1.Vitrectomy combined with inferior scleral buckling for a special kind of retinal detachment

AIM:To evaluate the efficiency of vitrectomy combined with inferior scleral buckling for a special kind of retinal detachment.
METHODS: Nineteen eyes of special retinal detachment had following features: 1 ) the course was more than 6mo;2) there were at least one hole located in the 5 : 00 - 7 : 00 of the marginal retina; 3 ) the detachment range of retina more than 270 degrees and retinal proliferation in retinal detachment. The 19 eyes who received vitrectomy combined with inferior scleral buckling, retinal anatomic reattachment and visual function recovery was observed.
RESULTS: Among these 19 eyes, all eyes retinal anatomic reattachment and best corrected visual acuity ( BCVA ) was improved in various degrees. The BCVA was 0. 01-0. 1 in 5 eyes, 0. 12-0. 3 in 9 eyes, and ≥0. 4 in 5 eyes.
CONCLUSION: It is an effective method to vitrectomy combined with inferior scleral buckling for the special kind of retinal detachment which has following features:1) the course was more than 6mo;2) there were at least one hole located in the 5:00-7:00 of the marginal retina;3 ) the detachment range of retina more than 270 degrees and retinal proliferation of cable in retinal detachment.

Herein, we document the discovery and biological profile of new anti-HIV-1 drugs approved by FDA between 2005 and 2008 and some drug candidates are also discussed.
3.Practice and thinking of assessment scale design for simulated clinical skills examination

This article described systematically the design basis,passing standard and influential factor of test score of the assessment scale of simulated clinical skills examination.Through developing and modifying OSCE examination scale design for many years,we have found that the use of 360-degree assessment for designing OSCE assessment rating scale can effectively reduce the impact on the score of examinee owing to the environment,the candidates,raters and question design,which has an important role for scientific development of OSCE assessment scales,as well as improving the reliability and validity for development of a scientific OSCE assessment.
4.Study on influences of Kuiyu dection on contents of IL-6 and IL-8 in patients with ulcerative colitis

ObjectiveTo observe the effect of Kuiyu dection on ulcerative colitis and to explore its mechanism.Methods120 patients with ulcerative colitis were randomly divided into a treatment group and a control group,60 cases in each group.The treatment group was treated with Kuiyu dection retention enema,1 time a day,15 days for a course of treatment; and the control group was treated with SASP orally,2 g every time,3 times a day,and 15 days for a course of treatment.All patients with a course of treatment for the next 7 days after the rest period of treatment,evaluated after 3 courses.Comprehensive clinical efficacy,the main symptoms,score of integral electronic colonoscopy examination,and the changes of IL-6,IL 8 levels were observed in both groups after treatment with complete remission and six months followed-up,besides,the recurrence rate was also observed.ResultsThe total effective rate was 93.33% in the treatment group,better than the control group 78.33% (x2 =9.027,P<0.05) ; the treatment group also showed better results in diarrhea,pus and blood improvement than the control group (x2=5.630、x2=5.972,P<0.05),the treatment group could significantly reduce UC Serum IL-6,IL-8 level (x2=8.314,P<0.01); electronic colonoscopy examination points,hyperemia,erosions,ulcers in both groups were significantly improved,but the treatment group was significantly better than the control group (x2=8.322,P<0.05) ; 6 months followed-up and the recurrence rate in treatment group were better than control group,and the difference was statistically significant (x2=5.857,P<0.05).ConclusionThrough decreasing the IL-6,IL 8 levels,Kuiyu dection can treat ulcerative colitis,with significant effect,non-toxic side effects,inexpensive,and easy for use.
5.Effects of exogenetic miR-34a on radiosensitivity of H1299 cells

Objective To investigate the influences of miR-34a on the radiosensitivity of H1299 cells.Methods CCK-8 kit was used to examine the viability of H1299 cells which were exposed to different doses (0,2,4,6 and 8 Gy) of 60Co γ-rays after transient transfection of pre-miR-34a.Apoptosis rate and cell cycle were measured with flow cytometry.The expression levels of miR-34a target genes,bcl-2,bax,CDK4,CDK6 and cyclinD1 were analyzed by real-time PCR.Results Compared to the control group of negative transfection,the cell viability in pre-miR-34a transfection group decreased significantly after irradiation at0,2,4,6,8 Gy (t=-2.39,-3.12,-4.98,-4.03,-3.06,P<0.05) in a dose-dependent manner.After being irradiated with 6 Gy γ-rays,the apoptotic rate in pre-miR-34a transfection group was significantly increased (t =7.06,P < 0.05) together with an accumulation of G0/G1 phase (t =3.94,P < 0.05) and a reduction of S phase (t =6.23,P < 0.05).The gene expression levels of bcl-2,CDK4 and CDK6 in pre-miR-34a transfection group were respectively decreased (t =3.39,12.88,6.21,P < 0.05) of negative control.cyclinD1 was also decreased but no significance,while bax was increased to 1.94 times of negative control (t =-4.35,P < 0.05) together with a decrease of bcl-2/bax.Conclusions miR-34a could promote cell apoptosis,induce G0/G1 phase accumulation,suppress cell activity,and in turn increase the radiosensitivity of H1299 cells.
6.Clinical analysis of the cause of local recurrence after Dixon operation for rectal cancer

Objectives:To discuss and analyse the causes of local recurrence after Dixon operation. Methods:Retrospective analysis was made on 72 cases after resection of rectal cancer by Dixon operation in our department from 1995 to 1999. Results:The local recurrence rate after Dixon operation was 12.5%(9 cases),the recurrence time was 3~26 months,and 16.2 months in an average after the operation.Seven cases of recurrence were within 2 years.The recurrence location occurred at the anastomotic stoma (6 cases),pelvic cavity (2 cases) and the perinum (1 cases) respectively.Based on Dukes classification, it showed one case of phase A, three cases of phase B and five cases of phase C.According to pathological classification, there were one case of papillary adenocarcinoma,five cases of rubiformadenocarcinoma and three cases of mucoid adenocarcinoma.A length from the lower margin of the tumors to the distal resection site,seven cases were within 3 cm,and two cases were beyond 3 cm. Conclusions:The causes for local recurrence after operation were related to Duke classification,pathological types,length from the lower margin of the tumors to the distal resection site,lymphadenectomy and operation on the tumor itself.
7.The effects of TPN on postoperative patients with colon or rectum cancer(clinical analysis of 42 cases)

Objectives:To study effects of TPN on postoperative patients with colon or rectum cancer,including gut function recovery,wound healing and immune function. Medthods:42 elder patients with colon or rectum cancer were divided into two groups at randomization,TPN group and control group.From POD+1 to POD+8,TPN group was given TNA daily and control group was given solutions of glucose and electrolyte.Gut function recovery and wound healing were observed and albumin,pre-albumin,IgG,IgA,IgM,CD3+,CD4+ and CD8+ were determined on POD+1 and POD+8. Results:The recovery of gut function and wound healing in TPN group were better than those in control group,and levels of pre-albumin,IgG,IgA were higher than those in control group. Conclusions:TPN has positive effects in gut function recovery,wound healing and immune function in postoperative patients with colon or rectum cancer.

OBJECTIVE:
To investigate the value of combination of thyroid imaging reporting and data system (TI-RADS) and enhanced flow (e-Flow) in the qualitative diagnosis of thyroid nodules; and to compare the diagnostic efficiency between e-Flow and color doppler flow imaging (CDFI.
METHOD:
The study chose 133 patients with 179 nodules (127 benign, 52 malignant nodules) who had per-operative ultrasound examination of their thyroid using grey-scale ultrasound, CDFI, e-Flow, and their noudles were graded by TI-RADS. Then the ultrasound diagnostic value were compared with their surgical pathology results by TI-RADS; TI-RADS and CDFI; TI-RADS and e-Flow, and diagnostic efficiency between e-Flow and CDFI were compared.
RESULT:
The results revealed that combination of TI-RADS and e-Flow had a higher sensitivity (94.23%, P < 0.05). There was no statistically significant difference between TI-RADS and the combination of TI-RADS and CDFI. Likewise, there was no significant difference between the combination of TI-RADS and e-Flow and the combination of TI-RADS and CDFI. The e-Flow showed a higher sensitivity, specificity and accuracy compare to the CDFI.
CONCLUSION
Combining e-Flow and TI-RADS could improve the sensitivity in qualitative diagnosis of thyroid nodules. Also, e-Flow has a better diagnostic efficiency of thyroid nodules.

10.Role of Leukocyte Activation and Inflammatory Reaction in Chronic Venous Insufficiency

Objective To discuss the role of leukocyte activation and inflammatory processes in the disease of chronic venous insufficiency(CVI).Methods The relevant literatures about the role of leukocyte activation and in- flammatory reaction in CVI were reviewed.Results The role of inflammatory reaction in occurrence and develop- ment of venous diseases has been studied a lot in recent years.It was found that the leukocyte activation and inflam- matory reaction are involved in the structural remodeling of venous valves and walls,leading to valvular incompe- tence and formation of varicose veins.Conclusion Leukocyte activation and inflammatory processes take important roles in the occurrence and progression of CVI.
