1.Clinical features of children's acute appendicitis

Objective To investigate the clinical features of children's acute appendicitis in order to reduce the misdiagnose rate.Method Collect and retrospective analyse 77 cases clinical data of children's acute appendicitis from January 2008 to December 2011 in our hospital.Results There are 77 cases abdominal pain,61 cases fever,48 cases vomiting in 77 children,22 cases(28.57%) misdiagnosis or missed diagnosis in total.17 cases with appendix perforation,the perforation rate is 22.07%.49 cases receive surgery,28 cases receive conservative treatment.All are improved and cured and discharged.Conclusions The main clinical features of children's acute appendicitis is abdominal pain,fever and vomiting.When clinical features is not classic,there is higher rate of misdiagnosis and missed diagnosis,especially for infants.Stress that for children with unreasonable abdominal pain and fever,it is necessary to check leukocyte,CRP and type-B ultrasonic,observe the abdominal sign continually,in order to diagnose and treat early.
2.Research Progress of protective effect of intestinal trefoil factor on the gastrointestinal tract

Intestinal trefoil factor (ITF or TFF3) is a member of trefoil factor family which is most studied , mainly in the in-testinal tract .TFF3 plays an important role in gastrointestinal mucosal protection and epithelial repair .Action mechanism includes in-teraction with mucin, migration, anti-apoptosis, angiogenesis, immune regulation, interaction with its receptor, etc.Recombinant TFF3 has been approved as a national drug and its therapeutic indications for repair mucosa , prevention and treatment of gastrointesti-nal ulcers and inflammation .Furthermore, the classic TFF3 receptor and signaling pathway is highly valuable to make TFF 3 become an effective treatment for gastrointestinal injury disease .
3.Advances in Study on JAK/STAT3 Signaling Pathway and its Role in Gastrointestinal Diseases

JAK/STAT3 signaling pathway exists in various organs and tissues and mediates multiple biological processes including cell proliferation,differentiation,migration,apoptosis and immunoregulation.Recently,it has been revealed that this pathway plays an important role in gastrointestinal diseases,promoting tumor growth,angiogenesis and inhibiting apoptosis in gastric and colorectal cancers,and being implicated in the pathogenesis of inflammatory bowel disease.Inhibitors targeting JAK/STAT3 pathway showed promising outcome in some disease models.In this review article,the advances in study of abovementioned issues were summarized.

Objective To explore the effect and mechanism of taking sibelium long term for treatment and preventing the attack again of cerebral infartcion.Methods 339 patients with cerebral infarction were divided into 2 groups randomly,namely preventing group(120 cases) and control group(119 cases).The basic treatment in the two groups are the same.The preventing group was taken siblium 10 mg once every evening,at least 6 months.To observe the clinical effect in 1 month and the rate of attack again in 1 year.The two groups were measured on hemorrheology,blood lipid and platelet aggregation rate before and after treatment one year.Results After treatment one month,the significant effective rate in sibelium preventing group is 80%,the total effective rate is 98.3%,but the control group is 54.6% and 74.8%(all P

6.Clinical Application in Diagnosis of Pulmonary Embolism by Multiple Spiral CT Angiography

Objective To evaluate the effectiveness of multiple spiral CT angiography in diagnosis of pulmonary embolism.Methods 16 patients clinically suspected of pulmonary embolism underwent multiple spiral CT angiography.Emboli were found in the 8 patients of all on multiple spiral CT angiography.5 of the patients were reexamined with this method 20 to 30 days after thrombolysis. Results In 8 patients with pulmonary embolism,43 vessels were involved with pulmonary embolism.The presence of pulmonary embolism on CT was seen as a total occlusion,partial filling defect,mural filling defect et al;dilated of pulmonary artery trunk;all signs with deceleration of pulmonary blood or “mosaic sign”. 5 patients were reexamined with this method.The emboli dissoved partly or entirely.Conclusion Multiple spiral CT angiography is a noninvasive and effective method for examining pulmonary embolism.
7.Expression and transport function of epithelial sodium channel in alveolar epithelial type Ⅱ cells of hyperoxia-exposed neonatal rat

Objective To investigate the effect of hyperoxia on the expression and transport function of epithelial sodium channel (ENaC) in neonatal rat alveolar epithelial type Ⅱ(AT Ⅱ) cells.Methods AT Ⅱ cells were isolated from neonatal rats,and primarily cultured under hyperoxic or normoxic conditions.Western blot was applied to examine the ENaC expression,and the amiloride-sensitive Na + currents were recorded using the whole-cell patch clamp technique.Results Hyperoxia upregulate the expression of β-ENaC and γ-ENaC subunits in the neonatal rat ATⅡ cells(β-ENaC:1 d:0.43 ±0.06 vs0.32 ±0.04,P =0.047;2 d:0.73±0.06 vs 0.50±0.08,P =0.019;3 d:0.72 ±0.08 vs 0.52 ±0.06,P =0.027;γ-ENaC:1 d:0.64±0.05 vs0.53 ±0.05,P =0.044;2 d:0.76 ±0.03 vs 0.52 ±0.04,P =0.001 ;3 d:0.77 ±0.06 vs 0.61 ±0.05,P =0.025).In addition,the amiloride-sensitive Na+ currents in hyperoxia-exposed AT Ⅱ cells were also increased (1d:13.71 ±2.77 vs8.92±1.38,P<0.001;2d:29.12±11.03 vs 10.41 ±1.80,P<0.001),which was consistent with the upregulated expression of β-ENaC and γ-ENaC.However,the expression of α-ENaC was inhibited by hyperoxia to some extent (1 d:0.31 ± 0.05 vs 0.46 ± 0.05,P =0.025 ; 2 d:0.30 ±0.01 vs0.38±0.02,P=0.002;3d:0.37±0.06 vs 0.37 ± 0.08,P =0.983).Conclusion Hyperoxia enhanced the transport function of ENaC in neonatal rat AT Ⅱ cells.Dysfunctional transport of Na + may not be a key factor involving in pulmonary edema at the early stage of bronchopulmonary dysplasia.
8.Multislice spiral CT virtual endoscopy: the clinical application in the diagnosis of vascular diseases

Objective To evaluate the display ability and the clinical application of multislice spiral CT virtual endoscopy (CTVE) in the diagnosis of vascular diseases. Methods Fifty-six patients were examined by CT virtual endoscopy.32 patients also had DSA for comparison.18 patients were confirmed by operation (3 patients also had DSA).9 patients with pulmonary embolism were confirmed by other examinations and treatments.Results 56 patients showed successful CT virtual endoscopy images.CT virtual endoscopy images could clearly show the inner wall and inner structures of the vessels,mural thrombus,calcification form,stenosis of the vessels,the true and false lumina of aortic dissections,the lesion of false aneurysm,and the anatomic relationship between the lesion and surrounding structures of the normal vessels.Out of the 32 patients with vessel abnormalities on DSA,24 patients (75%) showed the lesions on CTVE.17 out of 18 patients with operation showed the lesions on CTVE.All 9 patients with pulmonary embolism showed the abnormalities on CTVE.Chi-square test:? 2=5.2809,P=0.071 patients showed the vascular diseases on CTVE in 56 patients (84%).Conclusion MSCT virtual endoscopy could provide more information of the vessel by displaying the inner wall and allowing users to map the vessels in a display.It is helpful in correct diagnosis and guiding surgery.
9.Expression of Basic Fibroblast Growth Factor in Rats Model of Doxorubicin Induced Nephropathy

Objective To investigate the expression and importance of basic fibroblast growth factor(bFGF)in renal tissue of doxorubicin-induced nephropathy rats,and discuss its possible role in minimal change nephrotic syndrome(MCNS).Methods Fifty male Wistar rats were enrolled in this study.They were randomly divided into normal group(n=10)and nephropathic group(n=40).The nephropathic group was established by a single intraperitoneal injection of doxorubicin 5 mg/kg.The rats of nephropathic group were killed on the 3rd,7th,14th and 28th day and 24 h urinary protein of all rats was measured.Normal group rats were killed on the 28th day.bFGF level in renal tissue was determined with immunohistochemistry assays,and was quantitatively analyzed with color image analysis system.The SPSS 10.0 software was used to obtain t-test of various groups and correlation analysis of 2 variables.Results 1.The 24 h urinary protein of nephropathy rats increased gradually on the 7th,14th and 28th day,and it was greatly higher than that in control group(Pa
10.Evaluation of internal clinical skills assessment in five-year clinical interns

Objective To evaluate the assessment index system of internal medicine, and to analyze the factors of practical effects on internal medicine. Methods The assessment index system of clinical skills was developed by expert group based on the clinical syllabus and objectives. Internal medicine index system was divided into four items such as history-taking, physical examination, hos-pital records and case questions. 49 five-year clinical interns who took internal medicine exams were researched. Reliability, validity, discrimination, difficulty were used to analyze the quality of assess-ment index system. Gender, teaching hospital and the preparation of postgraduate examination were used to explore the influencing factors of practical effects with independent t-test and Wilcoxon signed rank tests. Results The reliability, discrimination, difficulty of the assessment index system were 0.50, 0.15 and 0.85, respectively. The removing item coefficient of history collection, physical ex-amination, hospital records, case questions were 0.38, 0.26, 0.47 and 0.53, separately, and the discrimination were 0.18, 0.17,0.27 and 0.37, separately, and the difficulty were 0.87, 0.89, 0.79, and 0.78, respectively. The scores of assessment index system of girls were higher than those of the boys(P<0.05). Moreover, the girls scored higher in history collection and physical examination, compared with the boys(P<0.05). The discrepancy scores were not statistically significant among teaching hospitals (P>0.05). The students who prepared the postgraduate examination scored lower than the others(P<0.05). Conclusions The assessment index system of internal medicine is reason-able and reliable, but the assessment of case questions still needs to be standardized. Teaching hospi-tals have little effect on internship while gender and the preparation for postgraduate examination influence practice effects in internal medicine.
