Chinese Health Economics 2017;36(3):12-15
Objective:To study the influence of transfer payment on the equalization of medical and health services. Methods:Based on panel data of China's 23 provinces from 2000 to 2014.the medical and health services equalization indexes were measured and developed as the explained index. The explanatory variables such as transfer payment, per capita GDP and health care input were involved. The threshold value of time and transfer payment were applied to developed to conduct the panel threshold regression. Results:Transfer payment significantly promoted the equal distrihntion of health service resources, but the elastic declined if transfered more than 33 billion yuan as the threshold.Meanwhile, there was a threshold effect that the elastic of transfer payment to the equalization of China's medical and health service declined after NCMC in 2003.Conclusion:To further increase transfer payment,it needed to enlarge the new farmers and scope, optimize the equal distribution of health service resources.
2.Research on the Spatial Spillover Effects of Medical and Health Resources on the Hospital Innovation
Chinese Hospital Management 2017;37(8):19-21
Objective To study the spatial spillover effects of medical and health resources on the hospital innovation. Methods Based on panel data of 31 provinces of China from 2000 to 2014,and with the method of Spatial Durbin Model(SDM),the paper does the regression with the sample of all provinces, eastern, central and western provinces, through using provincial patent number as explained variable, and using the provincial medical institutions of beds, per capita GDP,health care spending as explanatory variables. Results Medical and health resources have a significant role in promoting the hospital innovation, not only for local province but also for adjacent provinces, namely there is spillover effect;the promotion effect of medical resources for local and neighbouring province hospital innovation is almost the same;for eastern and central provinces, there is a positive effect of medical resources to hospital innovation, and for central province, the spillover effects exist. Conclusion The medical and health resources should be further increased,a provincial communication platform of resources and innovation be built, the east and western provinces spillover effect be promoted,to drive the reform of the supply side with innovation.

Chinese Journal of Interventional Imaging and Therapy 2011;08(5):394-397
ObjectiveTo investigate the ultrasonic features of phyllodes tumors of the breast (PTB) with different pathologic types.MethodsTwo-dimensional ultrasound and CDFI were performed in 16 patients with pathologically proved PTB,and the ultrasonic characteristics of different pathologic types PTB were reviewed retrospectively.ResultsAccording to WHO criteria, there were 12 benign PTB,2 borderline and 2 malignant PTB.Associated breast fibroadenoma was detected in 7 patients, including 6 benign and 1 borderline PTB.All of 16 PTB had clear boundary.Benign PTB had regular or irregular shapes, homogeneous or unhomogeneous internal echo and posterior acoustic enhancement in a few cases. Most borderline and malignant PTB showed regular or irregular shapes, mixed cystic and solid internal echo or irregular internal echo,as well as posterior acoustic enhancement. No calcification of PTB was found in all 16 patients. Conclusion The ultrasonograghic features of PTB with different pathologic types are characteristic to some extent, therefore being helpful to the differential diagnosis of PTB.

Objective To test the level of follicular helper T cells (Tfh) and interleukin (IL)-21,CXCL13 in the peripheral blood of patients with ankylosing spondylitis (AS),and to analyze the relationship between Tfh and clinic features and explore the possible immunological pathogenesis of AS.Methods The Tfh cells were obtained from patients and normal controls and detected by flow cytometry. While the levels of IL-21,CXCL13 in patients and normal controls were measured by enzyme-linked immunosorbent assay (ELISA) tests.Data analysis were performed by Student's t-test,Rank-sum test,Spearman's correlation test. Results The expression of CD4+CXCR5 qCOS + cells (Tfh) (mean rank 33.71) and IL-21 [(299±27) ng/L],CXCL13 [(5.8±1.0) μg/L] in the peripheral blood of AS was significantly higher than normal controls [mean rank 23.54,(176±26) ng/L,(4.2±0.8) μg/L] (Z=-2.258,t=17.221,t=6.464,all P<0.05).It was similar in AS with peripheral joint involvement compared with AS of non-peripheral joint involvement, and there was no difference between AS patients with positive HLA-B27 and those without HLA-B27.Mean -while,no correlation was found between the expression of Tfh,IL-21,CXCL13 and level of ESR,CRP,BASDAI.And there was no significant correlation between the expression of Tfh and IL-21,CXCL13 (P>0.05).Conclusion The expression of Tfh and the levels of IL-21,CXCL13 are increased significantly, but are not closely relatedto disease activity. These results indicate that the abnormality of Tfh may play an important role in the pathogenesis of AS.

In order to solve the problems of erratic drug absorption and low bioavailability after oral administration for poorly-water soluble drugs due to low solubility, a series of novel pharmaceutical dosage forms as solid dispersion, liposome, microemulsion, vesicle, cyclodextrin inclusion complexes and drug nanocrystal have been developed in recent years. Among which drug nanocrystal attracts more attentions for its simpler preparation method, higher drug loading and easier manufacturing technology in the design of dosage forms suitable for different administration routes. In this paper, the nanocrystals of the poorly-water soluble drugs prepared based on bottom-up and top-down technologies were introduced. The characteristics and applications of the nanocrystal-based dosage forms as suspension, tablet and capsule were also introduced and carefully evaluated with the focus on their pharmacokinetics, pharmacodynamics and tissue targeted drug distribution after delivery by oral administration, intravenous injection and pulmonary inhalation. The advantages of drug nanocrystals in their therapeutics effects over the bulk drugs were discussed together with the inherent mechanism. Finally, the problems existing in basic research and scaled-up manufacture of drug nanocrystal as well as the possible ways of solution were listed out so as to make the nanocrystal-based preparations exert their maximum therapeutic effect after clinical application.

Objective HupA is one of the potential drugs which can be used to treat Alzheimer's disease(AD).The aim of this study was to explore the pharmacokinetics and biodistribution of HupA in vivo by using 11C-HupA.Methods A total of 25 SD rats were studied. They were divided into 5 groups (5 rats in each group). All had intravenous injection of 22 MBq(in0.2 ml)11C-HupA through tail vein.Dynamic im-aging Was acquired from 5 to 90 minutes after injection.Venous blood and organ activities were collected at 5,15,30,60.and 90 minutes after injection.Percentage activity of injected dose per gram of tissue(%ID/g)was calculated to characterize the biodistribution of tracer in different brain regions: frontal, apical, temporal, occipital, cerebellum, hippocampus, striatum, thalamencephalon, and brain stem, Variance analysis using SPSS 11.5 software was performed and compared among the study groups. Results 11C-HupA was character-istic for its quick clearance from blood, with half time T1/2 of (14.61±1.77) min,and clearance rate (CL)macokinetics of 11C-HupA in rats corresponded to a one-compartment model.with an activity curve(area 11C-HupA distribution in different brain regions, being greater in cerebral cortex,hippocampus,hypothala-mus and brain stem. Conclusions Pharmacokinetic study of 11C-HupA in brain was fast.convenient and showed high specificity and sensitivity. Its ability to quantitatively evaluate brain function and its character-istic distribution in mice provided some evidence for monitoring therapy in AD patients.
