1.The analysis for the clinical features of 68 patients with Type 2 diabetes mellituscomplicated with cerebral infarction

ObjectiveTo analyze the association between type 2 diabetics and cerebral infarction and to guide the future clinical practice.MethodsA comparison study was conducted between 68 patients with Type 2 diabetes mellitus complicated with cerebral infarction(DCI) and 76 patients with cerebral infarction but no diabetic cerebral infarction (NDCI) .They were hospitalized during January 2007 and April 2010 and compared for the difference in blood pressure(BP), the cholesterol (TG), the triglyceride (TC), the lipoprotein cholesterol(LDC-C), the ages, the position of infarction, the amount of infarction, the clinical manifestations and the prognosis.ResultsThe patients with DCI had more lacunar infarction(41.2%)and multiple infarctions (33.8%)than those with NDCI(15.8% and 19.8%) .The difference was statistically significant (P <0.01) In the diabetic patients group, compared with the control group, the systolic blood pressure was ([155.8 ±24.0]mm Hg) vs.([138.5 ± 22.0]mm Hg), diastolic blood pressure was ([89.6 ± 15.0]mm Hg)vs.([84.7 ±14.0]mm Hg),the TG([1.6 ± 0.3]mmol/L vs.[1.2 ±0.2]mmol/L),the LDC-C(1.3 ±0.7]mmol/L vs.[2.7 ± 0.3]mmol/L) and the ages(50.6 ± 6.4) years vs.(57.8 ± 6.5)years.These parameters in DCI group patients were significantly higher than that of NDCI group(P <0.01) .The DCI patients had a longer hospitalization period ([17.8±5.7]and [14.5±6.3]d,t=1.67,P<0.05].ConclusionDiabetes is a risk factor of cerebral infarction and of the deterioration of cerebral infarction.Prevention or treatment at a early stage of diabetes and strict control of blood sugar,the blood pressure as well as blood lipids is essential to reduce the occurrence of ischemic infarction and improve the prognosis.
2.Influence of Propofol and Etomidate on Cognitive Dysfunction of Elderly Patients after Laparoscopic Chole-cystectomy

OBJECTIVE:To discuss the effect of etomidate and propofol on early postoperative cognitive dysfunction (POCD)of elderly patients after laparoscopic cholecystectomy(LC)and significance of serum protein S100β to the occurrence of early POCD in total intravenous anesthesia. METHODS:60 patients aged 65 years old above undergoing LC in total LMA intrave-nous anesthesia were selected and randomly divided into etomidate group(group E)and propofol group(group P),with 30 cases in each group. Anesthesia was induced by etomidate 0.3 mg/kg (group E) or propofol 1.5 mg/kg (group P),and additionally in-duced by sufentanil 0.4 μg/kg and vecuronium 0.12 mg/kg. Anesthesia was maintained with intravenous pump of remifentanil 0.15 μg/(kg·min),continuous target controlled infusion of etomidate(target concentration 1.0-1.5 μg/ml)(group E)or propofol(target concentration 3.0-4.0 μg/ml)(group P);the dual brain index(BIS)values were maintained between 40 and 50 throμgh adjusting target concentration of etomidate or propofol. The blood samples were collected 1 h before operation(T0),2 h(T1),24 h(T2), 48 h(T3)after operation,and the content of S100βprotein was detected and mini-mental state examination(MMSE)score were re-corded. Meanwhile,recovery time,laryngeal mask removal time,intraoperative dosage and the occurrence of intraoperative aware-ness were observed and recorded in 2 groups. RESULTS:There was no statistically significant difference in MMSE score between 2 groups at different time points(P>0.05);MMSE score of 2 groups at T1 and T2 was significantly lower than at T0,with statisti-cal significance(P<0.05). There was no statistically significant difference in the content of S100β protein between 2 groups at dif-ferent time points(P>0.05);The contents of S100β protein in 2 groups at T1 and T2 were significantly higher than at T0,with sta-tistical significance(P<0.05). The recovery time and larynge-al mask removal time were both short in 2 groups,with statis-tical significance (P>0.05). The amount of ephedrine in group P was significantly higher than in group E,with statisti-cal significance (P<0.05). No intraoperative awareness oc-curred in 2 groups throμgh postoperative follow-up. CONCLUSIONS:Etomidate and propofol total intravenous anesthesia can be safely used in elderly patients with LC,and they can cause short-term POCD at different degrees. The amount of S100β protein has some relevance with the occurrence of early POCD .
3.Progress in clinical research of asteroid hyalosis

Asteroid Hyalosis (AH) is a common clinical disease,which has been considered a benign disorder as it rarely impairs visual acuity.It was often discovered when the patient was treated for other eye diseases.The mechanism was unclear.Its characteristic B-ultrasound property makes the B-ultrasound a very helpful diagnostic technique.In the case of the patients with other fundus diseases associated with AH,optical coherence tomography (OCT) and fluorescein angiography (FA) may be used to reduce the interference from asteroid bodies,therefore improve the fundus visibility.Recent studies have shown that AH can incorporate with many other eye diseases.For example,in patients with cataracts,asteroid hyalosis can cause surface calcification of silicone plate intraocular lenses,which in most cases may lead to the need for explantation of the calcified intraocular lenses.The efficacy of pars plana vitrectomy (PPV),the removal of some,or all,of the eye`s vitreous humor for AH remains controversial.In this paper,we provide a review of the recent literature on AH disease: the etiology,diagnosis and treatment.We hope to thus improve the awareness and outcomes of AH disease.
4.Research Progress in Medicinal Plant Echinops Latifolius Tausch

Echinops latifolius Tausch is a plant of compositae echinops, whose dried roots are used as traditional Chinese medicine Yuzhou Loulu, and dried inflorescence is used as Mongolian medicine LanCitou. The studies on Chinese medicine Yuzhou Loulu and Mongolian medicine LanCitou in recent years were systemized and compared with each other in the paper, including chemical composi-tion, process research, quality control, pharmacological effects and so on. The results can provide scientific basis for the further studies and utilization of Echinops latifolius Tausch.
5.Far Infrared Thermography Analysis with Acoustic Neuroma

PurposeTo analyze the far infrared thermography characteristics before and after surgery in patients with acoustic neuroma.Materials and Methods Thirty-two patients with acoustic neuroma were included as observation group and 40 normal healthy individuals as control group. Un-cooled thermal imaging system (ATIR-M301B) was used with working temperature of 20-25℃. Craniofacial infrared images were collected to analyze temperature differences among different detection zones.Results Far infrared thermography revealed that there were no obvious temperature differences between both sides of supraorbital region, endocanthion region, frontal region and buccal region in the control group (P>0.05). The temperature differences in bilaterally symmetrical parts of supraorbital region, endocanthion region, frontal region, buccal region were significantly higher in observation group (t=1.557, 1.714, 1.483 and 1.569,P<0.05). The craniofacial temperatures of 32 patients changed after operation, and the differences reduced in supraorbital region, endocanthion region, frontal region, and buccal region (t=2.655, 2.462, 2.897 and 4.465,P<0.05).Conclusion Far-infrared thermography inspection can detect abnormal temperature changes.
6.The effect of the allocation of health resources on doctor-patient relationship in Chongqing

Objective To analysis the basic condition of the health resources in Chongqing ,and to explore the effect of the al‐location of health resources on doctor‐patient relationship .Methods Health statistics yearbook of Chongqing city was collected ,the fairness of the allocation of health resources in chongqing carried on the analysis and evaluation ,using the DEA model to analyze and evaluation the efficiency of the allocation of health resources in chongqing ,self‐designed questionnaire was used;hospitals ,doc‐tors and patients were investigated ,the descriptive and statistical analysis carried on .Results The distribution of health resources in Chongqing was inequitable on a regional .This inequitable distribution had been evaluated to the effect that ,as per the DEA mod‐el ,7 prefectures were effective with another 5 slightly effective and 26 as DEA ineffective .In the 8 hospitals that had been investiga‐ted ,the occurrence of medical disputes was undergoing a surge ,which indicates the tense relation between doctors and patients .Con‐clusion The total amount of health resources in Chongqing is fairly insufficient ,the high quality health resources are scarce .Unbal‐anced allocation of health resources and allocation inefficiency is the main reason for nervous doctor‐patient relationship and contra‐diction focus .
7.The contrast research between left ventricular three dimensional strain and coronary angiography for coronary disease patients without segmental ventricular wall motion abnormalities

Objective To evaluate the left ventricular diastolic function in coronary artery disease (CAD)patients without regional wall motion abnormality by real-time three-dimensional specke tracking imaging(RT3D-STI),and to investigate the value of RT3D-STI in detecting CAD.Methods According on coronary angiography,92 patients without regional wall motion abnormality were divided into two groups:control group(n = 21 ),without coronary artery stenosis;case group (n = 50 ),coronary stenosis group. Two dimensional images and the three-dimensional full volume dynamic images of left ventricle were collected.Routine parameters and end-systolic left ventricular globle longitudinal strain (GLS),globle circumferential strain (GCS),globle area strain (GAS),globle radial strain (GRS)and corresponding strain imaging diastolic index (SI-DI)were aquired,respectively.The parameters by RT3D-STI were analyzed to diagnose coronary artery stenosis.Results Compared with control group,there were no significant difference in routine parameters,GLS,GRS,GAS and GCS(P >0.05 ).Longitudinal SI-DI(L-SI-DI),area SI-DI(A-SI-DI),radial SI-DI (R-SI-DI),circumferential SI-DI (C-SI-DI)in case group were significantly reduced compared with control group (P <0.05).The sensitivity of L-SI-DI,A-SI-DI,R-SI-DI and C-SI-DI were 90.5%,81 .0%,85.7%,81 .0%,and the specificity of them were 82.0%,78.0%,68%,56%, respectively,L-SI-DI was the best in sensitivity and specificity.L-SI-DI,A-SI-DI,R-SI-DI were significantly correlated with Gensini score (r = -0.706,-0.547,-0.410 P < 0.05 ).Conclusions RT3D-STI can quantitatively assess the early changes of left ventricular diastolic function,which has important significance for early diagnosis of coronary artery stenosis.
8.Related Factors of Speech Intelligibility in Prelingually Deafened Children after Cochlear Implantation

Objective To investigate the related factors of speech intelligibility in the prelingually deafened children after cochlear implantation. Methods 47 prelingually deafened children who had received cochlear implantation and their families were investigated with questionnaires and analyzed with Fisher test. Results The age when cochlea implanted, the age when hearing aided, the time after the cochlear implantation and the time the cochlea opened were related with their speech intelligibility rating. Conclusion Suitable time for implantation, the hearing aid and making reasonable rehabilitation programme play a crucial role in the hearing rehabilitation of prelingually deafened children after cochlear implantation.
9.Effect of acute normovolemic hemodilution on apoptosis in hippocampal cells induced by global cerebral ischemia-reperfusion in rats

ObjectiveTo investigate the effect of acute normovolemic hemodilution (ANH) on the apoptosis in hippocampal cells induced by global cerebral ischemia-reperfusion (I/R) in rats.Methods Thirty-six healthy 50-60 day old male SD rats weighing 280-320 g were randomly divided into 3 groups ( n =12 each):group sham operation (group S); group global cerebral I/R (group I/R) and group ANH.Global cerebral I/R was produced by 4-vessel technique described by Pulsinelli et al.in groups I/R and ANH.ANH was carried out at 24 h after cauterization of bilateral vertebral arteries,before occlusion of bilateral carotid arteries.Blood was withdrawn from femoral artery until Hct was reduced to 30% and equal volume of hydroxyethyl starch 130/0.4 sodium chloride was infused into femoral vein simultaneously.Bilateral carotid arteries were blocked for 5 min at 10 min after ANH.The rats were sacrificed at 24 h of reperfusion and their hippocampi were isolated.Apoptosis was detected by flow cytometry.The expression of Apaf-1 mRNA and caspase-3 mRNA was determined by RT-PCR.Results Global cerebral I/R significantly increased apoptosis index and up-regulated Apaf-1 mRNA and caspase-3 mRNA expression in group I/R as compared with group S.ANH significantly attenuated apoptosis and down-regulated Apaf-1 mRNA and caspase-3 mRNA expression in group ANH compared with group I/R.ConclusionANH can reduce hippocampal cell apoptosis induced by cerebral I/R through down-regulation of Apaf-1 and caspase-3 expression in hippocampus.

Aim To observe the effect of methotrexate (MTX) on proliferation, migration and apoptosis of cultured vascular smooth muscle cells (VSMC). Methods Rabbit thoracaortic VSMC were cultured in vitro.VSMC proliferation was evaluated by cell counting and cell cycle analysis. Monolayer cell scrape was used to observe VSMC migration. Apoptosis was observed with flow cytometry, DNA gel electrophoresis and TUNEL stain.
