1.The clinical significance of the expression of XB130 in gastric cancer tissues

Objective To investigate the relationship between the expression of XB130 and the histodifferentiation,lymph node metastasis and survival time of patients with gastric cancer.Methods Seventy-two specimens of gastric cancer tissues and seventy-two specimens of paracancerous tissues were collected from June 2011 to June 2012 in Henan Province People's Hospital.The expression of XB130 protein in gastric cancer and paracancerous tissues was detected by immunohistochemistry.The expression of XB130 mRNA in gastric cancer and paracancerous tissues was detected by real-time quantitative polymerase chain reaction.The relationship between the expression of XB130 and clinicopathological features of gastric cancer was analyzed.Results There were 26 cases of positive expression of XB130 protein and 46 cases of negative expression of XB130 protein among the 72 cases of gastric cancer tissues,and the positive expression rate was 36.1% (26/72).There were 52 cases of positive expression of XB130 protein and 20 cases of negative expression of XB130 protein among the 72 cases of paracancerous tissues,and the positive expression rate was 72.2% (52/72).The positive expression rate of XB130 protein in gastric cancer tissues was significantly lower than that in paracancerous tissues (x2 =16.200,P < 0.05).The positive expression rate of XB130 protein in poorly and moderately differentiated gastric cancer tissues was significantly lower than that in well differentiated gastric cancer tissues (x2 =5.786,P <0.05).The positive expression rate of XB130 protein in gastric cancer tissues with lymph node metastasis was significantly lower than that in gastric cancer tissues without lymph node metastasis (x2 =4.281,P <0.05).The relative expression of XB130 mRNA in gastric cancer tissues and paracancerous tissues was 1.52 ±0.46 and 2.28-± 0.51 respectively,the expression of XB130 mRNA in gastric cancer tissues was significantly lower than that in paracancerous tissues (t =-21.744,P <0.05).The expression of XB130 mRNA in well differentiated gastric cancer tissues was significantly higher than that in poorly and moderately differentiated gastric cancer tissues (t =-13.982,P < 0.05).The expression of XB130 mRNA in gastric cancer tissues with lymph node metastasis was significantly lower than that in gastric cancer tissues without lymph node metastasis (t =-19.906,P < 0.05).The mean survival time in patients with high and low expression of XB130 mRNA was (37.040 ± 14.826) and (21.529 ± 11.789) months respectively,the survival time in patients with high expression of XB130 mRNA was significantly higher than that in patients with low expression of XB130 mRNA (t =9.121,P <0.05).Conclusion XB130 may be involved in the occurrence and development of gastric cancer,and it is associated with the differentiation,lymph node metastasis and survival time of patients with gastric cancer.
2.A multicenter analysis of bacteria distribution and antimicrobial resistance of bloodstream infection in Yunnan, 2017-2021

Abstract: Objective To analyze the distribution and drug resistance evolution characteristics of pathogenic bacteria of bloodstream infection in nine tertiary hospitals in Yunnan Province from 2017 to 2021, so as to provide reliable basis for rational selection of antibiotics in clinic. Methods Using the drug sensitive paper method or instrument method, the bacteria identification and drug sensitivity test were carried out in nine tertiary hospitals in different regions according to the unified technical scheme. The results were judged according to the Clinical and Laboratory Standards Institute (CLSI) breakpoint standard in 2021, and use WHONET5.6 for data statistical analysis. Results A total of 12 003 strains of pathogenic bacteria were isolated from bloodstream infection samples in the past five years, including 7 442 strains of Gram-negative bacteria (62.0%) and 4562 strains of Gram-positive bacteria (38.0%), with an increasing trend in the number of isolated strains; of these, 163 strains (1.4%) were isolated from outpatients and 11 840 strains (98.6%) were isolated from inpatients. The top three gram-negative bacteria were Escherichia coli, Klebsiella pneumoniae and Acinetobacter baumannii, of which 309 strains (4.2%) were carbapenem-resistant Klebsiella pneumoniae (CR-KPN), 29 strains (0.4%) carbapenem-resistant Escherichia coli and 19 strains (0.3%) carbapenem-resistant Enterobacter cloacae, and the number of CR-KPN was on the rise year by year. The top three Gram-positive bacteria were coagulase-negative staphylococci, Staphylococcus aureus and Enterococcus faecium, of which methicillin-resistant Staphylococcus aureus (MRSA) was detected for 213 strains, accounting for 27.7%, and decreased from 40.0% in 2017 to 23.4% in 2021, showing a downward trend year by year. No vancomycin-resistant staphylococci and enterococci were found. Conclusions The detection and composition of bloodstream infection pathogenic bacteria in multicenter have not changed much in the past five years, but each hospital has its own characteristics. The number of carbapenem resistant Enterobacteriaceae increased year by year, which should be paid more attention.
4.Effects of modified laparoscopic totally extraperitoneal prostheic in the treatment of inguinal hernia

Objective To investigate the application effects of modified laparoscopic totally extraperitoneal prostheic in the treatment of inguinal hernia.Methods 100 patients with inguinal hernia were selected,and they were divided into control group (50 cases) and observation group (50 cases) according to the digital table.The control group was treated with opening totally extraperitonea prosthetic,the observation group was treated with modified laparoscopic totally extraperitoneal prostheic.The operation time,hospital stay,leaving bed time,visual analogue scale (VAS score),adverse reaction and recurrence between the two groups were compared.Results The operation time between the two groups had no significant difference (P < 0.05).The leaving bed time,hospital stay and VAS score of the observation group were (13.54 ± 3.81) h,(5.04 ± 1.24) d,(3.46 ± 0.75) points,repectively,which of the control group were (25.76 ± 4.35) h,(8.39 ± 1.65) d,(7.19 ± 1.27) points,repectively,the differences were statistically significant(t =14.943,11.477,17.882,all P < 0.05).There was no statistically significant difference in the incidence of scrotal effusion,hematoma and wound infection between the two groups (all P > 0.05).The incidence rates of chronic pain(2.00% vs.16.00%) and adverse reactions(10.00% vs.30.00%) in the observation group were lower than those in the control group,the differences were statistically significant (x2 =5.983,6.250,all P < 0.05).After 6 months' follow-up,2 cases(4.00%) occurred recurrence in the observation gToup,3 cases(6.00%) occurred recurrence in the control group,the recurrence rate between the two groups had no statistically significant difference (x2 =0.211,P > 0.05).Conclusion Modified laparoscopic totally extraperitoneal prostheic can effectively relieve the postoperative pain degree and improve the adverse reaction rate,and with fast recovery.
5.Diagnostic value of combining detection of human epididymis protein 4 and CA125 in patients with malignant ovarian carcinoma.

OBJECTIVETo investigate the clinical value of combination of human epididymis protein 4 (HE4), CA125 and the Risk of Ovarian Malignancy Algorithm (ROMA) in diagnosis of ovarian carcinoma.
METHODSTo detect the serum concentration of HE4 using ELISA and CA125 using ECL in patients of ovarian carcinoma group (n = 119), borderline ovarian tumor group (n = 36), benign ovarian neoplasm group (n = 96) and female healthy control group (n = 53). The ROMA based on the serum level of CA125, HE4 and a woman's menopausal status was used to calculate the predicted probability (PP) and diagnostic results of ovarian cancers.
RESULTSThe receiver operating characteristic (ROC) analysis showed the cut-off value was 67.3 pmol/L (the AUC was 0.906, the sensitivity was 80.7% and specificity was 94.6%). The serum levels of HE4 and CA125 in the ovarian carcinoma group were significantly higher than that in the borderline ovarian tumor group, benign ovarian neoplasm group and female healthy control group (P < 0.01). The serum levels of CA125 and HE4 showed statistically no significant difference between the borderline ovarian tumor group and benign ovarian neoplasm group (P > 0.05). The levels of HE4 and CA125 were reduced significantly in ovarian patients after surgery therapy (P < 0.01). The sensitivity and specificity of HE4 + CA125 combination was 92.7% and 72.5%. The ROMA that can classify patients into high and low risk groups was established as 9.3% in premenopausal and 27.3% in postmenopausal women.
CONCLUSIONSHE4 is a helpful biomarker for ovarian carcinoma diagnosis. Biomarker combination of HE4 and CA125, and applying of the ROMA are helpful to improve the accuracy in diagnosis of ovarian cancers.

7.Ultrasonography of retroperitoneal ifbrosis

Objective To analyze the ultrasonographic features of retroperitoneal ifbrosis (RPF). Methods Totally 13 patients with retroperitoneal ifbrosis from February 2000 to October 2012 in the Long Gang central Hospital of Shenzhen were retrospectively analyzed. Results In all patients who underwent ultrasound examination, there were ten cases of idiopathic RPF and three cases of secondary RPF with abdominal tumors. In 11 cases, the masses were hypoechoic locating at retroperitoneum and surrounding the abdominal aorta without deifnitive margin. One case showed hypoechoic mass with clear boundary. In ten cases, the internal echogenicity of masses were uniform. In two cases, the internal echogenicity of masses were uneven with a small amount of ifbrous separator with slightly higher echogenicity. No blood was found in all masses. The encasement of inferior vena cava was found in three casesand the masses extended to iliac arteries in three cases. Hydronephrosis could be found in 11 patients (84.6%) and ureter dilatation was found in ten cases. Ureteral localized stenosis were found in two cases. Conclusion Ultrasonography is a preferred imaging method in diagnosing RPF.
8.Combination of single-port thoracoscopy and laparoscopy for the treatment of esophageal carcinoma: report of 6 cases.

OBJECTIVETo study the feasibility and early results of radical resection of esophageal carcinoma using single-port thoracoscopy combined with laparoscopy.
METHODSFrom March 2010 to December 2010, 6 patients with esophageal carcinoma underwent radical resection by single-port thoracoscopy combined with laparoscopy in the General Hospital of People's Liberation Army. With the patients at a supine position, laparoscopy was performed to complete stomach mobilization and abdominal lymph node dissection. Thoracoscopy was then carried out with the patients lying on the left to mobilize the esophagus and dissect thoracic lymph nodes. Finally, the stomach was pulled into the thoracic cavity via the hiatus of the diaphragm to construct a tube-like stomach, which was then anastomosed to the esophagus using the OrVil system.
RESULTSNo patient was converted to open surgery during the operation. The total operative time ranged from 200 to 320 min. The mean laparoscopic time was 75(range, 45-90) min, and the mean thoracoscopic time 160(120-240) min. The mean intraoperative blood loss was 220(160-300) ml. The mean lymph node retrieval was 12(9-18). No anastomotic fistula, chylothorax, lung infection were found postoperatively.
CONCLUSIONAfter esophageal resection using single-port thoracoscopic and laparoscopy, reconstruction using OrVil system is safe and feasible.

9.Inhibition of serine/threonine kinase 15 gene expression induces apoptosis of gastric cancer cells.

OBJECTIVETo observe the effect of inhibition of serine/threonine kinase15 (STK15) gene expression on apoptosis induction in gastric cancer cell line-MKN45 and discuss the role of STK15 in viability of gastric cancer cells.
METHODSThe STK15 expression was inhibited by chemically synthesized siRNA. The STK15 mRNA and protein level were respectively measured by real-time quantitative PCR and western blotting,the change of cell cycle distribution and apoptosis rate were detected by flow-cytometry, cell morphological change was observed by Hoechst staining,and pro-caspase 3 level was also detected by western blot.
RESULTSAfter treatment by siRNA targeting STK15 after 48 h, STK15 mRNA and protein level decreased obviously. More MKN45 cells accumulated at G(2)/M phase (P< 0.05). The apoptosis rate of STK15 siRNA treated MKN45 cells was higher than that of control cells(P< 0.05) with the pro-caspase 3 level decreased.
CONCLUSIONSInhibition of STK15 gene expression may induce apoptosis in MKN45 cells through the pathway of caspase3. STK15 gene play a key role in proliferation and viability of MKN45 cells.

10.Study of growth inhibition of gastric cancer cells by sRNA targeting polo like kinase 1 in vitro and vivo.

OBJECTIVETo observe the effect of polo like kinase 1 (plk1) gene depletion on the growth of gastric cancer cell line-MKN45 cells in vitro and vivo and discuss the feasibility and effectiveness of arranging plk1 as gene therapeutic target for gastric cancer.
METHODSThe plk1 expression of MKN45 cells was inhibited by RNA interference (RNAi). The plk1 mRNA and protein level were measured by real-time quantitative PCR and western blotting, and the change of cell cycle distribution and apoptosis rate were detected by flow-cytometry, and the MKN45 cells proliferation was measured by MTT method. MKN45 cells treated with plk1 siRNA were transplanted subcutaneously in nude mice and their tumorgenesis ability were observed, the plk1 protein levels of the samples from nude mice in different groups were compared.
RESULTSAfter treatment with plk1 siRNA, plk1 mRNA and protein level decreased obviously in certain time, more MKN45 cells accumulated at G(2)/M (P < 0.05). Apoptosis rate of MKN45 cells treated with plk1 siRNA was higher than that of control cells at 48 h and 72 h (P < 0.05), and MKN45 cells proliferated slowly than control groups (P < 0.05), while the tumorgenesis ability obviously decreased, but the plk1 protein levels of the samples from nude mice in different groups were not different.
CONCLUSIONSsiRNA targeting plk1 can inhibit the proliferation of MKN45 cells in vitro and vivo. Plk1 may be a novel therapeutic target for gastric cancer.
