1.Sequential invasive-noninvasive mechanical ventilation weaning strategy for patients after tracheostomy

BACKGROUND: Because the continuity and integrity of the trachea are likely damaged to some extent after tracheostomy, the implementation of sequential ventilation has certain difficulties, and sequential invasive-noninvasive ventilation on patients after tracheostomy is less common in practice. The present study aimed to investigate the feasibility of invasive-noninvasive sequential weaning strategy in patients after tracheostomy. METHODS: Fifty patients including 24 patients with withdrawal of mechanical ventilation (conventional group) and 26 patients with sequential invasive-noninvasive weaning by directly plugging of tracheostomy (sequential group) were analyzed retrospectively after appearance of pulmonary infection control (PIC) window. The analysis of arterial blood gases, ventilator-associated pneumonia (VAP) incidence, the total duration of mechanical ventilation, the success rate of weaning and total cost of hospitalization were compared between the two groups. RESULTS: Arterial blood gas analysis showed that the sequential weaning group was better than the conventional weaning group 1 and 24 hours after invasive ventilation. The VAP incidence was lowered, the duration of mechanical ventilation shortened, the success rate of weaning increased, and the total cost of hospitalization decreased. CONCLUSION: Sequential invasive-noninvasive ventilator weaning is feasible in patients after tracheostomy.
2.Analysis of mitochondrial DNA gene mutations in hearing loss induced by military noise

Objective To investigate the relationship between mitochondrial DNA gene mutations and the susceptibility to military noise,and to provide a criterion for screening the individuals susceptible to military noise through molecular biology diagnostic method.Methods An investigation of hearing loss was performed among a total of 1032 servicemen including 406 tankers and 626 artillerymen who have endured military noise in their military duty.A total of 122 blood samples were collected,of which 82 samples were collected from susceptible individuals(susceptible group) and 40 from tolerance individuals(tolerance group).DNA was then extracted from the leukocytes of blood samples.The gene fragments of mitochondrial DNA,which might associate with non-syndromal hearing loss,were amplified by polymerase chain reaction(PCR).The PCR products were analyzed by sequencing and contrasting.The gene mutations which were different from the standard sequences of mitochondrial genome were then compared between the susceptible and tolerance groups.Further auditory analysis was performed on those persons who had the gene mutations of hearing loss.Results The gene sequencing revealed there were definite differences in mitochondrial DNA between the susceptible and tolerance groups,and statistically significant difference was found in mitochondrial DNA COII gene T7684C and G7853A mutations between the two groups.Mitochondrial DNA gene mutations were found in the present study,such as A827G,T961insC(heterogeneity),T1005C,T1095C and G7444A,which were reported to be related to deafness in literature.Most of these mutations were detected from the samples of susceptible group,and the others from tolerance group.Conclusion The mitochondrial DNA COII gene T7684C and G7853A mutations might exist in those persons who are susceptible to military noise.

The premembrane and envelope proteins (prM-E),which contains the mainly protective antigen related with virulence and tropism,are the primary structural protein of flavivirus.However,prM-E in ZIKV is rarely understood.We have analyzed the structure and biological effects of prM-E in ZIKV by bionformatics methods.The prM-E proteins virus-like particles of dengue virus was introduced in the present.Then,the prM-E proteins virus like particles of ZIKV was prospected.
4.Decreases of progressive motility, total motility, and acrosin activity of sperm from oligoasthenoteratospermia males at different time points after sperm activation.

OBJECTIVETo investigate the progressive motility, (PR), total motility (progressive + non-progressive motility, PR + NP), and acrosin activity of sperm from normal and infertile men at different time points after sperm activation.
METHODSBased on the 5th edition of the WHO Laboratory Manual for the Examination and Processing of Human Semen and the results of modified Papanicolaou staining, we divided the semen samples into groups A (normal, n = 28), B (oligoasthenoteratospermia, n = 30), and C (asthenoteratospermia, n = 32). At 1, 24, and 48 hours after sperm activation, we detected sperm PR and PR + NP by CASA and chemical colorimetry, and determined sperm acrosin activity using the modified Kennedy method.
RESULTSSperm PR and PR + NP were significantly decreased in all the three groups at 1-24 hours and even more significantly at 24-48 hours after sperm activation as compared with the baseline (P < 0.05). Sperm acrosin activity showed remarkable reduction in group A (P = 0. 013) , even more significant at 1-24 hours than at 24-48 hours after sperm activation, but not in groups B and C (P = 0.519 and 0.979).
CONCLUSIONSperm PR, PR + NP, and acrosin activity are all decreased with the extension of time after sperm activation, each in a specific manner. Examination of sperm acrosin activity should be applied as a routine tool in the assessment of male fertility.

7.The effect of the allocation of health resources on doctor-patient relationship in Chongqing

Objective To analysis the basic condition of the health resources in Chongqing ,and to explore the effect of the al‐location of health resources on doctor‐patient relationship .Methods Health statistics yearbook of Chongqing city was collected ,the fairness of the allocation of health resources in chongqing carried on the analysis and evaluation ,using the DEA model to analyze and evaluation the efficiency of the allocation of health resources in chongqing ,self‐designed questionnaire was used;hospitals ,doc‐tors and patients were investigated ,the descriptive and statistical analysis carried on .Results The distribution of health resources in Chongqing was inequitable on a regional .This inequitable distribution had been evaluated to the effect that ,as per the DEA mod‐el ,7 prefectures were effective with another 5 slightly effective and 26 as DEA ineffective .In the 8 hospitals that had been investiga‐ted ,the occurrence of medical disputes was undergoing a surge ,which indicates the tense relation between doctors and patients .Con‐clusion The total amount of health resources in Chongqing is fairly insufficient ,the high quality health resources are scarce .Unbal‐anced allocation of health resources and allocation inefficiency is the main reason for nervous doctor‐patient relationship and contra‐diction focus .
8.A Rapid Filtration Sampling Probe for Fermentation Broth On-line Measurement

An inner-bioreactor rapid filtration sampling probe was designed to resolve the problems of continuous sampling the fermentation broth in on-line measurement the fermentation process. The probe consists of a support, a plug valve, an O-ring, a filter membrane and a pipe cover. It can be inserted directly into the bio-reactor withstanding high-temperature sterilization before fermentation, or preventing the bacteria's invasion during sampling. Fermentation broth in tank is cross-flow filtrated by a cylindrical ceramic membrane which is used as filtration membrane, and then the sterile permeated liquid is transported to the analyzer by a peristaltic pump. The sampling experiments using this device was described for sampling glucose solution indicating it is suitable for fermentation online sampling. The sampling rate is up to 3. 0 mL/ min, glucose permeability maintains 100% , and the delay time caused by the probe is about 2 min.

Objective:To study the relationship between expression of p53,nm23 and invasion and metastasis in human colorectal carcinoma,and the correlation between expression of p53 and nm23.Methods:Expression of p53 and nm23 were studied with SABC immunohistochemical technique Results:The positive rate of p53 and nm23 in 41 samples of human colorectal carcinoma was 58 5% and 53 7% respectively p53 over expression and nm23 low expression was significantly correlated with the depth of invasion and lymphatic metastasis The expression of p53 in pooly differentiated carcinoma was obviously higher than that in well differentiated carcinoma(P0 05) p53 over expression was correlated with nm23 low expression Conclusion:Over expression of p53 and low expression of nm23 might play an important role in invasion and metastasis of human colorectal carcinoma p53 over expression was related to the differentiation of colorectal carcinoma tissue
