1.Homocysteine in cerebrovascular disease

Over the last decade there is strong evidence indicating that hyperhomocysteinemia appears to be an important risk factor for vascular diseases, including cerebrovascular disease and myocardial infarction. Several multicenter-randomized trials have demonstrated that it is independent to other well known risk factors such as hypertension, diabetes mellitus, hyperlipidemia and smoking. Given the intense interest of our community concerning the prevention of cerebrovascular diseases we begin to measure homocysteinemia with a simple and accurate technique. The first results of our study raise the exciting possibility that homocysteinemia levels can be normalized by an inexpensive, well tolerated therapy with folate, pyridoxine hydrochloride and cyanocobalamin.

2.Hypertensive intracerebral hemorrhage: the role of surgery

From March 1997 – September 2002, study on 31 patients intracerebral hemorrhage (28 male, 3 female) and 16 patients of control group (14 male, 2 female). The mortality of surgical treatment group was 32% and control group was 56%. Most of the benefit was in patients with lobar hemorrhage and following criteria: volume of hematoma 60cc, age 70, GCS 8. These is some suggestion that in the next future the treatment of intracerebral hemorrhage will involve non-invasive, stereotactic aspiration of hemorrhage through a single burr hole within hours

3.Epidemiological features of Helicobacter pylori infection inchildren of five different ethnics in mountainous village (Banqua, Batxat, Lao Cai)

Background: There is nearly 3,5 billion people infected Helicobacter pylori in over the world. Objectives: To investigate Helicobacter pylori (HP) seroprevalence (by ELISA technique) and related risk factors (by questionnaire set) in children of different ethnics. Subjects and method: This cross - sectional study was carried out in Ban Qua, a mountainous village of a northern border province with a population of 408 children under 18 from 245 households (consisting of 781 members) with 5 ethnics. HP seroprevalence was determined by ELISA technique and risk factors by structure questionnaire. Results: HP seroprevalence in children under 18 was 26.7% without sex predominance but with significant ethnical difference: 16.1 % in H\u2019mong, 16.7% in Tay, 20.3% in Dao, 38.5% in Giay and 41.0% in Kinh (p < 0.001); and increased with age, particularly in 3 - 6 aged group, by the rate of 1-1.5%/year. The study results showed that age, HP infection in sibling, breastfeeding beyond 12 months and family history of gastro duodenal disorders were independently associated to HP infection in children. In addition, HP infection in parents and illiteracy in mothers were also significantly related to higher seropositivity in children (p<0.03 and <0.045, respectively). Conclusion: This study showed that although the rate of HP infection in minority ethnics was lower than in the majority (Kinh), but risk factors for the infection were somewhat similar to those identified in other studies in Kinh people.

4.Study on knowledge, attitude, risky behavior and HIV prevalence among The Khmer ethnic group in Kien Giang, An Giang and Hau Giang - 2007

Background: The rate of Khmer ethnic minority living in An Giang, Kien Giang and Hau Giang were 3%, 12.9% and 3%, respectively.These provinces had some specific factors such as low education, income and knowledge about HIV/AIDS, high risk behavior and it is difficult to practice intervention methods therefore the awareness of HIV infection risk on this group is required. Objectives: To determine the HIV prevalence, describe knowledge, attitude and risky behaviors that led to syphilis and HIV infection of the Khmer ethnic group, which are used to propose suitable intervention and prevention models. Subject and methods: A cross sectional survey was conducted from September 2006 to June 2007 in the Khmer ethnic group, including both genders at the age ranging from 15 to 49, living in some districts of An Giang, Hau Giang and Kien Giang. Results: A total of 877 households and 2.400 people were interviewed. 98.62% of the people agreed to take a blood test for HIV. The average age of first intercourse was 20. The condom usage proportion was very low. The number of men using condoms in the latest intercourse with their wives or lovers accounted for only 7.2%. Moreover, only 3% of the women used condoms in the last 12 months. The drug usage was 0.6% in Hau Giang and 0.1 % in Kien Giang. The knowledge about HIV/AIDS of both men and women were 9.2% and 4.5%. The prevalence of people, who made HIV test in the three provinces accounted for only 3 to 4.6%. HIV infection rate was 0.2% in An Giang, 0.5% in Kien Giang and 0.5% in Hau Giang. Syphilis infection rates were 1.6%, 1.4% and 1.5%, respectively. Conclusions: Appropriate communicative model is an effective way for the prevention HIV/AIDS among Khmer ethnic group.

5.Classification Technology of Traditional Chinese Medicine Based on Latent Semantic Analysis

Modernization of theories of traditional Chinese medicine (TCM) is a key issue for the development of Chinese medicine. As TCM concepts are too vague and lack precise definition, it is difficult to test those theories by modern technologies. Methods of TCM classification are not accepted by Western medicine so far. Because of the diversity and complexity of Chinese herbal medicines, the molecules of Chinese herbal medicines are difficult to be separated, and efficacy is difficult to be verified and classified by biological and chemical methods. In order to verify the scientificity of TCM classification, molecular properties can be studied by their spectrum, and be combined with latent semantic analysis technology and weighting method which are widely used in the Internet search engine. 48 Chinese herbal medicines belonging to four kinds of Chinese herbal medicines:nourishing yin, tonifying yang, invigorating qi, and enriching blood, are analyzed through NIRS, and the results are almost the same as the TCM classification.
6.Efficacy of intensive treatment by automatic continuous positive airway pressure in elderly patients with obstructive sleep apnea-hypopnea syndrome

Objective To investigate the efficacy of the hospital intensive treatment by auto-CPAP on elderly patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and to analyse the possible factors related to the effectiveness.Methods Subjects were selected from elderly OSAHS patients over 60 years old.The eligible subjects were divided into intensive treatment group and non-intensive treatment group (non-ITG).Patients in intensive treatment group were intensively treated using auto-CPAP for 3 consecutive days,assisted with targeted health education and guidance,and making proper adjustments to parameters of the auto-CPAP according to treatment outcomes at the previous day.But those in non-intensive treatment group were just treated with auto-CPAP for only 1 day,supplemented by regular health education and guidance.The apnea-hypopnea index (AHI),minimum oxygen saturation (SaO2min),mean oxygen saturation (SaO2mean) and the time spent at SaO2 lower than 90% (tSaO2<90%),the occurrence of clinical symptoms and adverse effect,Epworth sleepiness scale(ESS) were compared between both groups.The correlation analyses were also conducted.Results ESS (3.58±3.76 vs.6.84 ± 3.22),AHI [(7.85±6.53) time/h vs.(10.42±7.27) time/h] and tSaO2<90%[(5.65±15.43) min vs.(15.26±33.14) min] were lower(t=6.902,2.760,2.765,allP<0.05),while SaO2mean [(96.57±1.53)% vs.(94.63±1.38)%] and SaO2min [(88.24±4.43)% vs.(83.28±5.06)%]were higher(t=-9.870,-7.740,both P<0.05)in intensive treatment group than in non intensive treatment group.The occurrences of clinical symptoms and adverse effect (except for skin allergy) were decreased in intensive treatment group versus non-ITG (all P<0.05).The AHI after intensive treatment was decreased in females versus males [(7.85±5.19) times /h vs.(11.27±7.78) times/h,t=2.133,P<0.05].BMI,age,gender and the state of OSAHS were correlated to AHI,SaO2 mean,SaO2 min and tSao2<90% after intensive treatment (all P<0.05).Conclusions The hospital intensive treatment can improve the clinical results and decrease the occurrences of clinical symptoms and adverse effect for elderly OSAHS patients,and is expected to improve therapy compliance,and its curative efficacy is correlated to BMI,age,gender and state of OSAHS before therapy.
7.Occlusion of Esophageal Fistula with Medicinal Albumin Gel under Gastroscope

OBJECTIVE:To explore the method and efficacy of occlusion with medicinal albumin gel for esophageal fistula. METHODS:Over the recent 5 years,a total of 11 patients with esophagus -respiratory fistula or esophagus-neck fistula confirmed by iodine imaging and / or gastroscopy were occluded with medicinal albumin gel under endoscope. RESULTS:The occlusion with medicinal albumin gel for esophageal fistula was successful in all the 11 cases,and 9 out of 11 were completely healed after one time occlusion treatment and the other 2 cases were completely healed after the second occlusion treatment. The average hospital stay was 27 days and the average cost was 24,500 yuan. There was no recurrence after follow-up of 3 to 6 months,nor was there any adverse complication. CONCLUSION:Compared to the conventional conservative treatment,occlusion with medicinal albumin gel for esophageal fistula under endoscope can reduce the length of hospital stay and the cost of hospitalization.

9.Quercetin and X-Ray synergially inhibit the collagen synthesis of cultured keloid fibroblast

Objective To study the influence of quedcetin on the collagen synthesis of cultured fibroblasts and to explore the mechanism.Methods The inhibitory effect of quercetin and radiation on fibroblast proliferation was assayed using MTT assay.Collagen synthesis was detected by hydroxyproline colorimetric analysis.Immunocytochemical staining method was used to investigate the expression of collagen Ⅰ and Ⅲ.The mRNA expression of typeⅠ,type Ⅲ collagens and TGF?-1 were assayed by reverse transcriptionpolymerase chain reaction(RT-PCR) and real-time PCR technique.Results Keloid fibroblast cell proliferation and collagen synthesis of fibroblasts were inhibited by quercetin in a dose-dependent manner.Significant inhibition was observed by the treatment with quercetin and radiation together.Immunocytochemical staining indicated the IOD of type I and Ⅲ collagen protein was down-regulated by quercetin and radiation.Both collagen Ⅰ and collagen Ⅲ gene in the quercetin groups showed a significantly decreased mRNA expression compared with that in the untreated group,especially in the group treated with both quercetin and X-ray.Procollagen gene expression was inhibited and then decreased type Ⅰ and Ⅲ protein syntheses of fibroblsts,particularly type Ⅰ procollagen gene(P
10.Antithymocyte Globulin Combining Cyclosporin in Treatment of Pediatric Severe Aplastic Anemia

Objective To analyze the effectiveness of antithymocyte globulin(ATG) combined with cyclosporin(CSA)in treatment of pediatric severe aplastic anemia(SAA).Methods Seven children with SAA were treated with ATG and CSA,ATG 4-5 mg/(kg?d),5 days,methyprednisone 2-3 mg/(kg?d) to reduce anaphylaxis,CSA 3-5 mg/(kg?d),assisting with transfusion and G-CSF.Results The complete remission rate was 28.57%(2/7),the partial remission rate was 14.29%(1/7),and the obvious improvement rate was 28.57%(2/7),1 child died.The overall response rate was 71.43%.Transfusion volumes in 4 to 6 months post treatment decreased obviously comparing to the first 3 months(P
