2.Study Progress on Relationship between Intractable Epilepsy and Multidrug Transporter

Epilepsy is a common nervous system seizure disease in children,20%-30% of them is intractable epilepsy.The pathogenesis of intractable epilepsy in children is not yet identify.Some research think that this may be associated with resistance of a variety of diffe-rent mechanisms of antiepileptic drug and abnormalities of multidrug resistance gene expression.The most investigation of multidrug transpor-ter is P-glycoprotein,multidrug resistance-associated protein.They are present in cell membrane and belong to adenosine triphosphate-dependent membrane transport protein.Studies have showed that over-expression of multidrug transporter in temporal lobe brain tissue of patients with refractory epilepsy and animal model of chronic epilepsy reduce the concentration of therapeutic drug in cells.They result in resis-tance by releasing the energy and transferring large amounts of anti-epileptic drugs to exterior of brain capillary endothelial cells in way of active transport.The relationship between multidrug transporter′s structure,distribution,functions and drug-resistant epilepsy are reviewed.
3.The blood vessel damage and clinical prognosis in patient with type 2 diabetes mellitus and acute cerebral infarction

Objective To explore the effect of type 2 diabetes mellitus(T2DM) on vascular damage characteristics of acute cerebral infarction and the impact on the short term prognosis.Methods One hundred and fifty-six cases of acute cerebral infarction patients were selected and divided into the T2DM group with 64 cases and non-T2DM group with 92 examples.According to the whole cerebral angiogram,the results and the results of the treatment of acute cerebral infarction and prognosis were analyzed.Results The incidence of intracranial artery stenosis of the T2DM group was 79.69% (51/64),of non-T2DM group was 58.70% (54/92),the difference between two groups was significant(x2=12.856,P<0.05).The incidence of before andafter intracranial artery stenosis coexist,circulation narrow coexist in T2DM group was 50.00% (32/64),59.36%(38/64) respectively,in non-T2DM group was 21.74%(20/92),29.35%(27/92) respectively,the difference between groups was significant(x2 =9.652,8.659;P<0.05).The incidence of multivessel lesions and diffuse lesions and without collateral compensatory in T2DM group was 71.88% (46/64),65.63% (42/64) and 71.88%(46/64) respsctively,in non-T2DM group was 54.35%(59/92),39.13%(36/92) and 31.52%(29/92) respectively,thedifference between groups was significant(x2=8.625,9.354,11.053;P<0.05).The total effective rate after 2 weeks of treatment in T2DM group was 46.88%(30/64),in non-T2DM group was 90.22%(83/92),the difference between groups was significant(x2=8.061,P<0.05).Conclusion The incidence of intracranial artery stenosis in patients with intracranial artery stenosis combined with T2DM is higher than the control group,and lesion range widely,diffuse damage,vascular damage moderately severe stenosis and occlusion are significantly higher than in non-T2DM group,especially without collateral compensatory rates is higher than non-T2DM group,while the neural function damage of T2DM group is heavier and poorer prognosis.

Malignant gliomas cells grow invasively with no clear boundary,and a complete resection surgery is very difficult,and the pathological type is not sensitive to radiotherapy and chemotherapy,so the overall treatment effect is not ideal,and this disease is very prone to local recurrence after treatment.The treatment of malignant glioma is mainly composed of surgical resection with postoperative radiotherapy and chemotherapy.Other new treatments include targeted therapy,immune therapy,gene therapy and so on.
5.Pressure measurement and pathological study of the lumbar disc herniation

Objective To investigate the pathological and physiopathological difference between the ruptured” and the “degenerated” typed lumbar disc herniation (RDH and DDH) by measurement of the pressure of the herniated lumbar disc both in the herniated site and in the nucleus pulposus during the surgery, combined with the clinical,light microscopical and immunohistochemical properties. Methods Fifty seven cases of lumbar disc herniation undergone operation in the Tianjin Medical University Hospital were included in this study. The patients were divided into 2 groups. 1) Rupture typed lumbar disc herniation in which the herniated chip of the lumbar disc separated from the parent disc and was squeesed or drew out easily when the superficial posterior longitudenal ligament and anulus fibrosis were opened in the herniated site. 2) Degenerated typed lumber disc herniation in which no herniated chip of the lumber disc escaped; the herniated material can not be taken out unless the disc was resected by the instrument. Before the herniated disc was incised, the intervertebral disc pressure was measured using GENERAA710 multifunctional monitor. Both the light microscopical and the immunohistochemical difference between the RDH and the DDH was studied. Results 1) In the herniated site, the pressure in the RDH is higher than that in the DDH(P0.05). 3) In the RDH, the pressure in the herniated site is higher than that in the nucleus pulposus (P0.05). 5) The“lumbar pain score”and the pressure have obviously relationship in all of 35 patients(r=0.539,P

8.Brief Introduction of Pharmacovigilance System in the United States and Its Implication for China

Objective To make suggestions for China's drug safety supervision through an introduction of the status of pharmacovigilance system in the United States (US).Methods Through legal research and document research,the author introduced and analyzed the current situation of pharmacovigilance system in the US from the organization and responsibilities of Food and Drug Administration,system of laws and regulations,risk management and evaluation methods of post-marketing drugs etc.Results The US has now established a relatively mature pharmacovigilance system.This system has a perfect drug safety supervision organization structure,a sound system of laws and regulations and a scientific evaluation system of drug risk management.Conclusion The relevant experience of the US can be learned by China,such as the communication & collaboration between pre-and post-market drug regulatory agencies,instillation of drug risk management and establishment of guidance for industry to promote the development and improvement of China's drug safety supervision.
10.Expression and signifance of TRIM59,Twist and E-cadherin in hepatocellular carcinoma

Objective To investigate the expressions of TRIM59,Twist and E-cadherin in hepatocellular carcinoma and their clinical significance. Methods The expressions of TRIM59,Twist and E-cadherin protein were tested by immunohistochemistry in 80 cases of hepatocellular carcinomas and the adjacent paracancerous tissues. Results The positive rate of TRIM59 in hepatocellular carcinoma was significantly higher than that in the adjacent paracancerous tissue(76.3%vs. 8.0%,P<0.05). Significant difference was also observed in the expres-sion rate of Twist between the hepatocellular carcinomas and the paracancerous tissue(66.3%vs. 6.0%,P<0.05). The positive rate of E-cadherin in hepatocellular carcinoma was significantly lower than that in the adjacent para-cancerous tissue(27.5%vs. 90.0%,P<0.05). The differences of the expression of TRIM59 in hepatocellular carci-noma of pathological grading,tumor differentiation,vascular invasion and clinical TNM stage were significant(P<0.05). The differences of the expression of Twist in hepatocellular carcinoma of pathological grading ,differentia-tion,vascular invasion and clinical TNM stage was also significant(P<0.05,respectively). The differences of the expression of E-cadherin in hepatocellular carcinoma of pathological grading,differentiation,vascular invasion and clinical TNM stage were also significant(P<0.05,respectively). Significantly positive correlation was also found between TRIM59 and Twist by using spearman correlation analysis(P<0.05). Negative correlations were observed between TRIM59 and E-cadherin(P < 0.05),and between Twist and E-cadherin(P < 0.05). Survival analysis showed that TRIM59 expression was an independent prognostic factor in hepatocellular carcinoma. Conclusion TRIM59,Twist and E-cadherin protein expression might be associated with the development,invasion,and metas-tasis of hepatocellular carcinoma. TRIM59 may become a new target gene for the treatment of human hepatocellular carcinoma.
