1.Clarify some problems when selecting multifocal intraocular lens

Recently more and more ophthalmologists selected a multifocal intraocular lens (IOL) for their cataract patient in order to get good unaided reading vision. Although the design of IOL have made a great progress, we are still dissatisfied compared with our natural crystal lens. Understanding the design theory and improving our surgery technique are basic for better utilization the multifocal IOL. Does the multifocal IOL reduce contrast sensitivity? Should we implant the multifocal IOL in pediatric cataract surgery? How can we deal with the residual refractive error after multifocal IOL implantation? In this editorial, we try to clarify these problems to make us select and use multifocal IOL better.
2.Vascular intervention joint foot plastic surgery in the treatment of diabetic foot

Objective To evaluate the chnical value of intervention therapy combined with the foot plastic surgery in the treatment of diabetic foot.Methods 67 patients who accepted the intervention therapy combined with the plastic surgery for occlusive lesion of lower limbs arteries were selected.The clinical symptom,ankle brachial pressure index and hemodynamics were observed before and after treatment.All clinical indicators were compared between the therapeutic group and the control group.Results Among 67 patients,63 cases were successful in clinical treatment (the success rate:94.0%).One month,three months,six months after operation,the skin temperature,anklebrachial index,limp distance,clinical symptom,ankle brachial pressure index and hemodynamics were increased.Compared with preoperation,all above-mentioned indicators were improved obviously (P < 0.05,P < 0.01).Conclusion Intervention therapy for diabetic foot has the merits of less trauma,higher effectiveness of treating diabetic foot ulcers,defect repair,maximally reserved limbs and better shapes of limbs,which made both doctors and patients satisfied.It is a new direction for minimally invasive therapy.
3.Effect of Bronchoalveolar Lavage on the Inflammatory Mediators and Pulmonary Function of Patients with Refractory Pneumonia

Objective:To study the effect of bronchoscope lavage on the inflammatory mediators and pulmonary function of patients with refractory pneumonia.Methods:100 cases of patients with refractory pneumonia who were treated in our hospital from January 2010 to December 2015 were selected and randomly divided into two groups,the control group was treated with routine clinical treatment,and the observation group was treated with bronchoscope lavage.The curative effect and the changes of inflammatory mediators and pulmonary function of the two groups after treatment were compared.Results:After treatment,the effective rate of observation group was 90.00%(45/50),which was significantly higher than that of the control group (76.00%,38/50) (P<0.05);the levels of endotoxin,IL-6,TNF-2 and LPO in the bronchoalveolar lavage fluid of both groups were significantly decreased (P<0.05),which were decreased more significantly in the observation group (P<0.05),the pulmonary function of both groups were significantly improved(P<0.05),which improved more significantly in the observation group(P<0.05).Conclusions:Bronchoscope lavage had good therapeutic effect on the patients with refractory pneumonia,it could effectively reduce the level of inflammatory mediators and improve the lung function.
4.Preoperative serum cystatin C combined with dipstick proteinuria predicts acute kidney injury after cardiac surgery

Objective To evaluate serum cystatin C combined with dipstick proteinuria as early markers to predict AKI available before surgery.Methods We prospectively followed 616 patients undergoing cardiac surgery.Univariate as well as multivariate regression was performed.Cystatin C combined with dipstick proteinuria before surgery was assessed for its' predictive value of AKI using receiver operator characteristic (ROC) curves.Results Patients in higher cystatin C quartiles were older(P < 0.001),more often to have heavy proteinuria(P =0.021),hyperuricemia(P < 0.001),heart failure(P < 0.001)and recent MI (P =0.002).Those with heavy proteinuria were more often to have diabetes mellitus (DM) (P =0.010),hyperuricemia (P =0.043),worse cardiac function (P < 0.001),higher creatinine levels (P < 0.001) and lower eGFR levels (P <0.001).In a multiple logistic regression model,preoperative heavy proteinuria(OR =3.14) and preoperative cystatin C quartiles each associated with an increased odds of AKI,independent of advanced age (OR =1.04),hypertension (OR =1.88) and combined surgery(OR =3.47).The risk for adverse outcomes such as postoperative AKI,persistent AKI,severe AKI,dialysis and mortality were highest in patients with highest quartile of cystatin C(P <0.05,respectively) and heavy proteinuria (P < 0.05,respectively).The area under the ROC curve(AUCs) for preoperative cystatin C combined with proteinuria to detect AKI,persistent AKI and severe AKI were 0.695,0.753 and 0.718,P <0.001 respectively.Conclusion These data suggest that preoperative serum cystatin C combined with dipstick proteinuria may improve prediction of AKI among patients undergoing cardiac surgery.
5.Risk factors for acute kidney injury in overweight patients with acute Stanford type A aortic dissection following Sun's pocedure

Objective To identify the risk factors for acute kidney injury (AKI) in overweight patients who underwent surgery for the treatment of acute type A aortic dissection(TAAD).Methods A retrospective study including 108 consecutive overweight patients(body mass index(BMI) ≥24) between January 2010 and May 2013 in Beijing Anzhen Hospital was performed with Sun's procedure of total aortic arch replacement and frozen elephant trunk implantation.AKI was as defined according to Acute Kidney Injury Network (AKIN) criteria based on serum creatinine(sCr) or urine output.Results The mean age of the patients was(43.69 ± 9.66) years.During the postoperative period seventy-two patients(66.7%) developed AKI.The overall postoperative mortality rate was 7.4%,8.3% in AKI group and 5.6% in non-AKI group.There is no statistically significant difference between the two groups(P =0.32).A logistic regression analysis was performed to find out the independent risk factors for AKI:elevated preoperative sCr level and postoperative drainage volume.Renal replacement therapy(RRT) was performed in fifteen patients (13.9 %).Conclusion A higher incidence of AKI (66.7 %) in overweight patients following acute TAAD was identified.The logistic regression model found out elevated preoperative sCr level and 72 hour drainage volume as the two independent risk factors for AKI in overweight patients.More attention should be paid to prevent AKI in overweight patients following TAAD.
6.Estrogen regulates anoikis through extracellular signal-regulated kinase (ERK)-focal adhesion kinase (FAK) signaling in MCF-7 breast cancer cells

Objective To investigate the effects of estrogen ( E2) on the resistance to anoikis and a possible role of extracellular signal-regulated kinse ( ERK)-focal adhesion kinse ( FAK) signaling in the effect of estrogen to under-stand its underlying mechanism .Methods Poly-Hema-coated culture of human breast cancer cell line MCF-7 was used to induce anoikis .Cells were treated with E2 and/or pretreated with MEK or FAK inhibitors .Western blot was used to assess the phosphorylation of ERK and FAK , trypan blue staining and cell counting were employed to evaluate cell viability , and Hoechst staining was used to check apoptosis .Results Suspension culture greatly re-duced cell survival (P<0.01), and exposure of MCF-7 cells to E2 (10 nmol/L) led to a significantly increased resistance to anoikis and survival ( P<0.05 ) as compared to DMSO .Meanwhile , E2 induced increased phospho-rylation of both ERK and FAK .Pharmacological inhibition of MEK with U 0126 ( 10 μmol/L ) reduced E2-in-creased cell survival by 57.48%(P<0.01) and E2-decreased anoikis;Treatment with FAK inhibitor (10μmol/L) attenuated E2-enhanced cell survival by 53.59% ( P<0.01 ) and E2-reduced apoptosis .Conclusions E2 con-tributes to the enhanced cell viability and increased resistance to anoikis in MCF-7 breast cancer cells , and ERK-FAK signaling may be involved in the E 2-stimulated survival during suspension culture of MCF-7 cells.
7.Effect of reduced expression of Her2 by RNA interference on the biological characters of ovarian carcinoma cells

Objective To observe the effects of short hairpin RNA (shRNA) targeting Her2 on its gene expression when the shRNA was stably transfected into human ovarian cell lines, SKOV3 and SKOV3. ipl, which have different extent of malignancy and investigate the changes of the biological characters of the two cell lines after the stable transfection. Methods The plasmids expressing shRNA targeting Her2 gene were transfected into SKOV3 and SKOV3. ipl cells. The stably transfected cells were gained by antibiotic screening. The expression of Her2 before and after the transfection was detected by RT- PCR and western blot. The transwell experiment was used to observe the invasion ability of the cancer cells before and after the transfection, and the parent and the transfected cells were injected into nude mice to observe the tumor growth. Results After the stable transfection with Her2 shRNA, mRNA and protein levels of Her2 gene in SKOV3 and SKOV3. ipl cells were remarkably reduced. The expression of mRNA were (68.0±3. 1) %, (40. 8±2. 0) %, (99. 9±1.3) %, (42. 4±2. 5) %. The expression of protein were (72. 1±3.4) %, (36. 4±1.5) %, (98.2±1.7) %, (40. 7±2. 1) %. The invasion ability into basilar membrane of the transfected cells was greatly reduced compared with the parent cells. The invasion cell numbers were 7.6±1.1, 1.8±0. 8, 36. 2±9.7, 15.7±7. 2. The growth rate of the planted tumors was lower in transfected groups than that of the parent groups. Conclusions (1) The expression of Her2 gene in SKOV3 and SKOV3. ipl cells was remarkably reduced by RNA interference targeting Her2. (2) The biological characters of SKOV3 and SKOV3. ipl cells are changed when the expression of Her2 gene is reduced by RNA interference.
8.Analysis of point-of-care testing researches in China based on CNKI database

Objective:To identify current research hotspots, research trends and future trends of point-of-care testing(POCT) in China, by means of literature review.Methods:The " point-of-care testing" and " POCT" were used as the search terms, to retrieve in CNKI database the literature on point-of-care testing published during 2010-2019 in the method of subject retrieval. CiteSpace software was used to analyze and generate the research institutions and their cooperative networks, the keyword co-occurrence network, and the burst term analysis, for an analysis of the current researches in point-of-care testing in China.Results:A total of 588 literatures were retrieved, and 485 literatures were finalized as research samples, with exclusion of those obviously irrelevant. The research found that the point-of-care testing studies in China focused on portable blood glucose meters, quality control of POCT, comparison of POCT and biochemical analyzer results, and establishment of a POCT quality system. In terms of future research tend, the hotspots fell on " total quality management system of POCT" , " POCT in cardiovascular disease application" , " POCT in battlefield operation or emergency application" .Conclusions:Given the growing number of relevant documents on point-of-care testing, and the expanding scope of researches, regional research centers have yet to be formed.
9.Study on the sensitivity of different 3D detectors to MLC positioning errors in volumetric modulated arc therapy (VMAT) plan for nasopharyngeal carcinoma

Objective To compare the sensitivity of Deha4 and ArcCHECK 3D detectors to detect the multi-leaf collimator (MLC) positioning errors in the dose verification of volumetric modulated arc therapy (VMAT) plans for nasopharyngeal carcinoma (NPC).Methods Ten NPC patients receiving VMAT plans were selected.The positioning error of 0.5-4.0 mm was introduced into the leaves of each MLC segment in the original file to expand,contract or shift the whole segments.The possible positioning errors of MLC in the treatment of VMAT were simulated.The Delta4 and ArcCHECK were utilized to verify the measurements.The absolute gamma passing rate was compared between the calculated dose and the measured dose of the VMAT plan by using the paired t-test.Results When the evaluation criterion was taken as 3 mm/3%,the absolute passing rate verified by the original plans of two detectors was greater than 95%.The positioning errors of MLC expansion,contraction and shifting detected by Delta4 and ArcCHECK were 1.5 mm,1.0 mm,2.0 mm and 3.0 mm,1.0 mm,and 3.0 mm,respectively.When taking 2 mm/2% as the evaluation criterion,the absolute passing rate verified by the original plan was decreased significantly.Delta4 and ArcCHECK detected that the positioning errors of MLC expansion,contraction and shifting were 1.0 mm,1.0 mm,2.0 mm and 1.5 mm,0.5 mm,and 2.0 mm,respectively.Conclusions The dose verification of the VMAT plan for NPC by Delta4 and ArcCHECK can detect different types and sizes of MLC positioning errors,whereas the detection sensitivity slightly differs between Delta4 and ArcCHECK.Both of them are not sensitive to detect the MLC positioning errors less than 1.0 mm.It is fairly necessary to strengthen the quality assurance of MLC in the daily work.

The field of traditional Chinese medicine has a long history of applying modern science method in related research,and has made significant achievements in the field of disease and syndrome animal models.This paper reviewed the development history of the animal model of spleen deficiency syndrome,summarized and commented the modeling method,animal selection,evaluation index,digestive system disease and syndrome combination model.Small rodents,such as rats,are commonly used as representative animal models in research.Currently,the academic community mostly focuses on etiology and pathogenesis modeling,with limited use of chemical drugs and physical factors for modeling.The most common approach is the complex factor modeling method.Evaluation indicators include traditional Chinese medicine syndrome indicators,quantitative indicators related to"spleen"function and characteristics,as well as prescription counter-indicators.The focus of current research is the combination of disease patterns related to spleen deficiency,such as functional diarrhea/dyspepsia.The research on animal models of spleen.
