Non-coding RNA (ncRNA) is a class of RNA not coding the protein and plays a significant role in the process of growth and development of diseases.Barrett''s esophagus (BE) is considered as the precancerous lesion of esophageal adenocarcinoma.Several studies showed that ncRNA has the potential value in diagnosis, treatment and designing of targeting drugs.This article reviewed the role of ncRNA, especially microRNA, long non-coding RNA and circular RNA in the development and carcinogenic process of BE.

OBJECTIVEThis study aims to investigate the caries status of the first permanent molar and primary tooth among 7- to 9-year-old children in Tangshan city and the correlation between caries of primary tooth and the first permanent molar.

METHODSA stratified cluster sampling method was used on the basis of the Basic Approach of Oral Health Survey of WHO and the Guideline of the Third National Oral Health Survey. A total of 10 primary schools were randomly sampled in clusters that included 51 primary school sampling spots in Tangshan. The caries status of 1 794 children aged 7 years to 9 years was examined.

RESULTSThe prevalence rate of the first permanent molar caries was 47.49% (852/1 794), the mean number of DMFT was 1.30 ± 1.59, the mean number of DMFS was 1.96 ± 3.40, and the filling rate was only 2.35% (20/852). The preva- lence rate and mean DMFT of the first permanent molar caries showed significant difference between different ages and genders (P < 0.05). However, the mean DMFS of the first permanent molar caries only showed significant difference between the various ages (P < 0.05). The prevalence rate of the primary tooth caries was 83.11% (1 491/1 794), the mean number of dmft was 4.82 ± 3.55, the mean number of dmfs was 6.51 ± 5.61, and the filling rate was 12.41% (185/1 491). Caries of primary tooth was the first permanent molar caries risk factor [r = 0.277, OR = 5.736, P = 0.00, 95% CI (4.175, 7.881)]. Significant correlation was found between the caries of primary tooth and first permanent molar (r = 0.274, P < 0.001).

CONCLUSIONCaries status of the first permanent molar among 7- to 9-year-old children in Tangshan city is serious. Caries status of primary tooth may play an important role in predicting the first permanent molar caries in clinic and prevention.

Objective To investigate the protective effects and molecular mechanism of peroxisome proliferate-activated receptor α(PPARα) activation on acute myocardial damage induced by isoproterenol (Iso) in rats. Methods Thirty male Wistar rats, weighting 160～180 g, were randomly divided into control group, Iso group, fenafibrate(FF) group(each n=10) according to physique quantity. Acute myocardial injury caused by Iso abdomen cavity injection induced ischemia was established and the protective effects of peroxisome proliferate-activated receptor α activation were accessed by the level of ereatine kinase(CK), lactic dehydrogenase(LDH) in serum as well as the activities of myoperoxidase(MPO) in myocardium, and the protein expressions of PPABα in myocardium by Western blot. Results The level of serum CK in control group, lso group and FF group, was (62.41±9.47),(101.71±11.05),(75.64±11.73)kU/L, respectively(F= 34.34, P<0.01). Whereas the level of serum CK in Iso group and FF group was higher than that in control group(P<0.01 or<0.05), the level of serum CK in FF group was lower than that in Iso group(P<0.01). The levels of LDH in these three groups were (5912.20±204.44), (6365.78±137.10), (6089.76±169.60) U/L, respectively(F= 17.54, P<0.01). Compared with the control group, the levels of LDH in Iso and Fir groups were significantly increased(P<0.01 or<0.05). But the level of LDH in FIr group was decreased compared with that in Iso group(P<0.01). The activities of myocardial MPO in these three groups were (1.95±0.10),(3.89±0.17),(2.49±0.19)U/g, espectively(F=391.68,P< 0.01). The activities of myocardial MPO in Iso and FF groups were higher than that in the control group (all P< 0.01), while the activities of myocardial MPO in FIr group were lower than that in lso group(P<0.01). The protein expressions of PPARα in myocardium of these three groups were 251.57±10.95,191.97±10.74,215.08±9.61, respectively(F=82.69, P<0.01). Conclusion PPARα activation by its actor FF can exert protective effects on the acute myocardial ischemia injury induced by lso in rats through inhibiting the release of inflammatory cell factors.

Objective To investigate the relationship between interleukin (IL)-34 and bone erosions in psoriatic arthritis (PsA) patients.Methods Forty PsA patients,20 psoriasis (Ps) patients and 20 healthy volunteers were recruited into this study.The levels of IL-34 and osteoclast related cytokines [including tumor necrosis factor (TNF)-α,receptor activator of nuclear factor-κB ligand (RANKL),osteoclast precursors (OPG)] were detected in the serum samples of all subjects.The correlations among IL-34,the number of osteoclast precursors (OCP),disease activity and imaging scores were analyzed.All data were analyzed by graphpad prism 6.Differences between groups was analyzed with One-way analysis of variance,q tests,and Spearman's correlation was used to explore the relation between disease activity/radiographic scores and laboratory results and followed by linear regressions.Results The serum level of IL-34 in patients with PsA [(328±476) pg/ml] was higher than that in Ps [(33±52) pg/ml,q =3.92,P＜0.01] and healthy controls [(32±32) pg/ml,q =3.93,P＜0.01],the erosive PsA group were higher than the non-erosive PsA group [(449±527) pg/ml and (47±24) pg/ml,q=4.04,P＜0.01].The levels of TNF-α,RANKL and OCP in patients with PsA [(125±79) pg/ml,(488± 475) pg/ml and (17.7±4.8) 5 sigh views] were higher than those in PS [(40±22) pg/ml,(26±3) pg/ml and (5.2± 0.8),q=7.32,6.14 and 2.94,P＜0.01] and healthy controls [(41±19) pg/ml,(65±8) pg/ml and (6.2±1.8),q=6.67,5.62 and 2.71,P＜0.01],whereas the OPG/RANKL ratio in PsA patients (0.5±0.4) was significantly lower than Ps patients (4.3±2.7,q=-3.30,P＜0.01) and healthy controls (1.8±0.6,q=-1.72,P＜0.01).IL-34,TNF-α and RANKL levels were all positively correlated with OCP (r=0.10,P＜0.05;r=0.12,P＜0.05;r=0.13,P＜0.(5,respectively).Conclusion The level of IL-34 is not only high in patients with PsA but also positively correlates with the number of OCP.In PsA,IL-34 is probably related to the OCP and osteoclast differentiation,and further participates in the process of bone destruction.Therefore,IL-34 is promising to become a new target or alternative choice for the treatment of PsA.

Objective:To evaluate the effects of the flowable resin used as pit and fissure sealant using different etching adhesive systems.Methods:60 caries-free extracted human premolars were randomly divided into 4 groups(n =1 5)and treated by Gluma, NT and 3M-Z350 flowable resin(group A);Clearifil SE Bond adhesive and 3M-Z350 flowable resin(group B),phosphoric acid etching,3M-Z350 flowable resin and phosphoric acid etching(group C)and 3Mconcise sealant(group D)respectively.After pro-cessing the tooth surface the pit and fissue of 1 0 sample in each group were sealed.The microleakage was measured by 1 % methyl-ene blue staining(n =8).The material-enamel interface was observed by SEM(n =2).The shear bond strength of the column-shaped samples with the diameter and the height of 3 mm(n =5)on the mesial or dental surface was examined by a test machine. The sealant cartridges and flowable resin cartridges with the diameter and height of 4 mm were used for the crushing strength exami-nation(n =1 0).Results:There was no significant difference in the microleakage among the 4 groups.SEMobservation showed that the resin tags of group A were long and dense and the resin tags of group B were short and sparse,bubbles and cracks were found on the local site in group A and B.The resin tags of group C were long and thin,but combined with tooth tightly;the resin tags of group D were short and dense;the penetration was poor at the bottom of the fissures in the 4 groups.The shear bond strength of Group A was the highest(P <0.01 ),that of group C was higher than that of group B and D(P <0.05).There was no significantly difference between group B and D(P >0.05).The compressive strength of flowable resin groups was higher than that of fissure seal-ant group(P <0.05).Conclusion:The shear bond strength and compressive strength of all-etching bonding system combined with flowable resin is superior to that of self-etching bonding system combined with flowable resin and the traditional sealant.Using Prime&Bond NT bond after acid etching may improve the shear bonding strength.

Objective To investigate the value of bone morphogenetic protein-7 (BMP-7) and the kidney shear wave velocity (SWV) in the diagnosis and treatment of diabetic nephropathy.Methods 150 cases of patients with diabetic mellitus were divided into three groups according to 24h urinary albumin excretion (Ualb):normalbuminuria (NA) group,microalbuminuria (MA) group and clinical proteinuria (CA) group;50 healthy subjects were selected as the control group (NC).The levels of blood BMP-7 were detected and the virtual touch tissue quantification (VTQ) was used to detect renal SWV in four groups.Both the BMP-7 level and the renal parenchyma SWV were compared among four groups.Then correlation analysis was made between blood of BMP-7 level and renal SWV in diabetic nephropathy groups.Results Blood BMP-7 level:the lowest was CA group,the middle one was MA group,and the highest was NA group (P ＜0.05),the NC group and the NA group had no significant difference (P ＞0.05); SWV measured values:CA group was the highest,MA group was middle one and the NA group was the lowest (P ＜ 0.05),the difference between NC group and the NA group was not significant statistically (P ＞0.05).The blood levels of BMP-7 and renal SWV had a significant negative correlation (r =-0.612,P ＜0.05).Conclusions The blood BMP-7 levels downregulate with the progressing of diabetic nephropathy,but the renal SWV increases with the diabetic nephropathy deterioration,and may indirectly reflects blood BMP-7 levels.Both of them have important application values in the diagnosis or treatment of diabetic nephropathy.

Objective To study the effect of Pingchuan Formula(PCF)on Rho/Rock signaling pathway in the mice of asthmatic model, explore the mechanism of PCF on asthma. Methods Forty BALB/C mice were randomly divided into four groups:a control group, a model group,a dexamethasone group and a PCF group. After treated with medicine for 4 weeks, albumen expression of RhoA and RockI in lung tissues were detected by using Western blot, mRNA expressions of RhoA and RockI were detected by using Real-time PCR. Results The albumen and the mRNA expression of RhoA and RockI in the model group were(1.05± 0.20),(1.06±0.08),(6.60±1.09), (6.53±1.84), the control group were(0.76±0.08), (0.84±0.14), (3.82± 1.77), (3.65±1.46),there was a significant difference between two groups(P<0.01). The albumen and the mRNA expression of RhoA and RockI in the dexamethasone group were(0.78±0.11), (0.87±0.32), (4.19± 2.33), (4.09±1.08), there was a significant difference compared with the model group(P<0.01). The PCF group were(0.86±0.12), (0.93±0.14), (4.38±2.01), (4.50±1.13), there was a difference compared with the model group(P<0.05). These was no difference between the dexamethasone group and the PCF group(P>0.05). Conclusion The expressions of Rho/Rock signaling pathway were controlled significantly by PCF, and the curative effect was similar to dexamethasone.

OBJECTIVE: To establish an HPLC method for simultaneous determination of peoniflorin,calycosin and ferulic acid in Kangmin granules.METHODS: The separation was performed on a Kromasil C18 column (250 mm?4.6 mm,5 ?m),and the mobile phase consisted of methanol-acetonitrile-0.4% phosphonic acid (20 ∶ 10 ∶ 70) with flow rate of 1.0 mL?min-1.The detection was performed at wavelength of 254 nm.Column temperature was room temperature and the injection volume was 10 ?L.RESULTS: The linear ranges of peoniflorin,calycosin and ferulic acid were 180～3 600 ?g?mL-1(r=0.999 2),4.6～92.0 ?g?mL-1(r=0.999 1) and 8.0～160.0 ?g?mL-1(r=0.999 4),respectively.The average recoveries were 98.3%(RSD=1.8%,n=9),99.0% (RSD=2.0%,n=9) and 100.5%(RSD=1.1%,n=9),respectively.CONCLUSION: The method is simple,accurate,sensitive and reproducible for the quality control of Kangmin granules.

Objective To explore the application value of plastic biliary stent and fully covered self-expandable metallic stent (FCSEMS) in endoscopic retrograde cholangio pancreatography (ERCP) for treatment of benign biliary stricture after liver transplantation.Methods The retrospective cross-sectional study was conducted.The clinical data of 54 patients with benign biliary stricture after liver transplantation undergoing ERCP treatment who were admitted to the First Affiliated Hospital of Xi'an Jiaotong University between January 2010 and August 2016 were collected.Among 54 patients,44 had simple anastomotic stricture and 10 had non-anastomotic stricture.All the patients underwent stent implantation by ERCP.Patients with stricture within 1 month postoperatively initially selected single plastic stent or endoscopic nasobiliary drainage (ENBD),and then changed into multiple plastic stents at the second stent replacement.Patients with stricture after 1 month postoperatively selected multiple plastic stents,multiple plastic stents after balloon dilation or FCSEMS.Observation indicators:ERCP situations,stent implantation,time of stent indwelling,postoperative complications,stent dislocation,treatment outcome and follow-up situations.Patients were followed up by outpatient examination and telephone interview up to November 2016.Clinical symptoms of patients were observed within 1 month postoperatively and liver function and abdominal ultrasound were retested.Liver function and remission degree of biliary stricture were monitored regularly once every 3 months.Measurement data were described as average (range).Results All the patients underwent successful ERCP,of which 53 completed the process of ERCP and 1 rejected treatment due to economic problems.All the 54 patients received 140 times ERCPs with an average of 2.59 times per person,21 times ENBDs,11 times FCSEMSs and 108 times plastic stent implantations (including 35 times single stent implantations,46 times double stents implantations,23 times 3-stents implantations and 4 times 4-stents implantations).All the 54 patients were followed up for 3-143 months,with an average time of 73 months.Of 44 with anastomotic stricture,34 received plastic stent implantation and 98 times ERCPs,with an average number of stent implantation of 2 (range,1-4) and an average time of stent indwelling of 10.7 months (range,9.0-13.0months);the postoperative acute pancreatitis,biliary infection,hyperamylasemia and adverse stent implantation or dislocation were detected in 4 persons every time,7 persons every time,10 persons every time and 3 persons every time,respectively;26 patients were cured and 5 were improved,with an effective rate of 91.2％ (31/34);3 patients with noneffective treatment continued to undergo ERCP and 3 patients had recurrence of anastomotic stricture.Among 10 patients with initial FCSEMS implantation,12 times ERCPs were performed,with an average time of stent indwelling of 7.6 months (range,6.0-12.0 months);postoperative biliary infection,hyperamylasemia and stent dislocation were detected in 1 person every time,1 person every time and 1 person every time,respectively;8 patients were cured,with an effective rate of 8/10;of 2 patients with persistent stricture,1 patient received contrast examination after stent removal,showing a comparative stricture in level 1 branch of intrahepatic duct and considering combined ischaemia,and then underwent the second implantation using multiple plastic stents;the other patient had elevated level of jaundice at 3 months after stents removal and received ERCP,showing anastomotic inflammatory polyp,and then underwent FCSEMS implantation again.Ten patients with non-anastomotic stricture received plastic stent implantation and 30 times ERCPs,with an average number of stent implantation of 3 (range,2-4) and an average time of stent indwelling of 11.3 months (range,10.0-14.0 months);the postoperative acute pancreatitis,biliary infection,hyperamylasemia and adverse stent implantation or dislocation were detected in 2 persons every time,5 persons every time,2 persons every time and 1 person every time,respectively;3 patients were cured and 3 were improved,with an effective rate of 6/10;of 4 patients with noneffective treatment,2 died of gradually deteriorating liver function and 2 underwent the second liver transplantation.Conclusions Stent implantation in ERCP is safe and effective for treatment of benign biliary stricture after liver transplantation,single plastic stent should be used in the early period (within 1 month) and multiple plastic stents should be used in the later period.Although FCSEMS has a higher displacement rate,it should be recommended due to a better clinical effect,lower incidence of complications and simple operation.For patients with non-anastomotic stricture,plastic stent should be used for extrahepatic biliary stricture,with a good clinical effect,and there is worse effect in stent implantation through ERCP for multiple intrahepatic biliary strictures.

Objective To investigate the causes, diagnosis and surgical treatment of ERCP related duodenal per﹣foration. Methods Clinical data of 6 cases of surgical treatment of ERCP related duodenal perforation were retro﹣spective analyzed. All the 6 perforation patients underwent emergency surgical procedure, including 3 cases trans﹣fered from other hospital after duodenal perforation. 4 cases with a history of abdominal surgery. Preoperative con﹣firmed bravery manager stone 4 cases, 1 case of obstructive jaundice after gallbladder surgery, bile duct expansion in 1 case. Results Perforation causes include duodenum mirror improper operation related in 2 cases, duodenal papilla sphincterotomy related 3 cases (1 case of pre-dissection operation with needle knife), small endoscopic sphincteroto﹣my combined with endoscopic papillary balloon dilation lead to perforation in 1 case.4 cases of intraoperative found right kidney week pneumatosis, 2 cases of postoperative CT found after peritoneal pneumatosis, effusion. All patients with surgery including common bile duct exploration, T tube drainage, duodenal perforation repair, jejunum colostomy, among them 2 cases at the same time line of gastrointestinal anastomosis. 5 cases recovered, 1 case died. Conclusions Inappropriate duodenal papilla sphincter incision indications and Many previous abdominal surgery have higher perforated ration;Found in time, reasonable treatment is the most important;For serious typeⅠand typeⅡperforation, active surgical treatment in time, can effectively reduce serious consequences caused by the ERCP related perforation.
