1.Doctor Xuan Guiqi's Experience in the Diagnosis and Treatment of Childhood Asthma and the Thinking of Improving Efficacy

[Objective]To summarize Doctor Xuan Guiqi's experience in the treatment of childhood asthma and the idea of improving the curative efficacy. [Methods]By learning from Doctor Xuan and clearing up medical cases, the author finishes the summary on doctor Xuan's clinical experience on child-hood asthma based on the syndrome differentiation and clinical medication.[Results]The childhood asthma is divided into acute and chronic, the former is divided into cold asthma, heat asthma and wind-phlegm types. At the same time, we strengthen pertinence treatment of clearing heat and removing toxi-city, Qi spasmolytics, dispel ing wind and desensitization, activating blood and eliminating stasis, promoting Qi and regulating middle-jiao and relieving dyspepsia.[Conclusions] Doctor Xuan Guiqi's experience in the diagnosis and treatment of childhood asthma can significantly improve the clinical efficacy , and it is meaningful to clinic.
2.Functional food - nutraceutical product: Some opinions by view-point of management

The concept of nutraceutical food was introduced firstly in United State in 1989 by Stephen Defelice, the founder of the Foundation for Innovation in Medicine. It is defined as a product that considered as a food or a part of food, provided medical or health benefits, including disease prevention and treatment”. The health sector should review and re-evaluate the importance and potential of the nutraceutical food market and plan the development strategy for this novel avenue. The unique definition of nutraceutical product is needed. The Ministry of Health should direct the management of these products as medications rather than foods and the regulations of registration, labeling and take testing samples should be available

3.Allergic reaction to drugs in elderly person at Huu Nghi Hospital

Allergic reaction to drugs (ARTDs) occurs frequently in adults who are usually suffered from many diseases and who require multiple courses of medications. Investigative result showed that: The incidence of ARTDs in elderly is 6.74%, this incidence in patients of Asthma group is 14.7%. The main cutaneous manifestations of ARTDs in elderly patients included: urticaria and erythematous rashes (42.5%), erythrodermie (14.9%), erythematic multiform (21.3%), fixed drug eruptions (12.7%), vesiculobullous eruptions (6.4%).

4.Oily constituents of Meconopsis punicea

OBJECTIVE: To analyse oily constituents of Meconopsis punicea Maxim. METHOD: The oily constituents were analysed and identified by GC-MS. RESULTS: The sixty-three constituents were isolated and the forty-seven of that were identified, making up 74.6% of the total constituents. Ester compounds accounted for 57.5% of the identified compounds. The main constituents were ester octadecnate. CONCLUSION: Ester octadecenate has high biological activity, which agrees with curative effect of the plant.
5.Antiaddictive indole alkaloids in Ervatamia yunnanensis and their bioactivity

Objective:To investigate the constituents of indole alkaloids in Ervatamia yunnanensis and their antiaddictive bioactivities. Methods: The preliminary alkaloids(TEYA) were extracted by 95% EtOH from the stems of Ervatamia yunnanensis.TEYA was then purified by silica gel and Sephadex LH20 column chromatography repeatedly and its structure was identified with spectral analysis.The antiaddictive effects of these indole alkaloids were evaluated by Conditioned Place Preference(CPP) experiments.Results: Nine indole alkaloids were obtained and identified as: coronaridine(1),voacangine(2),3-R-ethoxycoronaridine(3),3-S-ethoxycoronaridine(4), 19-epi-heyneanine(5),heyneanine(6),19-epi-voacristine(7),coronaridine hydroxyindolenine(8) and 12-methoxyl-voaphylline(9).Among them voacangine (2) and 19-epi-voacristine(7) were effective in prevention and treatment of addiction induced by morphine in rats(n=8).Conclusion: Compounds 3,4,8,and 9 have been isolated from Ervatamia yunnanensis for the first time and coronaridine-type indole alkaloids are the main antiaddictive constituents of E.yunnanensis.
6.Applications of proteomics in SIRS and sepsis

Sepsis,the systemic inflammatory response syndrome(SIRS)induced by infection,is the first cause of death of patients in intensive care unit. Early diagnosis of sepsis with the actions of corresponding treatment can improve the prognosis of sepsis.It is an appropriate way to achieve early diagnosis of sepsis that using proteomics methods to detect unknown proteins during early stage of sepsis,in which we search for new protein biomarkers.
7.Analysis of the efficacy of compound desogestrel combined with metformin in treatment of adolescents with polycystic ovary syndrome

Objective To investigate the treatment of adolescent marvelon Metformin's clinical efficacy in patients with polycystic ovary syndrome.Methods 120 cases of adolescents with polycystic ovary syndrome were randomly divided into two groups,A group of 60 patients with single-agent oral therapy Marvelon, B group of 60 patients with Marvelon oral metformin therapy ,medication time was 3 courses; The levels of fasting insulin, fasting glucose, serum sex hormone levels were compared before and after treatment.Results The hormone levels in the two groups had significant difference before and after treatment (P <0.01) ;Two groups did not change in fasting blood glucose before and after treatment,no significant difference between the two groups(P >0.05) ;The insulin levels in two groups before and after treatment,was significantly different(P <0.05).Conclusion Metformin Marvelon adolescent polycystic ovary syndrome treatment can effectively reduce the T,LH,LH/FSH,insulin resistance,clinical results were satisfactory and better than single drug therapy Marvelon.
8.Research advances of antibody-dependent cell-mediated cytotoxicity in the resistance of rituximab

Rituximab,a chimeric monoclonal antibody targeted against the pan-B-cell marker CD20,comparing with chemotherapy has significantly improved the treatment outcome of all subgroups of B-cell lymphoma patients.However,not all patients respond to rituximab plus chemotherapy or relapse in a short time after response,which suggests the existence of resistance mechanisms.Antibody-dependent cell-mediated cytotoxicity (ADCC) is one of the main mechanisms of rituximab,so any abnormalities in ADCC pathway may affect the efficacy of rituximab.With the widely application of rituximab,its resistance mechanisms are attracting more attention.This review will summarize the factors within ADCC pathway that may affect the antitumor effect of rituximab.
9.Variations of brain edema and neurological function of rat models of cerebral infarction after hyperbaric oxygen therapy

BACKGROUND:Several studies have suggested that hyperbaric oxygen could better protect cranial nerve and brain cels. Hyperbaric oxygen therapy can make oxygen partial pressure rapidly diffusing toward relatively hypoxic brain tissue, so as to increase blood oxygen content in the brain tissue, reduce brain edema and promote the recovery of brain function. OBJECTIVE: To observe the effects of hyperbaric oxygen therapy on brain tissue sweling in rats after middle cerebral artery occlusion, and discuss the possible mechanism of action underlying the neuroprotective effects of hyperbaric oxygen therapy in rats with cerebral infraction. METHODS:Sixty adult female rat models of cerebral infarction were successfuly established by middle cerebral artery occlusion using suture method and then randomly divided into the sham, cerebral infarction and hyperbaric oxygen therapy groups (n=20 rats/group). At 3 days after middle cerebral artery occlusion, apoptosis of nerve cels in the infract area of rats in each group was detected by TUNEL method. At 72 hours after middle cerebral artery occlusion, the gene transcription and protein expression of aquaporin 4/9 and matrix metaloproteinases 9/2 in the peri-infarct area were detected by RT-PCR and western blot analysis. The pathomorphological change in the infract area was observed by hematoxylin-eosin staining. The expression level of glial fibrilary acidic protein was detected by immunohistochemistry. At 24 hours and 3 days after hyperbaric oxygen therapy and at 1 and 2 weeks after middle cerebral artery occlusion, neurological behaviors were evaluated using Longa behavioral scores. RESULTS AND CONCLUSION:After 1, 2 days of hyperbaric oxygen therapy, Longa behavioral scores in the hyperbaric oxygen therapy group were significantly lower than those in the cerebral infarction group (P < 0.05). At 3 days after middle cerebral artery occlusion, cel apoptosis index in the hyperbaric oxygen therapy group was significantly lower than that in the cerebral infarction group (P < 0.05). At 72 hours after middle cerebral artery occlusion, the aquaporin 4/9, matrix metaloproteinases 9/2 gene and protein expression in the hyperbaric oxygen group were significantly lower than those in the cerebral infarction group (P < 0.05). These results suggest that hyperbaric oxygen therapy can play its protective role by decreasing apoptosis of nerve cels in the infarct area and lessening the edema of brain tissue in rats with cerebral infarction.
10.The clinical significance of monitoring parameters for oxygen kinetics and metabolism in the diagnosis and treatment of shock

The importance of hemodynamic monitoring is reflected in the correct interpretation of the monitoring results and taking simultaneously the appropriate treatment strategy according to the correct interpretation of the results.The essence of the shock is body tissue hypoperfusion,whtich result in tissue hypoxia and organ dysfunction.The essence of the hemodynamic imbalance is the imbalance of oxygen supply and oxygen consumption.Therefore,the end point of the fluid resuscitation for shock is now increasing emphasis on monitoring mixed/central venous oxygen saturation and the concentration of blood lactate,the rate of lactate clearance.
