1.A Suggestion of Strengthening the Basic Pharmaceutical Supervision and Management to Realize the Ten Kinds of Strategic Changes

In the light of developing trend and reality of domestic pharmaceutical supervision and management we propose the guidance of drug market, using the scientific method, making supervision as center, taking emphasis on basic units, strengthening in vertical section, popularizing in horizontal section, expanding scale etc as guiding thoughts. In order to strengthen the basic pharmaceutical supervision and management the suggestion of ten kinds of strategic changes must be realized.
2.Changes of tear film after trabeculectomy in glaucoma

AlM: To learn the changes of the tear film before and after the trabeculectomy of glaucoma and explore the incidence of dry eye and the prevention and control measures.METHODS:The 36 patients (60 eyes) of glaucoma were examined in detail before 3d of trabeculectomy and after the surgery at 3, 7, 14 and 30d. The examinations include lower eyelid central river of tears, break-up time ( BUT) , Schirmer I test ( S I t ) and staining scores of corneal fluorescein under slit lamp microscope.RESULTS:The tear meniscus height of central lower eyelid was increased and the tear film BUT was shortened at the same time, the scores of S I t was reduced and corneal fluorescein staining score was increased at postoperative 3 and 7d compared with that of preoperation. The tear meniscus height of central lower eyelid, tear film BUT and Slt and score of corneal fluorescein staining began to recover in most of the affected eyes after surgery 14d. At 30d after surgery, 22%of patients tear film failed to recover to the preoperative level; dry eye occured in 18% preoperative eyes with normal tear film.CONCLUSlON:Trabeculectomy of glaucoma may affect the stability of the tear film and some patients showeing obvious dry eye and should be intervened and treatmented timely.
3.Effects of melatonin on retinal oxidative stress in short-term diabetic retina

Background Oxidative stress is one of the pathogenesis mechanisms of diabetic complications.Melatonin is the most powerful antioxidant in living organism.Seldom study on the effects of melatonin on diabetic complications is found in China now.Objective This study aimed to detect the expression and alteration of malonadehyde(MDA),glutathione(GSH)and glial fibrillary acidic protein(GFAP)in retina with experimental diabetes and explore the effects of melatonin on retinal oxidative stress in diabetic rats. Methods The animal models of diabetes were established by injection of 2% streptozocin via caudal vein(STZ,45 mS/kg)in 48 6-week-old clean SD rats.Isometric citric acid buffer solution wag injected in 24 normal.rats as control group.Seven days after injection of STZ,melatonin(10 mg/kg)was injected into the abdominal cavity of 24 diabetic rats in melatonin group dailv,and the equal volume of normal saline was injected in other 24 diabetic rats(model rats)and 24 rats in normal control group.The animals were sacrificed and retinas were obtained in three groups in 4,6 and 8 weeks respectively.Immunohistochemistry and Western blot were used to detect the expression of GFAP in rat retina.The levels of MDA and GSH in retina homogenate were assayed by spectrophotometer.This experiment complied the Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission.Results The expression of GSH in model group was lower than that in melatonin group or control group at 4,6 and 8 weeks (all P<0.05).However,no statistically significant difference was seen in the expression level of GSH between melatonin group and normal control group at 4 and 6 weeks(P>0.05),and in the eighth week,expression of GSH in melatonin group was lower than that in control group(P<0.05).The expressions of MDA and GFAP were more prominent in the model group than in melatonin group and control group in 4,6 and 8 weeks(P<0.05),but no significant difference was found between melatonin group and control group at 4 and 6 weeks(P>0.05).The expressions of MDA and GFAP in melatonin group were higher than those in control group at 8 weeks(P<0.05).Conclusion Melatonin plays a powerful effect in protecting the retina from oxidative stress in diabetes retina by downregulating the MDA level,upregulating GSH level and inhibiting GFAP expression.
5.Clinical research progress in adenocarcinoma of the esophagogas-tric junction

The adenocarcinoma of the esophagogastric junction (EGJA) is located in a unique anatomical position and at the junction of the squamous epithelium and the columnar epithelium. The biological characteristics of this disease are different from those of esophageal or gastric cancer. The diagnostic classification of EGJA has been subject to controversies, and no gold standard therapeu-tic regimens have been established, especially in the choice of treatment of locally advanced EGJA. Results from large-scale clinical tri-als and imaging technology development showed that the treatment of EGJA has been individualized. Furthermore, this problem high-lights the importance of multidisciplinary collaboration. This article focuses on current progress in studies on EGJA.
6.Clinical manifestation and outcome of severe ulcerative colitis

Objective To evaluate the clinical outcome of severe ulcerative colitis(UC)and to find the factors related to treatment and outcome.Methods Forty one hospitalized patients with UC during 1988-2004 were retrospectively reviewed.Data were recorded including the onset,symptoms,signs, laboratory,endoscopic,radiologic and pathologic findings,as well as the processes of clinical treatment. The patients who undergone surgery were also analysed.Results Forty one of 144(28.5%)hospital ized patients were suffered from severe UC,and among them 92.7%(38/41)had pancolitis.The patients who had first onset,chronic persistent,chronic recurrent type were account for 36.9%(15/41),36.9%(15/41) and 26.8%(11/41),respectively.The steroids treatment played the main role in the inducing remission of severe UC(61.0%).Thirty one cases(75.6%)could be relieved by drug therapy.Seven cases(17.1%) were progressed to have operation.The age of early onset,pancolitis,low hemoglobin and serum albumin levels and need of intravenous steroids treatment were associated with the need of surgery.Conclusions Most of the severe UC patients respond well to the medical therapy,but for some non-responding or steroids depending individuals,after a reasonable duration of treatment,surgery should be considered.
8.Study on the Preparation and Quality Control of Papaverine-nitroglycerin Ointment

OBJECTIVE:To prepare papaverine-nitroglycerin ointment and to establish a method for its quality control.METHODS:Nitroglycerin,papaverine hydrochloride and diclofenac potassium were used as basic remedies to prepare oint?ment.The contents of nitroglycerin,papaverine hydrochloride and diclofenac potassium were determined by UV-spec?trophotometric methods.RESULTS:The average recovery rates of Nitroglycerin,papaverine hydrochloride and diclofenac potassium were100.65%、99.86%、100.55%respectively,and RSD equal to0.4945%、0.1719%、0.3205%respectively(n=3). CONCLUSION:The ointment preparation is feasible in technique,reliable in quality control and stable in quality.
9.27-gauge microincision vitrectomy surgery system for clinical application

Objective To observe the preliminary clinical results and safety of 27-gauge microincision vitrectomy surgery for partial vitreoretinal diseases.Methods A total of 13 patients (13 eyes) who underwent 27-gauge microincision vitrectomy surgery were enrolled.The follow-up period was 6 to 12 months.Preoperative and postoperative visual acuity and intraocular,total operative time,cutting time for removing vitreous,wound healing status,intraoperative and postoperative complications were observed.Results Mean best corrected visual acuity improved from preoperative (1.26±0.66) logMAR (0.10±0.09) to postoperative (0.63±0.52) logMAR (0.35±0.24),and the difference was statistically significant (t=2.743,P=0.018).The difference of mean preoperative intraocular pressure (IOP),IOP of postoperative day 1,day 5,one month and final postoperative visit were not statistically significant (F=0.593,P>0.05).The mean total operative and cutting times were (36.38±14.97) min and (10.12±3.54) min respectively.Postoperative scleral incision showed linear closure,no cases of postoperative sclerotomyrelated complications such as wound dehiscence,vitreous incarceration and subcoujunctival fluid were observed.No intraoperative and postoperative complications of iatrogenic retinal breaks,endophthalmitis,choroidal detachment,retinal detachment and vitrous hemorrhage were observed.Conclusions The 27-gauge microincision vitrectomy surgery can improve postoperative visual acuity for treatment of vitreoretinal diseases and induce fewer sclerotomyrelated complications,which maybe a safer surgical approach.
10.Ultrasound thrombolysis in patients with ischemic stroke

Intravenous thrombolysis is the only treatment that has the evidence of evidence-based medicine in acute ischemic stroke. However, the narrow time window has limited the treatment opportunities of many patients. Transcranial ultrasound thrombolysis is a very promising thrombolysis-assisted method, and transcranial ultrasound plus microbubble-assisted thrombolysis is the research hotspot in recent years. At present, more suitable ultrasonic instruments for thrombolysis and a number of ways of ultrasound-assisted thrombolysis are being developed.
