1. Effects of different dosages of fluvastatin from different dosage forms on glycometabolism in rats with metabolic syndromes

OBJECTIVE: To evaluate the effects of fluvastatin on insulin resistance and hs-CRP level in metabolic syndrome rat model. METHODS: Sixty male KM rats were randomly divided into two groups: 10 rats in control (normal diet) and 50 rats in model (high lipid diet) group. By the end of the 4th week, 10 rats selected randomly from the model group were killed to verify the establishment of metabolic syndrome model. Since the 5th week, the remainder 40 rats were randomly divided into A, B, C and D groups, then were administered intragastrically with fluvastatin capsules at 6 mg · kg-1 · d-1, fluvastatin capsules at 12 mg · kg-1 · d-1, fluvastatin slow-release tablets at 12 mg · kg-1 · d-1 and saline, respectively, and all the rats were fed on normal diet for 4 weeks. All the rats were killed by the end of the 8th week. The levels of TC, TG, LDL-C, HDL-C, FBG, FINS, and hs-CRP were measured respectively and then homeostasis model assessment of insulin resistance was calculated. RESULTS: By the end of the 4th week, the metabolic syndromes model was established. At the end of the experiment, compared with group D, the levels of TC, TG and LDL-C were decreased significantly(P < 0.01) while HDL-C were increased significantly in A, B and C groups(P < 0.05); the levels of hs-CRP, FBG and FINS were decreased significantly(P < 0.01). Compared with group A, the levels of TC, TG and LDL-C were decreased significantly (P < 0.05), while the levels of hs-CRP, FBG and FINS had no change in group B(P > 0.05); compared with group B, the levels of TC, TG, LDL-C, hs-CRP, FBG, FINS and HOMA-IR were decreased significantly in group C (P < 0.05). CONCLUSION: Fluvastatin can reduce TC, TG, LDL-C, FINS, and hs-CRP levels and improve insulin resistance, the effects is more apparent at a large-dosage and with slow-release tablets especially, and that may attribute to its anti-inflammation effects.
2.Salivary cortisol response to electrical stimulation in TMD cases and its relation with pain catastrophizing

Objective:To analyse the salivary cortisol changes in patients with temporomandibular disorders(TMD)after electrical stimulation,and the relationship between cortisol and pain catastrophizing.Methods:1 1 TMD and 1 1 healthy subjects were evoked by electrical stimulation.The saliva samples were collested before test,50 s and 1 00 s of stimulation after test seperately.Salivary cortisol was analysed by ELISA kit.The pain catastrophizing scale(PCS)was answered by all the subjects before stimulation.Data were statis-tically analysed.Results:The salivary cortisol concentration in TMD patients was higher than that in the controls(P <0.05).After stimulation,it showed a downward trend.No interaction between time and groups.The total scores of PCS including three indexes(ru-mination,magnification,helpless)were positively correlated with salivary cortisol concentration.Conclusion:The unpredictable stress causes high level of cortisol.which is related to pain catastrophizing.
3.Clinical significance of serum homocysteine on peripheral vascular plaque in type 2 diabetes mellitus with subclinical hypothyroidism

Objective:To investigate the effect of homocysteine (Hcy) on peripheral vascular plaque formation in patients with type 2 diabetes mellitus (T2DM) combine subclinical hypothyroidism (SCH), and to provide guidance for clinical medication and prognosis judgement.Methods:A total of 125 patients with type 2 diabetes admitted to the NO.3201 Hospital from 2018 Jan to Dec 2019 were selected. 125 patients with type 2 diabetes mellitus were divided into T2DM without plaque group (40 cases), plaque group (23 cases), T2DM with SCH without plaque group (25 cases) and plaque group (37 cases) according to thyroid function and whether they had peripheral vascular plaque. Data were collected to analyze the influencing factors of peripheral vascular disease.Results:(1) The incidence of vascular plaque in T2DM group and T2DM with SCH group was 36.5%(23/63) and 59.7%(37/62), respectively, with significant difference ( P<0.05). (2) There were significant differences in serum thyroid stimulating hormone (TSH), apolipoprotein A (ApoA), triglyceride (TG) and Hcy between T2DM group and T2DM with SCH group ( P<0.05); there was significant difference in low-density lipoprotein (LDL-C) and Hcy between T2DM groups with or without plaque ( P<0.05); there were significant differences in ApoA and Hcy between T2DM with SCH group with or without plaque ( P<0.05). (3) Logistic regression analysis showed that Hcy was the risk factors for the occurrence of vascular plaque in T2DM and T2DM with SCH group ( OR=1.640, 2.695, P<0.05). (4) Receiver operating characteristic (ROC) curve showed that the area under the curve of Hcy in T2DM group was 0.842 and Youden index was 15.75 μmol/L; The Hcy's area under ROC curve was 0.945 and Youden index was 12.9 μmol/L in T2DM with SCH. Conclusions:Hcy is closely related to the presence of peripheral vascular plaque in T2DM patients with subclinical hypothyroidism. The detection of blood Hcy level can provide new ideas for early diagnosis and treatment of peripheral vascular lesions in T2DM patients with SCH.
4.Comparative analysis of FDG PET SUVmax cutoff values in detection of mediastinal lymph node metastasis and hilar/intralobar lymph node metastasis in patients with non-small cell lung cancer

Objective To study the FDG SUVmax cutoff values in detection of metastases in mediastinal lymph nodes (MLN) and hilar/intralobar lymph nodes (HILN) in pre-operative patients with N0-2 stage NSCLC.Methods A total of 69 patients with stage N0-2 NSCLC (40 males,29 females,age 36-87 years) were included in this retrospective study.18F-FDG PET/CT was performed and followed by lung cancer resection with lymph node dissection in 1 month.The excised lymph nodes were compared one by one between their SUVmax and histopathology.The SUVmax cutoff value in detection of lymph node metastases was determined by the ROC curve.Mann-Whitney u test,x2 test,and Fisher exact test were used for data analysis.Results Metastatic MLN and (or) HILN were found in 21 of 69 NSCLC patients.The histopathologic results demonstrated metastases in 61 of 339 lymph nodes.The SUVmax of metastatic lymph nodes (4.95(3.46,7.19)) was significantly higher than that of benign lymph nodes (2.10(1.59,3.22);z=-7.576,P<0.05).The SUVmax of metastatic HILN (6.32 (4.28,8.27)) was significantly higher than that of metastatic MLN (3.90(2.12,6.41);z=-2.921,P<0.05).With cutoff of SUVmax ≥2.5,the sensitivity,specificity and accuracy in detection of all metastatic lymph nodes were 83.6% (51/61),61.9% (172/278)and 65.8% (223/339) respectively,and the parameters were 74.2% (23/31),79.6% (160/201) and 78.9% (183/232) for metastatic MLN,and 93.3% (28/30),15.6% (12/77) and 37.4% (40/107) for metastatic HILN.The diagnostic sensitivities for metastatic MLN and for metastatic HILN were not significantly different (Fisher exact test,P>0.05),while the specificities and accuracies between the two groups were significantly different (x2 values:96.7 and 56.1,both P<0.05).According to ROC curves,cutoff values of SUVmax in detecting metastatic MLN and metastatic HILN were 2.78 and 4.93.With the specific cutoff value,the sensitivity,specificity and accuracy in detection of metastatic MLN were 71.0% (22/31),87.1% (175/201)and 84.9%(197/232),respectively.The corresponding data in detection of metastatic HILN were 73.3% (22/30),77.9%(60/77) and 76.6%(82/107).Conclusion Different cutoff values of SUVmax in detecting metastatic MLN and metastatic HILN should be considered for more accurate lymph nodes staging in patients with NSCLC.
5.Effect of Letrozole Combined with GnRH-a on Serum Estrogen Levels and Ovulation Quality of Patients with Polycystic Ovary Syndrome

Objective:To study the clinical effect of letrozole combined with triptorelin acetate (GnRH-a) on the serum estrogen level and ovulation quality of patients with polycystic ovary syndrome(PCOS).Methods:112 cases of PCOS patients in our hospital from August 2014 to January 2016 were selected and randomly divided into the control group (50 cases) and the observation group (62 cases) according to the wishes of patients.During the first 3~7 days of the menstrual cycle,the patients in the observation group were treated by LE,2.5 mg/d;the patients in the control group were given intramuscular injection of human menopausal gonadotropin (HMG),75 IU/d.when the diameter of the largest follicle (MFD)≥ 18 mm,the patients in the observation group were were given subcutaneous injection of 0.1 mg triptorelin acetate induced ovulation,patients in the control group were given intramuscular injection of Human chorionic gonadotropin HCG6000~10000IU induced ovulation.The ovulation induced ovulation effect and the serum hormone levels and pregnancy outcome were compared between two groups.Results:On the induced ovulation day,no significant difference was found in the endometrial thickness,the number of mature follicles,serum LH and P levels,rate of pregnancy and the luteal function between two groups of patients (P>0.05),the number of dominant follicles,serum E2 and T levels,multiple pregnancy rate,the incidence of OHSS and ovarian cyst in observed group were significantly lower than those of the control group (P<0.05).Conclusion:LE combined with GnRH-a could effectively improve the quality of ovulation in patients with PCOS,reduce the serum level of estrogen,and prevent the occurrence of OHSS,improve the outcome of pregnancy.
6.The expression of GDNF and GFRα1 in childhood nephroblastoma and its clinical significance

Objective To investigate the expressions of GDNF and GFRα1 in childhood nephroblastoma, and therefore its clinical significance. Methods The expression levels of GDNF and GFRα1 were examined by immunohistochemistry of 30 parrafin samples from nephroblastoma section as well as 16 parrafin samples of peritumor renal tissue. Results The pos?itive expression rate of GDNF and GFRα1 in nephroblastoma were higher than those in normal renal tissue(66.7%vs 6.3%;63.3%vs 6.3%). GDNF and GFRα1 were strongly expressed in epithelium and embryo bud, but weakly expressed in lobus intermedius of nephroblastoma. There was no correlation between the expressions of GDNF and GFRα1 with histological types, clinical stage and metastasis of nephroblastoma of patients. Conclusion High expression of GDNF and GFRα1 in childhood nephroblastoma might indicate its role in tumor development.
7.Efficacy analysis on combination use of endoscopy and enteral nutrition in the treatment of acute biliary pancreatitis in elderly patients

Objective To investigate the effectiveness of endoscopy and enteral nutrition (EN) in the treatment of acute biliary pancreatitis (ABP) in elderly patients and its impact on prognosis.Methods Eighty-four elderly patients with ABP were randomly divided into two groups,including 42 cases of combination treatment group,42 cases of routine treatment group.The routine treatment group was managed with normal comprehensive treatment,and in combination treatment group,patients were treated with additional endoscopic retrograde cholangiopancreatography (ERCP),endoscopic sphincterotomy (EST) and endoscopic naso-biliary (ENBD) drainage and EN.The clinical symptoms,serum inflammatory cytokines,complications and prognosis were compared.Results The CRP,TNF-α,IL-2,and serum amylase,glucose,AST after treatment were significantly lowered than those before treatment (P < 0.05).In addition,the levels of CRP,TNF-α,IL-2,and serum amylase,glucose,AST,ALB in combination treatment group were significantly lower than those in routine treatment group (P < 0.05).The time to abdominal pain cessation,time to fever cessation,hospital stay in combination treatment group were (4.6± 1.3),(3.7± 1.3),(13.5± 2.7) d,which were significantly lower than those in routine treatment group [(6.1 ± 1.3),(5.5 ± 2.1),(18.1 ± 3.8) d,P < 0.05].The APACHE Ⅱ score of combination group after treatment was (7.l ± 1.8) point,which was significantly lower than that in routine treatment group [(9.2 ± 2.6) point,t =3.41,P < 0.05)].The incidence rates of ARF,ARDS,peri-pancreatic infection and abdominal hemorrhage,in combination group were 7.1%,4.8%,4.8%,4.8%,respectively,which were significantly lower than those in routine treatment group (23.8%,19.1%,21.4 %,19.1%,P<0.05).The mortality in combiration group was 4.8%,which was lower than 16.7% in routine treatment group with no statistical significant.Conclusions It is a safe and effective to treat elderly ABP patients with endoscopy combined with EN,which can significantly reduce complications and mortality.
