The liposomes had homogeneous shape with the particle size at250nm~750nm and the envelope efficiency of matrine liposomes at47.25%.CONCLUSION:The optimized formulation of martine liposomes is reasonable in prescription,practicable in technics,high in envelope efficiency and perfect in stability.
2.Physical activity level in physical education class of elementary school in Xi an

Objective:
To assess physical activity level and characteristics in elementary physical education (P.E.) class among different weight status and grades students.
Methods:
ActiGraphGT3X was used to measure physical activity of 60 students in grade 1, 2, and 4 during three P.E. classes. BMI was calculated according to the student s height and weight in the latest physical fitness test results, and the weight status was divided according to the national Physical Fitness Standards for Students in different grades of elementary schools. Independent sample t test and analysis of variance were used to explore the difference of physical activity.
Results:
The proportion of moderate to high intensity physical activity (MVPA) in elementary school P.E. class was 21.9% (8.8 min). The proportion of MVPA in P.E. classes and the number of steps in normal weight students were 25.4% and (1 247.0± 326.3 ), respectively, which were significantly higher than those of underweight students, but not significantly different from those of overweight students. The MVPA of male and female PE classes in the second grade (27.0%, 26.4%) was significantly higher than that in the first grade, but there was no significant difference compared with that in the fourth grade ( P >0.05).
Conclusion
Moderate to high intensity physical activity is low in P.E. class in elementary school. Improving teachers awareness of promoting physical activity in PE class will benefit the MVPA in students.
3.Surgical treatment for 27 cases of malignant fibrous histiocytoma in the soft tissue of the neck

Purpose:An analysis of 27 patients who underwent surgery for their malignant fibrous histiocytoma in the soft tissue of the neck was done to show that compartment dissection of the neck should be the optimal choices of treatment. Methods:From 1 st Jan 1972 through 31 st Dec 1993,27 patients who were referred to the Cancer Hospital for surgical treat- ment were reviewed,including 19 males and 8 females in the age range from 15 to 67,with a mean of 44 years.Surgery was classified as wide resection and compartment dissection of the neck.The two groups were compared for local recurrence and 5 years survival rates.Results:Of the 27 operations,21 were wide resections and 6 compartment dissection of the necks.The average tumor diameter was6 millimeters in patients with compartment dissection of the neck.Local recurrence was encountered in 16 of 21 wide resections (76.2%),and in 1 of 6 compartment dissection of the necks (16.7%).The difference between the two groups was statistically significant (P= 0.015).The 5 years survival was 89% for patients without local recurrence and 52% for patients with local recurrence. The difference between them was significant statistically (Log Rank,P=0.0264).Conclusions:For malignant fibrous his- tiocytoma in the soft tissue of the neck,compartment dissection of the neck is recommended as the optimal operation for its ability to the local control of the disease locally.
4.Application of performance excellence management model in clinical nursing teaching

Objective To introduce the methods of performance excellence management model in clinical nursing teaching. Methods A total of 110 clinical nursing interns were selected before and after implementation of performance excellence model, traditional teaching method was used prior to the implementation of performance excellence model. The teaching quality evaluation, teachers teaching level feedback, examination of interns, teachers′ satisfaction degree with nursing work were investigated and compared before and after the implementation of performance excellence model. Results After the implementation of excellent performance management model, the assessment of interns with teaching quality scored from 3.75±0.51 (before the implementation) to 3.98±0.14, t=-6.738, P<0.01. Examination of interns by teachers scored from 92.44 ± 1.44 to 95.01 ± 2.20, t=-7.24, P<0.01. Teachers′satisfaction degree with nursing work scored from 3.91 ± 0.44 to 4.26 ± 0.46, t=-3.006, P<0.01. Conclusions The application of excellent performance management model in the hospital clinical nursing teaching can effectively improve the quality of teaching as well as satisfaction degree of nursing interns, teachers and departments.
5.The Application of Multi-slice Spiral CT Angiography in Pulmonary Lesions

Objective To study the applied value of multi-slice spiral CT(MSCT)in pulmonary lesions.Methods 68 cases with massive hemoptysis caused by lung tumor or other reasons underwent 16-slice spiral CT scan including plain and contrast-enhanced scan.Volume reconstruction(VR),multiplanar reconstruction(MPR),maximum intensity projection(MIP)and curved planar reconstruction(CPR)were performed using original images.The origin,number and shape of vessels feeding lesions were observed.Results Among 68 cases(two failed),the blood supply of pulmonary lesions was from bronchial artery in 60 cases,from the intercostals artery in 8 cases,from the internal thoracic artery in 5 cases,from the left subclavian artery in 6 cases,from the right inferoir phrenic artery in 5 cases,from the celiac trunk artery in 3 case and from the right renal artery in 1 case.The blood supply was from single artery in 50 cases,double arteriae in 10 cases and three arteriae in 6 cases.The supply arteriae were vascular plexus in 55 cases,meandering in 8 cases,net in 2 cases and aneurysm formation in one cases.Conclusion MSCT can clearly display the vessels feeding pulmonary lesions.

Objective To explore the effect of iron chelators on labile iron pool and expression of apoptosis associated genes in cells of K562, an erythroleukemia cell line.Methods K562 cells were incubated at 37 ℃ in RPMI 1640 containing 10% heat-inactived fetal bovine serum in an saturated humidity and 5% CO_2 incubator. Objective To observe the influence of the clinical pathway management intervention on medical service behaviors.Methods The clinical cases data in a grade A class 3 hospital of Zhangjiakou City,Hebei Province during 2011-2013 were retrospectively analyzed.The influence of implementing the clinical pathway management on the diagnosis results,medical records writing quality,clinical medication,operation,hospitalization time,medical costs,assisted examination items selection were observed and analyzed.Results The admission and discharge diagnosis coincidence rate and preoperative and postoperative diagnostic coincidence rate of 3 diseases entities in the two groups were 100%,the difference had no statistical significance (P>0.05);the medical record grade-A rate in the implementing clinical pathway group was significantly higher than that in the non-implementing clinical pathway group,the medical record grade-B rate was significantly lower than that in the non-implementing clinical pathway group,the difference was statistically significant (P<0.05);the antibacterial drugs cost and drugs proportion in the implementing clinical pathway group were significantly lower than those in the non-implementing clinical pathway group,the medication proportion within the directory in the implementing clinical pathway group was significantly higher than that in the non-implementing clinical pathway group,the difference was statistically significant (P<0.05);the preoperative preparation time in the implementating clinical pathway group was less than that in the non-implementing clinical pathway group,the difference was statistically significant(P<0.05),the grade-A healing rate had no significant difference between the two groups (P>0.05);the average hospitalization time,total costs,drug costs,laboratory fee and examination fee in the implementing clinical pathway group were lower than those in the non-implementing clinical pathway group,the difference was statistically significant (P<0.05);the operation fee,healthy material fee and nursing fee had no statistically significant difference was statistically significant(P<0.05).Conclusion Implementing the clinical pathway can standardize the doctor′s diagnosis and treatment behaviors and makes the medical work to develop to be more favorable for patients.
8.Soluble triggering receptor expressed on myeloid cells-1 as a diagnostic marker of ventilator-associated pneumonia in patients with acute ischemic stroke

Objective To investigate the potential role of soluble triggering receptor expressed on ayeloid cells-1(sTREM-1) expression in serum,endotracheal aspiration (ETA),bronchoalveolar lavage fluid (BALF) and exhaled breath condensate (EBC) as early biomarkers for the diagnosis of ventilator-associated pneumonia (VAP) in patients with acute ischemic stroke.Methods One hundred and thirty-two patients with clinically suspected VAP were prospectively included in this multicenter study.The levels of sTREM-1 in serum,ETA,BALF and EBC were analyzed for diagnostic evaluation at the time of VAP clinically suspected.The bacterial count over 104/CFU as a gold standard for VAP,and the receiver operating characteristic curves were used to identify the ideal cutoff values.Results VAP was confirmed in 76 patients (57.58%).In VAP patients (VAP group) and non-VAP patients (non-VAP group),the level of sTREM-1 in BALF was 32.35 (30.08-41.72) and 18.92(11.89-31.72) ng/L,and the level of sTREM-1 in EBC was 1.57 (1.02-2.61) and 0.41(0.19-1.61)ng/L respectively.The level of sTREM-1 in BALF and in EBC in VAP group was significantly higher than that in non-VAP group (P <0.05).The optimum cutoff value for sTREM-1 in BALF according to the maximum Youden index was 23.61 ng/L.This cutoff value had 85.5% sensitivity and 73.1% specificity,with 0.813 area under the curve.sTREM-1 in BALF had excellent correlation with that in EBC (R2 =0.78,P < 0.05).Conclusions The results of this prospective study suggest that sTREM-1 levels in BALF and EBC have better roles in facilitating the diagnosis of VAP and thus may be practically recommended to guide the administration of antibiotics when VAP is suspected.
9.Analysis of 32 cases of acute biliary pancreatitis with pancreaticobiliary maljunction

Objective To investigate the relationship between acute biliary pancreatitis and pancreaticobiliary maljunction and the role of magnetic resonance cholangiopancreatography(MRCP) in evaluation of pancreatico biliary maljunction.Methods To compare the liver function indicators of different groups of acute biliary pancreatitis patients(153 cases) associated with pancreatico biliary maljunction and without pancreatico biliary maljunction before and after the conservative treatment.Results The 32 acute biliary pancreatitis patients with pancreatico biliary maljunction detected by MRCP were compared with the 121 cases without pancreatico biliary maljunction.The ALT,AST,GGT after conservative treatment in both group of pancreatico biliary maljunction and Npancreatico biliary maljunction were significantly decreased (P < 0.05).ALT,AST and GGT of pancreatico biliary maljunction group were higher than that of Npancreatico biliary maljunction group with statistical significance (P < 0.05).Conclusions MRCP as a noninvasive cholangiopancreatography study of pancreatico biliary maljunction is a safe and reliable examination method,pancreatico biliary maljunction is one of the important causes of acute biliary pancreatitis.
10. Epidemiological characteristics and etiology of herpangina cases in Xi’an, 2016

Objective:
To analyze the epidemiological characteristics and etiologic agent of herpangina in Xi’an in 2016.
Methods:
Herpangina epidemiological data and random stool samples were collected from two sentinel hospitals. The data were statistically analyzed with Excel and SPSS 18.0 and samples were detected and enteroviruses were genotyped using real-time PCR and RT-snPCR.
Results:
Totally 421 cases were reported and the male-to-female ratio was 1.54∶1, age ranged from 3 month to 13 years, and 90.50% were under three years of age. Herpangina showed semiannual peaks in April to October, of which the highest was in June (23.75%, 100/421). Lab result showed that 71.05% were EV positive out of 152 samples, and further genotyping indicated 60 were non-typable, and other 48 comprised of eight serotypes of EV including EV-A71, CV-A4, CV-A5, CV-A6(6.48%), CV-A10(23.15%), CV-A12, CV-A16 and CV-B3.
Conclusions
Male children under 3 years of age were the main population and herpangina cases were most frequently reported in June in Xi’an in 2016. CV-A6 and CV-A10 were more frequently detected in the typable ones.
