Objective To compare the clinical effects of endoscopic saphenous vein harvesting and traditional surgical management in coronary artery bypass grafting(CABG).Methods Between march 2007 to June 2008,215 patients underwent CABG.Among them,87 patients underwent endoscopic saphenous vein harvesting.At the same time,128 cases were managed by traditional ways.Results The cases were foUowed up for 1-17 months.Endoscopic great saphenous vein harvesting was superior to traditional management in the healing of wound infection,relief of pain,improvement of edema and skin feeling(P＜0.05).Conclusion Endoscope saphenous vein harvesting is an efficient technique for CABG which is characterized by less trauma and fewer wound complications,and will not influcence the function of vein as well.On the other hand,the satisfaction is improved.

Oseteogenic sarcoma is a kind of malignanttumor of bone, it can occur at any age, teenagersare mose commonly involved. The conventionaltreatment of osteogenic sarcoma was high amputa-tion. Although the primary focuses were removedtotaliy, the subclinical minor focuses could not becontrolled effectively. Therefore. the prognosiswas very poor, the five-year survival rate was on-ly about 20 percent. With the rapid development inchemotherapy and orthopedical surgery tech-niques, multimodality therapy of osteogenic sarco-ma were carried out in 65 cases from 1982 to 1988in our hospital. Curative effect has been satisfac-torily improved. The fivc-year survival rate hasbeen raised to 67. 92%. The methods of multi-modality therapy were as follows: ①Peroperationand postoperation chemotherapy with p?liod ofbody. ②Local arteries were perfused with Nitero-gen mustard (HN_2). ③Inactivation of tumor celland replantation. ④Inactivation of tumor cells invivo. ⑤Amputation.

5srRNA was isolated and purified from the reticulocytes of rabbit. It labeled with ~(125)I, then incubated with mouse myeloma cells (SP2/0). By autoradiography it was observed that ~(125)I-SsrRNA could pass into the nuclei of the cells. In a separate experiment, it showed that the incorporation rate of ~3H-TdR into nuclei of SP2/0 after incubation with 5srRNA was decreased as compared with that f control group, hence the result indicates that 5srRNA inhibits DNA synthesis of the SP2/0 cells and it seems to play a role in the regulation of gene expression through its hybridization with DNA sequences of the SP2/0 cells. Thus it is likely that 5srRNA might act as "erythroid denucleation factor".

The present study reported that 5srRNA from reticulocytes of rabbit could pass into the nucleus of mouse myeloma cells SP2/0,and in the mean time DNA synthesis and cells division were markedly suppressed. In a separate experiment, the rRNA was extracted from embryonic liver and erythrocytes of rabbit or rat and analysed by agarose electrophoresis method. The result indicated that the amount of 5srRNA in various period of the development of erythrocyte is changed along with denucleation process. Thus it is likely that 5srRNA of mammalian erythrocyte plays a role in reversing malignant phenotype of tumor cells and promoting denucleation of mammalian erythrocyte through inhibiting DNA synthesis.

BACKGROUND: Most patients with posterior pelvic and sacroiliac metastasis are terminally ill. Their treatment is usually palliative and directed toward relieving pain and improving the quality of life with the least possible intervention.OBJECTIVE: To investigate the possibility of percutaneous CT-guided fixation of sacroiliac joint with cannulated lag screw treatment for pathologic pain of sacroiliac joint.DESIGN: Preoperative and postoperative auto-control clinical trial was conducted.SETTING and PARTICIPANTS: The study was completed in Ningbo Sixth Hospital. Eight patients(4 males and 4 females, aged 12 years to 83 years)with metastatic tumor of sacroiliac joint were selected for our study.METHODS: After treatment with percutaneous CT-guided fixation of sacroiliac joint with cannulated lag screw, the sacroiliac joint of the 8 patients became pathologically unstable. Enneking pain scale was obtained preoperatively and postoperatively.MAIN OUTCOME MEASURES: Pain recovery was assessed preoperatively and postoperatively.RESULTS: After 2. 5 years' follow-up, pain of the patients was relieved with a range of 0 to 5(mean 3.2).CONCLUSION: Percutaneous cannulated lag screws with CT guidance help alleviate the patients' pain due to pathologically unstable sacroiliac joint.

Objective To compare the perioperative safety and curative effects oflaparoscopic adrenal sparing surgery (ASS) with laparoscopic total adrenalectomy (TA) for aldosterone producing adenoma (APA).Methods An online systematical retrieval was performed with Pubmed,ScienceDirect,Springerlink,the Cochrane library,CNKI and China Biology Medicine disc for clinical comparative studies published before May 2016,these studies reported the treatment of ASS/partial adrenalectomy (PA) versus TA for APA.The selected studies were applied to Revman 5.3 software for meta-analysis.The main contents were perioperative outcomes (operative time,intra-operative blood loss,and length of hospital stay) and postoperative efficacy (cure rate,partial response rate,inefficiency rate).Results A total of 9 clinical studies (3 English documents and 6 Chinese documents) with 1036 patients were included into the final analysis,among which 544 patients were assigned to ASS group and 492 in TA group.The analyzed results demonstrated no statistical significance between ASS group and TA group on operative time (WMD:-2.09min,95％CI:-9.86-5.67,P=0.60),length of hospital stay (WMD:-0.10d,95％CI:-0.32-0.12,P=0.36),intra-operative blood loss (WMD:1.13ml,95％CI:-8.86-11.12,P=0.82),cure rate (OR=l.07,95％CI:0.73-1.58,P=0.72),partial response rate (OR=0.85,95％CI:0.57-1.27,P=0.43) and inefficiency rate (OR=2.15,95％CI:0.32-14.34,P=0.43).Conclusion For surgical treatment of APA,ASS is technically safe,can achieve reliable postoperative efficacy and a similar therapeutic effect compared with TA,so deserves further application in clinical practice.

Objective To describe the treatment experience of post-operative complications after total arch replacement for acute type A aortic dissection in 34 cases.Methods The subjects were 34 consecutive patients (Twenty-eight males and 6 females,age 34.0 -60.0 yrs) who received total arch replacement for acute Stanford type A aortic dissection from Jan.2005 to Oct.2010 in our hospital.The duration from the onset of the symptoms to the hospitalization ranged from 4 - 18 hrs.Pre-operative 2-D Echo revealed aortic valve regurgitation in 8 patients and mitral valve regurgitation in 1 patient.Results Three patients died after operation ( mortality 8.8％ ).Severe complications included acute kidney injury in 13 cases,respiratory dysfunction in 12 cases,paraplegia in 1 case,mental disorder in 10 cases and excessive post-operative bleeding in 2 cases.Conclusion The incidence of the complications after total arch replacement is still high and severe.Intensive care should be stressed peri-operatively and early diagnosis and treatment for post-operative complications are important procedures.

Objective To describe the follow-up data of 34 patients receiving total arch replacement with an open stent graft for acute type Ⅰ aortic dissection.Methods The subjects were 34 consecutive patients with type Ⅰ acute aortic dissection ( Twenty-eight males and 6 females,aged 34.0 - 46.0 yrs) who received total arch replacement with an open stent graft in our hospital from Jan.2005 to Oct.2010.Thirty of the 34 patients were followed up for 2 - 70 months.CT scanning was performed at the 3 and 12 months and then yearly after operation to detect the thrombus formation,absorption of thrombus,and obliteration of the false lumen after its exclusion by the stent graft.Results Three patients died peri-operatively with the mortality of 8.8％.One patient died during the follow-up period.Obliteration was recognized in all the patients at the distal side of the stent graft during the follow-up period.The false lumen remained in 10 patients at the distal part of descending aorta,but the diameter of the false lumen was not enlarged.Conclusion In patients with acute type Ⅰ aortic dissection,it is relative safe to perform extensive primary repair of the thoracic aorta by stent grafting.This method may enhance the obliteration of the false lumen and reduce the possibility for further operations to manage a residual false lumen.

Objective To compare the clinical efficacy and radiologic changes between constrained and non-constrained titanium plate in anterior cervical corpectomy and fusion (ACCF) in elderly cervical spondylosis patients.Methods A total of 58 elderly cervical spondylosis patients who underwent ACCF were divided into group 1 (patients treated with constrained titanium plates,n =30) and group 2 (patients treated with non-constrained titanium plates,n=28).The Japanese Orthopedic Association (JOA) score,fusion rate,the loss of segmental height and cervical lordosis were recorded.The clinical efficacy and imaging features were compared between the two groups.Results The improvement rate of JOA score had no significant differences between group 1 and group 2 [(77.7±18.6)％ vs.(75.8±23.2)％,t=0.340,P＞0.05].At 3 months after operation,the fusion rate was higher in group 2 than in group 1 (89.3％ vs.63.3％,x2 =5.327,P＜0.05).At 3,6 and 12 months after operation,there were no significant differences in the loss of segmental cervical height and lordosis between group 1 and group 2 [(2.42±3.05)mm vs.(0.98±2.86)mm,(3.95±3.65)mm vs.(2.34±2.97)mm,(3.60±4.33)mm vs.(2.40±2.96)mm,(1.64±2.33)° vs.(0.66 ± ±2.14)°,(2.13∧±±3.79)° vs.(0.70±2.99)°,(2.39±4.26)° vs.(0.86±3.25)°,respectively,all P ＞0.05].Conclusions The clinical efficacy is similar in ACCF with the two types of titanium plates.The non-constrained titanium plate can increase the fusion rate in early time,but may aggravate the loss of segmental cervical height and lordosis,which should be used with caution in elderly osteoporosis patients.
