1.Application of high pitch dual source CT in the diagnosis of acute aortic dissection

Objective To investigate the application value of low dose high pitch dual source CT angiography technique in diagno-sis of acute aortic dissection.Methods 21 cases of critically ill patients with suspected aortic dissection underwent total aortic angiog-raphy without using ECG by adopting the second generation dual source CT Flash Technology (pitch 3 .2 ,1 0 0 kV and 1 0 0 mAs), using a lower contrast dose (60 mL).Results All subjects accepted as low as possible radiation doses and iodine intake;at the same time,accurate information including aortic intimal tear position,intimal flap,true and false lumen,and lesions involving the range vessel signs and anatomic relationship were received,2 cases of Stanford type A,type B 1 9 cases (2 cases of aortic rupture).Average scan time 2.6 s,average effective radiation dose cm 270 mGy·cm.Conclusion High quality images by using high pitch dual source CT low dose angiography with lower contrast dose provide safe,reliable,real-time screening method for critically ill patients with aor-tic dissection caused by systemic severe compound injury.
2.Influence of metformin and paclitaxel in proliferation and apoptosis of ovarian cancer SKOV3 cells in vitro

Objective:To study the influence of metformin and paclitaxel in the proliferation and apoptosis of human ovarian cancer SKOV3 cells in vitro, and to clarify the synergistic effect of metformin and paclitaxel. Methods:The ovarian cancer SKOV3 cells were divided into blank control group, different concentrations (0.01, 0.50, 1.00, 5.00, 10.00mmol·L-1) of metformin groups and combined treatment groups(metformin combined with paclitaxel with different concentrations).The inhibitory rates of proliferation of SKOV3 cells after treated with different concentrations of metformin were detected by MTT method.The apoptotic rates of SKOV3 cells after treated with different concentrations of metformin were measured by flow cytometry.The inhibitory rates of proliferation of SKOV3 cells after treated with metformin and paclitaxel were determined by MTT method.Results:The MTT results showed that the inhibitory rates of proliferation of SKOV3 cells in different concentrations of metformin groups were increased in concentration-and timedependent manner;there were significant differences compared with blank control group (P<0.05).The flow cytometry results showed that the apoptotic rates of SKOV3 cells in different concentrations of metformin groups were increased;compared with control group, with the increasing of concentrations of metformin, the apoptotic rates of SKOV3 cells in experimental groups 48 h after treatment were increased significantly (P<0.05);the percentages of SKOV3 cells in G0/G1 phase were decreased with the increasing of metformin concentrations(P<0.05) and the percentages of SKOV3 cells in G2/M phase were increased(P<0.05).The MTT results showed that the inhibitory rate of proliferation of SKOV3 cells in 1.00 mmol·L-1 metformin+paclitaxel group was higher than that in 0.50 mmol·L-1 metformin+paclitaxel group was higher than that in 0.50 mmol·L-1 metformin+paclitaxel group(P<0.05),and the inhibitory rates of proliferation of SKOV3 cells in combined treatment groups were higher than those in paclitaxel alone groups(P<0.05).Conclusion:Metformin inhibits the proliferation of ovarian cancer SKOV3 cells through the induction of apoptosis.Metformin can enhance the cell proliferation inhibition of paclitaxel on ovarian cancer SKOV3 cells.The combination of metformin and paclitaxel has a synergistic reaction on SKOV3 cells.
4.Mutation analysis of the STAT3 gene in a patient with hyper-IgE syndrome

Objective To analyze mutations of the STAT3 gene in a patient with hyper-IgE syndrome (HIES).Methods Clinical data were collected and blood samples were obtained from a 14-year-old patient with hyper-IgE syndrome (HIES) and her parents.Genomic DNA was extracted and subjected to PCR for the amplification of the entire encoding and splice sites of the STAT3 gene followed by bidirectional sequencing.Meanwhile,amplified ribosomal DNA restriction analysis was carried out.Results A heterozygous missense mutation A1843G,which caused a K615E substitution,was found in exon 19 encoding the SH2 domain of the STAT3 gene in the patient,but not in either of her parents.The result of amplified ribosomal DNA restriction analysis was consistent with the findings mentioned above.Conclusion The novel K615E missense mutation in the STAT3 gene may contribute to the development of HIES.
5.The clinical analysis on the 956 cases with atypital squamous cells

Objective To analyze the clinical management of women with atypital squamous cells(ASC).Methods During Sep 2003 to Sep 2008,a totle of 956 women in our department were diagnosed with ASC by cytological test,they were further biopsied under the colposcope.The result of TCT,colposcopic test,biopsy and the highrisk factor were recorded and analysed;the based the biopsy were analyzed by the X~2-test;the high-risk factors and the odds Ratio(OR) were counted by SPSS10.0.Results Among the women with ASC,the rate of cervical intraepithelial neoplasia(CIN) diagnosed by the biopsy under the colposcope was 50.7% ,the rate of the high-grade lesion over CINII Was 13.3% ;if the colposcopic test were low-grade lesion,the rate of the cases which biopsys were the highgrade lesion over CINII,was 8.3% ;the high-risk factors which caused the cervical high-grade lesion were the number of sex-partners over 3,the age less than 20 yearold when the sex were made,the cervical erosion,the result of TCT with ASC-H.Conclusion Among the women with ASC,the biopsy shows multiformity;the rate of CIN and the highgrade lesion is also high;if TCT is ASC,the colposcopie test is advised;if the woman who has the high-risk factors,biopsy is advised.
7.Total knee arthroplasty with Gemini MKII rotating-platform prosthesis

BACKGROUND:Total knee arthroplasty mainly used fixed platform, but more and more movable platform was selected year by year in the clinic. Rotary platform theoretical y can apparently improve the curative effects, but it has not been verified in the clinic.
OBJECTIVE:To summarize the design characteristics and clinical results of Gemini MKII (Link, Germany) rotary platform.
METHODS:From January 2004 to January 2009, 108 patients (119 knees) including 31 males and 77 females were treated with total knee arthroplasty by using Gemini MKII system. There were 97 unilateral cases and 11 bilateral cases at the first stage of replacement. Preoperative diagnosis:1 case of hemophilic arthritis, 1 case of venereal Charcot’s disease, 8 cases of post-traumatic arthritis, 11 cases of rheumatoid arthritis, and 87 cases of degenerative osteoarthritis. The prosthesis was fixed with bone cement. None received patel ar replacement. RESULTS AND CONCLUSION:None experienced infection, femoral condyle fracture or nerve and vascular damage. Except bilateral replacement, the average operation time of unilateral replacement was (56±13) minutes. Average postoperative hemoglobin decreased (25±5) g/L. After replacement, hemoglobin maintained over 100 g/L in 83 patients (86%), without blood transfusion. Knee joint score and function score recovered to (92.0±4.7) points and (90.3±6.8) points at half years after replacement, and became (94.4±3.2) points and (91.6±5.9) points during final fol ow-up, which was significantly improved as compared with that pre-replacement (P<0.01). No revision surgery should be done for lining prolapse or spin out, osteolysis and prosthesis loosening. These results indicated that the early and midterm results of Gemini MKII rotating-platform prosthesis were satisfactory, but the long-term results remain to be confirmed.
8.Effect of House Indoor Air Pollution on Children Respiratory System in Lanzhou City

Objective To study the adverse effect of house indoor air pollution on children respiratory system health in Lanzhou city.Methods A retrospective questionnaire investigation on 1 700 children respiratory health was carried out in three kindergartens,three elementary schools involving lower grade pupils in three districts in Lanzhou city.At the same time,the information about the home environment of the children and family background was collected.Applying logistic regression model, the influencing factors were analyzed.Results The over-standard rates of indoor air quality were from 12.4% to 43.8% in Lanzhou.In the non-heating period,the over-standard rates of formaldehyde,benzene,SO_2,NO_2,CO and PM10 in indoor air were 22.6%,14.5%,13.3%,12.4%,0% and 23.3% respectively.In the heating period,the over-standard rates of the pollutants were 27.3%,16.4%,34.6%,28.5%,32.3% and 43.8% respectively.Indoor air pollution was statistically associated with children respiratory symptoms and diseases,the related risk factors were someone smoking in home,house decorated,opening windows, ventilation,heating in winter,sharing bedroom and maternal education.The outdoor air pollution was one of the influencing factors. Conclusion Children respiratory diseases and symptoms are related to many air pollution factors,such as outdoor and indoor air pollution,personal characteristic and family factors,so to prevent and control indoor air pollution,a comprehensive way should be taken in Lanzhou city.
9. Real-time transrectal ultrasound-guided seminal vesiculoscopy for the treatment of patients with persistent hematospermia: A single-center, prospective, observational study

This study aimed to describe endoscopic anatomy of the seminal tract and summarize our experience of transutricular seminal vesiculoscopy (TSV) guided by real-time transrectal ultrasonography (TRUS) in managing persistent hematospermia. A total of 281 consecutive patients with persistent hematospermia who underwent TSV with or without real-time TRUS were enrolled in this single-center, prospective, observational study. The median follow-up period was 36.5 (range: 8.0-97.5) months. TSV was successfully performed in 272 (96.8%) patients. The approach of a 4.5/6 F rigid vesiculoscope entering the seminal tract was categorized into four types on the basis of endoscopic presentation of the ejaculatory duct orifice and verumontanum. Seven (2.6%), 74 (27.2%), 64 (23.5%), and 127 (46.7%) patients had Types I (through the ejaculatory duct in the urethra), II (through the ejaculatory duct in the prostatic utricle), III (transutricular fenestration through a thin membrane), and IV (real-time transrectal ultrasound-guided transutricular fenestration) approach, respectively. In patients who successfully underwent surgery, bleeding occurred in the seminal vesicle in 249 (91.5%) patients. Seminal vesiculitis, calculus in the prostatic utricle, calculus in the ejaculatory duct, calculus in the seminal vesicle, prostatic utricle cysts, and seminal vesicle cysts were observed in 213 (78.3%), 96 (35.3%), 22 (8.1%), 81 (29.8%), 25 (9.2%), and 11 (4.0%) patients, respectively. Hematospermia was alleviated or disappeared in 244 (89.7%) patients 12 months after surgery. Fifteen patients had recurrent hematospermia, and the median time to recurrence was 7.5 (range: 2.0-18.5) months. TSV guided by TRUS may contribute to successful postoperative outcomes in managing persistent hematospermia.
10. Mean platelet volume might be an effective indicator of arterial erectile dysfunction

The aim of our study was to investigate the role of platelet parameters including mean platelet volume (MPV) and platelet count (PC) in the pathogenesis of penile arteriogenic erectile dysfunction (ED) and to evaluate the association between the platelet parameters and arteriogenic ED. There were 244 patients with ED (based on the International Index of Erectile Function [IIEF]-5 ≤21) and 60 healthy controls (IIEF-5 >21) enrolled. All participants were asked to undergo a laboratory examination, and penile vascular function was evaluated using penile color Doppler ultrasonography (pDUS). Among these ED patients, 24 patients with no abnormality on nocturnal penile tumescence (NPT) and 84 with normal vasculature or mixed vascular abnormalities were excluded. The other patients were classified into three groups as follows: control (n = 60), arteriogenic ED (n = 99), and venous leakage (n = 37) groups. MPV and PC were significantly higher in the arteriogenic ED group compared with the venous and control groups (P < 0.05). Receiver operating characteristic curve analysis revealed that the area under the curve for MPV to predict arteriogenic ED was 0.707. MPV ≥9.65 fl was recognized as a cut-off value for potential arteriogenic ED (sensitivity: 47.5%; specificity: 91.7%). A significant inverse correlation was detected between MPV and 10-min peak systolic velocity (PSV) (r =-0.34; P < 0.001) in the arteriogenic ED group. These findings suggest that the MPV might be a powerful indicator to predict and diagnose arteriogenic ED, and MPV may be a marker for ED when using pDUS.
