1.Spatio-temporal clustering analysis of pulmonary tuberculosis among the elderly in Shaoxing City

Objective:
To investigate the spatio-temporal clustering characteristics of pulmonary tuberculosis (PTB) aged 60 years and above in Shaoxing City from 2019 to 2023, so as to provide insights into prevention and control of PTB among the elderly.
Methods:
Data of PTB cases aged 60 years and above in Shaoxing City from 2019 to 2023 were collected from Tuberculosis Management Information System of Chinese Disease Prevention and Control Information System. The population data were collected from Shaoxing Statistical Yearbook. Vector map information was collected from Shaoxing Geographic Information Public Service Platform. The spatio-temporal clustering characteristics of PTB cases aged 60 years and above were analyzed using global spatial autocorrelation, local spatial autocorrelation and spatio-temporal scanning.
Results:
Totally 3 722 PTB cases aged 60 years and above were registered in Shaoxing City. The average annual registration rate was 61.71/105, showing no significant trend (P>0.05). Totally 2 548 pathogenetically positive cases were registered, with an average annual registration rate of 42.25/105. Spatial autocorrelation analysis showed there was a positive spatial correlation of PTB in 2019 and 2021 (both Moran's I>0, both P<0.05). Shengzhou City showed a high-low clustering, and Keqiao District and Shangyu District showed a low-low clustering. Spatio-temporal scanning analysis showed that a class Ⅰ cluster was located in Shengzhou City, with aggregation time from March 1, 2019 to August 31, 2021. The class Ⅱ clusters were located in Zhuji City, Shangyu City and Keqiao District, with aggregation time from March 1, 2021 to August 31, 2023, from April 1, 2021 to September 30, 2023, and from June 1, 2021 to November 30, 2023, respectively.
Conclusion
The PTB cases aged 60 years and above in Shaoxing City from 2019 to 2023 mainly concentrated in Shengzhou City, where the prevention and control of PTB among the elderly should be strengthened.
2.The effects of shenxiong injection on the expression of the endoplasmic reticulum stress associated molecules CHOP following cerebral ischemia-reperfusion injury in rats

Objective To investigate the expression of C/EBP homology protein(CHOP) in rat brain tissue after focal cerebral ischemia-reperfusion,as well as to observe the influence of the shenxiong injection on the expression of CHOP.Methods One hundred forty four male rats were randomly divided into three groups:the sham-operation group,operation group,shenxiong group.Focal cerebral ischemia and reperfusion rat models were established by using suture.Zea Longa method was introduced to evaluate neurologic behavioral changes.The levels of mRNA and protein of CHOP were measured with methods of immunohistochemistry and reverse transcription polymerase chain reaction.The neuronal apoptosis was detected by the method of terminal deoxynucleotidy1 transferase-mediated dUTP-biotin nick end labeling.Results The expression of CHOP was up-regulated in per-infarction after cerebral ischemia-reperfusion in rats.CHOP mRNA and protein levels peaked at the 12th h and 24th h after reperfusion,respectively.The apoptosis cell counting increased gradually after cerebral ischemia-reperfusion and peaked at the 24th h after reperfusion.Compared with the saline control group,treatment with shenxiong injection could reduce the neuronal apoptosis at the 6th,12th,24th,and 72nd h after reperfusion (P <0.01).Compared with the saline control group,treatment with shenxiong injection could decrease CHOP mRNA and protein expression at the 6th,12th,24th,and 72nd h after reperfusion (P <0.01).Conclusions Shenxiong Injection may prevent neurocyte from apoptosis by inhibition of the expression of CHOP induced by endoplasmic reticulum stress.
3.Effect of Shenxiong Injection on the expression of X-box binding protein 1 of endoplasmic reticulum stress following cerebral ischemia-reperfusion injury in rats

Objective To observe the effect of Shenxiong injection on the changes of brain X-box binding protein 1 (XBP1) gene expression following cerebral ischemia-reperfusion in rats and to investigate its neuroprotective effect and mechanisms. Methods One hundred forty-four male SD rats were randomly assigned to sham-operation, saline control and Shenxiong groups. A rat model of middle cerebral artery occlusion was induced using the suture method. Shenxiong injection 33.3 ml/kg was injected intraperitoneally during the reperfusion in the Shenxiong group. The same volume of saline was injected intraperitoneally in the saline control group. The animals were sacrificed at 6, 12, 24, and 72 hours after the reperfusion.Meanwhile, immunohistochemical staining and reverse transcription-polymerase chain reaction (RT-PCR) were used respectively to detect the expressions of XBP1 protein and mRNA in the cerebral ischemic areas. TUNEL was used to detect neuronal apoptosis. Results The levels of XBP1 protein and mRNA increased after the reperfusion, and it reached the peak at 12 hours; both the numbers of apoptotic cells and neurological scores in the Shenxiong group were lower than those in the saline control group at 6, 12, 24, and 72 hours after the reperfusion (all P <0.01); the levels of XBP1 and mRNA at 6, 12, and 24 hours after the reperfusion were alsosignificantly lower than those in the saline control group (all P < 0.01). Conclusions Shenxiong injection may have certain inhibition on the endoplasmic reticulum stress activated XBP1 pathway induced by cerebral ischemia-reperfusion.
4.Relationship between blocking of N-acetylglucosaminyltransferase V in hepatocarcinoma cells and cellular endoplasmic reticulum stress

Purpose:To study the connection between the blocking of N-acetylglucosaminyltransferase V(GnT-V) and endoplasmic reticulum stress(ER stress). Methods:The recombinant plasmid expressing siRNA of GnT-V was constructed and transfected into SMMC-7721. The key molecules such as GRP78,XBP1 and PERK during ER stress was analyzed in mRNA and protein levels. Results:GRP78 mRNA and protein levels were upregulated, the spliced forms of XBP1 mRNA and protein appeared and ER eIF2? kinase PERK was activated. Conclusions:These results suggest the blocking of GnT-V in SMMC-7721 induces cellular ER stress.
5.Study on chemokine receptor CX3CR1 gene T280M polymorphism in patients with ischemic cerebrovascular disease

Objective To investigate chemokine receptor CX3CR1 gene T280M polymorphism in patients with ischemic cerebrovascular disease(ICVD) and its frequency.Methods 165 patients with ICVD(cerebral infarction 85 cases,lacunar infarction 40 cases,transient ischemic attack 40 cases) and 150 age- and sex-matched healthy controls(normal control group) were involved in this study.The polymorphism of T280M was analyzed by multiplex polymerase chain reaction-restriction fragment length based(PCR-RFLB),the gene frequency was compared between two groups and each ICVD subgroups.Results There were TT and TM genotypes in normal control group,and TT,TM and MM genotypes in ICVD group.The genotype were significant differences between the two groups(P0.05).Conclusions There is MM genotype in the chemokine receptor CX3CR1 gene T280M in the patients with ICVD,and their M allele frequency obviously increase.It suggests that CX3CR1 gene T280M polymorphism may be associated with ICVD.

7.Efficacy of posaconazole in preventing invasive fungal disease in hematologic patients with neutropenia

Objective To evaluate the prophylactic efficacy and safety of posaconazole against invasive fungal disease(IFD)in hematologic patients with neutropenia.Methods Medical records of 18 hematologic patients with neutropenia received posaconazole for preventing IFD in a Beijing hospital between 2014 and 2015,the efficacy and safety was evaluated.Results There was no clinical diagnosis or confirmed diagnosis of IFD among 18 patients during posaconazole prophylaxis period,none of patients stopped posaconazole due to severe adverse reaction.Two patients with acute myeloid leukemia(AML) died of pulmonary infection,1 of whom isolated Stenotrophomonas maltophilia from sputum culture on the 12th day of posaconazole prophylaxis,the other isolated Escherichia coli from sputum culture on the 14th day of posaconazole prophylaxis.Other patients all adherence to posaconazole prophylaxis until granulocyte count recovered,patients were followed up until 100 days medication,no death occurred.The lowest peripheral neutrophil count was(0.00-0.27) x 109/L during posaconazole prophylaxis period,with the median of 0.02 x 109/L;the duration of posaconazole prophylaxis was 8-27days,with the median of 16 days;among patients without IFD breakthrough or received systemic use of other antifungal agents,there were 2 (11.1%) all-cause death within 100 days;there were no adverse reaction,such as liver function abnormalities≥grade 2 and kidney function abnormalities≥grade 2,as well as QTc prolongation.Conclusion Posaconazole is effective for preventing IFD in hematologic patients with neutropenia,adverse reaction is rare.
8.Thoracoscopic esophagectomy for esophageal carcinoma

Objective To investigate indications of video-assisted thoracoscopic surgery in the treatment of esophageal carcinoma.Methods Video-assisted thoracoscopic surgery was performed in 11 patients with esophageal carcinorma from January 2005 to November 2005.The tumors were located at the upper segment in 2 patients,middle segment in 8 patients,and lower segment in 1 patient.The TNM staging results showed T_1N_0M_0 in 1 patient,T_2N_0M_0 in 2 patients,T_2N_1M_0 in 1 patient,and T_3N_0M_0 in 7 patients.Under thoracoscopy via thoracic and cervical incisions,the esophagus and tumor were disconnected and the intrathoracic lymph nodes were cleared through the right thorax.Then a median incision was made at the epigastrium.Through the incision the stomach was disconnected and intraabdominal lymph nodes were removed.Afterwards,esophagectomy and gastroesophagostomy were performed by way of the cervical incision.Results The operation time was 220~300 min(mean,265 min).The operative blood loss was 150~400 ml(mean,240 ml).The number of removed lymph nodes was 4~10(mean,6.4).The drainage tube was taken out at 2~3 days after operation,with a drainage volume of 200~500 ml(mean,350 ml).One patient developed a temporary left recurrent laryngeal nerve palsy that disappeared at 3 months after operation.Follow-up checkups for 4~15 months(mean,8 months) in the 11 patients showed no recurrence or metastasis of tumor.Conclusions Video-assisted thoracoscopic surgery can be adopted in patients with esophageal carcinoma at stage Ⅰ or Ⅱ,
9.Research on design and clinical application of a detection equipment used in arteriosclerosis

Objective: To design device to measure D/T index which is used for evaluation of arteriosclerosis. Methods:By calculating the detected date of blood pressure values, pulse pressure, pulse rate, pulse patterns and SpO2 on the extremities, we get a set of analytical data in the system in order to analyze the arteriosclerosis situations. Results: The device use the ECG signal as it based and modulated signal, the computer process the date of blood pressure and SpO2, then output the result by the display and printing systems. Conclusion:The device provides an effective noninvasive detection means for extremity vascular.
10.Effect of Processing on Pharmacological Actions of Dipsacus Asperoides and Its Different Processed Products

Objective:To study the effect of processing on pharmacological actions Dipsacus asperoides and its different processed products.Methods:The analgestic effect of Dipsacus asperoides in mice was observed in hot plate and acetic acid-induced writhing test. The anti-inflammatory action was studied in mouse ear swelling test and capillary permeability test and anti-coagulation action in mice was also observed by mouse blood coagulation time test.Results:The experimental results show that all processed Products of Dipsacus asperoides can inhibit the pain and has the actions of auti-inflammation and auti-coagulation. Conclusion: The actions of its processed products with wine is the most powerful.
