1.The changes of bone mineral density and biochemical markers in male elderly patients with osteoporosis

Objective To investgate the changes of bone mineral density (BMD) and biochemical markers of bone metabolism in elderly male patients with osteoporosis (OP) . Methods BMD and bone mineral content (BMC) of the lumbar spines (L 2~4 ) and the biochemical markers of bone metabolism in serum and urine were measured. Results BMD and BMC in OP group was reduced by 21.6% and 25% respectively vs that in control group. Bone formation indicators of serum levels of ALP and BGP in OP group were increased by 25.4% and 222% respectively vs that in control. At the same time, bone resorption indicators of urine Hop/Cr and NTX/Cr ratios were significantly increased by 22.6% and 223% in OP group than that in control. Serum T level of OP group was lower than that in control group and serum 25 OH D3 was below the normal value in both groups. Conclusions BMD and BMC of the lumbar spines (L 2 4 ) are the main basis for diagnosing the osteoporosis in men. Some elder male patients with osteoporosis are due to high turnover of bone metabolism. Androgen plays an important part in maintaining the bone mass. Vitamin D deficiency in elder men is the main reason occurring osteoporosis.
2.Effect of dexamethasone on vascular endothelial growth factor in cerebrospinal fluid of rabbits with bacterial meningitis

Objective To investigate the effects of dexamethasone (DEX) on vascular endothelial growth factor( VEGF) in cerebrospinal fluid of rabbits with bacterial meningitis. Methods A total of 36 rabbits were assigned to study,which were randomly divided into meningitis model group (MOD) .dexametha-sone-treated group (DEXT) and control group( CON). CSF was sampled for determining at 6 h, 12 h and 24 h after injection of E. coli suspension. The concentration of VEGF in every CSF sample was determined quantitatively by ELISA. Results There were higher concentrations of CSF VEGF at6h,12h and 24hin M0D(( 1219 ±176) ng/L,( 1343 ±160) ng/L,(981 ±134) ng/L) than that in CON( (374 ±172) ng/L, (370 ± 169) ng/L,(367 ± 171) ng/L) (P<0.01). There was higher brain water content in M0D( (80.8 ± 0.5) % ) than that in CON( (80.0 ± 0.5) % ) (P < 0.01). There was positive correlation between the brain water content and the concentration of CSF VEGF at 24 h( r - 0.919,P < 0.01). Compared with MOD, the concentrations of CSF VEGF in DEXT at 6 h, 12 h,24 h ((941 ±147) ng/L, (1083 ± 123) ng/L, (825 ± 66) ng/L) were decreased significantly(P <0.05), the brain water content was less ((80.4 ±0.5) %) (P < 0.05). Conclusion The secretion of VEGF markedly increases in the pathological process of bacterial meningitis. VEGF contributes to the damage of blood brain barrier and the formation of brain edema. DEX can decrease the degree of brain edema by suppressing the generation of VEGF and lightening the damage of blood brain barrier.

4.Evaluation of diagnosis and curative criterion forconcussion patients by color three- dimensionaltrascrani doppler

Objective In order to offer a noninvasive and objective examination method for assessing the clinical diagnosis and eurative criterion of concussion patients, the patients color three - dimensional transcranial doppler( 3D - TCD) characteristic at deferent phases, parameter and clincal manifestation were studied Method Parameter of Median Cerebral Artery(MCA), Base Artery(BA), the average peak forwand velocity of cerebral blood flow, train figure were tested by 3D - TCD within 24 hours, 3 - 6 days and at the end of treatment, respectively. The results were compared with clinical diagnoses and curative criterion. Result Among 135 patients who had clinically diagnosed concussion, agreeable diagnoses 101 cases, basically agreeable diagnooses 12 cases, disagreeable diagnosis 22 cases(16. 2%) .At the end of treatment, according to clinical determination 96 cases(71.1%) were cured, 39 cases(28.9%) were improved. Wave train character, figure and diagnoses prameter of cerebral spasm caused by concussion were suggested Conclusion To be one of the diagnostic bases and curative criterion, 3D- TC D technology is performable. Meanwhile, it is a new testing objective technique for assessing curative and determining prognosis.
5.Absorption of flavonoids from Abelmoschus manihot extract by in situ intestinal perfusion.

To explore the mechanism of the absorption of flavonoids from Abelmoschus manihot flowers, in situ intestinal recirculation was performed to study the effect of the absorption at different concentrations and different intestinal regions. To evaluate the conditions of the absorption of six flavonoids from Abelmoschus manihot flowers, the concentrations of Abelmoschus manihot in the perfusion solution were determined by HPLC at predesigned time. And we have investigated the inhibitory effect of six flavonoids from Abelmoschus manihot flowers on P-glycoprotein (P-gp) drug efflux pump. The results demonstrated that the absorption rates of flavonoids from Abelmoschus manihot flowers are not significantly different (P > 0.05) at various drug concentrations, the absorption of flavonoids from Abelmoschus manihot flowers is a first-order process with the passive diffusion mechanism. The absorption rates of each of flavonoids are significantly different. The absorption rate of flavonoid glycoside was lower than that of aglycone; the flavonoids from Abelmoschus manihot flowers could be absorbed in all of the intestinal segments. The best parts of intestine to absorb hyperoside and myricetin are jejunum and duodenum, separately. Verapamil could enhance the absorption of isoquercitrin, hyperoside, myricetin and quercetin-3'-O-glucoside by inhibiting P-glycoprotein (P-gp) drug efflux pump.
6.Glucolipid Metabolic Disease and Precision Medicine

Diseases of glucose and lipid metabolism disorder,presented rather complicated pathological mechanism,often with clinical pattern of multiple concurrent diseases.Therefore,the traditional single-disease based on treatment methods need improving.In view of plenty of clinical practice,theatrical and fundamental researches,the pathological mechanisms of some chronic disorders,such as hyperlipidemia,nonalcoholic fatty liver disease,type 2 diabetes,hypertension,atherosclerosis and severe cardiovascular complications,resulted from the impairment in the metabolism of glucose and lipid were investigated using the method of integrated Chinese and western medicine.Overall,the features of these diseases and their common characteristics were discovered,and accordingly we defined the new concept of glucolipid metabolic disease (GLMD) and put forward the concept of pivot liver of metabolic regulation system.In addition,we developed the therapeutic strategy of modulating liver,starting pivot and cleaning turbidity,for the comprehensive and integrated treatment and prevention of these diseases.The theory of GLMD shared the critical characteristics with precision medicine,taking its own specialty.Finally,the content and approaches for the research of GLMD were proposed,and some essential and core fields in the precision medical research for GLMD were profoundly analyzed and prospected.
7.Clinical and endoscopic features in Henoch-Schonlein purpura

Objective To investigate the clinical and endoscopical features of patients suffered from Henoch-Schonlein purpura (HSP). Methods Retrospective analysis of the clinical manifestation in 197 purpura patients, of which 81 cases were HSP, and described the endoscopic features of 19 cases who underwent gastroscopy and/or colonoscopy. The specimens from 15 cases were evaluated histopathologically. Results Eighty-one patients were diagnosed as HSP, 51 (63. 0% ) of them had a history of upper respiratory tract infection 1 -3 weeks before, taking antibiotics or animal protein prior to the onset of the illness. In 21 patients, abdominal symptoms presented firstly, which occurred 1 -40 days prior to the appearance of skin rashes. Seventy- four (91. 4% ) patients experienced abdominal pain, 37(45. 7% ) patients had digestive tract bleeding. Endoscopic features included diffused congestive edema of gastrointestinal mucosa, widespread hemorrhagic spots, erythema, erosion and ulceration. Lesions were relatively severer in small intestine than those in large intestine. Histological manifestations showed massive neutrophilic infiltration in mucosa and submucosa, fibrotic necrosis of small vessels, focal bleeding, erosion and ulceration. There was prominent accordance in the extents of endoscopic and pathologic manifestation with the severity of gastrointestinal symptoms. Conclusions Forty one percent of purpura patients presented as HSP, of them 26% had the abdominal symptoms firstly. Small bowel lesions were severer than those of stomach or colon. The typical features of the illness and endoscopic findings are very helpful to the early diagnosis of HSP.
8.STUDIES ON THE STRUCTURAL RELATIONSHIP BETWEEN ACTIN FILAMENTS AND SERTOLI CELL BARRIER

The present study was designed to investigate the effects of cytochalasin E(CE) on the integrity of Sertoli cell barrier. The results indicate that: (1) In the CE-treated testis(1000-2000 ?mol/L CE/testis, 6-14 hr), actin filaments of the ectoplasmic specialization (ES) in area of Sertoli cell barrier were disrupted, the accumulation of amorphous material and fragmented small vesicles of SER were observed in cytoplasm of Sertoli cell. The above changes appeared to be dosage and duration dependent; (2)In the seminiferous tubules of animals receiving CE (1000-2000 ?mol/L testis, 6-14 hr) plus fixation with 10% hypertonic dextrose solutions, usually the germ cells shrinkage and exaggeration of intercellular spaces withint he basal as well asthe adluminal compartments were observed. The tight junction between Sertoli cells were also appeared to be separated by hypertonic action of dextros; (3) The results of tracing experiments showed that lanthanums as tracer could be seen to pass through the Sertoli-Sertoli cell junction of the barrier and enter into the adluminal compartment, the tracer that surrounding the spermatocytes and round spermatids were discernible readily. The above results suggest that cytochalsin E disturbed actin filaments of Sertoli ectoplasmic specialization thus altered the functional integrity of the Sertoli cell barrier. The relationship between the actin filaments and Sertoli cell barrier was discussed.

Objective To investigate the effect of comprehensive intervention therapy of losing weight on leptin and blood lipid,blood glucose,insulin in adolescent girl students with simple obesity.Methods Simple obesity adolescent girl students were accepted the losing weight therapy composed of the aerobic exercise,reasonable diet,behavior modification and medical supervision for 10 months.Then the changes of blood leptin and correlated hormone were examined before test,during test and after test,respectively.Results The leptin and correlated hormone levels were significantly higher in obesity subjects than that in normal subjects,and the levels of serum leptin was positively correlated with BMI and insulin.The blood leptin and blood lipid,blood glucose,insulin were decreased obviously after experimental losing weight.Conclusion Comprehensive intervention therapy of losing weight can significantly lose weight,leptin and insulin,so it plays an important role in modifying the metabolism disorder
