1.Roles of serum procalcitonin and C-reactive protein in the diagnosis of nonneutrocytic ascitic spontaneous bacterial peritonitis in liver cirrhosis

Objective To investigate the diagnostic value of serum procalcitonin (PCT) and C-reactive protein (CRP) in nonneutrocytic ascitic spontaneous bacterial peritonitis (NASBP).Methods From January 2012 to January 2015,patients with liver cirrhosis and ascites were restropectively enrolled.Patients were divided into NASBP group and non-infective ascites group.The receiver operating characteristics curve (ROC) was used for assessing diagnostic accuracy of serum PCT and CRP.Patients with conventional SBP were set as controls.Data between two groups were compared using t test,Rank sum test,x2 test or Fisher exact test.The areas under ROC curve or Youden indeces were compared using Z test.Results A total of 30 patients were collected in NASBP group,51 patients in non-infective ascites group and 33 patients in conventional SBP group.There were no statistically significant differences in PCT and CRP levels between NASBP group and conventional SBP group [0.70(0.25~2.45) μg/L,(21.85-±-16.46) mg/L;0.90(0.33~3.56) μg/L,(31.78-±26.74) mg/L] both P>0.05,and were both significantly higher than those of non-infective ascites group (0.20 (0.07 ~ 0.40) μg/L,Z=-3.38,t=4.64;(7.19±7.04) mg/L,Z=-3.89,t=-5.17,both P<0.05).The optimal cut-off value of PCT and CRP in the diagnosis of NASBP was 0.43 ng/mL and 12.76 mg/L,respectively.According to the cut-off value above,PCT,CRP and PCT plus CRP in series and in parallel in the diagnosis of NASBP,the areas under curves were 0.725,0.848,0.737 and 0.806,respectively,and there was no significant difference in pairwise comparison between groups.The sensitivities were 70.0%,70.0%,53.3% and 86.7%,respectively,there were statistically significant differences between inparallel and any other method (Fisher exact test,all P< 0.05).And there was no statistically significant difference (all P>0.05).The specificities were 76.5%,88.2%,94.1% and 74.5%,respectively.There was no statistically significant difference in PCT and CRP between combination in parallel and in series,while the differences in the other pairwise comparisons of combinations were statistically significant (Fisher exact test,P<0.05).The Youden's indexes were 0.465,0.582,0.474 and 0.612,respectively,there was no statistically significant difference in pairwise comparison between groups (all P>0.05).Conclusions All of serum PCT,CRP and the combination of them have good diagnostic value in NASBP with their own advantages which should be selected based clinical needs.

Objective To evaluate the effect on reliability with different sample amount when human corneal endothelial morpho- logic parameters estimated using center by center method analysis.Design Clinical study.Participants Thirty central corneal endothe- lium photo-micrographs for the same eye. Methods Noncontact specular photomicrographs were obtained.Each photo-micrograph was analyzed with center by center method and was analyzed for 9 times by pointing different amount cells,from 10 to 90.Compare the di- versity of corneal endothelial morphometric parameters between different groups. 3.Comparison of Disposcope endscope and GlideScope vedio laryngoscope in the application of double-lumen endobronchial intubation in difficult airway

Objective To evaluate and compare the clinical applications of Disposcope (DS)en-doscope and GlideScope (GS)video laryngoscope in double-lumen endobronchial tube (DLT)intuba-tion of the patients with difficult airway.Methods Forty patients scheduled for elective thoracic sur-gery after failure to place the DLT with modified general laryngoscope 2 attempts were randomly dev-ided into 2 groups,20 cases in each group.Patients underwent DLT with DS (group DS)or GS (group GS)intubation after failure to intubation.The causes of the failure of intubation,the intuba-tion of the video laryngoscope,the time of intubation and the location of left double lumen tube were recorded.And the postoperative injury of oral mucosa,teeth and respiratory tract were observed. Results The causes of the difficulty for DLT intubation included:long,high arched palate,large epi-glottis,reduced jaw opening,protruding or loose incisors,over bite,reduced neck extension.The in-tubating achievement ratio was significantly higher in group DS than in group GS (P < 0.05 ).But there was no obvious difference between the two groups in the location success rate and intubation time.The postoperative incidence of tooth loss and oral mucosal injury was significantly higher in group GS than in group DS (P <0.05).Conclusion Both DS and GS were great helpful to intubation and location of DLT in the patients with difficult airway.But DS was more superior than that of GS in the difficult airway of reduced jaw opening,protruding or loose incisors,over bite and reduced neck extension.

6.Effects of separation methods and culture conditions on biological characteristics of rabbit bone marrow mesenchymal stem cells

BACKGROUND:Bone marrow mesenchymal stem cells(BMSCs)are widely utilized as seed cells or carriers in bone tissue engineering and gene therapy.Thus,how to obtain BMSCs with high purity arose more attentions of researchers.OBJECTIVE:To observe the effects of different separation methods and culture conditions on biological characteristics of rabbit BMSCs.METHODS:BMSCs were obtained by whole bone marrow culture,density grand centrifugal and red blood cell lysis.At 48 hours after culture,the cell numbers were counted,the time of passage was recorded,in addition,the cell morphology was observed by phase contrast microscope,and the CD44 antigen expression was identified using flow cytometry.The 3rd and the 7th generation aging cells were cultured with DMEM-LG,MEM-HG,and DMEM/F12 culture medium.MTT and count cell plat were used to evaluate the growth of BMSCs.Phase contrast microscope was used to observe the morphological changes of aging cells.RESULTS AND CONCLUSION:BMSCs could be separated by each method.The adherent cells showed shuttle or multiple angle shapes,with rich cytoplasm,and positive for CD44 antigen.The more cell number and shortest primary culture time was presented in red blood cell lysis group(P < 0.05).DMEM/F12 could promote the proliferation of quiescent cells.And the cells prevented the better viability.The method of read blood cell lysis improving the efficiency of BMSCs adherent is an effective method of extraction of BMSCs.DMEM/F12 could promote the proliferation Maybe,DMEM/F12 is more suitable for BMSCs.
7.The Therapeutic Effect of Allergic Rhinitis by Different Acupoints Application Therapy for Wintertime Diseases in Summer Treatment

[Objective] To observe the effect of different acupoint application therapies :a summer treatment for the winter disease on allergic rhinitis ,and compare the relationship between the different acupoints and therapeutic effect. [Methods ] The 81 cases of allergic rhinitis who met inclusion criteria were divided into 12 acupoint group and 6 acupoint group. The time of application was in first ten-day period of the hot season. The duration of application was 2h. The days later, namely in the second ten-day period of the hot season, and twenty days later, in the third ten-day period of the hot season, the application was repeated respectively. Before treatment and half year after the treatment, the score of quality of life, symptoms of allergic rhinitis were observed to compare the relationship between the different acupoints and therapeutic effect. [Results] The effective rate was higher in 12 acupoints group than 6 acupoints group. After treatment, the score of quality of life, symptoms of allergic rhinitis were significantly improved( P<0.05 or P<0.01). And the comparison of the therapeutic effect of two groups showed significant difference in statistics( P<0.01). [Conclusion] The acupoint application therapy is effective for allergic rhinitis, and the comprehensive combination of acupoint is influential on the effect.
8.Identification of the regulatory effect of evodiamine on functional regulation gene expressing in murine dendritic cell by microarrays analysis

Objective:cDNA microarrary technique was employed to detect the effects of evodiamine on functional regulation of gene expressing in murine marrow-derived dendritic cells.Methods:Immature dendritic cells (iDC) derived from bone marrow of BALB/c mice were induced by GM-CSF for ten days in vitro and received corresponding treatment for 24 h.Then the cell were randomized into the control groups (Ⅰ),evoamine group (Ⅱ),LPS group (Ⅲ) and evodiamine+LPS group (Ⅳ).After 24 h’s treatment, total RNA of the cells was collected,and cDNA microarrary (GEArray S Series Mouse Dendritic & Antigen Presenting Cell Gene Array:MM-604) was used to screen the functionalg correlation genes of dendritic cells.Results:There were seven genes up-regulated and two genes down-regulated two-folds above in Ⅱ/Ⅰ;thirty four genes up-regulated and twelve genes dwon-regulated two-folds above in Ⅲ/Ⅰ;forty six genes up-regulated and seven genes down-regulated two-folds above in Ⅳ/Ⅱ;twenty four genes up-regulated and two genes down-regulated two-folds above in Ⅳ/Ⅲ.The function of these genes involve the secret of the cytokines and the expression of their receptors,antigen uptake,antigen presentation,cell surface receptors and signal transduction.Conclusion:The effects of evodiamine on dendritic cell involve in multi-regulation and expression of genes,these genes contribute to the functions,differentiation and maturation of the dendritic cells.The work provides a potentiality to seek the target of medicine.
9.Exposure Assessment of Vehicle Exhaust Pollution in Wuhan,Hubei

Objective To investigate the exposure level to vehicle exhaust pollution for population by different commuting modes in roadside environment in Wuhan. Methods Eight air monitoring sites were selected in the six main roads in urban area of Wuhan, Hubei province. The concentrations of NO2, CO, total hydro carbon (THC), PM10, benzene, toluene and xylenes (BTX) emitted from the vehicles were monitored and analyzed in winter and summer of 2007, respectively and combined with breathing rate and time spent of the population in traffic environments, exposure doses of different subpopulation were calculated as indicators in order to evaluate their exposure level to traffic exhaust. Results The PM10 was the primary pollutant on the roadside. Compared with the Ambient Air Quality Standard, the rate of exceeding standard limit for PM10 was 8% in summer and 100% in winter. 10.Effects of Kuijiening Plaster Point Application on Pro- and Anti-inflammatory Cytokines in Serum and Colonic Tissue of Ulcerative Colitis Rats with Spleen-kidney Yang Deficiency

Objective To observe the effects of Kuijiening Plaster point application on the levels of pro-inflammatory cytokines IFN-γ, IL-12 and anti-inflammatory cytokines IL-4, IL-10 in serum and colonic tissue of ulcerative colitis (UC) rats with spleen-kidney yang deficiency; To investigate the possible mechanism of Kuijiening for the treatment of UC with spleen-kidney yang deficiency. Methods Sennae Folium solution gavage, ice water stimulation and trinitrobenzene-sulfonic acid ethanol combinative methods were applied to establish the models of UC rats with spleen-kidney yang deficiency. Rat models were randomly divided into control group, model group, application group and SASP group, 10 in each group. The control group and model group were given medicine-free cataplasm matrix point application and perfused with NS. The application group was given Kuijiening Plaster point application and perfused with NS. SASP group was given medicine-free cataplasm matrix point application and perfused with SASP. The courses were 28 days. General conditions of rats in each group were observed during treatment. The general morphology and pathological form of colonic tissue were observed with naked eye and light microscopy through HE staining respectively in each group post-treatment. Pathological features were observed and scored by HE dyeing. Levels of IFN-γ, IL-12, IL-4 and IL-10 in serum and colonic tissue of rats in each group were detected with ELISA. Results The general conditions, gross morphology and optical microscope pathological morphology of colonic tissue improved and scores decreased in application group and SASP group. Compared with the model group, levels of IL-4 and IL-10 significantly increased, while the levels of IFN-γ and IL-12 decreased in serum and colonic tissue in application group and SASP group, with statistical significance (P<0.01). Moreover, levels of IL-4 and IL-10 in application group decreased not as much as those in SASP group, while IFN-γ and IL-12 concentration increased higher than SASP group, without statistical significance (P>0.05). Conclusion Kuijiening Plaster point application can play a role in treating UC with spleen-kidney yang deficiency through partially restoring the balance between pro- and anti-inflammatory cytokines in serum and colon tissue of rats.
