2.Optimization of the Extraction Technology of Paeonia lactiflora by Mixed Uniform Design

OBJECTIVE:To optimize the extraction technology of Paeonia lactiflora. METHODS:The surface figure made by mixed uniform design combined with the quadratic polynomial stepwise regression equation is adopted to optimize the extraction technology of P. lactiflora by using the content of paeoniflorin as index,with extraction solvent,the amount of extraction solvent, extraction time,extraction times as factors. RESULTS:The optimal extraction technology of P. lactiflora was as follows as 90%ethanol,12-folds extraction solvent,extracting for 150 min,extracting for 2 times. The measured value of 3 validation tests were 2.848 4%,2.795 7%,2.841 9%(RSD=0.82%,n=3),which was close to the predicted value 2.848 4%. CONCLUSIONS:The method is convenient and accurate,and can be used for the extraction of P. lactiflora.
3.The alteration of the bone metabolism in the elderly male patients with chronic obstructive pulmonary diseases

Objective To observe the alteration of bone metabolism and to study the pathogenesis of osteoporosis and factors in elderly patients with chronic obstructive pulmonary diseases (COPD). Methods The biochemical markers of bone metabolism, bone mineral density (BMD) of the lumbar spine and the right femur, parameters of calcaneal quantitative ultrasound(QUS), blood partial pressure and pulmonary functions in 39 male patients with COPD and 30 controls were measured. Results The broadband ultrasonic attenuation (BUA) and the speed of sound (SOS), BMD of the lumbar spines and the femur were significantly lower than that in control group. The biochemical markers of bone metabolism such as HOP/Cr, Ca/Cr, and PTH in the COPD group was significantly raised than that in control group〔(60 2?7 0)dB/MHz vs (66 5?4 9)dB/Mhz,(1 328 4?41 5)m/s vs (1 505 8?26 9)m/s,23% vs 34%,21% vs25%〕. 5.Modified Shengma Biejia Decoction Combined with CAG Program for Elderly Acute Myeloid Leuke- mia Patients with Yin Deficiency Toxin Stasis Syndrome.

OBJECTIVETo observe the efficacy and safety of modified Shengma Biejia Decoction (MSBD) combined with CAG program in treating elderly acute myeloid leukemia (AML) patients with yin deficiency toxin stasis syndrome (YDTSS).
METHODSTotally 46 elderly AML patients were assigned to the treatment group (24 cases; treated with MSBD + CAG) and the control group (22 cases; treated with CAG + placebos of Chinese medicine) according to random digit table. The therapeutic course of CM placebo or MSBD was 21 days. The clinical efficacy and adverse reactions were observed. Meanwhile, physical state (ECOG Score), transfusion dependency, and TCM syndrome score were compared before and after treatment.
RESULTS(1) The complete remission rate was 54% (13/24) and the objective response rate (ORR) was 71% (17/24) in the treatment group, obviously higher than those of the control group [36% (8/22); 54% (13/24)], with statistical difference (P = 0.036, 0.042). When comparing the efficacy based on risk level, the moderate and poor ORR was 71% (10/14) and 67% (6/9) in the treatment group, and 57% (8/14) and 33% (2/6) in the control group, with statistical difference between the two groups (P = 0.048; P = 0.010). (2) Compared with before treatment in the same group, the ECOG score significantly decreased, the average infusion time of red cells and platelets were markedly prolonged in the treatment group after treatment (P < 0.05). ECOG score, the average infusion time of red cells and platelets were significantly better in the treatment group than in the control group after treatment (P < 0.05). (3) Compared with before treatment in the same group, scores of fever, hemorrhage, and bone pain were markedly reduced in the control group (P < 0.05); scores of fever, fatigue, hemorrhage, dry mouth, and bone pain were markedly reduced in the treatment group (P < 0.05). Better effect in relief of fever, fatigue, hemorrhage, dry mouth, and so on was obtained in the treatment group than in the control group (P < 0.05). (4) In aspect of hematotoxicity, the incidence of neutropenia, anemia, thrombocytopenia was obviously lower in the treatment group than in the control group [29.2% (7/24) vs 54.5% (12/22); 16.7% (4/ 24) vs 45.5% (10/22); 33.3% (8/24) vs 63.6% (14/22); P < 0.05]. The incidence of fatigue and anorexia was obviously lower in the treatment group than in the control group [37.5% (9/24) vs 63.6% (14/22), 37.5% (9/24) vs 81.8% (18/22); P < 0.05].
CONCLUSIONMSBD combined with CAG program in treating elderly AML patients with YDTSS, with efficacy enhancing toxicity reducing effect, had distinct advantages in improving physical condition and clinical symptoms, and reducing transfusion dependency.

7.Analysis of the factors affecting the occurrence of myopia in children with myopia

AIM: To analyze the electronic product use time, writing time, playing piano time and outdoor activity time and the distribution of myopia in 586 cases of school age children in our hospital. ·METHODS: A retrospective analysis of 586 cases of children aged 6 to 12 years old in the outpatient department was established. Personalized files were used to record the uncorrected visual acuity, optometry, slit lamp, fundus mirror and strabismus. The cumulative use time of electronic products ( including computer, mobile phone, iPad) , writing time, whether to play the piano and outdoor activities time with the eye situation were recorded. Statistical analysis of the age group of myopia, the cumulative use of electronic age in different age groups, writing time, whether playing piano and outdoor activities and the distribution of myopia occurred. · RESULTS: ( 1 ) With the increasing of age, the distribution of uncorrected eyesight was in children mostly mild myopia, and the proportion of mild myopia was significantly higher than that of moderate and high myopia. ( 2 ) Electronic products use time distribution:the proportion of playing electronic products ( including mobilephone, computers, iPad) accounted for 76. 8%, of which 9 years old, 10 years old the cumulative use of electronic products with a long time was higher than other age groups. ( 3 ) Distribution of writing time: the proportion of write homework ≤1h was significantly lower than the proportion of writing homework > 1h (37. 2% vs 62. 8%), of which 9 and 10 years old children cumulative write time was higher than other age group. ( 4 ) Distribution of playing the piano: the proportion of playing piano time less than 1h was significantly higher than the proportion of playing piano time more than 1h ( 89. 1% vs 10. 9%). ( 5 ) Distribution of outdoor activities: the proportion of outdoor activities ≤1h in children at school age was significantly higher than that of outdoor activities > 1h (91. 8% vs 8. 9%). ·CONCLUSION: With the age exposure to electronic products becoming younger, heavy learning tasks and less outdoor activities, myopia occurred in advance of age. So health examination and eye guidance, reducing the amount of work appropriately, increasing outdoor activities will slow the development of early childhood myopia.
8.Effects of Morroniside on Cycloxygenase after Platelet Aggregation Induced by Adenosine Diphosphate in Rabbits

Objective To investigate the effect of morroniside on cycloxygenase (Cox) in the condition of platelet aggregation inducedby adenosine diphosphate (ADP) in rabbits. Methods The levels of Cox induced by ADP in different groups were detected by enzyme-linked immunosorbent assay (ELISA). Results Compared with the control group, all the morroniside groups significantly inhibited theincrease of Cox induced by ADP (P<0.001), which had concentration dependence, and the inhibition rate of high dose group was 30%. ConclusionMorroniside can decrease the level of Cox and it may be the mechanism of morroniside on inhibiting the platelet aggregation inducedby ADP in rabbits.
9.Effect of topically applied recombinant adenovirus of adiponectin on bone defect repair.

OBJECTIVETo investigate the effect of local administration of recombinant adenovirus of human adiponectin (hAPN) Ad-hAPN-EGFP on tibial defect repair of SD rats.
METHODSTibial defect (2 mm) models of 36 SD rats (72 sides) were randomly divided into three groups (A, B, and C; n=24). The three groups were injected with Ad-hAPN-EGFP, Ad-EGFP, and normal saline, respectively, during operation and the day after operation. A week after the operation, the expression of hAPN and osteogenesis-related factors were detected by real-time polymerase chain reaction. Three weeks after operation, the tibias were examined by micro-computed tomography, hematoxylin-eosin staining, and Masson staining to evaluate the restoration of bone defects.
RESULTS1) The hAPN expression was detected in group A but not in groups B and C. Osteogenesis-related factors expression of group A was significantly higher than that of the other groups (P<0.05). 2) Osteogenesis (including bone mineral density, relative bone volume, trabecular number and trabecular thickness) in group A were more evident than those in groups B and C (P<0.05). No significant differences were found between groups B and C (P>0.05).
CONCLUSIONLocal administration of recombinant adenovirus Ad-hAPN-EGFP may be an effective strategy to improve the restoration of bone defects in vivo.

10.Clinical analysis on surgical treatment of lumbar spinal stenosis in elderly patients aged 65 years and over

Objective To evaluate the surgical results of spinal stenosis in the elderly and to investigate the indications , surgical technique and factors which may contribute to the outcome. Methods A retrospective study was held, 304 patients aged 65 and over who had different kinds of decompression laminectomy for lumbar spinal stenosis between Jan 1990 and Jun 2005 were recruited. The mean patient age at surgery was 70.4 years (65-86 years old). Results One hundred and seventy-five cases had co-existing illnesses, 62 had concomitant degenerative spondylolisthesis, 138 had spinal instrumentation. Functional results were graded as excellent, good, fair and poor: 86. 8% excellent or good, 11. 2% fair and 6 cases poor. Four patients had re-operations because of post operation hematomas. Multivariate analysis revealed that age, sex, co-morbidity score, number of levels decompressed, and degenerative spondylolisthesis did not predict bad outcomes. Conclusions The surgical results of spinal stenosis in the elderly are favourable and comparable to those reported for the general population . Carefully perioperative preparation is very important in the treatment of elderly patient with lumbar spinal stenosis.
