Chinese Journal of Postgraduates of Medicine 2010;33(26):26-28
Objective To compare the long-term effects between procedurefor prolapse and hemorrhoids(PPH) and Milligan-Morgan hemorrhoidectomy( MMH ) in the treatment of Ⅲ and Ⅳ degree of hemorrhoids. Method The clinical data of 43 cases with Ⅲ and Ⅳ degree hemorrhoids received PPH (PPH group)and 71 cases received MMH (MMH group) were analyzed retrospectively from October 2001 to July 2003. Results Late bleeding in 5 cases of PPH group and no one of MMH group (P <0.05).Hemorrhoidal recurrence prolapsed in 9 cases of PPH group and 5 cases of MMH group (P < 0.05). The rectal neck pressure preoperative, after 6 months, 1 year and 3 years were (122.0 ± 11.3), (90.0 ± 10.4),(103.0 ± 13.8) and (113.0 ± 11.2) cm H2O (1 cm H2O =0.098 kPa) of PPH group, after 6 months was decreased obviously than preoperative (P<0.05),and they were ( 126.0 ± 13.5), (91.0 ± 12.4), (78.0 ±9.8) and (81.0 ± 7.5 ) cm H2O of MMH group, after 6 months, 1 year and 3 years were decreased obviously than preoperative (P < 0.05). Conclusion PPH is safety and effect at short time, but it brings more late bleeding and anal pain, and its rate of prospective hemorrhoidal recurrence is more higher than MMH.

BACKGROUND: Because the continuity and integrity of the trachea are likely damaged to some extent after tracheostomy, the implementation of sequential ventilation has certain difficulties, and sequential invasive-noninvasive ventilation on patients after tracheostomy is less common in practice. The present study aimed to investigate the feasibility of invasive-noninvasive sequential weaning strategy in patients after tracheostomy. METHODS: Fifty patients including 24 patients with withdrawal of mechanical ventilation (conventional group) and 26 patients with sequential invasive-noninvasive weaning by directly plugging of tracheostomy (sequential group) were analyzed retrospectively after appearance of pulmonary infection control (PIC) window. The analysis of arterial blood gases, ventilator-associated pneumonia (VAP) incidence, the total duration of mechanical ventilation, the success rate of weaning and total cost of hospitalization were compared between the two groups. RESULTS: Arterial blood gas analysis showed that the sequential weaning group was better than the conventional weaning group 1 and 24 hours after invasive ventilation. The VAP incidence was lowered, the duration of mechanical ventilation shortened, the success rate of weaning increased, and the total cost of hospitalization decreased. CONCLUSION: Sequential invasive-noninvasive ventilator weaning is feasible in patients after tracheostomy.

OBJECTIVE: To investigate the protection mechanism of volatile oil of Magnolia biondii Pamp. (VOMBP) against diabetic nephropathy in rats by observing its effects on level of soluble P-selectin (sP-selectin) in serum and expression of P-selectin in renal tissue. METHODS: Fifty SD rats were randomly divided into normal control group, untreated group, and low-, medium- and high-dose VOMBP-treated group. Diabetic nephropathy was induced in rats by intraperitoneal injection of 1% streptozotocin. Before and the 1st day, 4th, 8th and 12th week after the induction, random blood glucose (RBG) and 24-hour urinary micro-albumin were detected in different groups. At the 12th week, the rats were sacrificed to collect the blood samples and renal tissues. The contents of blood urea nitrogen (BUN) and serum creatinine (SCr) were detected and the pathological change in renal tissues was observed by light microscope; the level of sP-selectin was detected by enzyme-linked immunosorbent assay and the expression of P-selectin protein in renal tissues was measured by immunohistochemical method. RESULTS: Compared with the normal control group, RBG, 24-hour urinary micro-albumin, the contents of BUN, sP-selectin in serum and expression of P-selectin protein in renal tissue in the untreated group were significantly increased (P<0.01), the content of SCr was significantly decreased (P<0.01), and pathological change of renal tissues was also obvious. Compared with the untreated group, 24-hour urinary micro-albumin, the level of sP-selectin in serum and expression P-selectin protein in renal tissue of the three VOMBP-treated groups were all significantly decreased (P<0.01), and pathological change was lessened too. However, there were no significant differences among the three VOMBP-treated groups (P>0.05). CONCLUSION: VOMBP can protect the kidney in rats with diabetic nephropathy by inhibiting the expressions of P-selectin protein in serum and in renal tissue.

Object The immunoregulation function was studied in experimental animals to observe the effect of single and/or combined use with Radix Aconiti (RA) and Radix Stephaniae Tetrandrae (RST). Methods The response of delayed type hypersensitivity (DTH) in mice, the phagocytosis of abdominal macrophages (M?) in mice, and the production of special antibodies in mice were observed. Results The two Chinese materia medica (CMM) in combination use could induce the increasing of phagocytosic function of M? hypoimmunitic model mice, the inhibiting on the responses of DTH in the hyperimmunitic model mice and the non immunosuppressive treated mice. On the production of special antibodies in mice, the enhance effect was found in RA used in single, and the suppressive effect was found when using the combination of the two CMM. Conclusion In accordance with the mechanism of the disorder between the anti inflammation effect and the induce inflammation effect to arthritis in the immune system, these data indicate that the bidirectional effect of combination of the two CMM used on the immune responses, may be the involved mechanism on treating arthritis.

Objective To assess ventricular function of early stage neonates of different gestational ages by tissue doppler imaging(TDI).Methods Pulsed wave TDI velocities were obtained in 36 cases of premature infants with gestational ages of 32 to 36 weeks(premature group) and 33 cases of full-term infants(full-term group) aged 3 to 7 days at the lateral mitral annulus(MA),basal septum, and lateral tricuspid annulus(TA) during ventricular systole(Sa),early diastole(Ea),late diastole(Aa).Tansmitral and transtricuspid inflow were also obtained through pulsed doppler echocardiography. Results Ea and Sa in all of 3 locations were lower in the premature group compared with that of the full-term group, and Ea/Aa in TA was lower in premature group, but Aa and E/Ea showed no difference between 2 groups.Ea and Sa showed a positive correlation with gestational age and birth weight. Conclusions Ventricular systolic and diastolic function in premature infants are poorer than that in full-term infants in the early stage of neonatal period, and ventricular function is related to intra-uterus growth and maturity of the newborn.Diastolic function of the left ventricle in both groups develops rapidly during the early stage of neonatal period.

Generalized epilepsy with febrile seizures plus(GEFS+) is a new epilepsy syndrome proposed by International League Against Epilepsy. At present, the progress of genetic studies of GEFS+ focus on gene mapping based on family analysis, many researches indicate that GEFS+ is associated with voltaged-gated sodium channel(SCN) gene mutation. This paper intends to discuss the relationship beween GEFS+ and SCN1B,SCN2B,SCN1A,SCN2A genes, mutations in order to improve the cognition about GEFS+.
