1.The relationship of adhesion molecules, cytokines and vascular disease in patients with type 2 diabetes

Objective To explore the relationship of the levels of adhesion molecules and cytokines with vascular disease in patients with type 2 diabetes (T2DM).Methods Ninety-six T2DM patients were selected as case group and divided into non-vascular group (28 cases),micro-vascular group (33 cases) and major vascular group(35 cases) according to vascular disease.Meanwhile,30 healthy persons were selected as control group.Serum levels of serum soluble intercellular adhesion molecule-1 (ICAM-1),vascular cell adhesion molecule-1 (VCAM-1),interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α) and yon willebrand factor (vWF) were determined by Enzyme-Linked Immuno Sorbent Assay (ELISA),and the expression of adhesion molecule and cytokines of patients with T2MD at 2 weeks after treatment.Results Compared with control group,the levels of ICAM-1,VCAM-1,TNF-α and IL-6 in all subgroup of T2DM patients were significantly elevated(P < 0.05),and the levels showed an increasing trend with function deterioration (P < 0.05).The levels of ICAM-1,VCAM-1,IL-6 and TNF-α in case group before treatment were (505.34 ± 56.42) μg/L,(570.85 ±59.54) μg/L,(94.51 ± 18.04) ng/L,(70.57 ± 18.34) ng/L respectively,significant different from those of after treatment ((390.53 ±45.23) μg/L,(482.93 ± 69.85) μg/L,(77.31 ± 15.49) ng/L,(50.45 ± 12.66) ng/L; t =16.281,8.712,7.697,8.445; P < 0.05).There were significant positive correlation between ICAM-1,VCAM-1,IL-6,TNF-α and vWF in the case group(r =0.482,0.453,0.576,0.534 respectively,P < 0.05).Conclusion Adhesion molecules and cytokines are take part in the process of the deterioration of T2DM and the occurrence of vascular disease.
2.Clinical Observation of Reinforcing-reducing Needling Methods for Peripheral Facial Paralysis

Objective To observe the clinical efficacy of reinforcing-reducing needling methods in treating peripheral facial paralysis. Method Seventy facial paralysis patients were randomized into two groups to compare the reinforcing-reducing needling methods and conventional acupuncture. The House-Brackmann (H-B) scale was observed and compared prior to the treatment, right after the treatment, and respectively after 30-day, 3-month, and 6-month treatments, and the clinical efficacy was evaluated after 6-month treatments. Result The H-B score of the reinforcing-reducing manipulation group was superior to that of the conventional group (P<0.05), and the therapeutic efficacy of the manipulation group was more significant than that of the conventional group (P<0.05). Conclusion The reinforcing-reducing needling methods can produce a better therapeutic efficacy in treating peripheral facial paralysis compared to conventional acupuncture.
3.Monitoring results and epidemic analysis of gerbil plague in northern Ningxia in 2009

Objective To analyze gerbil plague in northern Ningxia and the monitoring results,to master the plague epidemic dynamics,and to provide the basis for developmenting countermeasures.Methods Monitoring data of gerbil plague focus in northern Ningxia were collect in 2009,counted main host density,rate of dye fleas,flea body index and bacteriology,serology detect and analyzed the epidemic situation.Results An average density of main host was 1.74/hm2,the average rate of infected fleas was 28.60%,and the average rat body flea index was 0.76.Monitoring found 4 plagues from rat plague epidemic,plague bacteria were found in 16 strains,of which gerbils inspection bacteria 10 strains,Meriones unguiculatus 2 strains,the same type cheopis 2 strains,and bald disease fleas 2 strains.Indirect hemagglutination (IHA) was used to test 529 copies of samples,2 copies were found positive,and hemagglutination-positive rate was 0.38%; eight copies were examined by reverse indirect hemagglutination(RIHA),7 material were found positive,and hemagglutination-positive rate was 87.50%.Conclusions In recent years,the disease is active among animals in gerbil plague focus Ningxia.The density of rodents is higher,and local plague epidemic is found in rats.Monitoring efforts should be strengthened and scope of monitoring should be expanded.We should pay close attention to the epidemic dynamics,control the prevalence and spread of animal disease,and prevent the occurrence of human plague.

5.Biocompatibility of cerebral intravascular stent

As a foreign body, intravascular stent will induce platelet aggregation on the stent surface and activate platelet system and coagulation system, resulting in thrombosis. Smooth muscle cells migrate to injured site and proliferate, leading to intimal hyperplasia and vessel wall remodeling, which induces restenosis. Incidence of intravascular restenosis is determined by stent type, disease of the host, severity of stenosis before implantation and remnant stenosis length following stenting. Inhibition of intimal hyperplasia or improvement of intima formation and vessel wall remodeling can effectively prevent restenosis. Currently, there are many drugs to prevent restenosis, including antiplatelet drug, anti-inflammatory growth factor inhibitor and anti-cell proliferation drugs.
6.The Impact of Global Budget on the Hospitalization Beneficial Degree under New Rural Cooperative Medical System

Objective:To analyze the effect of global budget of New Rural Cooperative Medical System (NCMS) on the per-visit inpatient compensations,inpatient compensation ratios,per-visit inpatient out-of-pocket expenses and inpatient out-of-pocket rates.Methods:The difference in difference method was used to control the non-intervention factors and estimate the net impact of global budget.Results:Global budget of NCMS decreased the per-visit inpatient compensations by 14.37 yuan,but it had no statistical significance.The compensation ratio of hospitalization increased by 5.23%,the average hospitalization self-payment decrease by 141.51 yuan,the self-payment decreased by 5.23%,which all had statistical significance while there were differences on the effects for specific diseases.Conclusion:Global budget of NCMS increased the inpatient benefit,but the effect was varies by conditions.In addition,measurement of global budget's standard still needed to be scientific and reasonable.

8.Analysis of the Changes of Blood Cell Count and D-dimer in Lung Cancer Patients with Thrombosis

Objective To analyze the changes of Blood Cell Count and D-dimer concentration in lung cancer patients with thrombosis.Methods ①554 cases of patients with lung cancer were included between January 2012 and November 2013,75 patients with other diseases which lead to high coagulation state were excluded,blood cell count and D-dimer concentration were compared between 64 lung cancer patients were the cases with thrombosis with 415 lung cancer patients were the con-trol group without thrombosis.②Color doppler ultrasonography was taken as the golden standard,receiver operating charac-teristic curves (ROC)were drawn for indexes which had significance in method.Results ①Compared with lung cancer pa-tients without thrombosis,lung cancer patients without thrombosis had decreased red blood cell count (RBC),increased white blood cell count (WBC)and D-dimer.And there were no significant change in platelet (PLT).②The area under curve (AUC)of ROC in RBC,WBC and D-dimer were 0.662,0.637 and 0.896,respectively.By thecut-off values of RBC>4.06× 1012 L,WBC<5.37×109/L and D-dimer<4.02 mg/L,the negative predictive values of RBC,WBC and D-dimer for diagno-sis of lung cancer patient with thrombosis were 93%,93% and 96%,respectively.By the cut-off value of D-dimer>4.02 mg/L,the positive predictive value of D-dimer for diagnosis of lung cancer patient with thrombosis was 6 6%.Conclusion RBC,WBC and D-dimer were related with the progress of thrombosis in lung cancer patients.RBC,WBC and D-dimer have good negative prediction effect on lung cancer patients with thrombosis.D-dimer had positive effect for diagnosis of lung cancer with thrombosis.
9.Endothelial repairing function in patients with symptomatic middle cerebral arterial stenosis after stent implantation

Objective To investigate the relation between endothelial repairing function and in-stent restenosis in patients with symptomatic middle cerebral arterial (MCA) stenosis after stent implantation.Method Sixty-six patients with symptomatic MCA stenosis underwent percutaneous stent implantation.Cranial CTA revealed that 23 patients had MCA restenosis (restenosis group) 1 year after stenting,including 14 cases with >50% stenosis and 1 case with MCA occlusion,and 43 patients had no restenosis (non-restenosis group).The number of endothelial progenitor cells (EPC) was examined by flow cytometry,the adhesion function of EPC was tested by adhesion assay,the migration ability of EPC was tested by Transwell method and serum vascular endothelial growth factor (VEGF) levels were measured by ELISA.The relationship of endothelial repairing function with restenosis was analyzed.Results The MCA stent implantations were successfully performed in all patients.The EPC number (33.7 ± 4.6 vs.61.6 ± 6.4),adhesion activities (26.1 ± 7.5 vs.56.3-± 9.6),migration activities (12.0 ± 3.9 vs.21.4 ± 6.5) and serum VEGF level [(56.7 ± 14.6) vs.(89.6 ± 17.32) ng/L] in restenosis group were significantly lower than those in non-restenosis group (t =18.48,13.09,6.34 and 7.73,all P < 0.05).Conclusion For patients with MCA stenosis after percutaneous stent implantation the increased risk of in-stent restenosis is associated with low level of EPCs and their migration ability,and low serum VEGF level.
10.Orthotopic transplantation of human amniotic mesenchymal stem cells for treatment of cerebral infarction in rats

BACKGROUND: Preliminary experimental study found that the human amniotic mesenchymal stem cells (hAMSCs)transplantation can improve nerve injury symptoms of rats with cerebral infarction.OBJECTIVE: To observe the survival, colonization and differentiation of hAMSCs in the infarct area of cerebralinfarction rats.METHODS: Sixty Sprague-Dawley rats were randomly assigned into hAMSCs transplantation, model or shamoperation groups (n=20/group). Animal models of middle cerebral artery occlusion were produced in the model andtransplantation groups by Zea-Longa method. One day after modeling, rats in the hAMSCs transplantation groupwere given in situ transplantation of 10 μL of hAMSCs (2×106) into the damaged striatum and cortex, while those inthe model and sham operation group were given the same volume of PBS. Within 1 week after transplantation, ratneurological defects were assessed and changes in their body mass were continuously monitored. Two weeks aftertransplantation, TTC staining was used to observe cerebral infarct size, hematoxylin-eosin staining was used forpathological observation of brain tissues, and immunofluorescent staining was used to detect expression ofneuron-specific nuclear protein.RESULTS AND CONCLUSION: With time, weight loss was increased while neurologic deficit scores were graduallyreduced in the hAMSCs and model groups. Compared with the model group, the weight loss and neurologic deficitscores were lower in the hAMSCs group,; however, there was a significant difference in the neurologic deficit scoresbut not in the weight loss between the two groups. Additionally, the hAMSCs significantly reduced infarct size,attenuated pathologic injury, and decreased the number of inflammatory cells. Immunofluorescence stainingshowed that the hAMSCs were observed at 1 week after transplantation under inverted luorescence microscope,and gradually differentiated into nerve cells at 2 weeks after transplantation. In conclusion, transplanted hAMSCsmay migrate to and survive in the cerebral infarct region, and differentiate into nerve cells in situ in rats with cerebralinfarction.
