1.Three years follow-up observation and analysis of caries status of primary and permanent teeth among 652 6-year- old children in Sichuan Province.

OBJECTIVEThis longitudinal study aimed to investigate the caries status of primary and permanent teeth among 6-year-old children in Sichuan Province from 2010 to 2012.
METHODSA sample that comprised 652 6-year-old children from six different elementary schools (three represented the urban areas, and the other three represented the rural areas) were examined according to a baseline, with follow-up examinations at 1, 2, and 3 years. Eruption and caries experience were re- corded using World Health Organization criteria.
RESULTSThe prevalence rates of primary tooth caries of 6-year-old children in Sichuan Province for 3 years were 74.23% (484/652), 75.61% (493/652), and 81.90% (534/652). The filling rate of the primary teeth was 5.87% (145/2,471) in 2012, with significant differences (P < 0.01) between the urban areas [10.84% (133/ 1,227)] and rural areas [0.96% (12/1,244)]. The total pit and fissure rate of the first molar was 14.11% (92/652) in 2012, with significant differences between the two areas (P < 0.01) [rural: 0.66% (2/303); urban: 25.79% (90/349)].
CONCLUSIONThe pre- valence of caries in the primary and permanent teeth of 6-year-old children was high. An increasing prevalence tendency was observed as the age increased. The prevalence of first molar caries indicated that prevention and control of dental caries should be performed as early as possible.

2.Interactional effects of corticosterone and stress in the formation of PTSD-like memory impairment rats

Objective To investigate the interactional roles corticosterone and stress played during the PTSD-Like memory impairment.Methods First we established the model of PTSD-like memory using three levels of electric shock:0 mA,0.8 mA and 1.4 mA.The freezing time percent of each group were calculated 24 hours later in order to find which group of rats obtained the PTSD-like memory,and generalizing tests was used to verify it.After the model was established,rats received adrenalectomy (ADX) were operated bilaterally in order to investigate the effect of corticosterone and fed with corticosterone in the drinking water 7 days before fear conditioning.These rats were divided into four groups:control group (no corticosterone supplement,0 mA shock),Cort group (corticosterone supplement,0 mA shock),shock group (no corticosterone supplement,1.4 mA shock),Cort-shock group (corticosterone supplement,1.4 mA shock).A two-way factorial analysis of variance was used to determine whether there was a significant interaction between corticosterone and shock.Results The freezing time percent of group 0.8 mA raised compared with the group 0 mA,whereas the freezing time percent of group 1.4 mA showed reversely and the generalizing effects appeared,compared with group 0.8 mA.In two-way factorial analysis design,the freezing time percent of Cort-shock group significantly increased (P<0.05) compared with other groups,and there was no significant variance among the other groups.Corticosterone (F=6.464,P<0.05) and stress (F=53.419,P <0.01) played parts in the formation of PTSD-like memory impairment,and they interacted with each other (F=11.580,P<0.05).Conclusion PTSD-like memory impairment can be induced on rats with high (1.4 mA) electrical shock,and they have interactional effect with each other in the process.
3.Short-term follow up of bipolar arthroplasties for aged cases with intertrochanteric fractures

Objective To evaluate the short term effect of bipolar arthroplasties in aged cases with intertrochanteric fractures.Methods There were 23 males and 15 females(at age of 70-93 years, average 76 years)with unstable intertrochanteric fractures treated with the third generation cementing techniques and bipolar arthroplasties that were followed up for average 2.4 years.The clinical effect was evaluate with Harris scale,X-ray films and complications.Results Of all,34 cases(89.5%)could walk freely.The average Harris scale was 84.2 points.The average period until walk with full-weight load was 5.6 weeks.The greater trochanters was united in 35 cases(92%)during average 4.2 months.No peri-prothetic ossteolysis and loosening or subsidence occurred.Conclusions Cemented bipolar arthro- plasty has good advantages of reduced laying up period and few complications.The short-term outcome is satisfactory hut the long-term outcome needs deeper observation.
4.Transcatheter arterial chemoembolization and delayed surgery for infant hepatoblastoma

Objective To evaluate the effect of transcatheter arterial chemoembolization (TACE) and delayed surgery for infant hepatoblastoma.Methods TACE was performed with the initial digital subtractive angiography (DSA) under general anesthesia 1-3 times in 8 infants with huge hepatoblastoma, whose age was 2 to 12 months. DSA was done via arterials in hepatoblastoma each time before chemoembolization. The arterials were perfused with chemodrugs and suspensions in ultrasome iodized oil , and were blocked with spring rings. DSA findings indicated that the tumor shrank without new tumorous arterials after 1 month in 6 cases, and 4 of them showed no tumorous staining, and the delayed surgery was performed successfully 1 week later in 6 infants. One boy underwent systemic chemotherapy alone during 6 months after 3 times of TACE. Results TACE therapy did not encounter any major technical problem or toxic reaction caused by chemotherapy. The following DSA test 4 weeks later did not detect any new tumorous vessels in 6 cases. Six children received TACE and surgery had been followed-up with no tumor recurrence for months averagely. The boy underwent TACE and venous chemotherapy for 6 months , without surgery , had been followed-up for 48 months until the present report. CT, AFP and DSA did not show any hints of tumor recurrence. Six cases receiving 3 times TACE combined with surgery survived without tumor recurrence. Conclusions TACE is a very effective, safe and helpful therapy for hepatoblastoma, which stressed the repeated use of spring ring to block tumor vessels lastingly if necessary. If surgery is required, DSA test is needed beforehand to detect new tumorous vessels or neoplasm. If there is any , TACE is repeated. TACE combined with surgery may provide an additional promising choice in the treatment of hepatoblastoma, and repeated TACE alone may cure hepatoblastoma in infants.
5.Basic Gait Characteristics of Healthy Adults

ObjectiveTo study normal gait characteristics of healthy adults in different age groups.Methods90 healthy adults were divided into three groups according to their ages. It was 20~39 year group, 40~59 year group and 60~70 year group. They received a gait analysis with the gait analysis system based on digital video and image processing which could provide temporal-spatial parameters and kinematic parameters. The gait data of the three groups were compared. Relationships between gait speed and other gait parameters were investigated.ResultsThere were statistically significant differences in some parameters among three groups. Gait speed was significantly correlated with stride time, stride length, stance time (%), cadence, maximum flexion of hip and knee in swing phase.ConclusionThe normal gait patterns of healthy adult established with the gait analysis system based on digital video and image processing can be used as the base line to compare with abnormal gait.
6.Analysis of risk factors of affecting intracranial infection complicating brain injury operation

Objective To discuss the risk factors attributing intracranial infection following brain injury operation. Methods Difference factors between intracranial infection group and non-infection group following brain injury operations were compared by retrospective analysis. Results Involved in the study were 770 cases in non-infective group and 142 cases (15.6%) in infective group, with the types of infection including nonbacterial meningitis, bacterial meningitis, cerebral ventriclitis and ventrical pus accumulation, cerebral abscess, subdural pus accumulation, subcutaneous or/and subskull flap pus accumulation or osteomyelitis, and incision infections. The rate of bacterial detection was 27.5%.Postoperative infections following brain injury surgery were related to hotter weather, advanced age of patients, severity of traumatic state, multiple operations in short time, successive or double-lateral craniotomies, long-time (>5 h) operation, microsurgery, operation on skull base or postfossa, cerebral ventricle draining, subcutaneous or epidural hydrops, and emergency operation(P<0.05). Conclusion It should be given more attention and prophylactic treatments for the brain injury with infective risk factors mentioned above.
7.Follow-up study on the serological reactivity of infants diagnosed with congenital syphilis in Shanghai

Objective To determine the status of false-positive report of congenital syphilis (CS),to analyze the possible causes of mis-diagnosis.Methods Basic information on CS in Shanghai in the past five years was collected.We identified infants diagnosed with CS and followed up the sero- logical reactivity of those patients and their mothers.The serological reactivity[rapid plasmin reagin (RPR)and treponema palidum hemagglutination assay(TPPA)]of infants was followed-up for up to 24 months,or until both antibodies turned to negative.The medical history of the mothers was col- lected,and their sera were examined for syphilitic antibodies.Results Total 99 infants diagnosed with CS were recruited. The major diagnostic method was treponemal antibody detection.Only 31.3% of the 99 infants exhibited clinical symptoms or syphilis-like symptoms at delivery.The cumu- lative RPR loss rates of the infants were 44.2%,64.0%,72.7%,83.9% and 87.1% at 1-3,3-6,6- 12,12-18 and 18-24 months after birth,respectively.The cumulative TPPA loss rates were 1.1%, 18.6%,44.6%,66.7% and 74.4% for 1-3,3-6,6-12,12-18 and 18-24 months after birth,respec- tively.TPPA remained positive in all mothers with syphilis.Conclusion The diagnosis of congenital syphilis determined solely by the positive tests of RPR and TPPA is unreliable and can be misdiagno- sis.The diagnosis and management of congenital syphilis should be urgently improved,and that the profes- sional health institutions should perform and closely monitor the quality controls in the diagnosis of CS and standardize the intervention strategy of maternal syphilis.
8.Influence of immunization dose schemes on immunoprotective response to recombinant signaling protein 14-3-3 of Schistosoma japonicum.

OBJECTIVETo discuss the optimal immunization dose by observing the immunoprotective effects of different doses of recombinant Schistosoma japonicum (Chinese strain) signaling protein 14-3-3 (rSj14-3-3).
METHODSSj14-3-3 gene was amplified by reverse transcriptase PCR (RT-PCR), subcloned into prokaryotic expression vector pET28a, then transformed into E.coli to express by inducing. Purified rSj14-3-3 was prepared through SDS polyacrylamide gel electrophoresis (SDS-PAGE), electroelution, dialysis, then BALB/c mice were divided into 5 groups and immunized in rSj14-3-3 protein followed by challenging infection (the 1st, 2nd, and 3rd groups were immunized in 50 microg, 100 microg and 300 microg antigen, respectively. The 4th, 5th groups were immunized in Freund's adjuvant and normal saline controls). After 6 weeks of challenging infection, the mice were killed and the worm and egg reduction rates were calculated. And the mice sera in different time were taken to examine the specific anti-Sj14-3-3 IgG.
RESULTSrSj14-3-3 protein was expressed successfully. After immunizing and challenging, worm reduction was found to be 28.20% in the 1st group, 43.10% in the 2nd group, 40.00% in the 3rd group, respectively. Number of eggs in liver tissue was reduced by 41.80%, 57.50%, 55.70%, respectively. Compared the results of the tested groups to the controls, the differences were of significance by t-test (worm reduction rate: t = 6.8 in the 1st group, t = 8.7 in the 2nd group, t = 7.3 in the 3rd group, P < 0.01 in all tested groups. Egg reduction rate at the group's number above: t = 11.23, t = 11.54, t = 7.99, P < 0.01 in all tested groups). As compared the results between the tested groups by chi(2), the differences were of significance between the 1st and the 2nd groups (worm reduction rate: chi(2) = 8.96, P < 0.05; egg reduction rate: chi(2) = 15.69, P < 0.05), between the 1st and the 3rd groups, the differences were also of significance (worm reduction rate: chi(2) = 6.52, P < 0.05; egg reduction rate: chi(2) = 12.52, P < 0.05). The difference was not of significance between the 2nd and the 3rd groups (worm reduction rate: chi(2) = 1.20, P > 0.05; egg reduction rate: chi(2) = 0.93, P > 0.05). In all tested groups, total anti-Sj14-3-3 specific IgG rose markedly. IgG(1) and IgG(2a) subtypes were high, but IgG(2b) and IgG(3) were near the background in four subtypes tested.
CONCLUSIONImmunoprotection of rSj14-3-3 should have some relations with immunization dose, and the protection obtained from immunizing mice by using 100 microg antigen was the best.

9.The comparison of clinical manifestation of organophosphorus pesticide poisoning (OPP) between oral exposure and occupational exposure in field work.

OBJECTIVETo investigate the differences of clinical manifestation and therapy of organophosphorus pesticide poisoning (OPP) between oral exposure and occupational exposure in field work.
METHODSFrom July 2007 to July 2010, 85 patients with acute severe OPP were treated in a hospital, which were divided into oral poisoning group (51 cases) and non-oral poisoning group (34 cases). The differences of clinical manifestations, curative effects and prognosis between two groups were compared.
RESULTSThe rates of myoclonus and ataxia in cases with moderate poisoning of oral poisoning group were 86.4% and 90.9%, which were significantly higher than those (50.0% and 55.0%) of non-oral poisoning group (P<0.05 or P< 0.01). The rates of myoclonus, lung fluid and coma in cases with severe poisoning of oral poisoning group were 100.0%, 89.7% and 93.1%, respectively, which were significantly higher than those (71.4%, 64.3% and 50.0%) of non-oral poisoning group (P<0.05). The mean detoxification hours in cases with moderate poisoning and cases with severe poisoning of non-oral poisoning group were (35.0 +/- 6.2) and (45.0 +/- 11.1) hours which were significantly lower than those [(49.0 +/- 7.7) and (77.0 +/- 10.3) hours] in cases with moderate poisoning and cases with severe poisoning of oral poisoning group (P<0.05). In 24, 48 and 72 h after treatment, the cholinesterase (ChE) activities of non-oral poisoning group were higher than those of oral poisoning group (P< 0.05 or P<0.01). The used doses of pyraloxime methylchloride (PAM-Cl) or atropine and the used total dose of atropine in non-oral poisoning group were lower than those in oral poisoning group (P<0.05 or P<0.01).
CONCLUSIONSThe clinical manifestation of non-oral poisoning group is different from the clinical manifestation of oral poisoning group due to the high morbidity of OPP occurred at field site in summer. The used doses of atropine and PAM-Cl are less and the ChE activity recovers quickly for non-oral poisoning group.

10.A study on clinical differential diagnosis between hemangioma and vascular malformation in infant.

OBJECTIVETo explore the main points of clinical differentiation between hemangioma and vascular malformation in infant.
METHODSBased on Mulliken and Waner's classification, from March, 1997 to February, 1999, 81 baby patients with hemangioma were included in this study. Thirty-eight cases, 43 cases received medical treatment of steroids.
RESULTSAll the patients were followed up from 5 to 7 years. Thirty-eight cases of red strawberry-like lesions limited in the skin began to involute within two years old. Of the 30 patients with strawberry-like lesions and subcutaneous mass, 20 cases involuted in varying degree; 10 cases' subcutaneous mass grew gradually and didn't involute, in 4 cases biopsy was performed, 3 cases were confirmed as hemangioma accompanied with venous malformation by pathology, 1 case was hemangioma accompanied with arteriovenous malformation. Of 13 cases with light blue or normal skin and subcutaneous mass, 7 cases involuted in varying degree; 6 cases grow gradually and didn't disappear, 2 cases were confirmed as venous malformation by biopsy.
CONCLUSIONSHemangioma in infant begins to involute within two years old. Vascular malformation or hemangioma with deep vascular malformation grows persistently and does not disappear. Skin temperature of lesion surface and dilative veins on the skin artery pulsation, are indexes compressibility, for differentiation between hemangioma and vascular malformation in clinical diagnosis.
