Objective To investigate the therapeutic efficacy of tiotropium bromide powder inhalant combined with meglumine adenosine cyclophosphate in the treatment of acute exacerbation of chronic pulmonary heart disease.Methods A total of 148 cases of acute exacerbation of chronic pulmonary heart disease were divided into observation group (n =74) and control group (n =74).The observation group received tiotropium bromide powder inhalant combined with meglumine adenosine cyclophosphate,while the control group was given meglumine adenosine cyclophosphate.Results Before treatment,the forced expiratory volume in one second(FEV1),forced vital capacity(FVC),arterial oxygen tension(PaO2),arterial carbon dioxide partial pressure(PaCO2) and pH of the observation group were no significant differences [(1.27 ±0.44)L vs (1.24 ±0.47)L,(2.49 ±0.74)L vs (2.43 ±0.77) L,(51.6 ±6.6) mmHg vs (51.2 ±6.7) mmHg,(69.1 ± 8.4) mmHg vs (69.5 ± 8.7) mmHg,7.27 ± 0.03 vs 7.26 ± 0.04,P ＞ 0.05].After treatment,The FEV1,FVC,PaO2,PaCO2 and pH were significant differences between the 2 groups[(1.45 ±0.39)L vs (1.31 ±0.41)L,(2.71 ±0.63)L vs (2.49 ±0.54)L,(72.8 ±5.3) mmHg vs (62.5 ±6.2)mmHg,(47.7±5.7)mmHgvs (56.2±7.2)mmHg,7.34±0.02 vs7.31±0.03,t =2.13,2.28,10.86,-12.65,7.16,P ＜0.05].Before treatment,the Ⅰ ～ Ⅳ NYHA cardiac functional grading were no significant differences between the 2 groups(0,17.6％,47.3％,35.1％ vs 0,18.9％,44.6％,36.5％,P ＞0.05).After treatment,theⅠ ～ Ⅳ NYHA cardiac functional grading of the observation group were 25.7％,44.6％,20.3％,9.4％ respectively,and the NYHA cardiac functional grading of the observation group was significantly better than that of the control group(20.3％,27.0％,36.5％,16.2％,t =2.27,P ＜0.05).Conclusions The tiotropium bromide powder inhalant combined with meglumine adenosine cyclophosphate showed good therapeutic efficacy in the treatment of acute exacerbation of chronic pulmonary heart disease,and this combined treatment program is able to improve the heart and lung function simultaneously.

Objective To study the adverse effect of mercury on the tibial nerves of rats. Methods Seventy SD rats (35 females, 35 males, weighed 160-200 g) were randomly divided into 4 groups. 40 rats in group 1 were used to test the acute toxicity of HgCl2, 10 rats in group 2 were given HgCl2 by gavage at 17 mg/kg(1/4 LD50) daily, in group 3 the dose was 8.5 mg/kg(1/8 LD50), 10 rats in group 4 were treated with 0.9% saline 2 ml by gavage daily, the experimental duration was (20?4) days. When the model had been developed, all the male rats were killed and the female rats in group 2 and 3 were given the chelator(DMPS) by intraperitoneal injection at 28 mg/kg for 2 periods of treatment, then were killed and the tibial nerves were examined with the electronic microscope. Results The oral LD50 of HgCl2 was 68.1 mg/kg. Seven rats in group 2 (7/10) were poisoned and two of them got pain manifestation. Six rats in group 3(6/10) were poisoned. Myelinoclasis and the changed axons were found in the tibial nerves of poisoned rats and it was more serious in the rats with pain. The chelator(DMPS) did not relieve the change. Conclusion Sub-acute poisoning of HgCl2 can induce degeneration of tibial nerves in SD rats .

Objective To study the effect of using handhold magnifier on reading speed in patients with low vision and normal-sighted people. Design Prospective,control study. Participants Low vision (13 patients)and normal-sighted readers (37 persons) who use handhold magnifiers or the first time. Method Nine-point text was read by normal vision subjects without magnifier and then with two magnifiers with different magnification (+10 D,+20 D) in different eye-to-magnifier distance (10 cm,35 cm). Nine-point text was also read by low vision people with magnifiers. The big printed text under the same magnification without magnifier was also read by low vision group. Reading speed was recorded. Main Outcome Measures Reading speed (words per minute). Result For normal vision subjects,reading speed without magnifier (194.6?45.2 words/min) is faster than that with a 10D magnifier in an eye-to-magnifier distance of 10cm (159.7?44.7 words/min) (P=0.001),and distance of 35cm (162.5?46.7words/min) (P=0.002),respectively. Objective To evaluate the effect on the corneal endothelial cell after phacoemulsification using the microburst mode with high vacuum. Design Prospective,observational case series. Participants 98 age-related cataract patients (120 eyes). Metheds All patients having phacoemulsification with the Alcon Legacy 20000 Everest software were assigned to 1 of 3 groups (the burst mode group,the pulse mode group and the continuous mode group). Main Outcome Measures Actual power,effect phaco time,visual acuity,corneal edema,and corneal endothelial cell loss. Results The mean actual power of the burst group was statistically lower than those of the other two groups,the effect phaco time of the burst group was statistically shorter than those of the other two groups. The corneal edema and the endothelial cell loss of the burst group were less than those of the other two groups. 1.5mm)and received CTR scleral fixation.Visual acuity before and after surgery were examined.IOL tilt and decentration were measured with PentaCam system.Complications were also recorded.Main Outcome Measures Postoperative visual improvement,IOL position and complications.Results At the last postoperative examination,best corrected visual acuities of all eyes were increased(P=0.000).The mean IOL tilt was 4.33?2.96?before and 4.11?2.05?after CTR fixation,the mean decentration was 2.56?0.90mm before and 1.19?0.71mm after.No significant differences were found in IOL tilt before and after CTR fixation(P=0.844),but there were significant differences in IOL decentration(P=0.009).Posterior capsular opacification was found in 8 eyes(20.5%)during the follow-up period,and 2 eyes(5.1%)received Nd:YAG posterior capsulotomy.1 eye received IOL scleral fixation because of capsule rupture during CTR implantation.2 eyes suffered from temporary IOP elevation after surgery.No other severe complications occurred.Conclusions Phacoemulsification with CTR and IOL implantation is effective and safe for congenital subluxated lenses.CTR scleral fixation is a necessary supplement for severely subluxated lenses and progressive cases.

Objective To investigate the visual impairment and its rehabilitation after stroke.Methods After diagnosis of stroke by the neurologists, the patients received examinations of ocular pathology and visual function, especially in vision correction and rehabilitation. Results Five hundred and forty-seven stroke patients were recruited. 47.0% of the patients had eye alignment/movement impairment, 34.0% had visual field impairment, 19. 0% had visual perceptual difficulties and 19.9% had low vision blind. After using telescope, 35.7% blind patient's distance vision was ≥0.05; 58.2% low vision patient's distance vision was ≥0.3. After refraction (average vision:0.36±0.34 vs. 0.48 ±0.36,t =- 3.949,P=0.03) or using telescope(average vision of patients had low vision:0. 13±0. 07 vs. 0. 46±0.26,t=-5. 208,P=0.01 ;average vision of blind patients:0. 01 ±0.01 vs. 0. 06±0. 05,t = -3. 628, P= 0. 05), the improvement of vision for patients was significant.Conclusions The 72% of stroke patients has some kinds of visual impairment, which will affect not only the patients' independent living but also the clinical rehabilitation. Refraction and using telescope are effective for vision rehabilitation. Doctors should pay much attention to the visual therapy and rehabilitation for the stroke patients.

OBJECTIVE: To observe the application of the tragus cartilage in tympanoplasty. METHOD: Thirty-eight patients with tympanoplasty carried out tragus cartilage-perichondrium repairing tympanic membrane. The air-bone gap changes and the recovery of patients'tympanic membrane were evaluated at pre-operative and two months after the operation. RESULT: Thirty-one patients finishing follow-up visit showed tympanic membrane healed and looked like normal appearance in 1 to 3 months after the operation. The postoperation narrow of the air-bone gap >18 dB, tragus looked normal. CONCLUSION Tympanic membrane reconstruction using the tragus cartilage-perichondrium is feasible for tympanoplasty, and the survival rate and postoperative hearing would be acceptable.

AIM: To decide the effect that selected siRNA degrades mRNA of IL-1? specifically and suppression of its expression after connected with target site with homology complementary sequence. METHODS: Synthesized DNA expression box aimed directly at target site through PCR reaction in vivo was purified, and transfected into lymphocytes stimulated by LPS. siRNA was transcribed by cellular endogenous RNA polymerase Ⅲ and then evoke the degradation of target mRNA. After 48 hours of transfection, the cell culture supernatant was collected and the concentration of IL-1? was assayed using ELISA. RESULTS: Compared with blank-control and negative-control, selected sequence decreased the expression of IL-1?. Rate of the suppression was about 15%. CONCLUSION: RNAi technology produces specific interference effect in mouse spleen lymphocytes in original culture and inhibits the excretion of IL-1?.
