2.Application of endoscopic saphenous vein harvesting in coronary artery bypass grafting

Objective To compare the clinical effects of endoscopic saphenous vein harvesting and traditional surgical management in coronary artery bypass grafting(CABG).Methods Between march 2007 to June 2008,215 patients underwent CABG.Among them,87 patients underwent endoscopic saphenous vein harvesting.At the same time,128 cases were managed by traditional ways.Results The cases were foUowed up for 1-17 months.Endoscopic great saphenous vein harvesting was superior to traditional management in the healing of wound infection,relief of pain,improvement of edema and skin feeling(P<0.05).Conclusion Endoscope saphenous vein harvesting is an efficient technique for CABG which is characterized by less trauma and fewer wound complications,and will not influcence the function of vein as well.On the other hand,the satisfaction is improved.
3.BIOAVAILABILITY OF SELENIUM IN SELENIUM-YEAST TO YOUNG RESIDENTS IN A KESHAN DISEASE AREA

Abstract To evaluate the bioavailability of selenium (Se) in Se-yeast to residents with low-Se status, fifty-two youth in a Keshan disease area were randomly divided into three groups, and they were given orally 200 ?g Se daily as So-yeast or sodium selenite, or ordinary yeast for 12 weeks, respectively. Se contents and activities of glutathione peroxidase (GSHpx) in blood were measured at wk 4 and 8 of supplements and 8wk alter the supplements were discontinued. The results show that: 1. the retention of Se in either plasma or erythrocyte is sigificantly higher in Se-yeast than in sodiurn selenite; 2 the effect of Se in Se-yeast is superior to that of sodium selenite for maintaining GSHpx activities; and 3. the relative bioavailabilities of Se in Se-yeast are 556% and 178% when using erythrocyte and plasma Se contents as the response critria, as well as 158% and 116% when estimated by platelet and plasma GSHpx activities. The results indicate that the bioavailability of Se to the residents in a low-Se Keshan disease area is greater when supplemented Se-yeast than selenite.
4.Diagnostic value of 128-slice spiral CT angiography for bypass grafts in patients after coronary artery bypass grafting

Objective To evaluate the diagnostic value of 128-slice spiral CT angiography( MSCTA)for bypass grafts in patients after coronary artery bypass grafting(CABG).Methods One hundred and thirty-three bypass grafts (44 IMA grafts,89 saphenous veins grafts) of 46 patients after CABAG operation for 12 to 76 months were examined by MSCTA.Then the coronary angiography(CAG) was performed on those patients 3 - 10 days after MSCTA examination.The MSCTA results were compared with the angiography results.Results Among the 133 bypass grafts,MACTA examination showed that 17 grafts were occluded and 20 grafts were severe restenosis( restenosis degree > 50% ).There was also 17 occluded grafts showed in CAG examination as in MSCTA results.But 21 restenosis ( restenosis degree > 50% ) bypass grafts were identified by CAG.Compared with the CAG results,there was 1 false positive and 2 false negative in the MSCTA results.The overall sensitivity and specificity of MSCTA on evaluating the bypass grafts were 94.7% and 98.9%.The positive predictive value and the negative predictive value were 97.3% and 97.9%,respectively.Conclusion As a noninvasive examination,128-slice spiral CT could accurately identify and evaluate the bypass grafts lesions after CABG.
5.The analysis on the causes and treatment stretagies of Stanford A aortic dissection

Objective To analyze the reasons of perioperative hypoxia in Stanford A aortic dissection,and summarize its management strategies.Methods From Dec.2005 to Jul.2011,sixty four patients underwent surgery for acute type A aortic dissection,of which 9 cases were with chronic dissection and 55 cases with emergent ones.Preoperative oxygen fraction ratio( PaO2/FiO2 )in 51 cases was lower than 200 mm Hg.All of them underwent the surgery with the help of deep hypothermia cardiac arrest technique.Results Three cases died.Thirty-three cases could not live without ventilation during the first 72 h because of continuous hypoxia ( PaO2/FiO2 < 200 mm Hg).One case underwent tracheotomy and auxiliary ventilation for 9 days.The rest were live without ventilation after auxiliary ventilator for 72 - 120 hrs.The data showed that postoperative hypoxia was related to preoperative hypoxia (oxygen fraction ratio < 200 mm Hg),BMI,acute onset,hypothermia cardiac arrest time,and transfusion more than 3000 ml ( P < 0.05 ).Conclusion Great attention should be paid to the perioperative hypoxia-related factors in Stanford A dissection,which will be helpful to improve prognosis.
6.Application of 3-dimensional CT reconstruction in the diagnosis of posterior urethral strictures or ankylurethria

Objective To study the diagnostic value of 3-dimensional CT reconstruction in posterior urethral strictures or ankylurethria.Methods Thirty patients with strictures or ankylurethria of posterior urethra caused by pelvic fracture underwent helical CT scan and 3-dimensional reconstruction of the urethral canal as well as radiographic urethrography before and post open urethral reconstruction to observe the urethral anatomy,the length and position of the urethral strictures,the depth of periurethral scar.Results The mean stricture or ankylurethria length measured by radiographic urethrography was 4.0 cm (range from 1.0 cm to 7.0 cm),and the correlation coefficient of stricture or ankylurethria length was 0.92,21 (70%) patients were diagnosed accurately by radiographic urethrography.The mean stricture or ankylurethria length measured by 3-dimensional CT reconstruction was 4.3 cm (range from 1.2 cm to 7.6 cm),and the correlation coefficient of stricture or ankylurethria length was 0.96,there were 28(93%) patients diagnosed accurately by 3-dimensional CT reconstruction.The mean stricture or ankylurethria length measured by open urethral reconstruction was 4.2 cm (range from 1.5 cm to 7.5 cm).Five patients with urethrorectal fistula were also diagnosed accurately by 3-dimensional CT reconstruction rather than by radiographic urethrography.Conclusions 3-dimensional CT reconstruction of the urethral canal can accurately evaluate the urethral anatomy,the length and position of the urethral strictures,as well as the depth of periurethral scar after crush injury and provide useful information for operation that may not be provided by radiographic urethrography.3-dimensional CT reconstruction may become the most valuable means for detecting posterior urethra strictures or ankylurethria with urethrorectal fistula.
7.Management of complications after endovascular repair for De Bakey type Ⅲ aortic dissection

Objective To summarize the experience in endovascular repair of De Bakey type Ⅲ aortic dissection in recent years and summarize the prevention and management of the related perioperative complications.Methods From January 2009 to January 2011,49 cases of endovascular repair for De Bakey type Ⅲ aortic dissection were performed under general anesthesia in our department.There were 45 male and 4 female.The follow-up was performed in the outpatient department or by telephone.Results There was no inhospital death and no paraplegia events.Severe complication included:coma,2 cases ( 4.1% ) ; endoleak,2 cases (4.1% ) ; upper limb ischemia,2 cases (4.1% ).Recurrent proximal aortic dissection,1 case.Fever was occurred in most of those cases.Conclusion Endovascular repair of aortic dissection improves the outcome of aortic dissection patients.But more attention should be pay to prevent the severe complications,It will help to improve the prognosis and life quality by reducing the risk of retrograde dissection,acute brain ischemia and endoleak.
8.THE RELATIVE BIOAVAILABILITY OF SELENIUM IN ZIYANG HIGH-Se WHEAT TO SELENIUM-DEFICIENT RATS

A three-phase depletion/repletion/depletion feeding study was designed to investigate the changes of Se levels and glutathione peroxidase (GSH-Px) activities in tissues and blood in rats fed for 6 weeks low-Se diet (6 ppb Se) from a Keshan disease area during the supplementations of Se as Ziyang high-Se wheat (1.175 ppm Se) or sodium selenite (dietary Se 219 and 223 ppb respectively) and after the supplements were discontinued, and to evaluate the relative bioavailability of Se in the wheat. The resalts showed that the average bioavailability of Se in high-Se wheat derived from the values at wk 2, 4 and 6 of supplement was close to that in selenite when plasma, erythrocyte, kidney, liver and cardiac Se contents were used as the response criteria, the relative bioavailabilities being 98%, 104%,100%,96% and 101% (sodium selenite = 100%) respectively. The bioavailability was lower for Se in high-Se wheat (70% or 90%) than for selenite when estimated by erythro-cyte or cardiac GSH-Px activities. However, the bioavailabilities of high-Se wheat Se in various tissues were not all the same at different stages of supplement. In addition, the effect of Se in high-Se wheat in maintaining either Se levels in heart, liver and erythrocyte or GSH-Px activity in heart was superior to that of selenite 3 weeks after the Se supplements were withdrawn.
9.pRNA: A Safe,Stable and Large Capacity Molecular Vehicle for Gene Therapy

The molecular delivery vectors used in gene therapy need provide the features of safety,stability, efficiency and capacity. The current studies on the structure and action mechanism of pRNA, a packaging RNA of phage?29, showed that pRNA with multiple binding sites can through cell membrane easily and escort exogenous molecules to target cell, without inducing immune reaction. As an ideal nano-scale gene therapy vehicle, pRNA presents a promising application in delivering multiple therapeutic components to detect and treat human diseases.
10.Fast measurement method based on near infrared spectroscopy in purifying process of Carthamus tinctorius extracts.

OBJECTIVETo really realize quality control of Chinese herb purifying process, near-infrared spectroscopy (NIRS) was used not only for fast monitoring quality-control index of the process, but also for fast judgment of absorption endpoint.
METHODThe purification process of Carthamus tinctorius extracts with nonionic macroreticular resin was selected as an example. HPLC was used as the reference method to determine the content of HSYA. Quantitative and qualitative detection modes of purification were developed by NIRS combined with partial least squares (PLS) and moving block of standard deviation (MBSD).
RESULTThe correlation coefficient of the calibration model was 0.999, and the RPD for calibration and validation were above 5, of 5.54 and 5.22, respectively. Based on acquisition spectra, absorption endpoint calculated by MBSD was close to that by HPLC, Only 1 min deviation.
CONCLUSIONThe method mentioned above is proved to be convenient, rapid and nondestructive, and is applicable for fast monitoring the content of HSYA and fast judgment of absorption endpoint in purifying process of C. tinctorius extracts.
