2.3-lead electrocardiography and pulse oximetry in early heart rate assessment of high-risk neonates

Objective:To compare the accuracy and timeliness of 3-lead electrocardiography (ECG) and pulse oximetry (POX) in neonatal heart rate (HR) monitoring after birth.Methods:This prospective study recruited 42 high-risk newborns with gestational age ≥37 weeks and birth weight >1 500 g who were born through cesarean section without resuscitation requirement in Xi'an People's Hospital (Xi'an Fourth Hospital) from October 2019 to August 2020. 3-lead ECG electrodes and POX sensors were attached to the neonates immediately after drying to continuously monitor the HR within 10 min after birth. All procedure was recorded by video camera, and data were independently analyzed by a clinician after the procedure was completed. Differences in time required to connect the devices, time to obtain a reliable HR and the interval between them, the time needed for obtaining a reliable HR after birth, the proportion of neonates with reliable HR obtained within 5 min after birth and the consistency in the reliable HR readings between the two devices were compared using Wilcoxon signed-rank test, McNemar test, Spearman's correlation coefficient, intraclass correlation coefficient or Bland-Altman bias analysis.Results:The median time required to connect POX and 3-lead ECG and to acquire a reliable HR were 13.0 s (10.0-17.0 s) vs 23.0 s (18.0-28.3 s) ( Z=-5.050, P<0.001), and 79.5 s (56.2-128.0 s) vs 11.0 s (10.0-13.3 s) ( Z=-5.646, P<0.001), respectively. The total time from the beginning of connecting the devices and birth to acquiring a reliable HR were both longer for POX than those for 3-lead ECG [92.0 s (71.3-139.0 s) vs 35.0 s (30.0-39.5 s), Z=-5.579, P<0.001; 110.5 s (85.8-153.5 s) vs 52.0 s (45.0-66.3 s), Z=-5.579, P<0.001]. Reliable HRs were obtained in 69.1% (29/42) and 2.4% (1/42) of the infants by 3-lead ECG and POX within 1 min after birth, respectively. The percentage of infants for obtaining a reliable HR detected by 3-lead ECG within 5 min after birth were more than those by POX, but with statistically significant differences only at the first 60 s, 90 s, 120 s and 150 s (all P<0.001). The median HRs obtained by 3-lead ECG and POX within 10 min after birth were 161 beats/min (147-175 beats/min) and 160 beats/min (146-176 beats/min), respectively ( r=0.966, P<0.001). The mean difference of HR detected by the two devices was 0.56 beats/min (95% CI:-4.3 to 5.4 beats/min). The intraclass correlation coefficient was 0.961, showing good internal consistency. Conclusions:Neonatal HR can be assessed accurately by 3-lead ECG within 1 min after birth, which is far earlier than that by POX. Therefore, 3-lead ECG can be an option for continuously HR monitor in neonatal resuscitation.

Genomic DNA was extracted from the blood samples of 3 patients from 1 pedigree with congenital nephrogenie diabetes insipidus (NDI) and their 12 family members.The whole coding region of the arginine vasopressin receptor 2 (AVPR2) gene was amplified by PCR and then directly sequenced,A mutation of AVPR2 gene [g1236T→C (L292P)]was found in 3 patients.The patients' mothers were found to have both mutant and normal alleles.
5.Diagnostic significance of detection of anti-citrullinated peptide antibodies in juvenile rheumatoid arthritis

Objective To assess the diagnostic value of anti-cyclic citrullinated peptide antibody(an- ti-CCP),rheumatoid factor,anti-perinuclear factor(APF)and anti-keratin antibody(AKA)for juvenile rheumatoid arthritis(JRA)and compare it with rheumatoid arthritis(RA).Methods Anti-CCP was determined by ELISA in 54 serum samples of JRA patients,31 from patients with other rheumatic diseases and 116 RA patients.RF was determined in the same samples by latex agglutination test.APF and AKA were determined by indirect immunofluorescent assay.Results The sensitivity of anti-CCP,RF,APF and AKA was 61.1%, 57.4%,37.0% and 18.5% and their specificity was 96.8%,93.6%,96.8% and 100%,respectively for the diag- nosis of JRA.The sensitivity of anti-CCP resembleed that of RF,Anti-CCP was more sensitivity than APF and AKA in JRA.The sensitivity of anti-CCP,RF,APF and AKA was 82.3%,78.3%,48.7% and 25.4% and their specificity was 95.7%,73.7%,91.6%,94.0% respectively,for the diagnosis of RA.Anti-CCP,RF,APF and AKA were less sensitive in JRA than in RA.There was no statistical significance in specificity of these anti- bodies for the diagnosis of JRA and RA.Conclusion The detection of anti-CCP,RF,APF and AKA are use- ful for the diagnosis of JRA,but are less sensitive than in adults RA.
7.Effects of Rehmannia glutinosa oligosaccharides on proliferation of 3T3-L1 adipocytes and insulin resistance.

OBJECTIVETo investigate the influence of Rehmannia glutinosa oligosaccharides (ROS) on the proliferation of 3T3-L1 adipocytes and insulin resistance.
METHOD3T3-L1 preadipocytes were cultured, the proliferation of 3T3-L1 preadipocytes was detected by MTT method. Insulin resistant 3T3-L1 adipocytes cell model was induced by dexamethasone and the change of glucose concentration in cell culture was determined after ROS treatment.
RESULTIn the high glucose DMEM culture media, MTT method showed that the absorbance at 570nm of 3T3-L1 preadipocytes was increased and that of 3T3-L1 adipocytes was decreased. ROS significantly increased glucose consumption in 3T3-L1 preadipocytes and adipocytes culture in a concentration-dependent manner. ROS improved the sensitivity of 3T3-L1 adipocytes to insulin.
CONCLUSIONROS can promote the proliferation of 3T3-L1 preadipocytes, inhibite the proliferation of 3T3-L1 adipocytes, and also, significantly improve insulin resistance induced by dexamethasone.

8.Change of memory function and decrease of nitric oxide level of whole brain in the transgenic mice expressing human tau 40 with P301L mutation.

OBJECTIVETo study the mechanism of learning and memory dysfuction in the transgenic mouse expressing human tau 40 isoform with P301L mutation (F10).
METHODSThe human tau protein expression and phosphor-tau protein levels were detected with Western blot method. The neurofibrillary tangles were observed with Bielshowsky silver stain. The behavior changes of learning and memory were observed by open field test and passive avoidance test. Acetyleholine level, activities of acetycholinesterase and choline acetyltransferase of whole brain was detected by colorimetry method. The nitric oxide level of whole brain was detected by nitrate enzyme reduction method.
RESULTSExogenous human tau gene was expressed and an elevation of phosphor-tau protein level in 7 and 3-month transgenic mice's hippocampus andcerebrocortex was observed. The neurofibrillary tangles were observed in cerebrocortex of 7-month transgenic mice; the 7-month transgenic mice also presented an evident reduction of learning and memory ability and nitric oxide level of the whole brain, but not changes in acetylcholine level, acetycholinesterase activity, choline acetyltransferase activity and expression in whole brain.
CONCLUSIONTau transgenic mice (F10) can still inherit their parents' biologiccal characters, and develop learning and memory dysfunction awnodh san obvious decrease in nitric oxide level of whole brain in the 7-month old mice, suggesting a decrease of nitric oxide level of whole brain would be involved in the mechanism of learning and memory dysfunction in these transgenic mice.

9.Value of urinary kidney injury molecule-1 protein in early diagnosis of radiocontrast- induced nephropathy in rats.

OBJECTIVETo assess the value of determination of urine kidney injury molecule-1 (KIM-1) protein content in the early diagnosis of radiocontrast-induced nephropathy (RCIN) in rats.
METHODSSeventy-two adult male SD rats were randomly divided into groups A, B, C (n=8) and D group (which was subdivided into 2, 6, 12, 24, 48 h and 7 days groups, n=8). Group A was subject to injections via the tail vein of PBS and normal saline (NS), group B received injections of PBS, NS, and contrast medium (CM), group C with indomethacin (INDO), nitric oxide synthase inhibitor (L-NAME), and NS, and group D with INDO, L-NAME and CM. Each injection was given at the interval of 15 min.
RESULTSIn group D, serum creatinine (Scr) and urea nitrogen (BUN) were significantly increased at 6 h after the injections (P<0.001), peaked at 24 h and recovered normal levels at 48 h. Histological examination revealed significant pathological changes in the kidneys at 6 h, showing diffuse tubulointerstitial hyperemia and hemorrhage, marked tubular necrosis and tubular structure destruction; at 12 h, significant tubular necrosis was still present with tubular structure destruction, but the tubulointerstitial hemorrhage was alleviated; at 24 h, tubular regeneration occurred in the renal medulla, but even till 7 days the tubular structures failed to show full recovery. In group D, urine KIM-1 level began to increase at 2 h, reached the peak level at 24 h, and lasted till 7 days (P<0.001); urine N-acetly-β-D-glucosaminidase (NAG) value began to increase at 6 h and became normal at 48 h. Urine MMP-9 level underwent no significant changes in group D over the time points of observation.
CONCLUSIONThe results show that urinary KIM-1 levels can be used as an indicator for early diagnosis of RCIN.

10.Diagnostic value of S100A6 mRNA expression level in endoscopic ultrasonography guided fine needle aspiration specimens in pancreatic cancer

ObjectiveTo investigate the feasibitity of detecting S100A6 expression at mRNA level in endoscopic ultrasonography guided fine needle aspiration (EUS-FNA) pancreatic ductal adenocarcinoma (PDA) specimens and its diagnostic value in PDA.MethodsA total of 18 PDA specimens and 22 normal pancreatic specimens were collected. RNA was extracted for reverse transcription.The expression of S100A6 gene was examined by real-time polymerase chain reaction.The cut-off value of S100A6 expression at mRNA level in PDA diagnosis was established through receiver operating characteristic (ROC) analysis. 28 patients with pancreatic head masses were selected for EUS-FNA examination,and the value of S100A6 mRNA expression level in PDA diagnosis was prospectively evaluated. The expression of S100A6 protein in PDA tissue was determined by immunohistochemistry staining.ResultsS100A6 mRNA expression in EUS-FNA and surgical PDA specimens (0.05023±0.10120,0.02083 ± 0.02848) was significantly higher than that of normal pancreatic tissues (0.00164±0.00202),both P<0.01.The expression of S100A6 in 22 EUS-FNA PDA specimens was significantly higher than that of 6 pancreatic benign disease biopsy specimens (0.00193 ± 0.00278,P =0.0009). There was no significant difference in S100A6 expression between 6 pancreatic benign disease biopsy specimens and normal surgical pancreatic samples (P=0.6143).When S100A6 mRNA expression in EUS-FNA specimens over 0.00525 was taken as positive diagnostic value,the sensitivity,specificity and accuracy in prospective pancreatic cancer diagnosis were 90.01%,100 % and 92.85 %,respectively.ConclusionThe high expression of S100A6 mRNA in EUS-FNA specimens of PDA has good preoperative diagnostic value.
