3.Influence of dexamethasone on airway inflammation and CD4+ CD25+ regulatory T cells of asthmatic rats

Objective To investigate dexamethasone on airway inflammation and CD4+ CD25 + regulatory T cells (CD4+ CD25 +Tr) of asthmatic rats,and elucidate the possible mechanism of dexamethasone in treatment of asthma.Methods 30 Wistar rats were randomly divided into control group,asthma group and dexamethasone-treated group.Bronchoalveolar lavage fluid (BALF) was collected,and cytology study was conducted.The lung tissue was obtained and pathologic analysis was done through HE stain.Flow eytometry was used to detect the CD4+ CD25 +Tr ratio in PBMCs.Results Total cells number,the percentage of lymphocytes,neutrophils and eosinophils (Eos)in BALF of dexamethasone-treated group were lower than that of asthma group (P<0.05,P<0.01).Compared with the asthma group,less infiltration of inflammatory cells in lung tissues was observed in the dexamethasone-treated group.CD4+ CD25 + Tr of asthma group was lower than that of control and dexamethasone-treated group (P<0.05).Conclusion Dexamethasone could suppress airway inflammation of asthmatic rats,which probably be due to increasing the number of CD4+ CD25 + Tr.
4.Influence of danshen injection combined with dexamethasone on CD4+CD25+ regulatory T cells of asthmatic rats

Objective To investigate the immunological mechanism of inhibitory effect of Danshen injection combined with dexamethasone(DXM) on asthmatic airway inflammation.Methods 50 Wistar rats were randomly divided into normal control(NC),asthma,Danshen,DXM and Danshen+DXM group.Cytology study of Bronchoalveolar lavage fluid(BALF) was conducted.Pathology of lung tissue was done through HE.Flow eytometry was used to detect CD4+CD25+ regulatory T Cells(CD4+CD25+ Treg) ratio in peripheral blood mononuclear cells(PBMCs).IL-4 and IL-5 levels in BALF were detected by ELISA.Results Total cells number,percentage of lymphocytes,neutrophils and eosinophils(Eos) in BALF of the three treated groups were lower than that in asthma group(P<0.05,P<0.01),particularly in Danshen+DXM group,which showed significant difference as compared with the other two treated groups(P<0.05).There was severe inflammation in lung tissue of asthma group,moderate inflammation in Danshen group and DXM group,and no inflammation of Danshen+DXM group.CD4+CD25+ Treg/CD4+ T ratio in the three treated groups were higher than that in asthma group,and the levels of IL-4 and IL-5 were lower than those in asthma group(P<0.05).In Dansben+DXM group,it showed significant difference on the change of CD4+CD25+ Treg,IL-4 and IL-5 as compared with other treated groups(P<0.05).Conclision Danshen injection combined with DXM could suppress airway inflammation in asthmatic rats,which may be through increasing the expression of CD4+CD25+ Treg,decreasing the levels of IL-4 and IL-5 and resuming the balance of Th1/Th2.
5.Use of gentamicin solution in transurethral ureteroscopic lithotripsy to prevent postoperative infection

Objective To evaluate the efficacy of gentamicin solution in transurethral ureteroscopic lithotripsy to prevent postop- erative infection.Methods Prospective clinical randomized control study was conducted.From July 2003 to June 2006,116 ca- ses of ureteral stones at high risk of postoperative infection were randomized into control group or gentamicin group.Patients in gentamicin group received gentamicin solution for washing in the operation.All the patients undergoing operation were followed up for 2 weeks after operation.Diagnosis of postoperative infection was based on clinical manifestations.Results A total of 109 patients received operation in all the 116 cases,including 58 cases in gentamicin group and 51 cases in control group.Thirteen cases of postoperative infection were identified in all the patients receiving operation (11.93%),3 cases in gentamicin group and 10 in control group.The incidence of postoperative infection was significantly different between the two groups (X~2= 5.3342,P=0.0209).Eight cases had positive bacterial culture.Of the microbiological isolates,2 were gram-positive bacteria, 5 gram-negative bacteria and 1 Candida albicans.Conclusions The most common pathogen causing postoperative infection after transurethral ureteroscopic lithotripsy is gram-negative bacteria.The use of gentamicin solution for washing in the operation can reduce the incidence of postoperative infection.

Objective To explore effects of heat stress response on neutrophil apoptosis and respiratory burst function. MethodsNeutrophils from each of 18 healthy volunteers were divided into 3 parts,and one part was served as control group,and the other two parts were induced by heat shock or cadmium chloride for heat stress response and named as heat shock group and cadmium chloride group.The neutrophils were incubated in culture medium.At 0,2,3,4,and 6 h following heat stress induction,the heat shock protein(HSP70) expression and the respiratory burst were detected in the neutrophils respectively by using PCR technique and flow cytometer.Level of apoptosis was observed by immuno-fluorescence and flow cytometer DNA ploid at 24 h after heat stress induction. The theoretical basis of the alcohol precipitation process control was provided, the alcohol precipitation was optimized and the relationship equation was got. The monod glycoside, loganin and paeoniflorin were used as the evaluation indexes to determine the impact factors of alcohol precipitation techniques of Liuwei Dihuang decoction by the Plackett-Burman experimental design and the levels of non-significant factors were identified. Then, Box-Behnken response surface methodology was used to research and discuss the critical process parameters influence the effect of alcohol precipitation and draw interaction between key process parameters and the correlation equation with index components. Through the establishment and solving the quadratic regression model of composite score, the optimum preparation conditions of alcohol precipitation techniques of liuwei were as follows: stirring speed was 580 r x min(-1), standing time was 17 hours, alcohol concentration was 34%, the density of Liuwei Dihuang decoction was 1.13. The response surface methodology for optimized alcohol precipitation techniques of Liuwei Dihuang decoction is. reasonable and feasible.

Objective:To evaluate the clinical efficacy of SKy bone expander system in percutaneous kyphoplasty for treat- ment of osteoporotie verterbral compression fracture.Methods:Twenty-two patients(aged 62-90 years,32 vertebrae)under- went percutaneous kyphoplasty using SKy bone expander system.The bone cement was injected into the collapsed vertebrae. The vasual analogue scale(VAS)and complications were recorded during follow up.Results:The operations were successful in all patients via unilateral or bilateral approach.The operation time ranged from 30 to 120 min.The mean volume of cement in- jected into each vertebra body was(4.8?1.1)ml,ranged from 3.1 to 6.8 ml.Extravertebral leakage of bone cement was ob- served in two vertebrae with no symptoms.All patients had their pain relieved;the VAS was 7.6?0.8 before operation,3.5?0.5 one day after operation,2.8?0.6 one week after operation,and 2.4?0.6 one month after operation,with significant difference found between preoperation and postoperation(P

Objective To determine the status of false-positive report of congenital syphilis (CS),to analyze the possible causes of mis-diagnosis.Methods Basic information on CS in Shanghai in the past five years was collected.We identified infants diagnosed with CS and followed up the sero- logical reactivity of those patients and their mothers.The serological reactivity[rapid plasmin reagin (RPR)and treponema palidum hemagglutination assay(TPPA)]of infants was followed-up for up to 24 months,or until both antibodies turned to negative.The medical history of the mothers was col- lected,and their sera were examined for syphilitic antibodies.Results Total 99 infants diagnosed with CS were recruited. The major diagnostic method was treponemal antibody detection.Only 31.3% of the 99 infants exhibited clinical symptoms or syphilis-like symptoms at delivery.The cumu- lative RPR loss rates of the infants were 44.2%,64.0%,72.7%,83.9% and 87.1% at 1-3,3-6,6- 12,12-18 and 18-24 months after birth,respectively.The cumulative TPPA loss rates were 1.1%, 18.6%,44.6%,66.7% and 74.4% for 1-3,3-6,6-12,12-18 and 18-24 months after birth,respec- tively.TPPA remained positive in all mothers with syphilis.Conclusion The diagnosis of congenital syphilis determined solely by the positive tests of RPR and TPPA is unreliable and can be misdiagno- sis.The diagnosis and management of congenital syphilis should be urgently improved,and that the profes- sional health institutions should perform and closely monitor the quality controls in the diagnosis of CS and standardize the intervention strategy of maternal syphilis.
