1.Doctor Xuan Guiqi's Experience in the Diagnosis and Treatment of Childhood Asthma and the Thinking of Improving Efficacy

[Objective]To summarize Doctor Xuan Guiqi's experience in the treatment of childhood asthma and the idea of improving the curative efficacy. [Methods]By learning from Doctor Xuan and clearing up medical cases, the author finishes the summary on doctor Xuan's clinical experience on child-hood asthma based on the syndrome differentiation and clinical medication.[Results]The childhood asthma is divided into acute and chronic, the former is divided into cold asthma, heat asthma and wind-phlegm types. At the same time, we strengthen pertinence treatment of clearing heat and removing toxi-city, Qi spasmolytics, dispel ing wind and desensitization, activating blood and eliminating stasis, promoting Qi and regulating middle-jiao and relieving dyspepsia.[Conclusions] Doctor Xuan Guiqi's experience in the diagnosis and treatment of childhood asthma can significantly improve the clinical efficacy , and it is meaningful to clinic.
2.Functional food - nutraceutical product: Some opinions by view-point of management

The concept of nutraceutical food was introduced firstly in United State in 1989 by Stephen Defelice, the founder of the Foundation for Innovation in Medicine. It is defined as a product that considered as a food or a part of food, provided medical or health benefits, including disease prevention and treatment”. The health sector should review and re-evaluate the importance and potential of the nutraceutical food market and plan the development strategy for this novel avenue. The unique definition of nutraceutical product is needed. The Ministry of Health should direct the management of these products as medications rather than foods and the regulations of registration, labeling and take testing samples should be available

3.Allergic reaction to drugs in elderly person at Huu Nghi Hospital

Allergic reaction to drugs (ARTDs) occurs frequently in adults who are usually suffered from many diseases and who require multiple courses of medications. Investigative result showed that: The incidence of ARTDs in elderly is 6.74%, this incidence in patients of Asthma group is 14.7%. The main cutaneous manifestations of ARTDs in elderly patients included: urticaria and erythematous rashes (42.5%), erythrodermie (14.9%), erythematic multiform (21.3%), fixed drug eruptions (12.7%), vesiculobullous eruptions (6.4%).

4.The relationship of multi-drug resistance associated protein and refractory epilepsy

The mechanism of drug resistance of refractory epilepsy has remained unclear.In recent years,studies have shown that the multi-drug resistance associated protein(MRP) is related with refractory epilepsy.The mechanism responsible for the drug resistance may have something to do with the higher expression of MRP1 mRNA in brain tissues of refractory epilepsy patients.Through the use of drugs in cells to cells from the pump,it can reduce the drug concentration in brain and make the creation bears the medicine and inhibitors specific for MRP,which can significantly improve the effect of some antiepileptic drugs.Thus,further studies may provide new ways for the treatment of refractory epilepsy and find targets for new antiepileptic drugs.
5.Surgical timing of severe infectious endophthalmitis

Objective To investigate the clinical efficacy and surgical timing of vitrectomy combined with silicone oil tamponade for severe infectious endophthalmitis. Methods Sixty-two patients (62 eyes)with endophthalmitis, diagnosed by the examinations of the best corrected visual acuity (BCVA),intraocular pressure, slit-lamp microscopy, direct and (or) indirect ophthalmoscopy and ocular B-ultrasound. There are 44, 17 and 1 eyes with posttraumatic, postoperative and endogenous infectious endophthalmitis, respectively. The patients were randomly divided into the group A (32 eyes) and B (30eyes). The former was treated immediately by vitrectomy combined with silicone oil tamponade after diagnosis, while the latter was treated by the same surgery after drug treatment depended on patients'choice. Vitreous purulence was taken in all patients before vitrectomy for bacterial, fungal culture and drug sensitivity test. 19/62 (30.65%) vitreous samples were positive for culture. The follow-up was ranged from 6 to 26 months. The visual acuity, intraocular pressure and eye retention situation before and after surgery were comparatively analyzed. Results In group A, endophthalmitis was controlled in all eyes after surgery;the visual acuity and intraocular pressure improved significantly after surgery (χ2=43.72, 6.83; P<0.05). In group B, endophthalmitis was controlled in 19/30 eyes (63.33%) after surgery; evisceration was performed on 11 eyes (36.67%) because of the atrophy of the eyeball. There was no significant difference of visual acuity before and after surgery. Conclusions Vitrectomy combined with silicone oil tamponade is an effective way to cure severe infected endophthalmitis. Performing the surgery immediately after the diagnosis is the key to achieve good effect.
6.Extracorporeal life support system for emergency and serious disease in children

Extracorporeal life support system (ELSS) can maintain the heart,lung,liver,kidney function and increased cerebral blood supply and reduce intracranial pressure.In addition,for children with emergency and serious disease combined multiple organ failure (MOF),ECLS has an irreplaceable role in gaining time for the treatment of protopathy,which can ultimately reduce mortality and sequelae for children with emergency and serious disease.This review briefly summarizes the indication,pathophysiology and timing of ECLS in pediatric emergency and serious disease.
7.Role of EGF family in development of endocardial cushion

During early embryonic development,these epithelial cells of cardiac atrioventricular and outflow tract undergo an epithelial-mesenchymal transformation to become mesenchymal cells that invade cardiac jelly,forming endocardial cushion.Epidermal growth factor family plays an important role in regulating epithelial-mesenchymal transformation.The family is composed of multiple ligands and four tyrosine kinase receptors ( ErbB),among which ErbB2 and ErbB3 are necessary for normal development of endocardial cushion.In addition,other receptors and ligands also play important roles in heart development.This review discusses the role of receptors and ligands of the EGF family in heart endocardial cushion development.
8.The similarities and differences between pediatric and adult septic shock

The children with septic shock could not be presented in hospitals with PICU facilities first.Furthermore,from a pathophysiologic perspective paediatric shock does not resemble adult septic shock.This suggests that every physician that could be faced with these children needs to understand how to recognize paediattic shock and have basic knowledge of the principles of primary management.

The importance of hemodynamic monitoring is reflected in the correct interpretation of the monitoring results and taking simultaneously the appropriate treatment strategy according to the correct interpretation of the results.The essence of the shock is body tissue hypoperfusion,whtich result in tissue hypoxia and organ dysfunction.The essence of the hemodynamic imbalance is the imbalance of oxygen supply and oxygen consumption.Therefore,the end point of the fluid resuscitation for shock is now increasing emphasis on monitoring mixed/central venous oxygen saturation and the concentration of blood lactate,the rate of lactate clearance.
10.Applications of proteomics in SIRS and sepsis

Sepsis,the systemic inflammatory response syndrome(SIRS)induced by infection,is the first cause of death of patients in intensive care unit. Early diagnosis of sepsis with the actions of corresponding treatment can improve the prognosis of sepsis.It is an appropriate way to achieve early diagnosis of sepsis that using proteomics methods to detect unknown proteins during early stage of sepsis,in which we search for new protein biomarkers.
