1.The comparative study of membrane oxygenator DDMO-100S and membrane oxygenator terumo SX-10 used in cardiopulmonary bypass in children

Objective To compare the gas and heat exchange performance of membrane oxygenator DDMO-100S and membrane oxygenator Terumo SX-10 in cardiopulmonary bypass(CPB)in children.Methods 39 children aged 3-12 yrs who suffered from congenital heart disease were selected,and the repair of ventricle septal defect or/and atrial septal defect was performed under cardiopulmonary bypass.The 39 children were randomly divided into Group DD(n =21)and Group SX(n = 18).Membrane oxygenator DDMO-100S(Made by Xi'an Daidai,China)was used in Group DD and Membrane oxygenator Terumo SX-10(Made by Terumo,Japan)in Group SX,respectively.The hypothermic cardiopulmonary bypass with high perfusion flow and the cool crystalloid cardioplegia were applied in all children.The artery blood samples were taken for measuring PO2 and PCO2 and the oxygenating index(OI)was calculated by the formulae(OI = PaO2/FiO2).The temperature at nasal pharynx was monitored and the rewarming time was recorded.Results The total priming volume in Group DD was bigger than in Group SX [(742.5 ± 107.3)ml vs(531.3 ± 84.3)ml,P < 0.05].The PO2,OI and PCO2 at 15 min after CPB or at 5 min before cease of CPB had no differences in both group(P >0.05).The rewarming time had no difference in both group [(31.5 ± 10.2)min vs(30.61 ± 8.2)min,P >0.05].Conclusions Under our observation condition,the gas and heat exchange performance of membrane oxygenator DDMO-100S were the same as membrane oxygenator Terumo SX-10,when it was used in cardiopulmonary bypass in children,but it had a little bit more priming volume.
2.Development of macroporous calcium phosphate cements as bone substitutes by using gelatin microspheres

[Objective]To develop macroporous calcium phosphate cements(CPCs) by incorporating the gelatin microspheres(GMs) and to evaluate the performance of the composite as a bone substitute in vitro and in vivo. [Methods]GMs were prepared by improved emulsified cold-condensation method and macroporous CPCs were developed using GMs with three different weight ratios (0%,2.5% and 5%). The porosity and the compression strength of the three composites were measured to determine the proper weight ratio of GMs. The morphologic changes of osteoblasts were examined with scanning electron microscope at two days after the cells were seeded on the CPCs and GMs/CPCs. The cells were also cultured with different leaching liquor of three kinds of materials (0%,2.5% GMs/CPCs and polystyrene).Cell viability analysis was performed by MTT assay,and alkaline phosphatase was measured with alkaline phosphatase kit. At six months after implanted in the goat vertebre ,the samples with CPCs or GMs/CPCs were obtained and evaluated by X-ray image and histological anatomy.[Results]Porosity of the GMs/CPCs increased with the increase of GMs,but compression strength decreased. The proliferation and differentiation of the osteoblasts were enhanced with the 2.5% GMs/CPCs compared to the CPCs. In vivo study showed that the degradation of GMs/CPCs was improved.[Conclusion]Macroporous calcium phosphate cements can be developed by incorporating gelatin microspheres,and biocompatibility and degradability of the composite can be enhanced. The composite can be used as a bone substitute under non-loaded circumstances.
3.Effect of leucine-rich α2-glycoprotein-1 on ischemic stroke by regulating transforming growth factor β signaling pathway

As one of the leucine-rich repeat protein family members, leucine-rich α-2 glycoprotein 1 (LRG1) affects many diseases by transforming growth factor (TGF)-β signaling pathway, and is closely associated with angiogenesis, endothelial cell apoptosis and autophagy, inflammatory reaction and blood-brain barrier damage after cerebral ischemia. It is expected to become a new marker and therapeutic target of ischemic stroke. However, at present, there are few studies on investigating the relationship between LRG1 and ischemic stroke, and the understanding of its molecular mechanism is not yet complete, resulting in controversy about the role of LRG1 in ischemic stroke. Therefore, this article reviews the research progress of LRG1-TGF-β signaling pathway and ischemic stroke, hoping to provide new ideas for the early diagnosis, prevention and treatment of ischemic stroke.
4.Disturbance of nivalenol on expression of adhesion molecule CD44 on surface of cultured chondrocytes

BACKGROUND: Deficit of nivalenol (NIV) and selenium (Se) is related with kashin-beck disease (KBD) to certain extent. Hyaluronic acid (HA) metabolism affects directly the polymerization of proteoglycans (PG) and normal structure and function of cartilage. The integration with HA receptor on surface of cartilage tissue is the key link in HA metabolism. Being the main receptor of HA on chondrocytic membrane, CD44 expression impacts directly HA metabolism, further affects cartilage matrix metabolism, which is extremely important to maintaining the structure and function of cartilage matrix.OBJECTIVE: To probe into the injury and protection of related etiology of KBD to target tissue cells and the mechanism on degenerative necrosis of chondrocytes.DESIGN: Randomized controlled observation was designed.SETTING: Department of Genetics and Molecular-Biology of Medical College of Xi' an Jiaotong University.MATERIALS: The experiment was performed in Key Laboratory Room on Ministry of Education associated with Environment and Disease of Xian Jiaotong University from October 2002 to July 2004. One New Zealand pedigree young rabbit aged 30 days was employed and its humerus, femurs and tibia were cut out in surgery.METHODS: With cell culture, the model of bone tissue was reconstructed in vitro, in which, NIV of various concentrations, KBD suspicious infectious agent and Se, the protective factor were added. HA receptor CD44 on chondrocytic membrane and soluble CD44 in cell culture solution were determined.MAIN OUTCOME MEASURES: ① Microscopic observation of adhesion molecule CD44 on chondrocytic surface. ② Soluble CD44 in chongrocytic culture solution.RESULTS: ① Microscopic observation of adhesion molecule CD44 on chondrocytic surface: CD expression in chondrocytic membrane was decreased with increasing of NIV concentration and it was in tendency of increasing with Se added. ② Soluble CD44 in chongrocytic culture solution:The concentration of soluble CD44 in cell culture solution was decreased gradually following the increased concentration of NIV, but it was increased in high concentration group and such tendency did not alter when Se added. Except blank control and Se control, significant difference was presented among groups (P < 0.05).CONCLUSION: NIV disturbs adhesion molecule CD44 expression on chondrocytic surface and further induces metabolic disturbance of cartilage extracellular matrix. Se supplementation can resist the injury of NIV to chondrocytes, but its action is limited.
5.Defects of fore foot repaired by reverse medial dorsal neurocutaneous flap of foot in emergency

Objective To investigate the possibility of repairing the defects of fore foot by reverse medial dorsal neurocutaneous flap on foot in emergency.Methods Flap was designed along medial dorsal cutaneous nerve of foot,the axis of the medial branch was the line between lateral part of malleolar middle point and medial part of first metatarsophalangeal joint,and the axis of the lateral branch was the line between middle point of two malleolars and the tip of the great toe webspace,the pedicle located on the distant part of the foot.Based on the size of wound,the flap was cut off from the deep fascia layer,and medial dorsal cutaneous nerve of foot was anastomosed with nerve of digitales plantares proprii.All patients were operated in emergency.Results Eleven patients with defect of fore foot were perfectively recovered and all the wounds healed primarily.The appearances and functions of the foots were satisfactory,with two point sensation of 2-3 cm after 6-11 months postoperatively and without any skin fester.Conclusion Medial dorsal cutaneous nerve of foot has advantages as follows:simply procedures,avoidance of sacrificing major arteries,less harm to donor area,good recovery of sensation.So it is a good method to repair tissue defects of fore foot.
6.Numerical simulation of chemico-biological particles transportation inside human micropassage

Objective To investigate the transportation of chemico-biological particles(CBP) through the micropassage inside the human body in order to improve chemico-biological protection.Methods Dissipative particle dynamics (DPD) method was used to study CBP transportation through micropassages inside the human body.Results The Poiseuille flow could be ensured by imposing boundary conditions including pressure gradient and no-slip.The axial velocity between fluid particles and CBPs was well matched except the area close to the passage wall.However, CBPs tended to accumulate and the density of CBPs slightly increased, leading to the jam effect and producing particle accumulation.Conclusion The characteristic of CBP transportation is better understood,which can help develop some chemico-biological protection devices according to movement of CBPs and improve the performance of CBPs during chemico-biological protection.
7.Design mechanism and application of quasi-zero stiffness isolator

Objective To develop a new quasi-zero stiffness isolator for the vibration isolation of the sick and wounded on the ambulance stretcher.Methods The isolator was designed and verified on the basis of the quasi-zero stiffness of slotted disk spring, vibration isolation design method with high static while low dynamic stiffness and co-laminated structure of the spring.Results The theoretical analyses and tests proved the force-displacement curves of the new quasi-zero stiffness isolator fit well. The frequency-weighted acceleration at the buttock and head under different speeds on the asphalt roads was less than 0.315 m/s2.Conclusion The new quasi-zero stiffness isolator developed behaves well in vibration isolation and comfortability for the ambulance stretcher sick and wounded.
8.Efficacy of Drugs with Different Mechanisms of Action on Functional Dyspepsia:A Comparative Study

Background:Oryz-Aspergillus Enzyme and Pancreatin Tablet is a double-deck digestive enzyme pellet containing Oryz-Aspergillus enzyme and pancreatin that has been widely used for treatment of functional dyspepsia (FD)in clinical practice. However,there is no randomized controlled trial focusing on the efficacy of this agent versus other drugs used for treatment of FD such as prokinetics and proton pump inhibitors (PPI). Aims:To compare the efficacy and safety among Oryz-Aspergillus Enzyme and Pancreatin Tablet,Domperidone Tablet and Esomeprazole Magnesium Enteric-coated Tablet, three drugs with different mechanisms of action on FD,in Chinese population. Methods:A total of 82 Helicobacter pylori-negative outpatients fulfilling the diagnostic criteria of FD in Rome Ⅲ were recruited from Nov. 2015 to Jun. 2016 at the First Affiliated Hospital of Zhejiang Chinese Medical University. These patients were randomly allocated into 3 groups,and received Oryz-Aspergillus Enzyme and Pancreatin Tablet (group A),Domperidone Tablet (group B)and Esomeprazole Magnesium Enteric-coated Tablet (group C)orally for 4 weeks,respectively. The improvement of dyspeptic symptoms and adverse events were observed and recorded. Results:After 4 weeks treatment,the overall efficacies for global symptoms in group A,group B and group C were 93. 1%,88. 9% and 69. 2%,respectively,statistically significant difference was existed among the three groups (P < 0. 05). Domperidone Tablet was effective for postprandial fullness and early satiety;Esomeprazole Magnesium Enteric-coated Tablet was sensitive for epigastric pain,epigastric burning,and belching and regurgitation;the efficacies of Oryz-Aspergillus Enzyme and Pancreatin Tablet for all five dyspeptic symptoms were in between. No adverse events were observed during treatment course. Conclusions:Digestive enzymes,prokinetics and PPI have different sensitive symptoms and optimal indications for treatment of FD. The overall efficacy of Oryz-Aspergillus Enzyme and Pancreatin Tablet is superior to that of prokinetics and PPI.
9.Analysis of plasma miRNAs expression profile in different subtypes of ischemic stroke

Objective To investigate the changes of circulating miRNAs expression profiles in different subtypes of ischemic stroke according to the Trial of Org 10 172 in Acute Stroke Treatment.Methods We selected 16 patients diagnosed as acute ischemic stroke at first time in the Department of Neurology,the Affiliated Hospital of Medical College Qingdao University from November to December in 2012.They were divided into large artery atherosclerosis (LAA) stroke group (n =8) and small artery occlusive (SAO) stroke group (n =8).At the same time,8 healthy checkup subjects were selected as control.High-throughput sequencing was used to detect the expression profiles of miRNAs in the plasma,and the high-throughput sequencing results were validated by quantitative real-time polymerase chain reaction.We performed the miRNAs variance analysis and associated bioinformatics analysis.Results Thirty hundred and sixty-nine miRNAs were detected in LAA group,SAO group and the control group.We found remarkable differences (fold change >2,P <0.01) in the expression of 12 miRNAs,including let-7a-5p,miR-744-5p,etc.,between any two groups of the three groups.Thirty-four miRNAs,containing miR-126-5p,miR-23a-3p,miR-143-3p,etc.,had a lower expression (fold change >2,P <0.01)in LAA group than that in the control group.In comparison with the control group,miR-1304-3p and miR-451a were significantly down-regulated (fold change > 2,P < O.01),while 27 miRNAs were significantly up-regulated (fold change >2,P <0.01) in SAO group.The expression levels of miR-146b-5p,miR-23a-3p and miR-451 a were validated in accordance with the results of real-time PCR.Target gene prediction and functional analysis revealed that target genes regulated by differential expression of miRNAs were mainly associated with cell proliferation,adhesion,metabolism and other biological functions.Conclusion miRNAs are differently expressed in the plasma of LAA group,SAO group and healthy control group,which suggest that miRNAs might play different roles in the pathogenesis of LAA stroke and SAO stroke by regulating downstream target genes.
10.Work engagement and its influencing factors in emergency nurses

Objective To investigate the current status and influencing factors of work engagement in emergency nurses, put forward a feasible strategy, improve the emergency department nurses work engagement for the emergency department to build effective human resource management to provide the reference. Methods A total 226 emergency nurses from 8 hospital of first-class in Tianjin city were recruited by the convenience sampling method and investigated with a self designed, the Medical Personnel′s Work Engagement Questionnaire. Results The total score of Medical Personnel′s Work Engagement Questionnaire was 111-166(137.32 ± 12.48) points and the scores were the highest in interpersonal concordance (4.20 ± 0.46) points and the lowest in work vigor (3.01 ± 0.61) points. Variance analysis and multiple linear regression analysis showed that gender, employment category and number of years as an emergency nurses were the influencing factors of work engagement, which could explain 51.3%of the variance. Conclusions Work engagement emergency of nurses is at a relatively moderate-to-high level, through establishing an effective incentive mechanism, increasing social benefits, accelerating the construction of emergency specialist nurse training system and paying attention to occupational development, work engagement level and quality of emergency medical care can be improved.
