The clinical data of 109 patients with rhegmatogenous retinal detachment were retrospectively analyzed and the relationships among retinal detachment，refraction and the shapes of hole in different age groups discussed. The results showed that myopia was more commonly seen in the patients less than 40 years old and the occurrence of myopia was gradually decreased in the patients over 40 years old. Round hole was more commonly seen in the patients less than 40 years old and horseshoe-shaped tears were more often found in the age group of over 40 years old.

AIM: To observe the efficacy and safety of small incision manual nuncleofragmentation technology in hard-nucleus cataract.METHODS: Fifty-four patients (54 eyes) were performed with small incision manual nucleofragmentation and intraocular lens implantation.Uncorrected-visual acuity,corrected-visual acuity and astigmatism were measured before and after surgery.Anterior segment of eye,retina,surgical complications were observed.RESULTS: Preoperative uncorrected vision(LogMAR) was 1.095±0.171,corrected vision (LogMAR) was 0.994±0.158.Postoperative uncorrected vision(LogMAR) was 0.321±0.214 and corrected vision(LogMAR) was 0.276±0.179 at 1wk after operation.Both had statistical significance compared with uncorrected vision(LogMAR) (t=27.052,P<0.01) and corrected vision (LogMAR)(t=29.211,P<0.01) after cataract operation.Average preoprative corneal astigmatism was 0.93±0.34D,average postoprative astigmatism was 0.95±0.29D at 3mo,there was no statistical significance of astigmatism before and after surgery(t=1.048,P>0.05).No severe complications occurred in all cases.CONCLUSION: The technology of small incision manual nuncleofragmentation in hard-nucleus cataract is proved to be safe and effective.

To evaluate the clinical effect of small incision cataract extraction with a manual nucleus division technique combined with intraocular lens implantation in cataract operation of large quantities.
●METHODS: Totally 105 patients (105 eyes) underwent small incision cataract extraction surgery. Visual acuity was observed, eye condition, fundus and intraocular pressure were examined by slit lamp at postoperative 1wk; 1mo.
●RESULTS: The visual acuity improved significantly at postoperative 1mo, 89 cases (89 eyes) with better than 0. 3( 85%), 101 cases ( 101 eyes) with 0. 1 and above (96%). lt had short operation time, fewer complications and good anesthetic effect.
● CONCLUSlON: This operation is suitable for primary hospitals, which visual acuity improved obviously, without serious complications, and economic and feasible.

Objective To evaluate the effect of polysaccharide nucleic acid fraction of bacillus Calmette-Guerin (BCG-PSN) on peripheral blood CD4+CD25+Foxp3+ regulatory T (Treg) cells in patients with condyloma acuminatum (CA).Methods Forty-two patients with first onset of CA were randomly assigned to receive either injection of BCG-PSN (0.35 mg every other day for 3 months) after fulguration (combination group,26 patients),or fulguration only (fulguration group,16 patients).Venous blood samples were obtained from all the patients at the initial visit and three months after the beginning of treatment,as well as from 30 healthy checkup examinees.The percentage of peripheral Treg cells in CD4+ T lymphocytes was determined by flow cytometry.The recurrence of CA was evaluated during the three months after the beginning of treatment.Results The percentage of peripheral Treg cells in CD4+ T lymphocytes was significantly higher in patients with CA than in the controls (8.31％ ± 1.24％ vs.5.15％ ± 0.72％,P ＜ 0.01),and in patients with clinical recurrence of CA than in those without (9.34％ ± 0.72％ vs.7.45％ ± 0.85％,P ＜ 0.01).The recurrence rate was significantly lower in the combination group than in the fulguration group (30.77％ vs.68.75％,P ＜ 0.05).After three months of treatment,the combination group showed lower percentage of Treg cells in CD4+ T cells compared with the fulguration group (5.87％ ± 1.05％ vs.6.60％ ± 0.75％,P ＜ 0.05).Conclusions The percentage of Treg cells has a close relationship with the progression of CA,i.e.,the higher the percentage,the more frequent the relapse.BCG-PSN may enhance the antiviral immune response in patients with CA and improve their prognosis by reducing the number of Treg cells.

Objective To study the relationship between EZH2 and Ki-67 expression and both in ameloblastoma and normal oral mucosa.Methods EZH2 and Ki-67 were detected by immunohistochemical SP method in 50 cases of ameloblastoma (30 cases primary,20 cases recurrence) and expression in 20 cases of normal oral mucosa.Results The positive expression of EZH2 protein in recurrent ameloblastoma was (36.25±7.24) ％,significantly higher than that of primary ameloblastomas (25.26±4.28) ％ (P ＜ 0.001).Ki-67 in recurrent positive expression in ameloblastoma was (34.96±5.28) ％,significantly higher than that of primary ameloblastomas (29.68±3.27) ％ (P ＜ 0.05).There was positively corTelated on EZH2 and Ki-67 (P ＜ 0.05),but not on the size of tumor and gender (P ＞ 0.05).Conclusions There are high expression of EZH2 and Ki-67 in ameloblastoma,and their play key role in the occurrence and development of ameloblastoma.They can be used as a reference index for the prognosis.The combined detection of EZH2 and Ki-67 protein expression with clinical evidence and its prognosis may contribute to the process of ameloblastoma and recurrence.

Objective To explore the change and the spectra of opportunistic infections in AIDS patients after antiretroviral treatment in He′nan province.Methods Inpatients of post treatment with disease in accordance with the criteria were selected from the Sixth People′s Hospital of Zhengzhou.A retrospective study was carried out, including the content such as gender,age,infection route,treatment time,the date of complication,and CD4+T lympho-cyte counts when the patient with complications.Disease were divided into four group:opportunistic infections,adverse drug reactions,tumor,cardio-cerebrovascular disease and diabetes mellitus.Statistic case number and incidence of every kinds of the disease in the different stages of HAART.Results (1)1 001 cases of patients were involved in the result analysis.People of 41-60 years old,the peasants and peasant workers,accounted for 57.4%and 78.9%.The infection route was mainly through blood transmission and blood collection, accounting for 83.45%.Most of the patients come from Zhumadian, Xuchang, Zhoukou, Nanyang, which were high -incidence area in He′nan.( 2 ) In 1 001 patients of post treatment, the rate of opportunistic infections was 58.4%, while adverse drug reactions was 30.5%.Bacterial pneumonia,extrapulmonary tuberculosis,oral cavity fungal infection and tuberculosis were common in opportunistic infections of patients of post treatment.The main adverse reactions were allergic dermatitis,liver injury and myelosuppression.The liver cancer case was the highest ( 8/29 ) .( 3 ) 1 001 cases were divided into 6 groups according to different count of treatment time,during 3 months of treatment,the incidence rate of opportunistic infec-tions and adverse drug reactions was the highest,accounted for 75.2% and 41.6%.After 5 years of treatment,the incidence rate of cardio-cerebrovascular disease and diabetes mellitus was 6.39%.With the prolonging of HAART time,incidence of opportunistic infections and adverse drug reactions remarkably reduced,but the occurence rate car-dio-cerebrovascular disease and diabetes mellitus increased.Conclusion In AIDS patients after HAART in He′nan at present,opportunistic infections and adverse drug reactions are main diseases and used to occur during three months after HAART.Attention will be focused on tumor, cardio -cerebrovascular disease and diabetes mellitus in AIDS patients of long-term survival.

Objective To observe the effects of low dose irradiation (LDI) on autoiogous CIK cell proliferation, phenotype and killing activity in tumor patients, and to provide the evidence for clinical application of adoptive immunotherapy with CIK cells. Methods Peripheral blood mononuclear cells (PBMC) were separated from 10 patients with malignant tumor, and CIK cells were cultured with different cytokines. (1) After 10 d culture, C1K cells were irradiated with different doses as 30, 50, 80, 100 and 200 Gy of X-rays was also detected. The CIK cell proliferation and killing activity were measured with 3H-TdR incorporation assay and 3H-TdR release assay, respectively and the percentage variation of CD3 +CD56 + were measured with flow cytometry after 24 h. ( 1 ) Autologous CIK cells were irradiated with 80 mGy X-rays. At different culture time ( 12, 24, 48, 72 h) after irradiation, the killing activity was measured with 3H-TdR release assay. (3) The effect of 3d low dose irradiation of 80 mGy X-rays on thekilling activity of CIK cells was also detected. Results After the CIK cells were irradiated with different doses as 50, 80, 100, 200 mGy of X-rays, the CPM values were 20 048.6 ± 2332. 2 ( t = 2.2, P ＜0.05), 21 832.2 ±2975.9 (t=3.5, P＜0.01), 21 000.3 ±2451.1 (t=3.3, P＜0.01), 19908.1 ±2051.0 ( t = 2.2, P ＜ 0.05 ), respectively and the proliferation of CIK cells were significantly higher than that of control group. The CD3 + CD56 + cell percentage of 50, 80, 100 mGy groups were ( 30.3 ±3.8)% (t=2.3, P＜0.05), (32.3±3.4)% (t=4.2, P＜0.01), (29.742.9)% (t = 2.4, P＜0.05 ), respectively. The killing activity of CIK cells of 80, 100 mGy groups were 55.2 ± 5.0 ( t = 3.3, P ＜ 0.01 ), 52.8 ± 4.1 ( t = 2.3, P ＜ 0.05 ), respectively. The killing activity of CIK cells up-regulated significantly at 24 h, dropped to normal levels at 48 h and 72 h. After 80 mGy X-ray irradiation for 3 consecutive days, the killing activity of CIK cells at different time points were 55.2 ± 5.3 (t = 2.6, P ＜0.05),61.9 ± 4.4 (t = 4.7, P ＜0.01), 67.2 ±5.7 (t = 5.7, P ＜0.01) for 24, 48, 72 h,respectively. Conclusions LDI might have the hormesis effect on CIK cells.

Hypertension is an important risk factor for cardiovascular and cerebrovascular diseases,which can damage structure and function of vital organs such as the heart,brain and kidney.Pseudo hypertension (PHT) refers to the blood pressure measured by ordinary cuffmanometry is higher than that of the arterial puncture,which is an essential factor of refractory hypertension.Recent findings have suggested that the elderly patients with concomitant history of atherosclerotic disease,renal insufficiency,and diabetes mellitus have the highest risk of pseudohypertension.The incidence rate ofpseudohypertension is about 1.7％-50％ in domestic and international studies.In the clinical treatment,the misdiagnosis of hypertension of patients with excessive blood pressure will lead to severe perfusion defects.In this review,we will focus on the diagnosis and pathogenesis of pseudohypertension.

Objective To compare the difference of the results of the total flavones acquired by three kinds of quantitative an-alytical methods in the People's Republic of China Pharmacopoeia(method A),Health Food Function Components and Health Indica-tors Inspection Specifications(method B),and Health Food Inspection and Evaluation of Technical Specifications(method C)respec-tively. Methods Twenty-one drug and food homologies,medicines and other samples were detected by using the above mentioned methods respectively. Comparisons of the results were established. Results The measured values by method A were the highest,that of method C were the lowest. Moreover,the determination values of method A were higher than those by the methed B(1.4 ± 0.12) times,and the method C(2.1±0.31)times. Conclusion There are significant differences among the results of the total flavonoids de-terminated by the 3 methods in the same sample and the numerical relationship will be helpful for the quantity control of homologous food and drug.

Objective To evaluate the relationship between Toll-like receptor 4 (TLR4)/nuclear factor kappa B (NF-κB) signaling pathway and propofol-induced inhibition of endotoxin-induced release of tumor necrosis factor-alpha (TNF-α) from alveolar macrophages (AMs) of rats.Methods AMs extracted from adult male Sprague-Dawley rats were cultured and inoculated in 6-well plates (1 × 106 cells/well)and in 96-well plates (1×104 cells/well).The cells were divided into 5 groups (n=18 each) using a random number table:control group (group C),dimethyl sulfoxide group (group D),lipopolysaccharide (LPS) group (group L),propofol group (group P) and LPS plus propofol group (group L+P).The cells were continuously cultured with phosphate buffer solution in group C.Dimethyl sulfoxide was added at the final concentration of 5 mg/ml in group D.LPS was added at the final concentration of 1 μg/ml in group L.Propofol was added at the final concentration of 25 μmol/L (4.46 μg/ml) in group P.LPS and propofol were added at the final concentration of 1 μg/ml and 25 μmol/L (4.46 μg/ml),respectively,in group L+P.At 24 h of culture or incubation,the cell viability was detected by CCK-8 assay,the morphological changes of cells were observed using Wright's staining,the concentration of TNF-α in the supernatant was determined by enzyme-linked immunosorbent assay,and TLR4 expression and NF-κB activities were measured by Western blot.Results Compared with group C,the cell viability and concentration of TNF-α in the supernatant were significantly increased,the expression of TLR4 was up-regulated,and the activity of NF-κB was enhanced in L and L+P groups (P＜0.05),and no significant change was found in the parameters mentioned above in D and P groups (P＞0.05).Compared with group L,the cell viability and concentration of TNF-α in the supernatant were significantly decreased,the expression of TLR4 was down-regulated,and the activity of NF-κB was weakened (P＜0.05),the morphological changes of cells were significantly attenuated,and the number of pseudopodia was reduced in group L+P.Conclusion The mechanism by which propofol inhibits endotoxin-induced release of TNF-α from AMs is related to inhibited activation of TLR4/NF-λB signaling pathway in rats.
