ObjectiveTo explore the application of the plastic principles in surgical treatment for diabetic foot ulcer and analyze the therapeutic efficacy.Methods51 cases of diabetic foot ulcer repaired with surgical treatment were analyzed.Results26 cases repaired with free skin graft,1 1 cases repaired with skin flap graft,3 cases repaired with amputation,the percentage of first stage healing rate was 91.4％.ConclusionApplication of the plastic principles in surgical treatment for diabetic foot ulcer,could improve the healing percentage and reduce the rate of amputation.

Objective To study the factors relating to failure of vacuum assisted closure (VAC)therapy for pressure ulcers control in order to guide the clinical indications.Methods A retrospective cohort study of 75 patients with pressure ulcers to undergo VAC therapy was conducted.Correlations between 12 risk factors and VAC therapy failure were analyzed.The statistical prognostic factors were screened and the sensitivity,specificity,positive predictive value and negative predictive value of them were analyzed.Risk factors for VAC failure were determined by using Logistic regression analysis.Results Among the 75 patients,17 patients failed to respond to the VAC therapy.The failure rate of VAC therapy had correlations with positive blood cultures,bony exposure,hypoalbuminemia (alb ＜25 g/L) and wound depths≥ 4 cm (70.0％,50.0％,38.5％ and 40.9％,respectively),but the Logistic regression analysis found that the 4 factors were not the independent risk factors for failure of VAC therapy.Conclusions Pressure ulcer patients with positive blood cultures,bony exposure,hypoalbuminaemia (alb ＜25 g/L),and wound depth≥4 cm tend to have a higher failure rate in VAC therapy,which are the relative indications for VAC therapy.

In this study,the loop-mediated isothermal amplification(LAMP)method was used to develop a rapid and simple detection system for porcine circovirus type 2(PCV2).According to the PCV2 sequences published in GenBank,multiple LAMP primers were designed targeting conserved sequences of PCV2.Using the DNA extracted from PCV2 isolates HUN-09 and SD-09 as the template,LAMP reactions in a PCV2 LAMP system was performed,the amplification products were detected by adding SYBR Green I and could be observed directly by the naked eye.The results showed highly-efficient and specific amplification in 30 min at 63℃ with a LAMP real-time turbidimeter.Furthermore,PCV2 DNA templates,with a detection limit of 5.5×10-5ng of nucleic acid,indicated that this assay was highly sensitive.The results obtained with the naked eye after SYBR Green I staining were consistent with those detected by the real-time turbidimeter,showing the potential simplicity of interpretation of the assay results.The LAMP assay appeared to have greater accuracy than PCR and virus isolation for the analysis of 18 clinical samples.In addition it offers higher specificity and sensitivity,shorter reaction times and simpler procedures than the currently available methods of PCV2 detection.It is therefore a promising tool for the effective and efficient detection of PCV2.

Objective To observe the effects of human embryonic fibroblasts ( FBs ) on the biological characteristics of diabetic FBs in vitro , and to explore the possible therapeutic mechanism on diabetic ulcers . Methods Diabetic FBs were isolated and cultured with embryonic FBs (experimental group), adult FBs(control group 1)and alone(control group 2)in transwell chambers.Diabetic FBs growth curves in the coculture systems were described.The cell shape was observed by microscopy with HE staining .On the 4th and 7th day of cocul-ture, diabetic FBs were isolated to measure the expression levels of hydroxyproline and transforming growth factor -beta 1(TGF-β1)in the supernatant of monoculture .The cell senescense was displayed by β-galactosidase histo-chemical staining on the 3th, 5th and 7th day of coculture.Eight patients with diabetic foot ulcers who were can-didates for embryonic FBs constructs after failed traditional treatment were included in the study .The pre and post self-control study was conducted by observing the wound healing process before and after embryonic FBs treat -ment.Results Diabetic FBs in the experimental coculture system displayed more typical pattern and faster grow -ing rate.The level of hydroxyproline and TGF-β1 in the supernatant of diabetic FBs monoculture was significantly higher in the experimental group than in the control group after treated 4 or 7 days ( hydroxyproline:4 d, 7 d :compared with control group 1:P=0.023,P=0.007;compared with control group 2: P=0.007, P=0.003;TGF-β1 4 d, 7 d:compared with control group 1:P=0.000, P=0.000;compared with control group 2: P=0.000, P=0.000).The expression of SA-β-gal tended to be more apparent in control group 1 and 2 than in the experimental group on the 3rd,5th,and 7th day.All of the 8 cases with diabetic ulcers were cured by using em-bryonic FBs constructs .Conclusion Embryonic FBs can promote the proliferation of diabetic FBs , correct cell senescence and increase the production of hydroxyproline and TGF-β1,which maybe enhance the process of dia-betic wound healing .

BACKGROUND: A majority scholars views that polymorphism of angiotensinogen T174M gene is one of the susceptible factors of inheritance of coronary heart disease, hypertension and myocardial infarction.OBJECTIVE: To probe into the relationship between the variation of angiotensinogen T174M gene and myocardial infarction.DESIGN: Case-controlled verified experiment.SETTING: Department of Cell Biology, Biochemistry and Molecular Biology, Biological Science Faculty of North China Coal Medical College,and Department of Cardiology in Affiliated Hospital of North China Coal Medical College.PARTICIPANTS: Fifty-five cases of myocardial infarction were collected from outpatients and inpatients in Department of Cardiac Vascular Internal Medicine of Worker's Hospital Affiliated to North China Coal Medical College in Tangshan from September 2002 to September 2003, of which,29 cases were males and 26 cases females, aged (60±8) years. At the same time, 60 cases (health control) were selected from the people who received clinical physical health check (without repeated physical check), of which,32 cases were males and 28 cases females, aged (60±10) years. The cases selected had no manifestation of coronary heart disease, without history myocardial infarction or cerebral infarction in family and the participants were in the know of the research.INTERVENTIONS: Polymerase chain reaction was used to amplify the genetic sequence of No.174 DAN residue involved in No.2 exon of angiotensinogen gene. Electrophoresis was used after variated with Nco I restriction endonuclease.Analysis of restriction fragment length polymorphism was carried on angiotensinogen genotype. Simultaneously,the relevant risk factors of coronary heart disease were detected in two groups, such as blood pressure, body mass, blood lipid, fasting blood glucose, etc.MAIN OUTCOME MEASURES: ① Distribution of genotype, frequency of genotype and frequency of allele in two groups. ② Analysis on risk factors of two groups.RESULTS: Totally 115 cases of objects all accomplished the design and entered result analysis. ① Frequency of angiotensinogen genotype: in myocardial infarction group, TT 75％ (41/55), TM 18％ (10/55), MM 7％ (4/55) and in control group, TT 83％ (50/60), TM 15％ (9/60), MM 2％ (1/60). Frequency of allele of M174 and T174 were 16％ (18/110), 84％ (92/110) and 9％ (11/120), 91％ (109/120) in myocardial infarction group and control group respectively. Frequency of allele of M174 in myocardial infarction group was significantly higher than that of control group (x2=5.79,P ＜ 0.05). By the division of sex, the frequency M and T alleles of both male and female in experiment group was basically identical to control group. Angiotensinogen 174MM genotype in myocardial infarction group was significantly higher than control group (x2= 7.55, P ＜ 0.025). ② Comparison of risk factors: The percentage of smoking history in myocardial infarction group was significantly higher than control group (P = 0.006). After correction of essential risk factors of coronary heart disease, angiotensinogen 174MM gene still increased significantly the risk of myocardial infarction (Odds ratio was 3.66, P= 0.018).CONCLUSION: Angiotensinogen genotype is related to the occurrence of myocardial infarction. M allele is one of the susceptible factors of inheritance of myocardial infarction and T allele prevents from myocardial infarction. The attack of myocardial infarction is not relevant to sex, but angiotensinogen 174TM gene is one of the essential risk factors of myocardial infarction.

Objective To analyze the MRI data of misdiagnosed and missed diagnosed of breast lesions and their histopathological features.Methods Data from 241 breast lesions within 121 patients were recruited in this study.The data included MRI images,uhrasounds and X-ray images were retrospectively interpreted by two radiologist and each lesion was assessed according to the BI-RADS classification.The pathologic features of miss or error diagnosed lesions on MRI were analyzed.Results In 241 breast lesions (malignance 120,bcnign 121),4 lcsions were miss diagnosed on MRI.Thcy were 2 intraductal papillomatosis and 2 fibroadenoma.All was benign.Twenty three lesions were misdiagnosed on MRI.Sixteen were overestimation,including 3 chronic inflammations,3 sclerosing adenosis,2 fibroadenoma,4 fibrocystic changes with or without atypical ductal hyperplasia (ADH),2 intraductal papilloma,1 infiltration of pectoralis major muscle and 1 axillary lymphnode metastasis.Meanwhile,there were 7 lesions were underestimation.These lesions included 2 invasive ductal carcinomas,1 mucinous adenocarcinoma,2 DCIS and 1 blunt duct adenosis with ADH and focal cancerous,1 inflammatory breast cancer underwent chemotherapy.The sensitivity and specificity and accuracy of breast MRI were 95.83 ％ (115/120),72.73 ％ (88/121),84.23 ％ (203/241),respectively.MRI findings had no difference with respect to mammogram or ultrasound was 75.10 ％ (181/241).Conclusion MRI misdiagnosis and missed often occurs in smaller breast lesions,morphologic and hemodynamic malignant manifestation atypical,especially intraductal lesions.MRI diagnosis should be combined with physical examination,X-ray mammogram and ultrasound to improve diagnostic accuracy and reduce missed diagnosis.

Objective To investigate the effect of theanine pretreatment on DNA repair function in neurons during brain ischemia-reperfusion (I/R) injury in rats.Methods One hundred and eight male Sprague-Dawley rats,weighing 290-310 g,aged 15 weeks,were randomly divided into 3 groups (n =36 each) using a random number table:sham operation group (S group),I/R group and theanine pretreatment group (T group).Global cerebral I/R was produced by 4-vessel occlusion method.Bilateral vertebral arteries were electrically cauterized,and bilateral common carotid arteries were clamped for 6 min.Theanine 1 g/kg was injected intravenously at 4 h before clamping bilateral common carotid arteries in T group,and the equal volume of normal saline was given in the other two groups.At 2,6,12,24,48 and 72 h of reperfusion,6 rats were selected in each group and sacrificed,the brains were removed,and the hippocampus was isolated for determination of the number of viable neurons in the hippocampal CA1 region (with a light microscope),apoptosis in neurons in the hippocampal CA1 region (by TUNEL),and expression of DNA repair protein X-ray repair cross-complementing group 1 (XRCC1) and Ku70 (by immunohistochemistry).The apoptotic index was calculated.Results Compared with S group,the number of viable neurons was significantly decreased,and the apoptotic index was significantly increased at 6,12,24,48 and 72 h of reperfusion,and the expression of XRCC1 and Ku70 was significantly down-regulated at 2,6,12,24,48 and 72 h of reperfusion in I/R group (P＜0.01).Compared with I/R group,the number of viable neurons was significantly increased at 12,24,48 and 72 h of reperfusion,the apoptotic index was significantly decreased at 6,12,24,48 and 72 h of reperfusion,and the expression of XRCC1 and Ku70 was significantly up-regulated at 2,6,12,24,48 and 72 h of reperfusion in T group (P ＜ 0.01).Conclusion The mechanism by which theanine pretreatment attenuates brain I/R injury is related to enhancement of DNA repair function and reduction of neuronal apoptosis in rats.
