1.Study on the peri-puerperium outcome and associated factors of patients with primary Sj?grenˊs syndrome

Objective To analyze the peripartum complications,frequency of neonatal abnormalities and the associated factors in patients with primary Sj?grenˊs syndrome (SS). Methods The chart of 39 patients with primary SS who were admitted to the hospital for delivery were retrospectively reviewed. The clinical data, obstetrical outcome and the frequency of neonatal abnormalities as well as the possible associated factors were analyzed. Results There were 76 pregnancies and 41 deliveries among these 39 patients. Two patients (5%) had pregnancy-related hypertension, 1 (3%) had gestational diabetes and 1 (3%) had eclampsia. Twenty-eight(72%) patients had at least one episode of complication. In which, 27(68%) were induced abortion, 2 (5%) were premature birth, 6 (21%) were amniotic fluid volume abnormalities, 2 (5%) were post-partum bleeding, 5 (13%) were premature rupture of membrane, 6 (21%) were intra-uterine distress and 3 (8%) had intra-uterine growth retardation. Fetal abnormality was detected in 7(18%) patients, in which 3 fetus (8%) died before delivery, 2 fetus (5%) were small than gestational age and 2 (5%) had fetal deformity. Compared to the reports in the literature, the rate of abortion and fetal death was higher, but the rate of growth retardation , induced abortion was lower. Conclusion The frequency of peri-partum complication and fetal abnormality is increased in patients with primary SS. Pregnancies in patients with primary SS should beclosely monitored by rheumatologists.
2.Quantitative Study on the Development of Basic Medical Insurance Schemes in China: A Mixed Model Based on Universal Health Coverage

Objective:Based on the perspective of Universal Health Coverage(UHC),a mathematical model was developed to conduct quantitative study on the development status of Basic Medical Insurance Schemes(BMIS) in China.Methods:A mixed model was developed to conduct quantitative study on the development of BMIS in the period of 2003-2015 from five dimensions:coverage of population,benefit package,reimbursement rate,risk pooling level and unity of the schemes.Sensitivity analysis was also performed.Results:The UHC scores for BMIS in China from 2003 to 2015 fluctuated obviously.Given the range of 0-100 percent,the UHC score in 2003 was 52.2%,28.5% in 2006,23.9% in 2010 and 26.5% in 2015.The integration and equalization of BMIS and scaling up the risk pooling levels were shown to contribute significantly to UHC.Conclusion:The construction of mixed models was developed to provide a new calculation assessment tool for measuring the UHC,which consisted of completed evaluation tool package with addition model and multiplication model.Considering the future development of UHC,there is a still long way to go for BMIS in China.Emphases should be given to integration and equalization of BMIS as well as scaling up the risk polling to provincial and national level.
4.Therapeutic Observation of Electroacupuncture plus Electro-spoon Needle-cupping for Lumbar-dorsal Myofascial Pain Syndrome

Objective To observe the clinical efficacy of electroacupuncture plus electro-spoon needle-cupping in treating lumbar-dorsal myofascial pain syndrome.Method Sixty patients with lumbar-dorsal myofascial pain syndrome were randomized into group A, group B, and group C, 20 cases in each group. With same acupoints selected, group A was intervened by electroacupuncture plus electro-spoon needle-cupping, group B was by electroacupuncture, while group C was by conventional acupuncture. Visual Analogue Scale (VAS) was observed before and after treatment, and the clinical efficacies of the three groups were compared.Result After treatment, VAS scores were significantly different from those before treatment in the three groups. (P<0.05). After treatment, the VAS scores in group A and B were significantly different from that in group C (P<0.01,P<0.05); the VAS score in group A was significantly different from that in group B (P<0.05). The total effective rate was 95.0% in group A, versus 90.0% in group B and 80.0% in group C, and the total effective rate of group A was significantly different from that of both group B and C (P<0.05).Conclusion Electroacupuncture plus electro-spoon needle-cupping is an effective method in treating lumbar-dorsal myofascial pain syndrome.
5.Effect of Mushao Composition on ?-EP and CCK-8 in Caudex Dorsalis of Rats with Pain after Fracture Operation

Objective To study the effect of Mushao composition on ?-EP and CCK-8 in the caudex dorsalis of rats with pain after fracture operation,and explore its mechanism.Methods The male Wistar rats were randomly divided into control group,model group,Mashao composition group and tramadol group.The Brennan model was adopted.The level of the ?-EP and CCK-8 were determined by the method of radioimmunity.Results Compared with model group,Mushao composition could increase level of ?-EP in the caudex dorsalis,while decrease level of CCK-8.Conclusion Mushao composition could increase the content of ?-EP and reduce the content of CCK-8,so to analgesia in rats model of pain after fracture operation.
6.Research of siRNA Inhibition Effect in HIV-1 Gene Expression

RNA interfering (RNAi) is a process of sequence-specific, posttranscriptional gene silencing in animals and plants. In mammalian cells, duplexes of 19~25 nts (nucleotides) RNAs efficiently inhibit gene expression. The pBS/H1SP vector expressing siRNA is used which inhibit specific HIV-1 gene expression. To assess the intracellular effect of this H1 promoter-driven siRNA, a reporter plasmid pEGFP-C1-HIV protein which expresses fusion protein of enhanced green fluorescent protein (EGFP) and HIV protein was used. The expression of the reporters can be easily visualized by fluorescence microscopy in living cells. siRNA-generating vectors targeted to several HIV-1 genomes were constructed and then co-transfected with respond reporter expression vectors into HEK293 cells. Cells transfected with pHIV-siRNA exhibited a significant inhibition of pEGFP-HIV expression compared with cells transfected with control vectors. By this way, it is successfully to select effective siRNA for silencing target HIV-1 genes. Then two or three siRNA transcripts targeted to different HIV genes were expressed by one plasmid, and a relative strong inhibition effect was observed.
7.Content Determination of 4 Index Components in Uygur Medicine Capparis spinosa by HPLC

OBJECTIVE:To establish the method for determining the contents of protocatechuic acid,rutin,gallic acid and kaempferol in Uygur medicine Capparis spinosa. METHODS:HPLC was performed on the column of Agilent C18 with the mobile phase of acetonitrile-0.1% phosphoric acid solution(7∶93,V/V)at the flow rate of 1.0 ml/min,the detection wavelength was 327 nm,column temperature was 30 ℃ and volume was 10 μl. RESULTS:The linear range was 0.260 0-50.0 μg for protocatechuic ac-id(r=0.995 6),3.109 0-102.0 μg for rutin(r=0.999 9),1.018 0-40.0 μg for gallic acid (r=0.998 9) and 0.063 0-36.0 μg for kaempferol(r=0.998 8);RSDs of precision,stability and reproducibility tests were all no more than 1.20%;average recoveries were respectively 101.51%(RSD=1.85%,n=6),99.70%(RSD=1.23%,n=6),98.28%(RSD=1.86%,n=6) and 100.97%(RSD=1.74%,n=6). CONCLUSIONS:The method is specific,and can fast and accurately determine the contents of protocate-chuic acid,rutin,gallic acid and kaempferol in Uygur medicine C. spinosa.
8.Celluar Immunity in Patients with Juvenile Rheumatoid Arthritis (JRA)

Objective To evaluate the change of cellular immunity and its clinical significance in JRA.Methods 7 lymphocyte swbpopulation was analyzed by immunofluorescein and interleukin 2 (IL-2) produced by peripheral blood mononuclear cells in vitro by MTT colorimetric assay. 29 times of various stage with JRA were examined, including 14 clinically active patients, 8 posttreatment or 7 clinically inactive ones. There are 19 healthy children of similar age in control group.Results In active patients, the number of OKT8, OKT4, the ratio of OKT4/OKT8 and the level of IL-2 decreased significantly compared with normal controls. These changes recovered matkedly in remission patients though they did alter affective treatment for (2~4) weeks.Conclusion Patients with active JRA are characterised by aberration of cellular immunity and the aberration reverses obviously slow in comparision with the clinical manifestetions and the routine laboratory investigation.
9.Effect of angiotensin Ⅱ receptors on collagen synthesis of fibroblasts derived from human hypertrophic scars

BACKGROUND: It has been reported that Angiotensin Ⅱ (Ang Ⅱ) is related to occurrence and development of dermatofibrosis; however, less is explored about the expression and effect of AT1 and AT2 receptors in the fibroblasts of human hypertrophic scar.OBJECTIVE: To observe the expression of Ang Ⅱ type 1 (AT1) and type 2 (AT2) receptors in human hypertrophic scars, and explore their effects on collagen synthesis of fibroblasts.DESIGN, TIME AND SETTING: Randomized control experiment was performed at the Experimental Center, Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA between August 2006 and November 2007. PARTICIPANTS: Samples of hypertrophic scare were taken from 18 patients (10 males and 8 females, 19-47 years Old). Seven specimens of normal skin served as control. All of the specimens collected were divided into two parts, one part for immunohistochemical staining after fixated by 4% paraformaldehyde, the other part for culturing fibroblasts.METHODS: The expression of both AT1 and AT2 receptors in fibroblasts of hypertrophic scare was detected with immunohistochemical staining and radioligand receptor binding assay. Collagen synthesis was examined in cultured fibroblasts of hypertrophic scars by measuring [3H]-proline incorporation into collagenous proteins.MAIN OUTCOME MEASURES: The expression of both AT1 and AT2 receptors in human hypertrophic scars; the [3H]-proline incorporation value in cultured fibroblasts.RESULTS: Positive staining signals of both AT1 and AT2 receptors were found in fibroblasts of hypertrophic scars. Similar results were also observed in cultured fibroblasts of hypertrophic scars, expression level of AT1 and AT2 receptors were (10.69±2.15) fmol/106 cells and (4.9±1.05) fmol/106cells, respectively. In cultured fibreblasts, Ang Ⅱ stimulation significantly increased collagen synthesis, which was inhibited by valsartan, an AT1 receptor blocker, but augmented by PD123319, an AT2 receptor antagonist.CONCLUSION: Both AT1 and AT2 receptors were expressee in the fibreblasts of hypertrophic scars, and Ang Ⅱ regulates collagen synthesis in hypertrophic scar fibroblasts through a negative cross-talk between AT1 and AT2 receptors, which might contribute, at least partly to formation and maturation of human hypertrophic scars.
