2.Effect of ventilation with low tidal volume and positive end-expiratory pressure in different periods on postoperative pulmonary function and short-term prognosis in elderly patients with abdominal surgery

Objective To observe the effects of ventilation with low tidal volume and positive end-expiratory pressure (PEEP)in different periods on the postoperative pulmonary function and short-term prognosis in aged patients undergoing abdominal surgery.Methods Sixty aged patients undergoing selective open abdominal surgery scheduled for general anesthesia,21 males and 39 fe-males,were randomized into 3 groups (n =20).Patients in group A received PEEP 1 h after the be-ginning of surgery;patients in group B received PEEP 1 h before tracheal extubation;patients in group C received PEEP intraoperatively.The secretion score in preoperative,postoperative 24 h and 72 h respectively,and the arterial blood gas analysis indexes (PaCO 2 ,PaO 2 ,A-aDO 2 ,PaO 2/FiO 2 calculation)in postoperative 1 h and 24 h were recorded.Results Compared with preoperative,in postoperative 1 h,PaCO 2 increased obviously in all groups,PaO 2 decreased in group B,A-aDO 2 in-creased in group A (P <0.05);in postoperative 24 h,PaCO 2 was significantly increased in group B and C,PaO 2/FiO 2 decreased in group B (P <0.05).Compared with postoperative 1 h,in postopera-tive 24 h,PaCO 2 and A-aDO 2 decreased obviously in group A (P <0.05).There were no differences in postoperative secretions score in between the 3 groups.Conclusion Low tidal volume combined short-range PEEP in different periods of surgery may improve postoperative pulmonary oxygenation. But they had no obvious help with postoperative pulmonary complications.
3.Determination of bisphenol A in water by truncated aptamer-fluorescence method

Objective:
To establish a fluorescence method based on turncated aptamer for the determination of bisphenol A in water.
Methods:
The bisphenol A truncated aptamer containing 38 bases was selected as a recognition module, and was modified with the fluorophore 6-FAM at the 5'end. The 3'end of the complementary sequence cDNA was modified with the quencher DABCYL. The standard solutions of bisphenol A and interfering compounds were configured. The detection system was established after optimizing the number of bases in cDNA, the concentration ratio of truncated aptamer to cDNA, the incubation temperature and time, and the pH of the buffer. The specificity and recovery experiments were carried out.
Results:
When the complementary sequence cDNA included 9 bases, the concentration ratio of the truncated aptamer to cDNA was 1:1.5, the pH value of the buffer solution was 7.5, the cDNA was incubated at 55 ℃ for 60 minutes, in the concentration range of 10-75 pmol/L, the linear regression equation was y=2 230.7x+110 825, the correlation coefficient was 0.926. The limits of detection was 3.3 pmol/L. The difference values of fluorescence intensity between tetrabromobisphenol A, estradiol, estriol, bisphenol S and bisphenol A were obviously different, so there was no significant interference to the test result. The recovery rates were 97.8%, 98.8% and 102.3% with the spiked concentrations of 20.0, 40.0 and 60.0 pmol/L. The relative standard deviations were 4.4%, 2.1% and 2.6% (n=5), respectively.
Conclusion
The fluorescence method based on turncated aptamer has the advantages of easy operation, high sensitivity and specificity, which can be used for the determination of bisphenol A in water.
4.The effect comparison of anterograde reamed intramedullary nail and dynamic compression plate for the management of diaphyseal fractures of the humerus

Objective To compare the clinical effects in patients treated with anterograde reamed intramedullary nail (IMN) and dynamic compression plate (DCP) for diaphyseal fractures of the humerus.Methods Forty-seven patients with diaphyseal fractures of the humerus,were divided into two groups by random digits table:IMN group (23 cases) and DCP group (24 cases),IMN and DCP were performed fixation,respectively.After the surgery,perioperative,functional outcomes of shoulder and elbow,postoperative complications between two groups were compared.Results The average follow-up time was (22.3 ± 5.7)months,3 cases were lost in 12-month-follow-up,1 case in IMN group and 2 cases in DCP group.The operation time,intraoperative blood loss in IMN group were significantly shorter than those in DCP group [(67.7 ± 15.3) min vs.(87.0 ± 12.5) min,(106.3 ±57.6) ml vs.(226.7 ±60.2) ml,P<0.05].The incidence rate of subacromial impingement in IMN group was significantly higher than that in DCP group [18.2% (4/22) vs.0,P < 0.05].The incidence rate of wound infection in IMN group was significantly better than that in DCP group [4.5%(1/22) vs.22.7%(5/22),P < 0.05].There were no statistically significant differences between two groups in other indexes (P > 0.05).Conclusions DCP has some advantage in perioperative period,but they are comparable in the functional outcomes of shoulder and elbow and complications.
5.The expression and role of BAMBI in pancreatic cancer

Objective To investigate the expression of BAMBI in pancreatic carcinoma tissue and its clinical value.Methods Expression of BAMBI was detected by immunohistochemistry in 69 cases of pancreatic cancer and 15 normal pancreatic samples.The survival and its association with clinicopathologic parameters were analyzed.Results The expression of BAMBI in the cancer tissue was much higher than that in the normal tissues (56.5% vs 16.7%,P <0.01).The expression of BAMBI was not associated with age,sex and tumor size,differentiation,but expression of BAMBI in patient with lymph node metastasis was higher than that in patient without lymph node metastasis (67.4% vs 38.5%,P =0.019),the expression of BAMBI in TNM stage Ⅲ was higher than that in stage Ⅰ + Ⅱ (75.0% vs 31.0%,P =0.001).The median survival of BAMBI-positive group (6 months) was lower than that of BAMBI-negative group (10 months).Conclusions BAMBI is over-expressed in pancreatic carcinoma,and its expression is associated with lymph node metastasis,TNM staging,differentiation and prognosis.
6.Ultrastructural changes of reconstructed mandibular condylar cartilage under continuous mandibular advancement

BACKGROUND:Previous animal experiments have demonstrated that mandibular advancement can cause the remodeling of temporomandibular joint tissue of young SD rats. This is mainly characterized by accelerated growth rate of the condyle tissue and secondary growth of mandible. But the ultrastructural remodeling of condylar chondrocytes remains poorly understood. OBJECTIVE:To observe the histological and ultrastructural variations of reconstructed condylar cartilage of young rats under the effect of continuous mandibular advancement. METHODS:SD rats aged 4 weeks were randomly divided into control and experimental groups. Rats in the experimental group were subjected to mandibular advancement for 24 hours and sacrificed at 3, 7, 14, 21 and 30 days of intervention. Condylar cartilage samples were harvested and their histological and ultrastructural changes were observed under optical microscope and transmission electron microscope. RESULTS AND CONCLUSION:After 14 days of intervention, the thickness of condylar cartilage in the experimental group increased first and then became thin in the period of observation. The cartilage thickness variations in the postmedian condylar were significant (P < 0.01). After 7 days of intervention, the ultrastructure of condylar chondrocytes was reconstructed, including intracelular karyopyknosis, rough endoplasmic reticulum compartment sweling, smaler even absent lipid droplets, less and irregular microfilaments around the nucleus, broadened and increased extracelular matrix and the emergence of large gaps. These results demonstrate that under continuous mandibular advancement, the rat condylar cartilage wil become thick or thin with the endurance time, and chondrocyte matrix synthesis ability wil be significantly enhanced.
7.Design of a portable oxygen supply respirator

Objective To design a portable respirator that can facilitate both cardiopulmonary resuscitation and auxiliary oxygen inhalation.Methods Positive pressure design as well as pneumatically powered and controlled design were adopted for this respirator.Theoretical calculation of key components was carried out.The virtual prototype was established and the structure checked using the three dimensional design software Solidedge.Finally,the prototype was processed,assembled and tested.Results Experimental tests proved that the designed respirator was capable of cardiopulmonary resuscitation and auxiliary oxygen inhalation when oxygen supply pressure reached 3 -5.5 bar.It could achieve the expected technical indicators,such as the oxygen flow rate that was between 10 and 12 L/min.Conclusion The portable oxygen supply respirator has the advantages of a small size,light weight,simple operation,and automatic power supply.It has broad prospects of application.
8.Expression and diagnostic value of 14-3-3 protein in brains of patients with Creutzfeldt-Jakob disease

Objectives To investigate the expression and diagnostic value of 14-3-3 protein in brains of patients with sporadic Creutzfeldt-Jakob disease(sCJD).Methods 14-3-3 protein was immunohistochemically analyzed in tissue from the frontal lobe of 5 patients with sCJD and 4 non-CJD eases Using 14-3-3 ?and ?antibodies with reference to the results of KB,GFAP and PrP detection.Results The expressions of 14-3-3 protein in five brains of sCJD were more obviously,mostly in gray matters and astrocytes in three cases.The concentration was related to PrP deposition type,but not related to prion protein genotype.Except few expression of 14-3-3 protein in neurous of two cases of acute contusion,there were no expression in the other two cases in control group.Conclusions The expression of 14-3-3 protein in brain is useful to pathological diagnosis of CJD.
9.Research process of reactive oxygen species-based tumor immunomodulation

Reactive oxygen species (ROS) is defined as the electron reduction product of oxygen with high reactivity which can maintain normal physiological functions and redox homeostasis. The tumor microenvironment is in a state of oxidative stress. ROS can affect multiple processes of tumor immune response by modulating the phenotype and functions of tumor cells and immune cells. With the rapid development of immunology, ROS-based tumor immunomodulation has been widely concerned and studied. In this review, the mechanism of ROS participating in tumor immune response is elaborated. Meanwhile, the research process and application of ROS in tumor immunomodulation in recent years are reviewed and analyzed.
10.Surgical strategy for treatment of type A aortic dissection with small true lumen of the descending aorta

Objective:To summarize the results and methods of surgical treatment for type A aortic dissection with small true lumen of the descending aorta.Methods:9 patients underwent surgical treatment for type A aortic dissection with small true lumen of the descending aorta between January 2017 and December 2019 were analyzed retrospectively. There were 7 males and 2 females, mean age of (41.6±9.2) years. Acute dissection were 2 cases, and chronic dissection were 7 cases. Preoerative computed tomography was used to diagnose the dissection and evaluate the true lumen of the descending aorta. This procedure was done in all patients via a median sternotomy under hypothermic CPB with SCP. 4-branched prosthetic graft was used to replace the ascending aorta and aortic arch. The procedures involving the descending aorta: Hybrid surgery using TEVAR. Distal intimal flap fenestration. Implanting the intraoperative stent-graft or prosthetic graft at false lumen for second-step operation.Results:There was no in-hospital mortality. Stroke, Spinal cord, visceral ischemia and lower limbs malfunction were not observed. Reintervention was not found in case with acute dissection during follow-up. One patient who reveived fenestration underwent TEVAR, others with chronic dissection underwent thoracoabdominal aortic replacement 3 months after surgery.Conclusion:Hybrid or staged procedures was a suitable alternative to patients with type A aortic dissection with small true lumen of the descending aorta.
