1. Clinical study of two-trocar laparoscopy-assisted ventriculoperitoneal shunt in the treatment of hydrocephalus

Objective • To compare the clinical outcomes between two-trocar laparoscopy-assisted ventriculoperitoneal shunt (LAVPS) and conventional open ventriculoperitoneal shunt (OVPS), and explore the clinical practice and experience of the placement of distal catheter in two-trocar LAVPS. Methods • A total of 308 patients with hydrocephalus who underwent ventriculoperitoneal shunt (VPS) from January 2016 to December 2018 in the Department of Neurosurgery at Renji Hospital, Shanghai Jiao Tong University School of Medicine were analyzed retrospectively. Among them, there were 90 patients in the LAVPS group (Group L), and the two-trocar method and the original suture loop method were adopted to place the distal catheter in the right hepato-diaphragmatic space. For the other 218 patients in the conventional OVPS group (Group O), the laparotomy approach was adopted to put the distal catheter into the left lower abdominal cavity through the midline incision. The operation time and complications of the distal catheter between the two groups were compared. Results • Compared with Group O, the mean operation time was significantly reduced in Group L (54 min vs 90 min, P=0.000), and the incidence of distal catheter complications was also significantly decreased (0 vs 9.6%, P=0.002). In Group L, only one case of visceral injury, one case of distal catheter migration and one case of proximal catheter obstruction occurred. The incidences of infection and obstruction of the distal catheters were 0 in Group L, significantly lower than those in Group O (0 vs 4.6%, P=0.039; 0 vs 5.0%, P=0.030). In Group O, 10 cases of distal catheter infection, 11 cases of obstruction, 4 cases of visceral injury and 2 cases of incisional hernia occurred. Conclusion • The modified two-trocar LAVPS, of which the distal catheter was guided by a suture loop method and placed in the right hepato-diaphragmatic space, is a safe, economical and simple surgical procedure that is more effective in treating hydrocephalus of various origins than conventional OVPS.
2.Effect of SiteRite5 vascular ultrasound and micro intubation scabbard technology on indwelling double channel power peripherally inserted central catheters ( Power PICC) in upper arm

Objective To observe the successful puncture rate,complications,the liquid flow rate and the high-pressure-injection status of upper arm indwelling double-channel Power PICC with SiteRite5 vascular ultrasound and micro inmbation scabbard technology.Methods Totals of 106 patients received PICC randomly divided into control group(53 cases) and observation group(53 cases),and two groups were assessed vessel by SiteRite5 vascular ultrasound of American Bard and used micro intubation scabbard technology in indwelling double-channel Power PICC.Control group used there valve type double-channel PICC catheter of Bard,while observation group used double-channel Power PICC catheter.Then successful puncture rate,complications ratio,the way and effects of high-pressure-injection when received enhanced CT examination in two groups were compared.Results The successful puncture rate of two groups was 100%,respectively.Complications ratio of control group was 7.41% and observation group was 0.00%,the difference was statistical significant( x2 =5.76,P<0.05).Observation group received enhanced-CT examination that can directly connected with highpressure-injection,while 100% patients of control group needed peripheral large veins puncture again; liquor exosmosis and local swelling ratio and local pain,unwell ratio of control group was 17.02% and 25.53%,and that of observation group was 0.00%,and the difference was statistical significant ( x2 =18.60,29.27 ; P <0.01 ).Conclusions Upper arm indwelling double-channel Power PICC with SiteRite5 vascular ultrasound and micro intubation scabbard technology has a safety operation,wiresoft,high successful puncture ratio,low complication ratio,liquor velocity and flow reach clinical requirement,and can use enhanced-CT to make up the deficiency of existing PICC puncture,no risk for liquor exosmosis,and can increase Catheter utilization ratio,effectively decrease the workload of nurses,and improve the satisfaction of patients,which has a better clinical utilization value.
3.Development and preliminary application of the measurement scale for medical students' professionalism cognition

Objective:To explore the present situation of medical students' professionalism cognition and provide reference for medical personnel training and medical education.Methods:A cross-sectional survey was conducted on medical students among 2 affiliated hospitals of a medical university in H province by cluster sampling. Exploratory factor analysis and reliability and validity test were carried out and a descriptive analysis of the present situation of professionalism was conducted by SPSS 20.0. Amos21.0 software was used for a confirmatory factor analysis on the samples.Results:The measurement scale had good reliability and validity, and the Cronbach alpha coefficient of the scale was 0.901. The average professionalism score of the medical students was about 80 points. The score of "physical and mental status and self-development ability" was the highest (83.65 points), and the lowest score was "academic ability" (72.21 points). There was a little difference in the professionalism cognition between the two hospitals, with significant differences between the dimension "respect and care" and "responsibility".Conclusion:This study has initially formed a medical professionalism measurement scale with good reliability and validity. The professionalism of medical students in the 2 affiliated hospitals of a medical school in Province H is in good condition as a whole. In the future, medical education should pay more attention to the combination of basic professional knowledge and clinical practice of medical students, and change the training model of medical students in order to improve the academic ability of medical students and medical students' overall cognition of professionalism.
4.Implementation of undergraduate tutorial system of biomedical engineering specialty in medical college

Objective To explore the tutorial system of biomedical engineering students in medical colleges and universities,and provide useful references for the implementation of undergraduate tutorial system in medical colleges and universities in China.Methods Based on the compulsory professional knowledge and skills of biomedical engineering students,the undergraduate tutorial system of biomedical engineering specialty was analyzed and summarized with thatof School of Biomedical Engineering of the Fourth Military Medical University taken as an example.Results The necessity,problems and new mode were pointed out for the undergraduate tutorial system of biomedical engineering specialty.Conclusion The implementation of undergraduate tutorial system is a new idea of deepening the reform of undergraduate education in biomedical engineering specialty,and is of important significance to improve the professional skills of students.
5.Significance of No.14v lymph node dissection for advanced gastric cancer undergoing D2 lymphadenectomy.

OBJECTIVETo elucidate the necessity of No.14v lymph node dissection in D2 lymphadenectomy for advanced gastric cancer.
METHODSClinicopathological data of 131 cases of advanced gastric cancer receiving D2 or D2+ plus No.14v lymph node dissection were reviewed retrospectively. Clinicopathological factors associated with No.14v lymph node metastasis were analyzed and prognostic value of No.14v lymph node metastasis was evaluated.
RESULTSOf the 131 patients, 24 (18.3%) had positive No.14v lymph node. The incidence of 14v metastasis was associated with tumor location, tumor size, depth of invasion, N staging, TNM staging, No.1, No.6, and No.8a lymph nodes metastasis. Tumor location and N staging were independent risk factors for No.14v metastasis (all P<0.05). The 5-year survival rate was 8.3% and 37.8% in patients with and without No.14v metastasis respectively. The difference was statistically significant (P<0.01). Multivariate analysis revealed that metastasis of No.14v was an independent prognostic factor for advanced gastric cancer after D2 lymphadenectomy (P=0.029, RR=1.807, 95%CI:1.064-3.070).
CONCLUSIONSFor advanced middle and lower gastric cancers, especially those with larger size, serosa invasion and possibility of No.6 lymph node metastasis, it is necessary and feasible to remove the No.14v lymph node.

6.Influencing factors for posttraumatic hydrocephalus in patients suffering from severe traumatic brain injuries.

OBJECTIVETo detect the influencing factors for posttraumatic hydrocephalus in patients with severe traumatic brain injuries and provide theoretical reference for clinical treatment.
METHODSRetrospective study was made on 139 patients with severe traumatic brain injuries in our hospital. The patients were divided into two groups: hydrocephalus group and non-hydrocephalus group. Single factor analysis and multiple factor analysis were used to determine the related factors and hydrocephalus. Multiple factor analysis was conducted with logistic regression.
RESULTSPosttraumatic hydrocephalus was found in 19.42% of patients. Age(OR equal to 1.050, 95%CI: 1.012-1.090), decompressive craniectomy (OR equal to 4.312, 95%CI: 1.127-16.503), subarachnoid hemorrhage(OR equal to 43.421, 95%CI: 7.835-240.652) and continuous lumbar drainage of cerebrospinal fluid (OR equal to 0.045, 95%CI: 0.011-0.175) were screened out from nine factors as the influencing factors for posttraumatic hydrocephalus.
CONCLUSIONSRisk factors for PTH are as follows: age, decompressive craniectomy and subarachnoid hemorrhage (SAH). Continuous lumbar drainage of cerebrospinal fluid can greatly reduce posttraumatic hydrocephalus.

7.Prognosis analysis of 247 cases of gastrointestinal stromal tumor.

OBJECTIVETo study the clinicopathologic features and prognostic factors of gastrointestinal stromal tumor (GIST).
METHODSClinicopathologic data of 247 patients with GIST from January 2003 to November 2012 in the Affiliated Hospital of Qingdao University Medical College, and the prognostic factors were evaluated retrospectively by univariate and multivariate analysis with Log-rank test and Cox proportional hazard model.
RESULTSPatients were followed up with a median time of 26 months (1 to 113 months). Twenty-six patients developed recurrence or metastasis, and 18 died of GIST. The 1-, 3-, 5-year survival rates were 94%, 91% and 83% respectively. Univariate analysis showed that age, tumor location, tumor size, mitotic count and tumor rupture were predictive factors of survival after resection of primary GIST (all P<0.01). For patients at intermediate and high risk to relapse, imatinib group had a higher 5-year overall survival rate than non-imatinib group (85.7% vs. 81.0%, P<0.05). Multivariate analysis revealed that tumor size (RR=2.248, 95%CI:1.081-4.677, P=0.030), mitotic count (RR=2.220, 95%CI:1.032-4.776, P=0.041) and tumor rupture (RR=5.183, 95%CI:1.677-16.017, P=0.004) were independent prognostic factors.
CONCLUSIONSTumor size, mitotic count and tumor rupture affect the prognosis after resection of primary GIST independently. Imatinib adjuvant therapy can improve overall survival of patients at intermediate and high risk to relapse after surgery.

8.Preparation and identification of anti human myocardium troponin I monoclonal antibodies

Objective: To prepare monoclonal antibodies (McAb) with cardiac troponin I (cTnI) which was purified from fresh human cardiac muscle within 6 h. Methods: (1) Extraction and purification of human cTnI: cTnI was purified by high salt extraction, saltless precipitation, 65℃ treatment, ammonium sulfate fractionation and DEAE-cellulose chromatography, etc. (2) Preparation of anti human cTnI McAb: The purified cTnI was injected into the spleen of BALB/c mice. The cTnI-primed spleen cells were fused with Sp2/0 myoloma cell. The McAbs anti human cTnI were obtained by screening with indirect ELISA and 3 times clone. (3)The identification of anti cTnI McAb. Results: Five hybridoma cell lines, named 3A7,3A11,3D2,3F10 and 1H9 were developed, which could secret McAb stably. The 5 McAbs all were demonstrated to be IgG2a by double gel diffusion test. The number of hybridoma chromosomes was between 92 to 110 and the chromosomes were mainly telocentric. Five kinds of ascites had no cross-reaction to LDH,CK,CK-MB ,AST and cardiac troponin T(cTnT), and their titers were between 3.2×10-6 to 1.6×10-7. Conclusion: 3D2,3F10 and 3A7,3A11,1H9 react to different epitopes of cTnI.

10.Clinical Observation on the Modified Huangan Lipi Decoction Combined with Acupuncture at Sifeng Points in the Treatment of Children with Tic Disorders

Objective To observe the clinical efficacy of the modified Huangan Lipi Decoction(mainly composed of Sclerotium Poriae Pararadicis,Paeoniae Radix Alba,Atractylodis Macrocephalae Rhizoma,Polygalae Radix,Acori Tatarinowii Rhizoma,and Curcumae Radix)combined with acupuncture at Sifeng(EX-UE10)points in the treatment of children with tic disorders(TD).Methods Seventy cases of TD children with spleen deficiency and liver hyperactivity syndrome were randomly divided into the treatment group and the control group,with 35 cases in each group.The control group was treated with conventional western medicine of Tiapride Hydrochloride Tablets,while the treatment group was treated with the combination of the modified Huangan Lipi Decoction and acupuncture at Sifeng points.Both groups were treated for 8 weeks.The changes of the Yale Global Tic Severity Scale(YGTSS)item scores of tic muscle group,tic frequency,tic intensity,complexity and interference degree as well as their total scores in the two groups were observed before and after treatment.Moreover,the clinical efficacy and safety in the two groups were evaluated.Results(1)After 8 weeks of treatment,the total effective rate of the treatment group was 88.57%(31/35),and that of the control group was 68.57%(24/35).The intergroup comparison showed that the total effective rate(tested by chi-square test)and the overall therapeutic efficacy(tested by rank-sum test)of the treatment group were significantly superior to those of the control group,and the differences were statistically significant(P<0.05).(2)After treatment,the YGTSS item scores of tic muscle group,tic frequency,tic intensity,complexity and interference degree as well as their total scores in the two groups were significantly lower than those before treatment(P<0.01),and the effect on lowering the above scores of the treatment group was significantly superior to that of the control group,the differences being all statistically significant(P<0.01).(3)During the treatment,the incidence of adverse reactions in the treatment group was 2.86%(1/35)and that in the control group was 8.57%(3/35).The intergroup comparison showed that the incidence of adverse reactions in the treatment group tended to be lower than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion Modified Huangan Lipi Decoction combined with acupuncture at Sifeng points exert certain effect in the treatment of TD children with spleen deficiency and liver hyperactivity syndrome,and its efficacy is superior to that of the western medicine Tiapride Hydrochloride Tablets.
