1.Clinical Observation of Mental Disorder for Young Adult Stroke in Acute Stage

Objective To investigate mental disorder and related factors for young adult stroke in acute stage.Methods 98 young patients with stroke were evaluated with Symptom Checklist-90 (SCL-90), Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA). The mental disorder and related factors of these patients in acute stage were analyzed.Results Of 98 patients, 62 cases (63.3%) had mental disorder in acute stage and incidence rate of male was significantly higher than that of female ( P<0.01). The related factors included self-character, activity of living, family relationship, the degree of nervous functional defects and recent negative life events, etc.Conclusion The factors related with mental disorder of young adult stroke in acute stage are mainly biological and social psychic.
2.Effect of microRNA-217 on pain behavior and neuro-inflammatory factors in rats with partial sciatic nerve ligation

Objective:To observe the effect of microRNA-217 (miR-217) on neuropathic pain and neuro-inflammation in rats with partial sciatic nerve ligation (pSNL).Methods:Twenty-four healthy adult male SD rats were randomly divided into three groups: pSNL group (partial sciatic nerve ligation), Sham group (Sham surgery), and normal group (no treatment) with 8 in each group. The mechanical withdrawal threshold (MWT) and paw withdrawal thermal latency (PWTL) were measured on the 1st, 2nd and 3rd day before surgery and on the 1st, 3rd, 7th and 14th day after surgery.The mRNA expression changes of miR-217 and TLR5 in the spinal cord of rats were detected by qRT-PCR. Additionally, 24 healthy adult male SD rats were randomly divided into three groups: pSNL- LV-miR-217 group (sciatic nerve part ligation + intrathecal injection of LV-miR-217), pSNL- LV-NC group (sciatic nerve part ligation + intrathecal injection of LV-NC), and Sham operation group with 8 in each group. MWT and PWTL were measured on the first, second and third day before surgery and on the 1st, 3rd, 7th, 14th day after surgery.Results:The results of pain behavior showed that the MWT of SNL group decreased significantly on the 7th day ((6.72±0.15) g, t=11.79, P<0.05) and the 14th day ((5.72±0.22) g, t=17.83, P<0.05), compared with sham group (7th d (13.58±0.30) g, 14th d (14.54±0.51)g). PWTL of pSNL group was (6.78±0.21) s and (5.99±0.22)s respectively on the 7th and 14th day after operation, which was lower than those of Sham group ((14.86±0.35) s, (14.50±0.51) s), the difference was statistically significant ( t=4.86, 7.13, both P<0.05). The qPCR experiment showed that the expression of miR-217 was down-regulated and the expression of TLR5 was up-regulated in the spinal cord of rats in the pSNL group, showing significant differences compared with the Sham group ( t=8.94, P<0.01; t= 7.54, P<0.01). The MWT of the pSNL- LV-miR-217 group were significantly increased on the 7th and 14th day after the intraoperative injection of miR-217, showing a significant difference compared with the pSNL-LV-NC group (7th d: t=14.79, P<0.01; 14th d: t=15.83, P<0.01). The PWTL of the pSNL-LV-miR-217 group were significantly increased(7th d: t=14.95, P<0.01; 14th d: t=16.27, P<0.01). qPCR assay showed that compared with the pSNL-LV-NC group, the expression of miR-217 in the pSNL-LV-miR-217 group was significantly increased ( t=7.49, P<0.01), and the expression of TLR5, COX-2( t=8.82, P<0.05), IL-6( t=6.42, P<0.05) and TNF-α( t=7.46, P<0.05) was significantly decreased. Western blot assay showed that the protein expressions of TLR5, COX-2, IL-6 and TNF-α in pSNL-LV-miR-217 group were significantly decreased on the 7th day after the operation. Conclusions:Intrathecal injection of miR-217 can significantly alleviate sciatic nerve ligation neuropathic pain and may inhibit neuroinflammation by regulating TLR5.
3.Correlation of polymorphisms at the matrix metalloproteinase-3-1612 position of the promoter region with the inflammatory response and oxidative stress in elderly patients with cerebral ischemic stroke

Objective:To investigate the association of polymorphisms at the matrix metalloproteinase(MMP)-3-1612 position of the promotor region with the inflammatory response and oxidative stress in elderly patients with cerebral ischemic stroke.Methods:In this retrospective study, 129 elderly patients with cerebral infarction diagnosed and treated in our hospital between March 2019 and March 2021 were enrolled as the study group, and 110 healthy subjects were selected as the control group.Polymorphisms of the MMP-3-1612 position in the promotor region, the inflammatory response and oxidative stress were examined using appropriate parameters and the associations between them were analyzed. Results:Compared with the control group, the proportions of patients with hypertension, diabetes, and smoking history in the study group were significantly higher( χ2=16.05, 17.19, 14.19, all P<0.05), and the levels of fasting blood glucose, low-density lipoprotein, and homocysteine were also significantly higher( t=6.22, 3.64, 2.69, all P<0.05).Meanwhile, compared with patients carrying the MMP-3-5A/6A or the MMP-3-6A/6A genotype, the levels of serum inflammatory markers such as high mobility group box-1 protein(HMGB1), fractalkine(FKN), tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β)and interleukin-17(IL-17)in patients carrying the MMP-3 gene 5A/5A genotype were significantly higher(all P<0.05).In addition, the expression of serum oxidative stress-related molecules Kelch-like ECH-associated protein 1(Keap1), nuclear factor erythroid-2 related factor2(Nrf2), antioxidant response element(ARE), quinone oxidoreductase 1(NQO1), and heme oxygenase-1(HO-1)was also significantly increased(all P<0.05), but there was no difference in these markers between patients carrying the MMP-3-5A/6A genotype and patients carrying the MMP-3-6A/6A genotype( P>0.05).Patients carrying the 5A/5A genotype and the 6A/6A genotype exhibited only one 97 bp band and one 120 bp band, respectively, while the patients carrying the 5A/6A genotype exhibited two 97 bp bands and two 120 bp bands.There was no statistical difference in the number of patients carrying the 5A/6A genotype in the cerebral infarction group compared with the control group( P>0.05), and the number of patients carrying the 5A/5A genotype in the cerebral infarction group was higher than that in the control group(69% or 53.49% vs.35% or 31.82%, χ2=11.34, P<0.05).Polymorphisms of the MMP-3 gene had a positive correlation with the risk of stroke( r=0.25, P<0.05). MMP-3-1612 gene polymorphism( OR=7.21, 95% CI: 1.13-1.83, P=0.01), elevated blood glucose( OR=1.27, 95% CI: 1.18-2.06, P<0.001), high homocysteine( OR=1.05, 95% CI: 1.08-1.58, P<0.01), hypertension( OR=5.414, 95% CI: 1.140-4.46, P<0.01), elevated low-density lipoprotein( OR=4.03, 95% CI: 1.03-2.35, P=0.02), coronary heart disease( OR=1.17, 95% CI: 1.47-3.19, P<0.01)and diabetes( OR=8.52, 95% CI: 1.32-4.71, P<0.01)were risk factors for cerebral infarction. Conclusions:In elderly patients with cerebral infarction, polymorphisms of the MMP-3-1612 position in the promotor region is closely related to the risk of cerebral ischemic stroke, the inflammatory response and oxidative stress.MMP-3 gene polymorphisms are risk factors for stroke.
